3EMPLOYEE BENEFITS OVERVIEW

BENEFITS ELIGIBILITYYou are eligible to participate in Centurion's benefit plans if you are scheduled to work a minimum of 30 hours per week. Most benefits are effective on the first day of the month following your hire date. Eligibility for short- and long-term disability begins on the first day of the month following six months of continuous employment. You are eligible to participate in the 401(k) savings plan beginning the Monday after receiving your first paycheck.

The plans offer coverage for eligible dependents, including: Your legal spouse (opposite and same-sex). Your children to age 26 (age 25 for the critical illness benefit), including a stepchild, legally-adopted child, a child placed with you for adoption, a foster child, or a child for whom you are the legal guardian.

Your dependent children of any age who are physically or mentally unable to care for themselves and who became disabled before age 26.

You can sign up for benefits or change your benefit elections at the following times: Within 31 days of your initial eligibility date (as a newly-hired or newly-eligible employee; e.g., change from part-time to full-time status)

During the annual benefits open enrollment period Within 31 days of experiencing a qualifying life event

The choices you make at this time will remain the same through December 31, 2020. If you do not elect benefits during your initial eligibility period, you will not be able to elect coverage until the next plan year.

DEPENDENT ELIGIBILITY AUDITIf you enrolled dependents as a newly hired employee after February 1, 2019, you must participate in the annual dependent eligibility audit. This audit is held to ensure that only qualifying dependents are enrolled in the medical, dental, or vision plans. If you are covering a dependent, you will be required to submit documents to prove eligibility, including a marriage or birth certificate.

Ineligible dependents include, but aren’t limited to, children over the limiting age, grandchildren, nieces, nephews, parents, sisters or brothers, girlfriends or boyfriends, unless they are a legal dependent.

CHANGING YOUR BENEFITS DURING THE YEARCenturion allows you to pay your portion of the medical, dental, and vision plan costs, and fund the flexible spending or health savings accounts, on a pre-tax basis. Due to IRS regulations, once you have made your elections for the plan year, you cannot change your benefits until the next annual open enrollment period. The only exception is if you experience a qualifying life event. Election changes must be consistent with your life event.

To make changes due to a qualifying event, visit www.YourCenturion.com within 31 days of the qualifying event. You will need to submit proof of the change, such as a marriage certificate or record of birth. Change requests submitted after 31 days cannot be accepted. 5MEDICAL INSURANCE PLANS

Centurion offers three medical plan options through Anthem: the KeyCare Standard PPO Plan, the KeyCare Choice PPO Plan, and the Lumenos High-Deductible Health Plan (HDHP). All three plans offer in- and out-of-network benefits, providing you the freedom to choose any provider. However, you will pay less out-of-pocket when you choose a network provider. Locate an Anthem network provider at www.anthem.com.

MEDICAL INSURANCE PLANSThe table on page 6 summarizes the key features of the medical plans. The coinsurance amounts listed reflect the percent you pay. Please refer to the summary plan descriptions for additional information on coverage and exclusions.

PREVENTIVE CAREThe Anthem medical plans cover in-network preventive care at 100%. This includes routine screenings and checkups, as well as counseling to prevent illness, disease, or other health problems.

You won’t have to pay anything—no deductible, copay, or coinsurance—for preventive services when: You get them from a doctor or other health care provider in the Anthem network.

The main purpose of your visit is to receive preventive care.

Talk to your primary care physician to find out which screenings, tests, and vaccines are right for you, when you should get them, and how often. Please be aware that you will be responsible for the cost of any non-preventive care services you receive at your preventive care exam. Learn more about preventive care at www.anthem.com.

Preventive PrescriptionsAll three medical plans offer a preventive prescription benefit. Preventive drugs are maintenance medications that help you manage your chronic conditions such as blood pressure, cholesterol, and diabetes, and are available to you at no cost. Preventive and formulary drug lists can be found online at www.anthem.com.

