Daily Fit Log Assessment Guide I. Introduction The Daily Fit Log Assessment Guide will provide step-by-step instructions that will demonstrate the best approach towards using DFL to help create meaningful and personal fitness experiences for your students. Each grade level will engage in a similar learning process in order to streamline your students’ fitness experience across all grade levels – however, as students progress through their academic career and continue using DFL, the learning process will become more comprehensive and students will delve deeper into the site. By doing so, your students are challenged to use their personal health and fitness data to make informed decisions when choosing a course of

action to improve both their physical activity habits and personal fitness levels.

II. Rationale As the United States stands toe- to- toe with an obesity epidemic, it has become clear that our country is at a crossroad. Though we are now facing one of our biggest public health crises experienced in the last 50 years, lethargic American lifestyles are still supported in an environment that promotes sedentary behavior for our youth. Unsatisfied Americans don’t strive for a permanent change – many would rather pursue quick and effortless ways to manage weight and achieve what they think to be, a healthier lifestyle. This idea is senselessly supported by the billions of dollars spent per year on diet related products. Unfortunately, research has shown that even proper diets are not sufficient enough in reducing and maintaining a healthy weight. In a 2005 addition of the Journal of Applied Physiology, Hill and Wyatt suggest that, “It is likely that physical activity is so low in most people that total energy expenditure has been reduced below the point where it is feasible to match it with a low energy intake.” With that in mind, DFL has set out to re-energize our youth by positively altering both their physical activity habits and fitness levels through the use of standard-based assessment with our cutting-edge personal fitness planning technology.

III. Goals Below you will find the goals of the Daily Fit Log Assessment Guide:

IV. The Daily Fit Log Learning Process Our assessment guide reflects a cyclical learning process that is both authentic and personalized for your students. The sequence of activities at each grade level will challenge students to continually analyze their personal health and fitness data, and also implement a plan for improvement that reflects each student’s current level of fitness. Below is a visual display of the process that your students will engage in as they use DFL and its benefits:

This process reflects the learning of a number of essential skills for maintaining optimal fitness levels:

Personal Fitness Skills • Self-Assessment • Goal Setting • Planning an Exercise Routine • Tracking Progress • Assessing the Plan’s

Effectiveness

V. The Student Experience As you integrate the Daily Fit Log learning process into your instruction (as laid out in this assessment guide), your students will be responsible for a number of site tasks using DFL on a daily, weekly, bi-weekly, or monthly basis. These site tasks will be vital for the success of DFL at your school, as well as for student learning. These site tasks include:

Logging physical activity on a daily basis Logging Health Data Setting Goals Planning Physical Activity Charting Physical Activity Checking Bulletin Board Evaluating Physical Activity Levels Filling out Reflection Sheets

VI. Role of the Teacher Your role in this process will be as a facilitator, since DFL by nature is a student-driven program that promotes students taking ownership of their physical activity habits. As the teacher, it will be important that you facilitate an experience for your students that is both organized and properly sequenced – each instructional situation is different due to varying contextual factors at your school. With this in mind, developing a routine for your students and properly articulating expectations regarding timelines, necessary site tasks (ex. logging daily activity, setting goals, planning activities, completing and handing in reflection sheets), etc. will be pivotal. In order for you to communicate with your students, we provide a bulletin board feature that enables your students to easily receive information from you outside of school. This will help you communicate important information to your students about DFL (and any other Health and PE related materials) within the DFL environment, rather than in the gymnasium or health classroom amongst a myriad of other topics. Our guide will include a pre-populated calendar which lays out exactly what tasks students and teachers will need to complete. We recommend posting this calendar to each Class Bulletin Board in order to express the critical timelines and site tasks to your students. We understand that academic calendars can vary from school to school - a blank calendar is available for customization.

