Medical
You have access to VillageMD Primary Care and Affiliate Providers as well as Cigna’s national network of contracted providers, hospitals and specialists. You can take advantage of a variety of resources tailored to help you navigate through your specific health care need.
Dental
Cigna administers our dental PPO plan, giving you access to a large network of qualified dentists and dental specialists.
Virtual Health Visits
You can access non-emergency medical or behavioral health care virtually through your in-network provider, Village+* or MDLIVE. Virtual visits can happen anywhere via video or phone 24/7/365. Schedule a behavioral/mental health virtual care appointment online in minutes.
* Village+ available in select markets.
Behavioral Health
Whether you’re struggling with stress or anxiety or just need to talk to someone, Cigna’s behavioral health services are here to help. The Fast Access network guarantees first-time appointments in five business days. For more intensive care for mental health or substance abuse issues, Cigna has a nationwide network of Centers of Excellence.
Employee Assistance Program (EAP)
You and your family members can take advantage of 6 face-to-face counseling sessions per issue per year, at no cost. Video-based sessions are available to fit your busy schedule. The EAP also offers legal and financial consultations, caregiving resources and referrals, even help to resolve identity theft.

Higher Life Insurance Options
This change brings more options and higher maximums for employee-paid supplemental life and accidental death and dismemberment (AD&D) insurance:
• Employee: $10k increments up to $750,000; $300,000 guaranteed issuance
• Spouse: $5k increments up to $100,000; $50,000 guaranteed issuance. (Note that the amount of spouse coverage cannot exceed the employee’s coverage.)
• Child (up to age 26): $10k
Your One-Time Annual Enrollment Opportunity
During 2022 Annual Enrollment only, EOI will be waived for supplemental coverage up to the guaranteed issuance amounts.*
If you don’t take advantage of this one-time opportunity and want to enroll during a future Annual enrollment, EOI will be required to elect more than $10k of new or increased employee coverage, or more than $5k of new or increased spouse coverage, up to guaranteed issuance amounts.
* EOI will be required if you had a previous application declined.
TERMS TO KNOW
Guaranteed Issuance (GI): The dollar amount of coverage for which you are guaranteed acceptance without providing EOI.
Evidence of Insurability (EOI): RSLI will require proof of good health before approving your application for coverage above the guaranteed issuance level.
Disability Insurance
Employees eligible for the employee-paid Long-Term Disability (LTD) plan will not be required to complete EOI to be approved for coverage. The pre-existing condition limitation will still apply. See your Benefits eGuide for details.
INCOME PROTECTION PLANS
We’re moving from Dearborn National to Reliance Standard Life Insurance (RSLI) for administration of our life insurance plans, disability benefits and absence management programs.

Financial and Work/Life Benefits
These new benefits can help you protect yourself, your finances and your personal information in a variety of situations.
• Emergency Travel Assistance (On Call International): If you experience a personal emergency when traveling more than 100 miles from home, you have around-the-clock access to services such as help with an illness or injury, lost passport, missing luggage or even a prescription refill.
• Identity Protection (InfoArmor): Team members and family members receive powerful monitoring and security tools. Should you experience fraud, InfoArmor’s comprehensive recovery services will help you resolve your case and restore your identity, with up to $25K in identity fraud expense reimbursement.
• Legal and Financial Service (ACI Specialty): Initial assessment and a 60-minute consultation about your legal or financial issue. Receive free financial coaching, DIY document preparation, and document assistance services at a reduced fee.

Benefit-eligible team members are automatically enrolled in these programs at no cost.
Caring in Difficult Times
If you find yourself managing through a significant loss, VillageMD has resources available to support you and your loved ones during your journey.
• Grief Support and Counseling: You and your household members can have up to 3 Reliance counseling sessions at no cost.
• Bereavement Support Services (ACI Specialty): Unlimited, confidential telephonic grief counseling, along with online funeral planning and beneficiary financial and legal consultation.
IRS ANNUAL CONTRIBUTION LIMIT UPDATES
The only plan contribution limits that are changing for 2022 are as follows:
• HSA contribution limits (if in HSA Medical Plan):
— $3,650 individual coverage — $7,300 family coverage — +$1,000 catch-up (age 50+)
• 401(k) Retirement Savings Plan contribution limits:
— $20,500 — +$6,500 catch-up (age 55+)

Who’s Eligible for VillageMD Benefits
To be eligible for benefits, a VillageMD-eligible team member must work at least 30 hours per week.
Who You Can Cover• Your legal spouse, civil union or domestic partner.
• Your dependent child(ren) under age 26, or 26 and older who are permanently disabled (birth, legally adopted or court appointed legal guardian).
When Benefits Are Effective
The benefit elections you make during Annual Enrollment will be in effect January 1 through December 31, 2022. Mid-year changes are not allowed unless you have one of the following qualified life events that affects your coverage needs:
• Marriage• Divorce or legal separation• Birth or adoption• Gain or loss of coverage• Death of a dependent• A dependent is no longer eligible for coverage
If you have a qualified life event, you must provide written documentation and notify the VillageMD Benefits Team at [email protected] within 30 days of the event. Otherwise, you must wait until the next Annual Enrollment period to make any changes.
2022 BENEFITS ENROLLMENT CHECKLIST
Before You Enroll• Review the 2022 Benefits eGuide to see your
benefit options, costs, HSA/FSA contribution limits and employer HSA contribution amounts.
• Review and update your personal information (i.e. home address) in ADP at workforcenow.adp.com.
How to Enroll
Enroll online at workforcenow.adp.com from October 25 through November 8, 2021.
• If you decide to opt-out of VillageMD’s medical, dental or vision coverage, you are required to reflect in the ADP enrollment system that you are waiving coverage.
• You must add your beneficiaries for your life insurance plan.
• Upon completing your benefit elections, it’s important you validate your elections and click “SUBMIT ENROLLMENT” to complete the process.
• You will receive an email confirmation of your benefit elections. If you find an error, immediately contact the VillageMD Benefits Team at [email protected].
NEW TEAM MEMBERS
If you were hired after October 25, 2021, you must enroll for 2021 and 2022 benefits
separately.
