Other health coverage may disqualify you - including Medicare, Tricare, or a traditional health plan
• You do not have access to a full purpose FSA

ALL full-time employees are required to select or decline medical, dental and vision
benefits for the upcoming year
New and existing HSA, FSA Medical and FSA Dependent Care participants must make an
election for 2020; current deductions will not carry forward

Plan elections are binding for the 2020 plan year, unless you experience a qualifying life event:
Marriage
Birth/adoption
Divorce
Death
Change in employment status
Change in dependent status
If you are enrolling a new dependent for 2020, proper documentation must be provided no
later than November 30, 2019 (copy of marriage license, birth certificates)
Life event changes allow you to make benefit election changes
The Department of Human Resources Benefits Division must be notified within 31 days of life
