    IMPORTANT INFORMATION ABOUT THE SCHOOL Holy Name Catholic Primary School is a Voluntary Aided Catholic School provided by the Archdiocese of Liverpool and the Parish community of Holy Name in Fazakerley, with the assistance of the Local Authority and Liverpool Children’s Services. It is hoped that the following information proves useful to you, as we welcome you and your child to our school family. The education of your child is of paramount importance to us, as well as you. It is a partnership between Parents/carers, Governors, Staff, the Parish Community, the Archdiocese of Liverpool and Liverpool Children’s Services, where we are all working for the benefit of all the children entrusted to our care. Our school building is one storey and has ramps to access the building. Signs in the school are also in Braille and pictorial as part of our Dyslexia Friendly School status. We also have a portable loop, which can be made available, on request. There is a bay in the car park for disabled access. We operate a policy of Inclusion and have been awarded the Inclusion Mark (Advanced Level). Pupils with disabilities will not be treated less favourably than other pupils. We have an Accessibility Plan to increase access for pupils with disabilities to be DDA compliant. If you ever have any concerns, please do not hesitate to contact school. On occasion it may be necessary for an appointment to be made. COMPLAINTS PROCEURE Parents/carers have the right under Section 23 of the Education Reform Act to complain to the Governing Body if they believe that the Governors have not carried out their duty in respect of the School Curriculum, Religious Education and Collective Worship. We would hope that matters are resolved first with the Executive Head Teacher/Head of School and if this fails, the matter should be referred to the Governing Body as a formal complaint, in writing, addressed to the Chair of Governors, care of the school. If matters are still not resolved then the school will follow the LEA Complaints Policy.     AIMS OF HOLY NAME CATHOLIC PRIMARY SCHOOL At Holy Name we aim to:

    CHARITABLE DEEDS As a Catholic School we undertake a charitable deed each term to demonstrate our Christian beliefs in helping others less fortunate than ourselves. SAFEGUARDING AND CHILD PROTECTION All members of staff who work at Holy Name Catholic Primary School are cleared by the Disclosure and Barring Service, including Governors, as are all visitors to school who work with the pupils without supervision. Any infrequent visitors to school are given a copy of the school’s Code of Conduct to read and informed that by signing in, they are agreeing to abide by the document. All staff and Governors receive Safeguarding and Child Protection Training. There is a strictly adhered to policy for providing protection to safeguard all children. The Child Protection Officer at Holy Name is Ms P Sloane. The Deputy Child Protection Officer is Mrs H Perry. All staff receive annually, updated INSET in Safeguarding and Child Protection. There are two Governors for Safeguarding and Child Protection, one for staff and one for pupils. These people also receive the appropriate training. A copy of the Safeguarding and Child Protection Policy is available on request. SCHOOL INSPECTIONS Holy Name Catholic Primary School was last inspected by OFSTED in March 2017. This was a section 8 one day inspection and it was deemed to still be a good school. The most recent Religious Education inspection took place in May 2017 when the school was rated as a good school.     PLEASE NOTE: DOORS WILL BE OPEN AT 8.45 AM This is to give children the opportunity to read independently and to alleviate traffic congestion.

    We ask that ALL Key Stage One children are collected from school by an adult. If there is a change in the person who usually collects your child, please notify school. We also ask that you collect your child promptly at the end of the session. If there is an occasion when you will be late in collecting your child, please notify us, as any delay may lead to a child becoming distressed. School will contact Children’s Services if a pupil is NOT collected by 4.00 pm. If you have occasion to collect your child before the end of a session, you will be required, for Health & Safety reasons, to “Sign out” your child in the Early Collection Book. OUT OF HOURS CLUB This is a facility offered to parents/carers to provide care for their children from 7.45 am at the Breakfast Club. There is a charge of £3.00 per day which includes breakfast. After 8.00 am the charge is £2.00 per day. There is also a facility from 3.00 pm to 5.45 pm at a charge of £7 per day which includes refreshments. Both facilities are available on the days that the school is open. Parents/carers requiring further details should contact Mr. Moore, the Manager of the Out of Hours Club. Please note, on the days when school closes early, there will be no afternoon session available. ATTENDANCE AND PUNCTUALITY Children should only be absent from school if they are SERIOUSLY ILL. The Government has set a Target of 97% attendance. We ask that you encourage good attendance habits from an early age. Attendance is closely monitored by the LEA Attendance Officer. Persistent Absenteeism will be challenged and could result in prosecution. There are strict Government Guidelines on unauthorised absence from school. If your child is absent from school, we need to be notified by letter or telephone call. Parents/carers receive a termly Attendance Report. The Government states that holidays are not permitted during Term Time. The Governing Body will NOT authorise holidays in term time. Please refer to the Holiday List enclosed. Parents/carers are required to complete an Application Form, which is available from the Admin Office. The LEA is

