Country groupings:
Low-income countries (<US$ 1,025 per capita GNI) pay
US$ 0.20 per dose
Intermediate countries (US$ 1,025–US$ 1,550) increase co-payments by 15% per year
Graduating countries (>US$ 1,550) increase payments steadily to reach sustainability after five years
