• Benefits are effective the 1st of the month following your date of hire.
• Full-time, regular employees are eligible for benefits. Temporary and part-time employees (working 29 hours or less) are not eligible to participate in the benefit program.

QUALIFIED STATUS CHANGE
CompuGroup Medical US
The elections you select when you enroll in the Benefits Plan will be in effect until December 31, 2015.
The only way to make a change is if you have a qualified event status change such as a:
*You must notify Human Resources within 30 days of this change. Supporting documentation must be submitted upon receipt, however, do not wait for it.
Changes to benefit coverage will begin on the first of the month following the change in status.

HOW TO ENROLL
CompuGroup Medical US
• Enrollment for all CGM benefits, except the 401(k), can be done online via the online ADP portal available to you 24/7.
• You have 15 days from your start date to enroll.
• If you choose to waive coverage, please still complete the online enrollment process via ADP this way you receive the company paid benefits such as STD and LTD.

participants who elect an H.S.A