    encouraging the use of Fixed Penalty Notices for holidays taken in term time. School will enforce this as requested. MEDICAL APPOINTMENTS Medical appointments should be made out of school hours, if possible. If this is unavoidable, then prior notice would be appreciated. Children MUST BE COLLECTED AT THE SCHOOL OFFICE on the day of the appointment, and “signed out” in the Early Collection Book. Evidence of an appointment is required. e.g. Appointment card. Punctuality is also important. When children are late they miss valuable TEACHING and LEARNING time.

    HOME–SCHOOL AGREEMENT Parents/carers are invited to sign the Home–School Agreement when their child starts at Holy Name. It forms a bond between children, school and parents/carers so that all will work together for the benefit of the child. HOMEWORK It is proven that children who complete their homework achieve better results. Homework is an integral part of a child’s learning process. It reinforces the learning in class. It also demonstrates the link between home and school. We ask that parents/carers support us in this policy. All children are given homework every night. However, it may not always be written homework. The completion of homework is monitored by each class teacher and parents/carers are notified of any problems. NB READING EVERY NIGHT (around 10 minutes is sufficient). This really does improve your child’s accuracy, fluency and skills of inference and deduction. Please note: If your child is struggling with an aspect of the homework set for them, please contact their teacher to discuss the matter.

    SPLIT AGE CLASSES Due to the fact that the Standards Admission Number is 45, it may be necessary to have split age classes. The reasons for having a split age class are: the number of pupils in all age groups throughout the school, number of classrooms, the budget and the number of teaching staff. ASSESSMENT There is a Nursery Assessment for Nursery pupils. Children on entry to Reception undertake a Baseline Assessment Test (new pilot in September 2019). Teachers complete a Foundation Stage Profile for Reception pupils throughout the school year. Teachers regularly assess a pupil’s progress using statements from the New National Curriculum as to whether a child is has reached the age expected level (expected) or is below (emerging) or above (exceeding). EXTRA CURRICULAR ACTIVITIES We aim to offer as many educational opportunities to our pupils as possible. Educational Visits are arranged to enhance Teaching and Learning of a topic. MOBILE PHONES The use of mobile phones is not permitted in school. Pupils who walk home from school alone or pupils who are going home with other family members can bring in mobiles but they should turned off and

    handed into the office when pupils arrive. They can be collected at the end of the day but must not be turned on until pupils are leaving school. TRANSITION Much work is done to ensure the smooth Transition for pupils from Nursery to Reception classes, and likewise for Y2 pupils “moving up” into KS2. Support is offered to Year 6 parents/carers in selecting a High School for their child. We work closely with the High Schools, providing information about all pupils, and facilitate attendance at “Taster Days” and “Open Days”. References are given when High Schools request them. We are also willing to help and support parents with Appeals. SATS In the Summer Term pupils in Years 2 and 6 undertake the Government Tests. Parents/carers receive the results with their child’s Annual Written Report. Parents/carers are notified of the school’s performance in the Prospectus. PHONICS SCREENING In the Summer Term, pupils in Year 1 undertake the Government’s Phonics Screening Check. Any pupils who were not successful in the Phonics Screening in Year 1 will get extra phonics support in Year 2 and retake the Phonics Screening Test in the Summer Term of Year 2. Parents/cares are informed whether their child has been successful or not.

    Due to Covid 19 and the National and Local Lockdowns in 2020, SATs were not undertaken. However, Teacher Assessment still took place. Local and National Averages were not available for comparison.