Medical Plan CostsListed below are the per-pay-period deductions (26 pay periods per year) for medical insurance. The amount you pay for coverage is deducted on a pre-tax basis.

10 WELLNESS PROGRAMS

WELLNESS PREMIUM CREDITCenturion supports employees in their total health and wellbeing, and believes that employees should take responsibility for personal choices that may impact their health status. If you complete an annual wellness exam before October 31 each year, you will earn the Wellness Premium Credit. The premium credit is a reduction in your payroll deductions for medical insurance premiums. If you get a wellness exam annually, you will continue to receive this credit year after year. There is no paperwork required to receive this benefit. Anthem will automatically notify Centurion if you fulfill this requirement.

WELLNESS ACTIVITY SUBSIDYCenturion encourages participation in ongoing fitness and weight management programs designed to promote overall health and wellbeing. We are pleased to offer a Wellness Activity Subsidy of 50% up to $200 per year to benefit eligible employees (working 30+ hours per week) who participate in either a fitness-related facility or weight management program.

To qualify, you must have participated in an activity program to improve your overall health and have been in an active full-time status for at least three consecutive months. Deadline for submission is June 30 of each year. To find more information, please go to the Centurion Portal and click on HR Benefits.

DISEASE MANAGEMENTAnthem offers disease management programs to help members manage chronic conditions. If you are enrolled in an Anthem medical plan, an Anthem representative may contact you to help you manage your chronic condition. Their goal is to help you improve your health. This service is confidential and no personal information is shared with Centurion. You can decline participation in this program.

You must complete an annual physical (wellness exam) by October 31 each year with your physician to qualify. The exam is covered 100% by all Anthem medical plans if performed by an in-network doctor. The premium credit will go into effect in the new plan year.

11DENTAL INSURANCE PLAN

DENTAL INSURANCECenturion offers a dental insurance plan through Delta Dental of Virginia. The PPO Plus Premier Plan provides in- and out-of-network benefits, allowing you the freedom to choose any dentist. The amount you pay varies based on whether you see a Delta Dental PPO dentist, Delta Dental Premier dentist, or out-of-network (non-participating) dentist. A current list of participating providers can be found at www.deltadentalva.com. Please refer to the summary plan description for additional information on coverage and exclusions.

Preventive exams do not apply toward your annual maximum benefit—NEW BENEFIT in 2020! You will pay less out-of-pocket when you choose a Delta Dental PPO dentist. Delta Dental PPO and Premier dentists file claims directly with Delta Dental and accept Delta Dental’s reimbursement in full. When you choose a PPO or Premier dentist, you will only be responsible for your deductible and coinsurance, as well as any charges for non-covered services up to Delta Dental’s approved amount.

If you choose to visit an out-of-network dentist, you will incur additional out-of-pocket expenses, as you will be billed the total amount the dentist charges (called balance billing).

When you choose a Delta Dental PPO or Premier dentist, you are protected from balance billing. The coinsurance amounts listed below reflect the amount you pay.

12 VISION INSURANCE PLAN

VISION INSURANCECenturion offers a vision insurance plan through VSP. The plan offers in- and out-of-network benefits, providing you the freedom to choose any vision provider. However, if you select an out-of-network provider, you may be responsible for paying in full at the time of service and submitting a claim for reimbursement. VSP does not issue ID cards; instead, you will use a participating provider and your Social Security number to determine eligibility. To find a participating provider, go to www.vsp.com.

13HEALTH SAVINGS ACCOUNT

HEALTH SAVINGS ACCOUNTIf you enroll in the Anthem High-Deductible Health Plan (HDHP), you may be eligible to open and fund a health savings account (HSA). An HSA is a personal health care savings account that you can use to pay out-of-pocket health care expenses with pre-tax dollars. Your contributions are tax free, and the money remains in the account for you to spend on eligible expenses no matter where you work or how long it stays in the account. Please note, PNC bank will automatically deduct $2.45 per month from your account for an administrative fee. You will automatically receive an HSA debit card if you enroll in the HDHP; however, you must also enroll in the HSA to use the HSA debit card.If you enroll in an HSA, you cannot contribute pre-tax dollars to a traditional health care FSA. However, you may participate in a limited purpose health care FSA (for dental and vision expenses only).

