The 1st Singapore Quintessence Grand Prix in 2011 attracted several hundred participants from 15 countries worldwide together with 12 exhibiting companies representing 51 international brands. With 18 international speakers sharing their knowledge and expertise over 19 lectures and one hands-on workshop, this event provided participants close and enthusiastic discussions and high level networking opportunities, and has now made its mark as The Quintessential Platform in the Asia Pacific region for the sharing of knowledge, skills and information and for active networking.

As technology and material science continue to advance in the field of Dentistry, our approach to patient care continues to be holistic with a greater multi-disciplinary approach. 09.00 - 10.15 Dr. Tidu MankooTopic: Restoration of ‘Failing’ Teeth with Implants in the Aesthetic Zone – The Keys to Success

The aesthetic rehabilitation of multiple failing teeth and the periodontally compromised dentition remains a challenge despite the numerous advances in the treatment of periodontitis, regenerative and dental implant therapies. This presentation will review the contemporary surgical and prosthetic concepts in management of implants in the aesthetic zone with a view to achieving optimum long term aesthetics and stability. With this in mind, novel approaches for aesthetic rehabilitation of periodontally compromised teeth in the aesthetic zone will be presented.

The outcomes depend on the clinical management and an understanding and application of the biological factors that influence our treatment outcomes. The optimal management of the interplay between bone, soft tissues and prosthetics determines the aesthetic outcome and the long-term stability of soft tissue aesthetics. An interdisciplinary approach is the key to success.

10.15 - 10.45 Prof. Richard LeeSungBokTopic: Roles of Restorative Dentistry for the Disabled and Elderly person

Considering that we are now entering into an aged society, dentures for the disabled and elderly play a crucial part in life satis-faction as well as heath related quality. Nutritional care is essential when they suffer from chronic and severe disorders. Dental care and treatment, denture treatment are the most important issues for elderly people. Followings are general characteristics of elderly patients.

Prevalence of completely edentulous or partially edentulous patient population1. Fully edentulous patients are steadily decreasing due to the advancement of dental health and care. According to the national oral health survey which was released in 2000, old patients aged between 65 and 74 years old presented fully edentulous maxil-la(26.8%) and mandible(19.63%). In 2006, the number of fully edentulous patients has been decreased to 21.6% in maxilla and 14.3% in mandible. More than 5% decrease was observed in the number of the patients who were completely edentulous for just six years. 2. As dental implants are becoming increasingly popular, implant overdentures in fully edentulous cases showed higher patient satisfaction and functional improvement than conventional complete denture. 3. The number of fully edentulous patients is steadily declining worldwide and the number of partially edentulous patients has been rising and the period of partial edentulism has been prolonged. 4. With the increase of partially edentulous patients and extended period of partial edentulism, the treatment paradigm for partial edentulism is changing.

The change of treatment paradigm for partial edentulism1. Novel classification for partial edentulism Since dental implant has been applied as an artificial root for a replacement of tooth in edentulous site, treatment planning for pros-theses has been significantly changed. When there was no alternative but to employ removable denture on the distal extension of partial edentulism in the past, dental implants have been a revolutionary tooth-replacement option for those patients. If dental implants are placed on the partial edentulous site, they will alter the conventional treatment method(A,B) for removable partial denture and eventually evolve into a new treatment paradigm(C,D) that natural teeth and implants co-exist in the same oral cavity.A. What is a universal design?The term “Universal design” refers to the design that can be used for all. Barrier-free designed device, facility, and equipment can be readily used by non-disabled people as well as by disabled people. Universal design in device, facility, and equipment can be accessed by everyone without difficulty regardless of disability.

7 principles of universal design:1. Equitable use2. Flexibility in use3. Simple and intuitive4. Perceptible information5. Tolerance for error6. Low physical effort7. Size and space for approach and use.

In a case study which was conducted with 184 patients who used denture over 5 years in Kyung Hee University Hospital at Gang-dong(Department of Biomaterials and Prosthodontics, Richard Leesungbok, Lee Suk Won, Ahn Su Jin), universal design(one or two additional implants with magnet attachment which are placed in the edentulous mandible) showed higher patient satisfaction compared to that of conventional denture.

The placement of dental implant is no longer seen as a major procedure in the daily dental practice. The fact that some dentists are doing more implants than they are doing wisdom teeth surgery is a testament of the growing popularity of this treatment modality.However, placement of dental implants can be arduous and full of difficulties. This may have deterred some dental surgeons from embracing this fulfilling science and art.

