Twelve cases of administration of drug for which there was a known allergy history, have been reported. Among these, eight cases of administration of contraindicated drug occurred due to drug allergy information not described in the predetermined site on the medical chart, but described in different site on the medical chart or other documents, have been reported (information collection period, from January 1, 2006 to January 31, 2009; the information is partly included in "Medical Adverse Event Information to Be Shared" in the 12th Quarterly Report). Administration of allergic drug to patientwith previous known allergy history

When the patient was hospitalized last time, physician A described an allergic reaction to Cravit on the progress record of the electronic chart, but did not input the allergy information in the predetermined site on it. Consequently, physician B in charge of the patient prescribed Cravit to the patient.