    ARCHDIOCESAN LEVY A Levy of £10.00 per child/family per year is set by the Archdiocese. School is asked to collect this money from parents/carers and then forward it to the Archdiocese. If parents/carers fail to pay this levy, then the school has to find the shortfall. CHARGING POLICY Parents/carers are asked to make a voluntary contribution towards the cost of an Educational Visit organised by the school. No child will be excluded due to Family hardship. Such matters should be discussed with the Executive Head Teacher, and will be treated with confidentiality and discretion. However, if there are insufficient voluntary contributions, then the activity might not take place. PARENTAL INVOLVEMENT IN SCHOOL LIFE We welcome and encourage you to take an active part in your child’s education. There are opportunities for you to become involved. If you are interested in working alongside the teachers, you might like to consider listening to children read. Please contact the Executive Head Teacher or Mr Morris to express your interest. In the Autumn and Spring Terms parents/carers are invited to a Parents’/Carers’ Meeting. In the Summer Term you will receive your child’s Annual Report. You will also be given the opportunity to discuss its contents with your child’s class teacher by arrangement if you wish to. Meetings to discuss certain aspects of the Curriculum or on special issues will be made known to you throughout the year. Each half term you will receive an outline of the work planned for your child to learn. Parents/carers also receive monthly Newsletters. These will also be available on the school website. PASTORAL CARE AND DISCIPLINE At Holy Name we are committed to the use of positive reinforcement of good behaviour. We adopt an Assertive Discipline Policy and reward scheme called Good to be Green. This is evident in every day life and formally recognised at the weekly Golden Assembly each Friday morning and the Golden Assembly at the end of the school year. The classes with the best weekly attendance and the best behaviour and attitude are also awarded. We strive to celebrate the children’s achievements in as many ways as possible recognising achievement, progress and effort. Praise and encouragement are always at the forefront. We believe that parents/carers are the prime educators, and we wish to work in partnership with you. If you ever have a concern, please do not hesitate to contact school in order that the matter may be shared and resolved. Occasionally it might be that an appointment may be necessary. Unacceptable behaviour is recorded in an Incident Book. Parents/carers will be notified, by the Executive Head Teacher, regarding serious incidents. In serious cases, where children are not co-operating, they will be asked to go home for their lunch. We use these sanctions to maintain a calm, safe atmosphere where children can thrive. It is always made clear that it is the behaviour which is unacceptable. We respect each child, and each child is respected and loved for his or her uniqueness. Every child is encouraged to develop self-discipline from an early age.

    We rely upon the firm foundation of a strong partnership between home and school in order to help our pupils grow and develop into responsible and caring, independent young people. Parents/carers need to acknowledge that the school has the power to use reasonable force, in exceptional circumstances, as outlined in the policy on Restraint. Parents/carers are advised that all school policies are available on request. PARISH LINKS Our school enjoys close links with the Parish of Holy Name. Our Parish Priest, Fr. John Hindley, is a regular visitor to school. There are also close links with the Parish in the preparation of the children for the Sacraments. Every Sunday, at the Parish 11.00 a.m. Mass, there is a “Children’s Liturgy” or “Children’s Church”. This helps the children in their understanding of the Eucharist and Faith. It develops a greater sense of belonging in the Parish. As a school, we strongly recommend attendance at the “Children’s Church” each week as it nurtures a stronger bond between the school, home and Parish. At the beginning of each school year there is a Mass to dedicate our year’s work to Almighty God. Parents/carers and members of the Parish are warmly welcomed to join us. At the end of the year we take time to thank Almighty God for all that we have achieved together with the help of His grace and love. HEALTH AND SAFETY For the safety of all, but in particular the children: Dogs are NOT allowed on the playground. Bicycles are NOT Allowed to be ridden on the playground but can be left in the designated areas as long as they have a bicycle lock on them. Smoking is also NOT allowed anywhere on the school premises (building and/or grounds).