This paper will outline the ways and means on how to simplify the clinical processes of dental implantation as well as to describe some simple anecdotes to solve common problems in surgical as well as prosthetic path in a successful ocutcome in the therapy of rehabilitation of patient with dental implants.

11.45 - 13.00 Dr. Jaime Jimenez GarciaTopic: What is the right time for Implant placement? Indications for Delayed, Early & Immediate implant placement in the Esthetic Area

The Survival rate in implant dentistry has been well documented in several studies. Today, if we analyze the literature carefully, there are still many questions concerning long term success rate (esthetic results) , without clear conclusions. For that reason, many times clinicians can be confused about when is the right moment to place an implant and achieve our goal, which should be to obtain a good long term esthetic result. The waiting time period in implant dentistry is nowadays a critical aspect. We have passed from waiting times of months, to new techniques reducing the period of healing, where in the latter, the same day of the tooth extraction, an implant might be placed (immediate implant dentistry) achieving a good final esthetic result. During the pres-entation, different protocols will be discussed to obtain clear clinical guidelines, of what to analyze before making the decision of when and how to place an implant, all based in the scientific evidence.

14.00 - 15.15 Dr. Federico FerrarisTopic: Esthetic restorations with different kind of metal-free materials

Progress in materials and a minimally invasive approach represent the guidelines of the present tendencies for esthetical resto-rations.

The choice of materials on market nowadays is huge and generally these materials can offer a good prognosis.

Knowing the different characteristics and the working protocols to apply to each of them, can help orientate the clinician to make the best choice according to each clinical case.

Compared to the past, composites allow better esthetics thanks to their new composition, for example with innovative character-istics. It should be underlined that the possibility of obtaining excellent esthetics is often with total conservation and respect of the healthy tissue.

Silica-based ceramics always consent excellent esthetics while offering good translucency; however they require a very strong adhesion to the dental tissue which can be achieved following a strict protocol as minor mechanical-physical characteristics are present and often they are used with non-retentive preparations.

Polycrystalline ceramics respond better to mechanical stimulation and are protagonist of the major evolutions of the last years. Through CAD CAM technology these materials are representing a simplification of some protocols, a standardised result of preci-sion compared to conventional procedures, guaranteeing however good esthetics.

The clinician can therefore find himself having to manage different restorative approaches with metal free materials according to the clinical indications, chewing and esthetical needs.

Topic: Challenges along 1/3 Cervical

1/3 Cervical, title of the presentation, is refering to the most unique area of a living natural “tooth anatomy” , it is the area of ex-tended anatomical cervical line including all the root and surrounding supporting tissue, which function as a connector of “embed-ded”anatomical root part to the “exposed” anatomical coronal part of a tooth in the oral cavity.

Back in 1778, we may read scientific books with a chapter of discussion on this part of tooth. GV Black, father of modern dentistry, wrote on his book about “wasting” a term that he used for unique cervical lesion which we know as NCCL today.

A journey through clinical finding on this area of the natural tooth and corresponding situation on dentistry-optimized tooth up to natural replacement will be presented to raise our awareness on the continuous challenges which become much more complicated along with advances on dentistry technique, material and knowledge.

16.15 - 17.30 Dr. Domenico MassironiTopic: Precision in Dental Esthetic

During the lecture the focus will be put on the main prosthetic aspects of the work at greater magnification concerning precision and aesthetics. In the last years the field of working of stereomicroscope is enlarged to the different fields of Dentistry: from endodontics to periodontology, from prosthesis to dental technique. We use the stereomicroscope in our surgery and in laboratory since 1988. Mega magnification is very important and fundamental in the different steps of fixed prosthesis. In fact, the possibility of analysis at the greater magnification allows the control of the correctness of all the steps of the different prosthetic phases.

The phase of preparation and marginal placing must come in the correct relation with marginal soft tissues and the use of micro-scope allows to obtain a marginal finish line more regular and with a bigger detail; furthermore, the use of the stereomicroscope in finishing phases allows us to use sonic oscillating instruments.

Precision, function and aesthetics are the main aims of prosthetic dentistry.

