I wish to become a Master Gardener™ Volunteer and would like to be accepted into the 2020 training program. I am enclosing a completed application and deposit of $200 cash/check. (Please make check payable to Oregon State Extension Service). I understand that if I am accepted into the Master Gardener (MG) Volunteer training, I agree to: 1. Attend the orientation session prior to start of class and complete required paperwork. 2. Attend all classes, class activities, projects, demonstrations, orientations and trainings for Master

Gardener Plant Clinic. 3. Complete 40 hours of on-the-job Plant Clinic service plus 20 hours of approved Master Gardener

volunteer activities to the MG Program in Lane County (April-October 2020). 4. Pay class tuition, (additional) $230 by November 13, 2019 No refund after first day of class. 5. I agree to work with a team of volunteers in class, plant clinic, booths, and events. 6. If I must drop out of the class for any reason or don’t complete the 60 hours of required volunteer

service by Nov 1, 2020, I will not receive the $200 deposit refund.