Contributions to an HSA (including the Centurion contribution) cannot exceed the 2020 IRS contribution maximums: Employee-only coverage: $3,550. All other coverage tiers: $7,100.

Employees age 55+ by December 31 of the plan year may contribute additional funds to their HSA (up to $1,000 in 2020).

HSA EligibilityYou are eligible to open and fund an HSA if: You are enrolled in the Anthem HDHP. You are not covered by a non-HSA plan, health care FSA, or health reimbursement arrangement.

You are not eligible to be claimed as a dependent on someone else’s tax return.

You are not enrolled in Medicare or TRICARE for Life. You have not received Veterans Administration Benefits in the last three months.

Maximize Your Tax Savings Contributions to an HSA are tax free and can be made through payroll deduction on a pre-tax basis if you open an account with PNC, Centurion’s partner institution.

If you open an account through an institution other than PNC, you will need to make post-tax contributions directly to the institution and take an “above the line” deduction on your federal income tax filing for any amounts you contribute to your HSA.

The money in your HSA (including interest and investment earnings) grows tax free.

As long as you use the funds to pay for qualified medical expenses, the money spent is tax free.

Pay for Qualified Medical Expenses You can use your HSA money to pay for eligible expenses now or in the future.

Funds in your HSA can be used for your expenses and those of your spouse and eligible tax-code dependents, even if they are not covered by the Anthem HDHP.

Eligible expenses include deductibles, doctor’s office visits, dental expenses, eye exams, prescription expenses, and LASIK eye surgery.

A complete list of eligible expenses can be found in Publication 502 on www.irs.gov.

Accessing Your HSA FundsImportant: Claims incurred before your HSA is open are not eligible for reimbursement. Debit card—Draws directly from your HSA and can be used to pay for eligible expenses at your doctor’s office, pharmacy or other locations where you purchase health-related items or services.

Pay bills online—Send payments directly to your health care providers, pharmacy or other payees for eligible expenses you paid out of your pocket.

Reimburse yourself—Request a check or schedule an electronic account transfer to pay yourself back for eligible expenses you paid out of your pocket.

If You Fund an HSA Through PNC

Centurion will match your HSA contribution to a maximum of $750 for employee-only coverage and $1,500 for all other coverage levels. The match will be prorated and distributed in equal per-pay-period installments.

14 FLEXIBLE SPENDING ACCOUNTS

FLEXIBLE SPENDING ACCOUNTSCenturion offers three flexible spending account (FSA) options—the health care FSA, the limited purpose health care FSA, and the dependent care FSA—which allow you to pay for eligible health care and dependent care expenses with pre-tax dollars. The FSAs are administered by TRI-AD.

How Does an FSA Work?You decide how much to contribute to each FSA on a plan year basis up to the maximum allowable amount. Your annual election will be divided by the number of pay periods and deducted evenly on a pre-tax basis from each paycheck throughout the year.

You will receive a debit card from TRI-AD, which can be used to pay for eligible health care expenses at the point of service. If you do not use your debit card, or if you have dependent care expenses to be reimbursed, submit a claim form and a bill or itemized receipt from the provider to TRI-AD. Keep all receipts in case TRI-AD requires you to verify the eligibility of a purchase.

Health Care FSA (not allowed if you fund an HSA)The health care FSA allows you to set aside money from your paycheck on a pre-tax basis (before income taxes are withheld) to pay for eligible out-of-pocket expenses, such as deductibles, copays, and other health-related expenses, that are not paid by the medical, dental, or vision plans. Over-the-counter (OTC) medications are not eligible for reimbursement without a prescription.