Precision in metal ceramic prosthetic fixed restoration provides the best marginal seal. The phase of preparation and marginal repositioning must come in the correct relation with marginal soft tissues and in finishing phases allows us to use sonic oscillating instruments. Methods used to produce good marginal integrity in metal-ceramics may also be adopted for all-ceramic restorations and laminate veneers. This presentation will discuss the concepts of precision and aesthetics pertinent to a full spectrum of resto-ration type and materials. In our opinion the most important step is the preparation of the tooth with rotating and non rotating instruments. In our daily practice the magnification vision with the operative microscope is fundamental specially to achieve the good marginal preparation using sonic technology without rotating movement.

Topic: The CAD/CAM Technique - Does it really make a difference in adult esthetic orthodontics?

Over the last 2 decades, Lingual Orthodontic treatments have dramatically risen in popularity due to the abundance of worldwide courses and conferences, the large variety of lingual brackets, the improvement in laboratory procedures, updated CAD-CAM technologies and the implementation of 30 years of experience. Yet, most clinicians still assert that the results are not predictable and not as good as seen with the buccal appliance. The reasons are multifactorial, but are mainly attributed to ignorance and the lack of knowledge of the biomechanics limitations of the orthodontic appliances and the importance of treatment planning. Various CAD-CAM techniques will be surveyed with numerous clinical cases.

10.15 - 10.45 Dr. Masana SuzukiTopic: Soft Tissue Augmentation for maximum Implant Esthetics

Soft tissue augmentation around implants in the aesthetic zone.Implant placement and restoration in the aesthetic zone is an es-pecially challenging area for dental clinicians. One of the major complaints of unsatisfied patients is change in the appearance of restorations caused by gingival recession. Root coverage and ridge augmentation have favorable prognosis in 10 year follow-up cases. Based on these results, adapting connective tissue grafts to the implant abutment’s exposed surface might be a valid alternative for soft tissue augmentation in the aesthetic zone. This lecture will discuss current aesthetic methods of soft tissue reconstruction around implants in the anterior zone according to a classification of gingival recession around the implant and ways to prevent gingival recession.

11.15 - 11.45 Dr Chou Cherng TzehTopic: Ridge Preservation - a key factor for long-term success for Aesthetic implant therapy

Many factors may affect the outcome of success of osseointegration, such as the hard tissue height and width, also the keratinized mucosa. Many surgical methods had been proposed to achieve aesthetic result but may not last long. The key factors still the combination of hard and soft tissue and the surgical timing. This presentation is trying to discuss an easy and predictable method to meet all the aesthetic criteria, which has been used over ten years, no matter how less the buccal plate left at the time of extraction. The rationale,bio-material, histological and clinical outcome will be discussed.

Considering the analysis of recent literature reviews concerning patients expectations on implant treatment, augmentation proce-dures seem to be the least favoured option of choice. Furthermore literature also shows that implant placement in pristine bone sites seem to have a better longterm success. Following these facts, the use of angulated implants offers an alternative treatment option, making the best use of the existing bone. In this presentation we would like to give an overview of research and literature on this topic and also present data our on work on this subject. Finally we will present treatment option with for the final restoration of these patients.

09.30 - 10.30 Dr. Haider Al-WaeliTopic: Promising Non-Invasive Therapy Concept for Management of Periodontal and Peri-implant Diseases with antimicrobial Photodynamic Therapy

Fifty narrow Sky dental implants in eight consecutive patients were evaluated retrospectively following the described modified split-crest osteotomy with bone regenration. Inclusion criteria were: inadequate maxillary buccolingual dimension, 3–4 mm of crestal width, and sufficient height from alveolar ridge tip to maxillary sinus floor and in the anterior zone. Primary stability was calculated using resonance frequency analysis (RFA). Alveolar bone height was measured in the panorex pre- and postoperatively. Histolog-ical bone examination was assessed following trephine bone harvesting during the second operation. Mean follow-up was 28.33 months. Bone regeneration of the inter-cortical gap occurred in 98% of implant sites (implant survival rate 100%). Mean implant stability quotient (ISQ) for the whole series of implants was 69.48. At the second operation, mean loss of the alveolar bone height was 0.542 mm. Predictable results are obtained using the modified split-crest osteotomy with Narrow implants. This technique provides an acceptable inter-cortical gap, decreases the risk of necrosis of the outer cortex, and provides a firm-wall box for the placement of particulate Biphasic biomaterial Ossceram NAno bone grafting.

