Only the mesh repair revealed acceptable recurrence rates with high patientcomfort. From a surgical point of view, the most important prognostic factorfollowing mesh repair is the surgeon's experience.

Evidence from randomised clinical trials andmeta-analysis indicate that a continuousrunning non-absorbable or slowly absorbedsuture such as polydioxanone is the methodof choice for abdominal wall closure.

Prophylactic subfascial non-absorbable mesh reinforcement of midline closure in high-riskpatients can be used safely and effectively to provide extrinsic strength of the woundwithout relying too much on the defective development of its own intrinsic strength and toprevent subsequent incisional hernia.

Preventing parastomal hernia with a prosthetic mesh.Jänes A, Cengiz Y, Israelsson LA.Arch Surg. 2004 Dec;139(12):1356-8.
Does prophylactic biologic mesh placement protect against the development of incisional hernia in high-risk patients?Llaguna OH, Avgerinos DV, Nagda P, Elfant D, Leitman IM, Goodman E.

No further trials should be conducted for evaluation of technique and available materials forelective midline abdominal fascial closure, according to the results of our cumulative meta-analysis. Future trials will have to define the optimal closure strategy in the emergencysetting and relevance of new suture materials and other strategies such as the use ofprophylactic mesh in targeted subpopulations.