    PARKING AREA The area around school is for “Residents’ Parking only”. For the safety of the children, Parents/carers are asked not to park in Moss Pits Lane or in the School car park. Parents/carers are also asked not to obstruct the entrance to the car park. We would hope that Parents/ carers would park considerately and safely on nearby roads, and walk into school to collect their children. MEDICATION Holy Name School has staff trained in the administering of First Aid. Parents/carers are asked to provide the school with details of their child’s medical condition(s) if appropriate. This is recorded in the school’s “Medical Room” as well as the child’s personal file. For Health & Safety reasons, we require written instructions from a Medical Practitioner regarding any Medication. This includes inhalers, where children administer their own medication under the supervision of an adult. Inhalers should be clearly named. We ask that parents/carers obtain one inhaler for school use and one for home use. INJURY & SICKNESS We request that the school has an emergency contact telephone number for each child for use in the eventuality of an accident occurring whilst the child is in school.     school’s accident book. If your child is involved in an accident you will be notified. If it is a serious accident you will be contacted immediately. All head injuries are considered serious and you will be advised to take your child home for close monitoring. In the unlikely event that your child requires hospital attention arrangements will be made to meet you at the hospital. No treatment can be administered to your child without your permission and signature at the hospital. It is imperative that school records are kept up to date in case of an emergency. SCHOOL NURSE The school nurse holds a drop in session for parents/carers. This will be an opportunity to discuss, in confidence, any medical issues your child may have and get advice. SCHOOL MEALS Infants (Reception and Key Stage One) All children in the Infants are offered a free meal under a Government initiative. Children may stay for a school dinner, provided by our school cook. Meals are provided by Absolute Catering. However, if your child is entitled to free school meals we ask that you complete the application as the school will receive extra funding for each child. Thank you. Juniors (Key Stage Two) Dinner Money (£1.90 per day = £9.50 per week) MUST be PAID WEEKLY IN ADVANCE on a MONDAY MORNING to your child’s class teacher. It should be brought in a sealed envelope with your child’s name and the amount enclosed written on it. Meals cannot be provided without payment. It is the responsibility of parents/carers to make alternative arrangements either by taking their child home for lunch or providing them with a packed lunch. If your child is absent then a credit for the absence will be made the following week. Should your child be entitled to free school meals, application forms are available at the One Stop Shop to enable you to make an application to the LEA. Please make prompt application in readiness for the beginning of the academic year. Remember that the Council has thousands of applications to process. Please be advised that the matter will be dealt by the school with the utmost discretion and respect. Without the proper authorisation from the LEA, school cannot provide a Free School Meal. Until school receives this authorisation, parents will be required to make alternative arrangements by paying for their child’s school meal, taking their child home for lunch or providing a packed lunch for their child. PACKED LUNCHES Children may bring a packed lunch if you prefer but we would ask parents/carers to support the school’s work in promoting “a healthy lifestyle” by giving their child “a healthy packed lunch.” If children wish to change from school meals to packed lunch, we require you to give half a term notice.     PLAYTIME SNACKS In order to promote healthy eating, crisps, biscuits, chocolate and sweets are NOT permitted for break time. The children are encouraged to bring a healthy option such as fruit, raisins or cheese. Fizzy drinks are also NOT permitted. Fruit is available for KS1 pupils. WATER BOTTLES Throughout the school day, pupils have access to water to ensure they do not get dehydrated. Please supply your child with a plastic water bottle, labelled with their name which they will bring home at the end of each day for you to clean out. Water fountains are in place around the school for pupils to refill them. SPECIAL EDUCATIONAL NEEDS AND DISABILITY (SEND) In line with our Mission Statement, which guides us to fulfil each child’s potential, we strive to maintain a high quality curricular provision for each child, of every ability. A proportion of our children may have special needs or disability and we aim to meet their requirements through a flexible, appropriate and varied provision. We aim to identify a child’s needs as early as possible and inform parents/carers if their child’s name is put on the Special Needs Register. Parents/carers are invited to a termly review of their child’s progress towards their targets on the One Page Profile. Holy Name Catholic Primary School is legally bound to follow The Code of Practice.     Holy Name is a Catholic School under the trusteeship of the Archdiocese of Liverpool. It is maintained by Liverpool City Council. As a Voluntary Aided School, the Governing Body is the Admissions Authority and is responsible for taking decisions on applications for admissions. The co-ordination of admissions arrangements is undertaken by the Local Authority. For the school’s year commencing September 2020, the Governing Body has set its admissions number at 45. Our principal role as a Catholic school is to participate in the mission of the Catholic Church by providing a framework which will help children to grow in their understanding of the Good News and in the practice of their faith. The school will help the children develop fully as human beings and prepare them to undertake their responsibilities as Catholic in society. The school asks all parents applying for a place here to respect this ethos and its importance to the school community. This does not affect the rights of parents who are not of the faith of this school to apply for and be considered for a place here. ADMISSIONS TO THE SCHOOL will be determined by the Governing Body.     The distance will be measured by straight line distance between the child’s permanent home address and the school using the local authority’s computerised measuring system. Children with a Statement of Special Educational Needs or Education Health Care Plan that names a school will be offered place without using the admission criteria and will count as part of the school’s published admission number. Notes: a. All applications submitted before the national closing date will be considered equally and included