10.30 - 11.30 Dr. Georg BayerTopic: SKY fast & fixed Therapy Concept - long-term stable and successful oral rehabilitation in minimal invasive implant treat-ment with reduced number of implants

Parodontal compromised patients refuse dental treatment, due to the fear of removable and uncomfortable prosthetic reconstruc-tion. Also the proposed grafting techniques resulting in unkown post-operative discomfort. The placement of implants for fixed res-torations without grafting procedures, requires a detailed preoperative planning and the possibility to reduce the risk of complication especially for immediate implant placement. Immediate loading with tilted implants requires components, which are optimized for angulated implant treatment. In the last years clinical experience has shown that the adjunctive treatment with the antimicrobial photodynamic treatment reduces significantly the risk of wound healing disturbancies. Immediate loading in the combination with three-dimensional planning and prefabricated components is possible directly after surgery and is the prerequisite for a long-term stable restoration. Comprehensive augmentation procedures are not necessary, which results in a very high patient acceptance, due to the reduced surgical efforts. The SKY fast & fixed-System in combination with the Helbo-Disinfections shows a reliable procedure for the edentulous or nearly edentulous patient in the mandible and maxilla with a fixed implant borne superstructure.

BioHPP stands for High Performance PolymerBioHPP is sealed against humidity and comes in a transparent tube. The required quantity can be dosed by the gram, looked up from the wax to BioHPP conversion chart. Only that amount of material is used.

Unlike framework materials used so far, BioHPP features elasticity adapted to the bone. Ceramic and NPM are about 20 times, gold and titanium about 10 times more rigid than bone material. This similarity with bone offers particular advantages especially for long span substructures.

Using a tension-compression testing machine, the fully anatomical bridges and the bridges veneered with visio.lign were loaded until fracture occurred. The metal ball transmits the masticatory force!After masticatory simulation, all bridges revealed slight traces of abrasion at the contacts toward the human antagonist but no damage or decementing.

14.45 - 15.45 DT. Jeong-ho JeonTopic: No chipping and long-term aesthetic solution with innovative hybrid materials and technology

The 73-years-old female patient came to the department of prosthodontics, Dental hospital of Yonsei University. Chief complaint was to make new dentures. The things that should be considered when establishing treatment planning were dentulous state of upper jaw and edentulous state of lower jaw, moderate alveolar bone resorption and difficulty in implant surgery on posterior region, loss of soft tissue. Treatment planning that could distribute and support the occlusal force was needed. The possible options were to make implant-supported overdenture using four fixtures as removable type or to treat using implant-fixed prosthesis. There are two types of fixed prosthesis which are the ceramo-metal and the hybrid fixed prosthesis. In case of ceramo-metal prosthesis, there were difficulties in implant surgery and non-aesthetic result was expected because of lack of alveolar bone on posterior region, loss of soft tissue and inappropriate interocclusal space. Furthermore, multiple pores can be formed as a result of thickening of metal superstructure which leads to fracture of prosthesis. Consequently, the hybrid restoration using alveolar bone anterior to mental foramen was more appropriate. The patient wanted the fixed type with implants. Thus, we decided the hybrid restoration as a final treatment plan. This enhances masticatory ability and psychological stability to the patients than removable type. It also has the advantages such as proper soft tissue profile, esthetic outcome and is proved to have clinically long-term stability.

Dr. Otto Zuhr studied dentistry at the University of Aachen from 1986 until 1992. Dr Firdaus Hanapiah graduated from University of Otago, New Zealand in 1990. He then returned to Malaysia to take up position as a clinical tutor at the Dept of Oral and Maxillofacial Surgery, Faculty of Dentistry, University of Malaya. In 1993 he then joined the Eastman Dental Hospital, University College London where he was a part time honorary senior house officer.In 1996 he completed an MSc in Oral Surgery from the Eastman and also was awarded the Fellowship in Dental Surgery of the Royal College of Surgeon, England. He then once again returned to Malaysia to take a position as a lecturer at the Dept of Oral and Maxillofacial Surgery, Faculty of Dentistry, University of Malaya.

He is now in private practice based in the Selangor Medical Centre, in Shah Alam, Selangor , Malaysia, as the consultant Oral & Maxillofacial surgeon as well as head clinician for the Zahida and Sharina Dental Clinics in the Klang valley.He has since placed more than 1000 dental implants. Registration:-On-site registrations will be accepted only if room capacity allows.-For on-site registration, only credit card and cash payment in Singapore Dollars (SGD) will be accepted.-Online, mail and fax registration are only available from 22nd Feb 2013 onwards and close on 5th Sept 2013 (Thurs-day) at 1800 hrs (GMT +8).
