Please read all instructions first!It is your (the student’s) responsibility to: pick up the local packet, fill out the application, make copies, request a transcript, write essays and provide thank you cards.

YOU WILL BE RESPONSIBLE FOR MAKING ALL YOUR OWN COPIES.(LPHS COPIERS WILL NOT BE AVAILABLE)

Step One- Read through all the Local Grant descriptions in the packet. Highlight or circle any and all grants which you meet the requirements for.

Step Two- Each grant application MUST contain the following in this order and be stapled.1. Cover Page (see sample cover page in this packet)2. Application Form3. Essay4. Transcript

Step Three- Turn in to Ms. Vazquez by 3:00 p.m. on Tuesday, March 19, 2019.!!!NO EXCEPTIONS!!!

You will find an example of the cover page layout in this packet. If it says, “Local Grant Application Form”, it requires the local form (which is included in this

packet). You will fill out the application and leave the top 2 lines blank, make your copies of the application, then fill in the top two lines for each grant that uses this form.

If it says “Their Application Form” please see Mrs. Vazquez in the Counselor’s office and pick up from her. It will help if you have the grant number when picking it up. If you have more than one application to pick-up, please make a list of all numbers needed.

Typed Essay-see essay requirements for each scholarship listed in packets. Not following the essay requirements for each listed scholarship can result in scholarship disqualification.

AFTER THE APPLICATIONS ARE TURNED IN…If you have received any other scholarships, let us know by May 3RD so you may be recognized

for your achievements.

Local Recipients will be announced at the Scholarship Program on Thursday, May 16th at 6:30 p.m. in the Sonja Angelo Theatre.

Each Recipient will need to fill out a Thank You form for each grant received. These forms will be attached to the recognition certificate that you will receive the night of the program.

You are also required to turn in a “Thank you” card to the sponsor. Turn in your form and thank you cards together. We will mail the thank you cards to the sponsors for you.

All money will be sent to the college that is listed on the Thank You Form. Please be sure that it is where you will be attending in the fall. If the money has to be sent back to us and we have to

send it back out, your money may not be at the right place when you need it in the fall.

Ms. Vazquez must have all forms and thank you notes by May 20th.
