Accessing Your Educator Evaluation Forms

Getting Started — MPS main page — Quicklinks — My Learning Plan — Login

1. On the left side, choose

My Evaluations

2. Choose Evaluation

3. Choose an Educator Eval-uation component by click-ing on the + sign to the left of the component title

4. The component will open and status information will appear

5. Click on the “Document” icon and the selected form will open

2. Choose Artifact Files

3. Choose Add Artifact

An Upload file box will appear

You many upload:

Word documents, Excel files,

jpeg images or PDFs

4. Fill out the following areas:

Name: Choose a name that will help iden-

tify the artifact.

Description: Explain briefly why you feel

this artifact is appropriate as evidence.

Share what you would like the evaluator

to know about this artifact. If you feel it

reflects a level 3 score, begin with

“LEADERSHIP ROLE” so they know.

5. Choose Types:

Click on the arrow to the right

A drop down menu will appear

Click on the appropriate category

6. Categories — This will send artifact to the

chosen documents

Professional Foundations — N/A

SLOs — Choose EdEval #1 Goal/Objective

for SLO #1 or EdEval #2 Goal/Objective for

SLO #2 etc.

Prof Growth Goal — Choose EdEval #1

Goal/Objective for Goal #1 etc.

7. Alignment:

Choose a Rubric (Prof Found only)

Click on the arrow to the right

A drop down menu will appear

Click on the appropriate category

THEN

Choose a Criteria (Prof Found only)

Click on the arrow to the right

A drop down menu will appear

Click on the appropriate criteria — Criteria

choices are based on the Rubric selection

made previously

8. Click Add Criteria* and the Rubric compo-

nent and Criteria you selected for this artifact

will show

* IMPORTANT! — Add Criteria will cause

the artifact to be aligned with the correct

document (Prof Foundations, Prof Growth

Goal, or SLOs) and save you the time and

trouble of uploading the artifact again!

11. Artifacts that have been successfully up-loaded will be reflected on the screen

On the right, the following icons:

Tells what type of file

Click on it to open file

Pencil — Edit the Artifact entry (Not the actual artifact)

Delete the Artifact entry

Uploading Artifacts from My Learning Plan Professional Development

(Saving a file as a PDF from My L Plan and uploading it as an Artifact)

1. To create a PDF from My L Plan activities:

Open to the main page that lists all the activities in

which you have participated

Select the one you would like to use as an Artifact

by double clicking on it

Another screen (Activity Status) will open that

gives the details of the activity

9. To Upload a File:

Click on the Blue Arrow in the lower right corner

Your documents library will open

Double click on the file you would like to upload

to open it

If successful, you will see the file name in place

of Select a file

10. When finished, click Save

3. Scroll to the bottom of the District Pro-

fessional Development screen and choose

Print

4. A grey screen will appear along with a

Print box

Choose PDF creator

Then click Apply at the bottom of the

Print box (it will fade out)

Then click Print — it will not print, but

will open a document box (step 5)

2. At the bottom of the Activity Status screen, choose

View/Print Form

Another screen will open —

You can close this screen— the

document has been converted and

7. To upload this new PDF file to your Artifact File, follow the directions for

Uploading Artifacts from Your Document Library (M Drive)

5. A PDF Creator box will appear

Document Title: Create a name that reflects

what the document is (for example, PD_ELA Cur-

riculum)

Then click Save at the bottom of the PDF Creator

box

Your Documents Library (MDrive) will open

Choose which folder you will save these PD Arti-

facts in and double click to open

Click Save at the bottom

A small PDF Creator sign will appear to let you

know that the document is saving

You can minimize/maximize each section by clicking on the arrow in the upper right corner

3. Evaluator will score the submitted artifacts

4. Evaluators may add a rationale, justifying the score

5. You can upload artifacts here for each indi-vidual PF component OR you can link artifacts from your Artifact Files (see the instructions for Uploading Artifacts)

6. If you upload items into Artifact Files and note that they align with Prof Foundations, the artifacts will show up here at the very bottom of the form

8. Once you have received your scores, you are asked to ACKNOWLEDGE that you received them — this does not mean you agree or disa-gree, simply that you received the form back with scores from your evaluator. The Evalua-tor will then FINALIZE the document as com-pleted.

7. When you have everything uploaded, you should click SAVE if you are still working on it or SAVE and NOTIFY to let your evaluator know you are ready for them to score. (See notes to the right.) Your evaluator will score the arti-facts and will submit the scored document back to you.

There are 3 SLO forms — Initial, Mid-Point, Results. The information for the Mid-Point and Results SLO forms pulls from this SLO form.

Complete the following sections:

1. Objective Title — What is the focus for this SLO?

2. Content Area — Which grade levels does this SLO apply to?

3. Objective Statement — Describe the over-all objective, including whether it focuses on progress (i.e., expecting students to make a certain amount of progress within an interval of instruction) or mastery (i.e., expecting stu-dents to meet a particular bar or standard).

4. Rationale — Describe the reasoning for this objective, including whether it is aligned to a school-wide SLO, and what data informed this decision.

5. Align Standards —Specify the CCSS, RI GSEs/GLEs, or other RI/national standards to which this objective is aligned

6. Students — Specify the number of and grade/class of students to whom this objec-tive applies.

7. Interval of Instruction — How long will this SLO be measured?

8. Baseline Data — Describe the pre-test or baseline information/data available for this student population that informed the target (e.g., are students entering without, with, or above the necessary prerequisite knowledge or skills?).

Complete the following sections:

9. Target — Describe where you expect students to be at the

end of the interval of instruction. If baseline data suggest mean-

ingful differences in prerequisite knowledge or skills, targets

should be tiered to be both rigorous and attainable for students

at various levels.

10. Rationale for Target— Explain how the target was

determined (e.g., pre-test, baseline, or historical data on your

current students, or historical data from past students). Explain

why the target is appropriate (both rigorous and attainable) for

all students.

12. Evidence Source Level of Standardization —

Indicate the appropriate level of standardization; High:

assessments administered and scored in a standardized

manner; Medium: assessments with lacking standardiza-

tion in administration and may have subjective scoring;

Low: assessments not ad ministered and scored in a

standardized manner.

13. Evidence Source Description — Describe what

assessment you will use to measure student learning and

why the assessment is appropriate for measuring the

objective. At least one assessment as a source of evi-

dence is required, but multiple sources may be used. If a

common assessment exists, it must be used as the prima-

ry source of evidence.

14. Evidence Source Administration — Describe how

the assessment will be administered (e.g., once or multi-

ple times; during class or during a designated testing

window; by the classroom teacher or someone else).

15. Evidence Source Scoring — Describe how the as-

sessment identified in the Description section will be col-

lected and scored (e.g., scored by the classroom teacher

individually or by a team of teachers; scored once or a

percentage double-scored).
