KEY POINTS TO KNOW ABOUT THE AFLAC ACCIDENT ADVANTAGE ELIGIBILITY The current Accident Indemnity Advantage (Policies A35B24, A35100 and A35200) and Off-the-Job Accident Only Indemnity Advantage (Policies A35BOF, A35300 and A35400) will be withdrawn from payroll, union and association sales within 30 days of introduction in your state. Accident Indemnity Advantage plan (Series A35000) will remain available on a direct basis only. Accident Advantage policies (Series A36100, A36200, A36300, A36400 and A363OF) may be issued to qualified applicants ages 18 through 75 on Payroll/Employee Direct Bill (EDB),Union or an Association basis. There are two optional riders that may accompany the Accident Advantage: Additional Accidental-Death Benefit Rider and the Aflac Plus Rider (issue ages for additional riders are 18 – 70) – please refer to your state specific administrative guidelines for detailed information as rider availability may vary by state. The plan is Guaranteed Issue; an underwriting question will only apply to Association and Associate Only applications. The plan is eligible for pre-tax deduction under Section 125 Cafeteria Plan on a payroll basis only. Associations and Unions are only eligible for after-tax deductions. Aflac’s Accident Advantage (Series A36000) plan can be solicited in accounts where other Aflac Accident products have been sold in the past. The Accident Advantage cannot be added to a payroll account during a mid-year enrollment unless (1) it is added on an after-tax basis or (2) plan documents are amended and Aflac receives an M0138 with the hold harmless agreement section completed for existing accounts. BENEFITS (Enhanced & New)

• The Accident Treatment Visit, formerly the Emergency Treatment Benefit, has been enhanced and is now a tiered benefit

for physician and ER visits. Additional Accident Treatment Visit benefits include:

- Added treatment by nurses and Physician’s Assistants

- Added previous standalone x-ray benefit to the benefit

• The Wellness benefit has removed its 12 month waiting period and is now based on a calendar year rather than a policy

year (once per policy, per calendar year as long as they’re covered under the policy).

• Follow-Up Visit benefits have been enhanced to include added treatment by nurses and Physician’s Assistants.

• The enhanced Therapy Visit, formerly called the Physical Therapy Benefit, includes occupational and speech therapy in

addition to physical therapy.

• The Appliance benefit is now tiered with higher benefit payouts for higher prices appliances such as wheelchairs, etc.

• The Prosthesis benefit has added an additional benefit that will pay for initial loss as well as the replacement of existing

prosthesis previously paid under the Prosthesis Benefit or replacements due to a covered injury which requires repair/

replacement of existing prosthesis.

• NEW Organized Sporting Activity benefit will pay an additional 25% of the total benefits payable while participating in an

organized sporting activity. Limited to $1,000 per policy, per Calendar Year. NOTE: Organized Sporting Activity defined

as – A competition or supervised organized practice for a competition. The competition must be governed by a set of

written rules, be officiated by someone certified to act in that capacity, and overseen by a legal entity such as a public

school system or sports conference. The legal entity must have a set of bylaws and competition must be on a regulation

playing surface. Participation must be on an amateur basis.

• NEW Family Support benefit pays $20/day up to 30 days per accident, per covered person when qualified for benefits

under the Accident Hospital Confinement Benefit; this benefit can be used to help pay for daycare, dog sitting, etc., while

confined in a hospital.

• NEW Home Modification benefit due to catastrophic loss of limb or sight to help with any home modifications such as

wheelchair ramps, lowering countertops, etc.

• NEW Waiver of Premium benefit waives premium if total disabled for 180 consecutive days; please see your state

specific administrative guidelines as most states will be limited to a total max of 36 months per eligibility of the benefit.

• NEW Option 4 has been added to provide a more robust product offering.

ASSOCIATE INFORMATION

• Accident Advantage is available for purchase by the field force through the agents’ account (00316), paper only.

• Production for the Accident Advantage will count towards nationally sponsored contests. For standard associate and

union commissions please refer to your state specific administrative guidelines.
