    Background: The Jharkhand Rural Health Mission Society (JHS) has expressed its intent towards the

    operationalization of 10 Primary Health Centres (PHCs) through Public Private Partnership. The PHC / PHCs will

    be handed over to the NGOs/ Corporate / Private Agencies based on the strengths of the proposals received

    against the clauses mentioned in Expression of Interest floated by JHS. The government, as of now, plans to hand

    over 7 out of the 10 PHCs to NGOs. Before the final handing over, JHS wants a third party assessment of the

    PHCs to be done. The physical infrastructure of the PHCs will be handed over as per the inventory jointly agreed

    upon by JHS and concerned NGO.

    Scope of Work: The Consultant will provide technical support in carrying out the Facility survey of the

    identified PHC/PHCs which will include the following activities:

    1. To examine the status of infrastructure

    2. To examine the status of human resource

    3. To examine the status and functionality of equipment

    4. To examine the status of type of services being provided as per the IPHS to the community.

    The questionnaire/ check-list to be used for the mentioned survey have been developed by ITAP and

    encompass Indian Public Health Standard (IPHS) guidelines and other relevant questions required for

    contracting out of facility.

    Facilities visited:

    Above team has carried out the Facility survey of the identified three PHCs which are as follow:

    Report on Singhari Primary Health Centre

    A team of consultant visited Singhari Primary Health Centre which comes under Meharama block of

    Godda district on 6th

    July 2011. This PHC is located 3 kilometres away from Meharma CHC. Team of

    consultant is also accompanied by District Programme Manager and Medical Officer of respective areas.

    Status of infrastructure:

    This PHC is located within village locality. PHC building is a designated government building available

    for PHC only with staff quarters handed over by District collector on 11th

    April 2010. The PHC is just a

    building, which has functioned occasionally to host eye surgical camps. The building is of very good

    quality, and has been designed as a hospital with residential quarters for doctors and other staff. PHC

    have partial compound wall. Walls and floors are in good condition. Good cleanliness is maintained in

    OPD, Rooms, Wards, Toilet and entire premises. A prominent display board regarding availability of

    services was not there. There are separate registration counters but not functioning. There is no

    pharmacy for drug dispensing and drug store. There are separate toilets for male and females but not

    functioning. Separate wards for male and females are there. Separate OPD rooms are available. There is

    no suggestion/complaint box available at PHC. Family welfare clinic, Newborn care corner, DOT

    centres are not there. No operation theatre is available although there is space for same . Labour room and

    laboratory are not available. The main water supply is hand-pump/tube well and there is a over head tank on PHC.

    Sewerage facility is there with soak pit. There is no electric connection. Generator is not available. Laundry

    facility, communication facility, and vehicles are not available. There is a separate space for office, store room

    and kitchen. There are staff quarters but only two 4th grade staffs are staying.

    Status of Human resources:

    Mr. Rajpathi Tandan (Male ward attendant) from Meharama CHC and Mr. Sanath Lal soren fromBlawadah PHC is deputed to Singhari PHC.

    Status and functionality of equipment:

    No equipments are available there.

    Status of type of services being provided as per the IPHS to the community:

    Occasionally eye camps were held at PHC building. There are no services being delivered to community

    at Singhari PHC.

    Report on Teenpahaar Primary Health Centre

    A team of consultant visited Teenpahaar Primary Health Centre which comes under Rajmahal block of

    Sahebganj district on 7th

    July 2011. This PHC is located 13 kilometres away from Rajmahal CHC. Team

    of consultant was also accompanied by District Programme Manager and Medical Officer of respective

    areas.

    Status of infrastructure:

    The PHC building was built under the MP fund and handed to the Health Department by the District Collector

    recently. This PHC is located within village locality. PHC building is a designated government building

    available for PHC. PHC doesnt have compound wall. Walls and floors are in good condition. Good

    cleanliness is maintained in OPD, Rooms, Wards and entire premises except toilets. A prominent display

    board regarding availability of services was not there. There are separate functional registration counters.

    There is a functional pharmacy for drug dispensing and drug store. There are separate toilets for male

    and females but not clean. Space for separate wards for male and females are there but functional.

    Separate OPD rooms are available. There is no suggestion/complaint box available at PHC. There is a

    separate waiting room for patient. Family welfare clinic is functional. Newborn care corner, DOT

    centres are not there. No operation theatre is available. Labour room is available and not functional and not

    well equipped. Deliveries are not conducted due to lack of staff. Laboratory is available and functional but for

    only few diagnostic purposes. The main water supply is hand-pump/tube well and there is no over head tank on

    PHC. Sewerage facility is there with soak pit. There is electric connection lines connected to all parts of PHC.

    One inverter is available and functioning properly. Laundry facility, communication facility, and vehicles are not

    available. There is no separate space for office but space available for store room and kitchen. There are no staff

    quarters.

    Status of Human resources:

    Two medical officers deputed to this PHC. Dr. Mahmood Alam and Dr.Nishant Kujur. Two ANMs were

    posted named Mrs. Sohagini Murmu and Madhuri Kumari along with one MPW male- Manindar Nath

    Yadav on contractual basis. One health educator named Ganesh Prasad Mandal, one clerck Mohd. Tufail

    Ahmed Ansari and one 4th

    grade staff Mr. Chakrawarthi Paaswan were also posted.

    Status and functionality of equipment:

    One binocular microscope with oil immersion is functional. New born care corner is not available, B.P.

    apparatus, stethoscope are available in 2 no. out of which 1 each is functioning. Clinical thermometer,

    hub cutter, examination table, writing tables, plastic chairs, full size steel almirah, fans and one vaccine

    carrier are available.

    Status of type of services being provided as per the IPHS to the community:

    Only primary treatment is available, MCH and RCH services are not available due to non availability of

    staffs and equipments.

    Report on Atka Primary Health Centre

    A team of consultant visited Atka Primary Health Centre which comes under Bagodar block of Giridih

    district on 11th

    July 2011. This PHC is located 11 kilometres away from Bagodar CHC. Team of

    consultant was also accompanied by District Programme Manager and Medical Officer of respective

    areas.

    Status of infrastructure:

    This PHC is located within village locality. PHC building is not a designated government building

    available for PHC and operated in building of veterinary department. PHC doesnt have compound wall.

    Walls and floors are in good condition. Good cleanliness is maintained in OPD and Rooms. There is no

    toilet facility. A prominent display board regarding availability of services was not there. There are

    separate functional registration counters. There is a functional pharmacy for drug dispensing and drug

    store. There is no space for separate wards for male and females. Separate OPD rooms are available.

    There is no suggestion/complaint box available at PHC. There is a separate waiting room for patient.

    all parts of PHC. Laundry facility, communication facility, and vehicles are not available. There is no separate

    space for office but space available for store room and kitchen. There are no staff quarters.

    Status of Human resources:

    The PHC has two doctors, two ANMs, a Laboratory Technician apart from support staff. It has a HMSand a bank account.

    Status and functionality of equipment:

    One functional binocular microscope with oil immersion, one vaccine carrier, IUD insertion kit, normal

    delivery kits, 1 fridge (165 ltr.) are available in functional condition.

    Status of type of services being provided as per the IPHS to the community:

    Only primary treatment is available, MCH and RCH services are limited available due to non

    availability of staffs and equipments.