VII. Grading The recommended best practice for grading your students’ use of DFL will be similar throughout all grade levels. The basis for which you grade students should not be about quantity, but rather, quality. While we encourage students to participate in as much physical activity as possible, we do realize that the appropriate level of physical activity for each student can drastically vary in frequency, intensity, type and time. For this reason, we do not promote a grading methodology based on reaching a certain level of activity – but rather, simply participating in physical activity that is deemed appropriate for each student. The tools available to students and teachers alike allow for the planning of appropriate amounts of activity for the individual student while visibly displaying that information to you. DFL's advanced design will simplify the assessment process, allowing you to easily scan your class roster and measure each student's planned vs. actual activity in seconds. A following critical component in the assessment of your students will be the completion of their Reflection Sheets. These sheets will provide you with an understanding of how well your students are internalizing their experience and should represent a portion of their grade for using DFL.

5th Grade Introduction

5th Grade Focus The focus of the 5th grade experience will be:

Achieving 60 minutes per day of physical activity

Participating in activities from all fitness categories

Goal Setting to improve and/or maintain suitable physical activity levels

5th Grade Daily Fit Log Learning Process

Focusing on the points above will allow your students to explore and learn ways to meet the activity requirements set forth by the Centers for Disease Control and Prevention (CDC). More importantly, your students will learn the basics of goal setting and self-monitoring in order to reach those requirements. Students will focus on achieving these objectives through the completion of a number of tasks on the site.

**The instructions in the guide will recommend posting each of the following documents to your Class Bulletin Boards. In order to make this process easier for you, we have posted each individual document to the DFL Site Bulletin Board so you can download and save as needed.**

1. Important Site Tasks for Students 2. DFL Student Contract 3. DFL Grading Rubric 4. Student Task Calendars 5. DFL Reflection Sheet Month 1 6. DFL Monthly Reflection Sheet 7. DFL Midway Reflection Sheet 8. DFL Final Reflection Sheet

5th Grade Resource Sheets The sheets listed below are intended to help introduce the program to you and your students as well as to efficiently manage your students’ usage throughout the year.

1. Important Site Tasks for Students This sheet is meant to be distributed to your students and reviewed in class prior to the start of the program. It details all of the important components of the 5th grade DFL experience for a student and provides them with an understanding of what will be expected each month. During this review, also allow time to go over the DFL Student Contract and DFL Grading Rubric. Once the entire review is complete, these sheets should be distributed via the Bulletin Board.

2. Important Site Tasks: Role of the Teacher

This sheet serves as your guide in order to help facilitate a meaningful experience for your students. It chronicles the necessary steps that you, the teacher, should take in order to help your students stay organized and up-to-date with their DFL assignments.

3. DFL Student Contract The contract should also be handed out to the students at the start of the program. By signing this contract and handing it back into you, students are stating that they understand the expectations for using DFL and will adhere to them at all times.

4. DFL Grading Rubric

This rubric will be used to grade your students each month based off their completion of their assigned DFL tasks. The rubric should be handed out to your students at the start of the program in order for them to have an understanding of how they will be graded for their use of the program.

5. Student and Teacher Calendars Student and Teacher Calendars easily articulate the pacing of the program to you and your students in order to keep them on the same page. By using the calendars you and your students will have a day to day overview of the program with important due dates highlighted.

Important Site Tasks for Students:

1. Daily Fit Log Activity Contract & Rubric Student Responsibility:

Completing the DFL Student Contract and returning it to your teacher.

Reviewing the Grading Rubric.

2. Daily Physical Activity Logging Student Responsibility:

Logging of physical activity should take place daily. o Back logging is available. If a day is missed of imputing physical activity, DFL will allow you to go

back and log your physical activity data. o *We do not, however, recommend getting into a habit of back logging. It may leave a less

accurate depiction of your actual physical activity habits.*

3. Long-Term Physical Activity Goal: Student Responsibility:

Creating a long-term physical activity goal on the DFL Goals Page.

This goal should represent the level of physical activity that you would like to achieve or maintain through the end of the school year.