The health care FSA maximum contribution is $2,750 for the 2020 plan year.*

Limited Purpose Health Care FSA (if you fund an HSA)If you fund an HSA, you are not eligible to fund a health care FSA. However, you can fund a limited purpose health care FSA. A limited purpose health care FSA can only be used to reimburse dental and vision expenses. In addition, a limited purpose health care FSA can only be elected if you fund an HSA.The limited purpose health care FSA maximum contribution is $2,750 for the 2020 plan year.

Dependent Care FSAThe dependent care FSA allows you to set aside money from your paycheck on a pre-tax basis for day care expenses to allow you and your spouse to work or attend school full time. Eligible dependents are children under 13 years of age, or a child over 13, spouse, or elderly parent residing in your house who is physically or mentally unable to care for himself or herself. Examples of eligible expenses are day care facility fees, before- and after-school care, and in-home babysitting fees (income must be reported by your care provider).

You may contribute up to $5,000 to the dependent care FSA for the 2020 plan year if you are married and file a joint return or if you file a single or head of household return. If you are married and file separate returns, you can each elect $2,500 for the 2020 plan year.

Things to Consider Before Contributing to an FSA: Per IRS regulations, health care FSA, limited purpose health care FSA, and dependent care FSA dollars are use it or lose it (no roll over allowed).

You cannot take income tax deductions for expenses you pay with your FSA(s).

You cannot stop or change your FSA contribution(s) during the plan year unless you experience a qualifying life event.

15LIFE AND AD&D INSURANCE

BASIC LIFE AND ACCIDENTAL DEATH & DISMEMBERMENT INSURANCECenturion automatically provides basic life and AD&D insurance through Cigna to all benefits-eligible employees at no cost. If you die as a result of an accident, your beneficiary would receive both the life benefit and the AD&D benefit. Additionally Centurion automatically provides basic life insurance for your dependent spouse at no cost.

Employee life and AD&D benefits: 1x annual salary up to a maximum of $150,000.

Spouse life benefit: $10,000.

VOLUNTARY LIFE INSURANCECenturion provides you the option to purchase voluntary life insurance for yourself, your spouse, and your dependent children through Cigna. You must purchase voluntary coverage for yourself to purchase coverage for your spouse and/or dependents. Voluntary life rates are age banded. Benefits will reduce to 65% at age 65, 45% at age 70, 30% at age 75, and 20% at age 80. Coverage for your spouse ends when he/she reaches age 70.

If you elect coverage when first eligible, you may purchase up to the guarantee issue amounts without completing medical evidence of insurability (EOI). If you do not enroll when first eligible, and choose to enroll during a subsequent annual open enrollment period, you will be required to complete EOI for any amount of coverage. Coverage will not take effect until approved by Cigna.

Employee: $10,000 increments up to $1,250,000 or 4x annual salary, whichever is less.

Spouse: $5,000 increments up to 50% of the employee’s election or $100,000, whichever is less.

Dependent children: $10,000.

Voluntary Life Insurance CostsListed below are the per-pay-period rates per $1,000 of coverage (26 pay periods per year) for voluntary life insurance. The amount you pay for coverage is deducted on a post-tax basis. Employee life rates are based on your age as of January 1, 2020. SHORT-TERM DISABILITY INSURANCECenturion automatically provides short-term disability (STD) insurance through Cigna to all benefits-eligible employees at no cost effective on the first day of the month following or coinciding with six months months of continuous employment. STD insurance is designed to help you meet your financial needs if you become unable to work due to an illness or injury. Benefits will be reduced by other income, including state-mandated short-term disability plan benefits. This is a company-paid benefit and any STD benefit payments you receive will be taxable.

