1. INTRODUCTION

HFE Close-out Report is a DCAF deliverable for the Execute phase. The purpose of this report is to document how the Project applied HFE Principles for the Project Cycle from Select to Operate. Therefore the close-out report is devided into 2 Main Sections Pre-PSUA and Post PSUA.

2. OBJECTIVES2.1 Pre-Startup

HFE Close-out Report shall be started prior to Pre-Startup Safety Audit and include the following:

HFE Design Studies Completed in Define. HFE ALARP review (if required). HFE Design Studies Closeout and HFE Verification Approval

2.2 Post-Startup

HFE Close-out Report shall be completed after all of the HFE Actions from the Pre-Startup Safety Audit have been closed.

1. The Screening report/HFE Strategy should be regarded and treated as any CMF deliverables for the project. It should be distributed for comment and review; at least to the individuals who participated in the screening, and approved by the project manager. It should be kept in project documentation for auditing purposes. 2. The Actions adopted should be put into a tracking register, either stand-alone or as part of a master HSSE register. 3. The project (ideally through the HFE AP, but at least by the TSE) should track the progress and timeliness of the Actions and the necessary measures to close them. 4. The project should verify compliance with the Actions and the design requirements, through design reviews, and during construction. A final HFE close-out report or summary confirming close out of all Actions is required.

1. Are there known issues with operability and maintainability with similar equipment or have there been incidents where poor HFE design was a contributing factor to the incidents?

The Project should ensure relevant learning from incidents and known O&M issues are addressed in the design by proposing specific actions or design requirements for implementation by the project team. The Project should apply EMIS.PMQ.02 (Task Requirements Analysis (TRA)).

N.B. TRA will require support from a HFE Specialist – contact the PDO HFE TA (MSE42 / MCOH1) for more advice.

2. Will the project involve procuring and locating any of the following:• Manually operated valves• Automatic valves that require status verification or quick access

If valves are included in the project scope then the HFE Strategy should ensure the design allows for safe access and operation of all valves according to their criticality. This could be achieved by requiring the following• Identification of critical valves and defining the accessibility requirements (including vendor supplied skid packaged unit valves).• Determining which valves require actuators or remote operators. • Identifying critical valves within P&lDs to facilitate design verification • Verifying that the Project complies with the design requirements

The objective here is to ensure the risk to health, safety and human reliability during operation or maintenance of vendor supplied packaged units is minimized. The key activities necessary to support such a design is by: • Identifying relevant HFE design requirements and including them in Vendor Package Specifications. • Ensuring the Vendor provides adequate and early opportunity for design reviews.

6. Will the project involve new equipment items, or modifications to existing equipment that require human intervention during operation, maintenance and/or inspection?

The objective here is to ensure efficient and safe access to equipment for operation, maintenance and/or inspection activities under all operating conditions. This can be achieved by identifying and incorporating relevant HFE design requirements and verifying compliance throughout the design process.

The objective here is to ensure signs and labels are visible and legible, that where applicable they indicate the presence of hazards and dangers, that they minimize the risk of equipment misidentification and support personnel to execute tasks safely and efficiently. This can be achieved by identifying and then specifying the relevant design requirements as well as approving the content, design and positioning of signs and labels when appropriate.

8. Has the Project defined how it will verify compliance with the HFE requirements and who will be responsible?

HFE design verification is essential for both technical and quality assurance. The verification has to be done by someone with the appropriate HFE competence (i.e., the HFE Authorized Person) and has to be done against the project HFE standards and any HFE design requirements identified in the project HFE Strategy Report. It should start early to avoid costly modifications at a later stage. It is important to liaise with operations and maintenance (O&M) during these reviews. Verification activities can take many forms, for example: • Review of plot plan or general arrangement drawings• Review of equipment specifications and other design documentation • Review of detail layout drawings, in particular vendor supplied packages and control panels/cabinets • Participation in 3D model reviews (30/60/9096) (see DEP 30.10.05.11-Gen.) • Validation during construction inspections and walk-through to ensure that the HFE design intent is assured and is not compromised by decisions made to meet conflicting construction requirements.