Your goal can be generated by evaluating your first month of baseline physical activity logging via the Charting Page and Student Activity Overview.

4. Monthly Reflection Sheets & Short-Term Goal Setting Student Responsibility:

Completing your Monthly Reflection Sheets and short-term goal by the assigned due date of your teacher.

The Reflection Sheet and goal/goals setting should be completed by evaluating the prior month’s physical activity logging via the Charting Page and Student Activity Overview.

Your goal should be set on the DFL Goals Page and it should be in relation to your long-term activity goal. Each month’s short-term goal should bring you closer to achieving your long-term goal.

5. Midway and Final Reflection Sheets Student Responsibility:

Downloading the Midway and Final Reflection Sheets from the Class Bulletin Board.

Completing your Midway and Final Reflection Sheets by the assigned due date of your teacher.

The Reflection Sheet should be completed by evaluating the prior month’s physical activity logging via the Charting Page and Student Activity Overview.

Important Site Tasks: Role of the Teacher

1. Daily Fit Log Activity Contract & Rubric The Teacher’s Role:

Reviewing the DFL Student Contract and DFL Grading Rubric with students.

Posting the Contract and Rubric to the students’ Class Bulletin Board.

Providing students with a Student Activity Overview, which can be generated from your Teacher Account under the Class Reporting Page.

2. Daily Physical Activity Logging The Teacher’s Role:

Mandate that physical activity must be logged DAILY by students! o In order for the program to be successful, students must have data to review and analyze.

Therefore, mandating that activity must be logged daily will be the most critical element to the success of the program at your school.

Monthly grading of students activity logs. o Create accountability by assigning a grade for whether or not a student has logged activity each

day. This can easily be done once a month from the Class Reporting Page in the Teacher Account. (Recommended grading scale = 1 pt/day for activity logged)

DO NOT require students to meet certain levels of activity. Please note that even if a student accumulated 0 minutes of physical activity, it should

still be logged. By doing so, students will more easily create a habit of using DFL and will get them to think about their fitness habits on a daily basis.

3. Long-Term Physical Activity Goal The Teacher’s Role:

Setting a due date for the creation of a student’s long-term goal.

Instructing your students to view their Student Activity Overview, which can be found in the Student Account as a Fit Log sub-tab.

Encouraging students to strive to achieve 60 minutes worth of physical activity each day, which is recommended by the Centers for Disease Control and Prevention.

4. Monthly Reflection Sheets & Short-Term Goal Setting The Teacher’s Role:

Posting the Monthly Reflection Sheet to your students’ Class Bulletin Board. o Require student responses for months 1-4 & 6-9. (Months 5 and 10 will be the Midway and Final

Reflection Sheets, which are explained directly below).

Instructing your students to view their Student Activity Overview, which can be found in the Student Account as a Fit Log sub-tab.

Setting a due date each month for the Monthly Reflection Sheets and goal setting that is clearly articulated to the students.

Providing students with one week's notice to the due date of their Reflection Sheet and goal setting.

Provide daily reminders as closure to class that students’ Reflection Sheets are due and their goals need to be set.

5. Midway and Final Reflection Sheets The Teacher’s Role:

Posting the Midway and Final Reflection Sheets to the students’ Class Bulletin Board. o *These reflection sheets will be more in depth than the monthly reflection sheet.*

Instructing your students to view their Student Activity Overview, which can be found in the Student Account as a Fit Log sub-tab.

Setting a due date for the Midway and Final Reflection Sheets that is clearly articulated to the students.

Providing students with one week's notice to the due date of their Reflection Sheet.

Providing daily reminders as closure to class that students’ Reflection Sheets are due.

DFL Student Contract

By signing this contract, I, ______________________, agree to the following terms:

I have reviewed both the Site Tasks for Students Sheet as well as the DFL Grading Rubric. I understand what my responsibilities are as a student using the Daily Fit Log program and I am aware that my grades will be a reflection of my use of the DFL program.

