This focus has yielded signif- icant improvements in the quality and safety of our care as evidenced by the “Report Cards” generated by several authorized and qualified organizations, including federal and state government agencies. In “Patient Care,” there are a number of meaningful measures routinely collected and analyzed. Some represent to us points of pride, while others represent challenges for improve- ment. Because you have entrusted your health- care to us, it is our tendency to NEVER feel that the work is “good enough.” Equally important to us is our deep commitment to holding ourselves totally accountable for your safety and the quality of our work. As you might expect, we also are committed to sharing our results with you, honestly and often. Consistent with this philosophy, all of us at Charlotte Hungerford Hospital want every person who provides, regulates or utilizes our services to feel strongly that our care is safe, effective, patient-centered, timely, efficient and equitable. To achieve care that can be character- ized as such requires all hospital and medical staff members to work together and requires hospital leadership to constantly reinforce and support this collaboration. Consequently, more and better information about the safety and quality of care across each of these six character- istics will continue to be developed and shared. Fortunately, over the last few decades important work has been done to develop measures and increase the capacity to collect and analyze quality and safety data. Still, it can be a daunting task to sift through so many clinical performance measures, some of which may not mean much to the layman. Nonetheless, it is widely agreed that the Joint Commission is one of the hospital indus- try’s key patient care auditors. And, of the many measures they evaluate, the Patient Safety Accountability Composite Rate is the key statistic that gives readers of their reports a useful measure of the hospital’s overall per- formance over time. Below is a description of some of their best work: Maintaining quality hospitalist care CHH expanded its Hospital Medicine Department, increasing the number of regular, full-time hospital-based doctors, APRNs and physician assistants who treat patients admitted to the hospital. Hospitalists, who specialize in acute hospital care, manage your care and treat- ment with your primary care doctor. Delivering rapid emergency care An innovative, new model of care was introduced in the CHH Emergency Department, bringing physicians and physician assistants into the triage function immediately, resulting in more rapid assistance and a huge decrease in time from patient arrival to seeing a physician. In the first three months of its implementation, we have seen an 11 percent decrease on average in our door-to-doctor time as compared to our FY 2012 average. As our staff becomes more comfortable with this new treatment model, our door-to-doctor time will continue to decrease. Sustaining Rapid Response Teams A Rapid Response Team is a group of med- ical professionals from different specialties and areas of the hospital that come together when called to bring their critical care expertise to a patient’s bedside during an emergency with a goal of avoiding a “crisis” situation. Our rapid response team has responded to 145 calls in the last year. This demonstrates our staff recogniz- ing and responding to signs of instability soon- er, allowing quick intervention to treat patients, and to stabilize and transfer some patients to a higher level of care if needed. Introducing heart saving technology Our Emergency Department has enhanced its cardiac care program by implementing a state-of-the-art hypothermia system. A new “ThermoSuit ® ,” which places the patient in an induced hypothermic state after experiencing a heart attack, uses a water-circulating system that lowers a patient’s inner core temperature. Thanks to this revolutionary system, we can cool the body and preserve brain function while the heart is receiving lifesaving treat- ments. After cooling the patient and providing the immediate emergency treatments, we transfer the patient to one of our tertiary center partners for further care. But our improvements continue…We’re also investing in new projects and partner- ships that will greatly enhance the size and scope of the medical services we offer. improve facilities, technology and equipment.With the ever-evolving healthcare environmentand the shift from inpatient to outpatient care,our Emergency Department needs to growphysically, technologically and in the level ofpersonalized care and comfort it provides topatients. To achieve this, plans are under wayfor a proposed multi-year, renovation and mod-ernization of the hospital’s EmergencyDepartment. Plans include an enhancedentrance for walk-in patients and ambulances, afriendly, welcoming waiting area and 27 privatetreatment rooms, including a GYN room, anorthopedic room, two triage rooms, two resus-citation rooms, several rooms dedicated tobehavior health treatment, and four “flex”rooms to help handle high volume peaks.

● Northwest Connecticut cancer care servicesCharlotte Hungerford Hospital, Yale-New

Haven Hospital, and Yale Cancer Center areexploring ways to enhance outpatient cancerservices in Litchfield County that would helpincrease access to care and ensure the efficient,effective and high-quality delivery of a wide-variety of cancer services in a single facility. Theinvolved parties anticipate this new collaborationwill greatly enhance the existing servicespatients have come to expect in the local deliveryof cancer care. We are simultaneously working tointegrate and streamline the center’s services and

Shared goals for the new partnership toenhance patient care include building upon thestrength of our current physicians and staff,enhancing research and complementary cancertherapies, supporting the expansion of the earlydetection and breast navigation services andoffering greater access to cutting-edge treat-ment options, technology and drug therapies.

● Greater Winsted emergency andmedical servicesAfter years of deliberation, the CHH

Board of Governors reached the conclusionthat the future needs of patients, staff, areaphysicians and the public will best be met bybuilding a new, easily accessible, and patient-friendly facility on Route 44 on the town linebetween Winsted and Barkhamsted. Thissoon-to-be-constructed 19,000-square-footmedical building will permit CHH to offeradditional, technologically advanced servicesto better meet the needs of those it serves, justone mile from its current location on SpencerStreet in Winsted. Plans call for the new spaceto be built within the next two years andaccommodate the relocation of existingservices including the emergency department,cardiac and pulmonary rehabilitationprograms, blood drawing, laboratory, X-ray,digital mammography services, a sleep center,

physician offices and more. A CT scanner isalso being proposed for the location.

Charlotte Hungerford Hospital Auxiliary EmployeeService

2012 was another busy year for the CharlotteHungerford Hospital Auxiliary whose 23 boardmembers and hundreds of member supporters gavetheir support and time by raising money through theoperation of the CHH Gift Shop, membership dues,special vendor sales, special events, the Artist of theMonth Program and a holiday season’s greetingspromotion.

The auxiliary delivered its third $10,000installment of its $50,000 pledge to the hospital’sinformation technology initiative and participated inand pledged $1,000 to the CHH Front CircleRevitalization Project. The auxiliary also approved arequest for $15,000 toward the purchase of newchairs for the hospital cafeteria and $10,000 for aninfant warmer with a resuscitator system forMaternity Services. In addition, the auxiliaryawarded 12 $1,000 scholarships to students enteringthe medical field including the Dr. Alfred FebroScholarship and one in memory of Joseph Curi, M.D.

The auxiliary held several successful fundraisersincluding a murder mystery dinner, which wasattended by 160 people. Thanks to the CHH physi-cians and nurses who joined in the fun. The annualfashion show was attended by 185 women with fash-ions provided by Dress Barn. Thanks as well to thenumerous local businesses and organizations whodonated items to the very profitable raffle.

The CHH Gift Shop continues to serve thepublic and hospital employees with a wide range ofunique and wonderful products, jewelry, toys,clothing and accessories. Please stop by duringnormal hospital visiting hours and support thiswonderful place for fine gifts and clothing.