    in the Local Authority initial allocation of school places. Applications received after the national closing date will be processed in accordance with Liverpool admissions scheme for schools detailed in the Liverpool admissions information booklet.

    b. A Looked After Child is a child who is (a) in the care of a Local Authority, or (b) being provided with accommodation by a Local Authority in the exercise of their Social Services functions (under section 22(1) of the Children Act 1989. A previously Looked After Child is one who immediately moved on from that status after becoming subject to an adoption, child arrangements order or special guardianship order. Written evidence of their reception into full communion with the Catholic Church can be obtained by recourse to the Register of Receptions, or in some cases a sub-section of the Baptismal Registers of the Church in which the Rite of Reception took place. The Governing Body will require written evidence in the form of a Certificate of Reception before applications for school places can be considered for categories of ‘Baptised Catholics’. A Certificate of Reception is to include full name, date of birth, date of reception and parent(s) name(s). The certificate must also show that it is copied from the records kept by the place of reception.

    Those who have difficulty obtaining written evidence of baptism for a good reason, may still be considered as baptised Catholics but only after they have been referred to the parish priest who after consulting with the Episcopal Vicar, will decide how the question of baptism is to be resolved and how written evidence is to be produced in accordance with the law of the Church.

    d. Home Address is considered to be the address where the child normally lives. Where care is split and a child moves between two addresses, the household in receipt of the child benefit would normally be the address used but the admission body reserve the right to request other proofs as fit the individual circumstance. Applicants should not state a childminder’s or other relative’s address.

    e. Sibling is defined in these arrangements as full, half or step brothers and sisters, adopted and foster

    brothers and sisters who are living at the same address and are part of the same family unit. This does not include cousins or other family relationships.

    f. A waiting list for children who have not been offered a place will be kept and will be ranked according to the Admission Criteria. The waiting list does not consider the date the application was received or the length of time a child's name has been on the waiting list. This means that a child's position on the list may change if another applicant is refused a place and their child has higher priority in the admissions criteria. The waiting list will be retained until at least the end of December of the relevant year of the admissions process.

    g. For ‘In Year’ applications received outside the normal admissions round, if places are available

    they will be offered to those who apply. Direct application to the school can now be made under this heading. If there are places available but more applicants than places then the published oversubscription criteria for the relevant year group will be applied. A waiting list for those who have not been offered a place will be kept until the end of the relevant academic term.

    h. If an application for admission has been turned down by the Governing Body, parents can appeal to an Independent Appeals Panel. Parents must be allowed at least twenty school days from the date of notification that their application was unsuccessful to submit that appeal. Parents must give reasons for appealing in writing and the decision of the Appeals Panel is binding on the Governors.

    i. The Governing Body reserve the right to withdraw the offer of a school place where false evidence is received in relation to the application.

    j. It is the duty of governors to comply with regulations on class size limits at Foundation Stage and Key Stage One. The Governing Body may exceed the regulations for twins and children from multiple births where one of the children is the 30th child admitted. This also applies to in-year applicants who are looked after/previously looked after, children of UK service personnel or children who move into the area for whom there is no other school available within a reasonable distance.

    k. If a child is a “summer born child”, parents can request that the date their child is admitted to school is deferred to later in the school year.

    Parents of children who are offered a place at the school before they are of compulsory school age can defer their child’s entry until later in the school year or until their child reaches compulsory school age in that school year.

    Parents cannot defer entry beyond the term after the child’s fifth birthday, nor beyond the academic year for which the original application was accepted.