Benefit: 60% of base weekly pay up to $1,000 per week

Elimination period: 14 days

Benefit duration: Up to 13 weeks for an approved disability from original date of disability less the elimination period

VOLUNTARY LONG-TERM DISABILITY INSURANCECenturion provides you the option to purchase voluntary long-term disability (LTD) insurance through Cigna. LTD insurance is designed to help you meet your financial needs if your disability extends beyond the STD period. For newly hired employees, this coverage, if elected, will begin on the first day of the month following or coinciding with six months of continuous employment:

Benefit: 60% of base monthly pay up to $6,000 per month

Elimination period: 90 days

Benefit duration: 2 years own occupation then any occupation to age 65

VOLUNTARY CRITICAL ILLNESSCenturion provides you the option to purchase voluntary critical illness coverage through The Hartford. The critical illness plan helps protect you and your family in the event of a critical illness. The plan pays a cash benefit if you are diagnosed with a covered illness while insured under the policy. Critical illness insurance covers a range of illnesses, including cancer, heart attack, and more.

You may use the payment in any way you choose, including paying for deductibles and coinsurance, everyday living expenses, recovery and rehabilitation, child care or caregiver expenses, and travel expenses to and from treatment center(s). As part of the plan, members receive a benefit of $100 for completing an annual health screening. The health screening benefit provides a flat reimbursement for each covered member after a 90-day waiting period from the effective date of the policy. View the summary plan description for specific rates. You will not be required to submit evidence of insurability for coverage.

You can choose the amount of coverage available at the time of enrollment. Coverage options for yourself are: $5,000, $10,000, $20,000, or $30,000. Here are some specifics about the plan: Full- and part-time employees may elect to defer up to 75% of their compensation on a pre-tax and/or after-tax basis up to the IRS annual limit ($19,500 for 2020). If you are age 50+ by December 31, 2020, you may make additional contributions ($6,500 for 2020). Centurion matches 50% of the first 3% you elect to defer, up to a maximum of 1.5% per-pay-period for eligible earnings. Employees are immediately vested in the matching contributions. COMMUTER BENEFITSCenturion allows you to pay up to $270 per month for commuter transit costs and $270 per month for commute-related parking costs with tax-free dollars through TRI-AD. Below is a description of the commuter products available.

Public Transportation and Vanpools Commuter check vouchers: Commuter checks can be used to purchase transit passes, tickets, fare cards, or other fare media for the transit authority of your choice. They can also be used to pay for vanpool expenses. Commuter checks come in flexible denominations to meet your transit needs and are valid for 15 months.

Commuter check card: This card is accepted at transit agencies or designated transit retail centers where only transit and vanpool passes, tickets, and fare cards are sold.* The card can be also used at fare vending machines. This saves you time waiting in line and time locating a customer service desk or staffed sales area.

Fare media: Various transit passes, tickets, and fare cards are offered by participating transit authorities (tolls are not eligible) across the nation. They are sent directly to your home, so you avoid waiting in line to purchase your transit passes.

*For compliance reasons, the commuter check card can only be accepted at designated outlets that sell transit products exclusively, such astransit stations and kiosks. Stores that sell other products, such as station sundry shops, will not accept the commuter check card.

Parking Monthly direct pay: This solution allows TRI-AD to send payments directly to your parking provider each month, so you don’t have to worry about writing checks and mailing payments.

Commuter check for parking vouchers: These vouchers are made payable to the parking provider of your choice and can be used to pay for parking expenses. Commuter check for parking vouchers may be used to purchase one or more types of parking, and you can order as many vouchers as needed for multiple parking providers. They come in flexible denominations and are valid for 15 months.

Commuter check card: This prepaid card offers the functionality and convenience of a reloadable, personalized debit card for use at parking facilities nationwide. No receipts are necessary with this solution—just swipe and go.

Parking cash reimbursement: You can elect a monthly amount to be set aside from your paycheck into a pre-tax spending account. Once you have incurred out-of-pocket parking expenses, simply complete a claim form and submit it with receipts for reimbursement.