I understand that I may be responsible for using the tools within Daily Fit Log to plan out my physical activity weekly.

I understand that as I form and continue to build a healthy routine, I am responsible to create both short and long-term goals. My monthly short-term goals should help bring me closer to my final long-term goal.

I understand that I am expected to log my activity daily. I will be honest with my logs so my grades will be a valid result of my work.

I understand that if it is suspected that my logging is altered or untrue, my grades will be negatively affected.

I understand that I must complete Monthly Reflection Sheets, as well as one Midway and one Final Reflection Sheet. They will contain a review of my past performance.

I understand that Daily Fit Log can help me form a healthy lifestyle through daily use of the program. As long as I understand and follow these terms, I can expect positive results.

Overview and Instructions for Grading Overview This grading rubric reflects grading on a monthly basis in order to keep consistency with the month-to-month pattern of the DFL Program. However, this does not mean that the grading structure cannot be altered to fit the needs of your school. If you feel that it would be better to grade your students quarterly in order to coincide with your marking periods, that is certainly a possibility. You would simply need to alter the rubric to reflect the following:

Daily Logging: How many days were in the marking period? (1 pt/day)

Reflection Sheets: How many DFL Reflection Sheets were completed? (10 pts/sheet & 20 pts for Midway and Final)

Instructions 1. Daily Logging: To grade your students’ daily logging, please choose the following options in your Teacher Account:

a. Click the Class Reporting tab. b. The page will default to the Student Activity Overview sub-tab. c. Click the “Custom Date Range” button and enter the date range for the month you are grading. d. The “Number of Days Active” field will display the student’s grade. An example of what you might see would be: “20 of 30” –

meaning that the student has created at least one Fit Log entry for 20 out of the 30 possible days in the date range.

2. Reflection Sheets: Your students’ Monthly Reflection Sheets will be worth 10 points. The Midway and Final Reflection Sheets will be worth 20 points. Different schools will be using DFL to address unique issues within their respective school communities; therefore, we have left the option up to you for determining the point values of each question.

Student Reflection Sheets

As previously stated, our assessment guide reflects a cyclical learning process that is both authentic and personalized for your students. One critical component of this process is the completion of Monthly Reflection Sheets. DFL provides four different reflection sheets for your students to complete on a monthly basis.

1. The "Month 1 Reflection Sheet" will be distributed to students upon completion of the first month of

the DFL program. This sheet will be used by students to help assess their baseline health and fitness

levels.

2. The "Midway Reflection Sheet” will be distributed to students upon completion of the 5th month of

the DFL program. This sheet will help students assess their progress at the midway point of the school

year and determine whether or not any changes to their long-term goals are necessary.

3. The "Final Reflection Sheet" will be used to culminate the DFL experience and ask students to assess

their physical activity choices and fitness strategies that worked for them throughout the year.

4. The "Monthly Reflection Sheet" will be the most frequently used sheet as it is used upon completion of

the remaining months. This sheet will help students assess their monthly health and fitness progress

and address any changes that need to be made to their physical activity choices.

Daily Fit Log: 5th Grade Reflection Sheet Month 1

1. Were you able to consistently achieve 60 minutes per day of physical activity? Why or why NOT?

2. What could you start to do differently to ensure that you can steadily achieve 60 minutes per day of physical activity? If you have already

been successfully performing 60 minutes worth of activity daily, what habits do you believe you should continue? Why?

3. Which areas of fitness were you most active in? Which areas could you improve your activity time in? What exercises or activities could

you add into your daily routine that would allow you to improve in your weaker areas?

4. Based on your above answers, please create a long-term goal that you would like to accomplish by the end of the school year. Also,

please create one short-term goal that you would like to complete by the end of next month. Remember, your short-term goal should be

leading you towards achieving your long-term goal. Once this sheet is completed, please use the “DFL Goals Page” to officially set your