The expectation would be that parents have decided that their child miss the reception year and would be applying for a year 1 place, depending upon availability.

    A request for a full year deferral with a start in Reception would be considered to be an application to admit outside the normal age range (see below)

    If a parent wishes their child to be educated out of their normal age group (e.g. if the child is

    gifted and talented or has experienced problems such as ill health or is a summer born child) they must discuss this with the school before applying for a place. The decision rests with the school as admission authority as per para 2.17 of the Admissions Code.

    Where entry is deferred, the school will hold the place for that child and not offer it to another child during the remainder of the school year.

    Parents cannot defer entry beyond the term after the child’s fifth birthday, nor beyond the academic year for which the original application was accepted.

    Parents can also request that their child attends part-time until their child reaches compulsory school age.

    Parents of a summer born child, born between April and August, may choose not to send that child to school until the September following their fifth birthday. The expectation would be that parents have decided that their child miss the reception year and would be applying for a year 1 place, depending upon availability.

    A request for a full year deferral with a start in Reception would be considered to be an application to admit outside the normal age range (see below)

    l. If a parent wishes their child to be educated out of their normal age group (e.g. if the child is

    gifted and talented or has experienced problems such as ill health or is a summer born child) they must discuss this with the school before applying for a place. The decision rests with the school as admission authority as per para 2.17 of the Admissions Code.

    If there is more than one application for an available place reference will be made to the oversubscription criteria.

    NURSERY CLASSES IN DIOCESAN PRIMARY SCHOOLS

    Admission to the Nursery class at Holy Name Catholic Primary School will be on a part time basis and made by the Governing Body in accordance with the parental applications subject to the following set of ADMISSIONS CRITERIA forming a priority order where there are more applications for admission than the Nursery has places available. For the school year commencing September 2016 the Governing Body has set its admissions number at 52 part time, which means 26 maximum for each of the morning and afternoon sessions. 1. Looked After Children and previously Looked After Children 2. Baptised Roman Catholic children resident in the parish of Holy Name. 3. Baptised Roman Catholic children who have a sibling in the Primary School at the time of admission 4. Baptised Roman Catholic children resident in other parishes 5. Looked After Children and previously Looked After Children other than Catholic. 6. Other children. NOTES a) A Looked After Child is a child who is (a) in the care of a Local Authority, or (b) being provided with accommodation by a Local Authority in the exercise of their Social Services functions under section 22(1) of the Children Act 1989. A previously Looked After Child is one who immediately moved on from that status after becoming subject to an adoption, residence or special guardianship order. b) For a child to be considered as a Roman Catholic evidence of a Roman Catholic Baptism will be required. c) The decision with regard to the allocation of a morning or an afternoon place rests with the Headteacher. d) Admission to the Nursery Class does not necessarily secure admission to the Primary School. e) If in any category there are more applications than places available, priority will be given on the basis of distance from home to school. Distance will be measured in a straight line from the front door of the child’s home address (including the community entrance to flats) to the main entrance of the school using the Local Authority’s computerised measuring system with those living nearer to the school having priority. In the event of distances being the same for two or more applicants where this distance would be last place/s to be allocated, a random lottery will be carried out in a public place. All the names will be entered into a hat and the required number of names will be drawn out. f) Where a child lives with parents with shared responsibility, each for part of a week, the child’s “permanent place of residence” will be determined as the address of the parent who ordinarily has responsibility for the child, as determined as the address of the parent who normally has responsibility for the majority of school days in a week. g) Sibling is defined in these arrangements as children who live as brother or sister in the same house, including natural brothers or sisters, adopted siblings, stepbrothers or sisters, foster brothers and sisters or the child of a parent/carer’s partner where the child for whom the school place is sought is living in the same family unit at the same address as that sibling.

    h) The Governing Body reserve the right to withdraw the offer of a school place where false evidence is received in relation to baptism, sibling connections or place of residence. Pupils who are admitted to Holy Name Catholic Primary School will enter the reception classes on a specified day in September 2016, which is forwarded to parents individually. This is to assist with the Admissions procedure, following parental contributions to the Foundation Stage Profile. IMPORTANT INFORMATION PARENTS MUST COMPLETE the Local Authority Common Application form. They are also strongly advised to complete the school’s Supplementary Form.
