As course director for 2019 Annual Conference and President of the American Medical Marijuana Physicians Association (AMMPA), I would like to welcome each and every one of you to this year’s conference.

We are committed to outreach locally, and nationally on topics that affect you, your practice, and your patients. We strive to bring together an incredible faculty of thought leaders, and conference attendees who are our colleagues and friends who truly want to improve the science, patient outcomes, and quality of care. We hope to bring discussions

and presentations on the best that science has to offer, and we encourage feedback and recommendations to make the program truly what it sets out to be.

The AMMPA Annual Conference continues to grow as we deliver consistent quality education for physicians interested in cannabis science and research. We provide an opportunity to network with like-minded physicians from around the world that are also valued members of the American Medical Marijuana Physicians Association. We have a full two and half days of educational presentations, networking, and panel discussions with thought-leaders researching and utilizing medical cannabis to improve the health of their patients.

I sincerely appreciate the input and energy invested in this conference by our board members, planning committee, scientific education committee, faculty members, and conference staff in an effort to continue to make this program educational, informative, enlightening, and most of all, fun. Raphael Bones, M.D.

Richard Boxer, M.D.

Dr. Raphael Bones was born and raised in beautiful Guayanilla Bay, Puerto Rico. He grew up in a family that enjoys fishing and water sports. While attending the University of Puerto Rico, he joined the US Navy as he felt committed to serving his country. At the birth of the new millennia he graduated from the Universidad Central Del Caribe Medical School. After completing two years of the General Surgery residency program in Puerto Rico he changed his goals and decided to complete his residency in Internal Medicine in 2005. As a physician, he has experience in various settings including Emergency Room, as a Hospitalist, and as a Primary Care Provider in outpatient clinics. Dr. Bones is currently a member of The American Medical Association, The Florida Medical Association, The American Association for Physician Leadership, and The American Academy of Anti-Aging Medicine. In 2015 Dr. Bones joined Prixus Medical to practice primary care with integrative functional medicine. Now he enjoys practicing medicine the way he envisions it and in his spare time he loves playing golf and boating in the Florida coast waters.

Dr. Richard Boxer is currently a Clinical Professor of Urology and Visiting Scholar at University of California, Los Angeles and was formerly Professor of Clinical Urology at the University of Miami, and the University of Wisconsin. Dr. Boxer was a finalist for the position of Surgeon General of the United States in both the Clinton and Bush administrations and has served as a senior health policy advisor to Presidents, Cabinet Secretaries, Governors, Senators, and Members of Congress. Dr. Boxer has been a member of the Board of Directors at the National Cancer Institute and the National Institute of Diabetes, Digestive and Kidney Diseases. He represented the U.S. at the World Health Organization and is a recipient of a Presidential Citation from the American Urological Association. Dr. Boxer has had 150 publications and has delivered hundreds of invited lectures over his distinguished career. He is dedicated to the advancement of research and improvement in quality of medical cannabis.

Brian Evander, JD

Kenneth Brummel-Smith, M.D.

Brian has been admitted to practice law in the State of Florida since 2010. In his role as Partner with Lowe & Evander, P.A., Brian represents the interests of physicians, physician groups and their respective insurance carriers in numerous areas of health care law and litigation, such as defending physicians and providers against professional licensure investigations and disciplinary processes (including against the Florida Department of Health, the Florida Agency for Health Care Administration, Florida’s Medicare Administrator, and the U.S. Department of Health and Human Services – Offices for Civil Rights), preparing business documents (such as physician employment agreements, operating agreements, partnership agreements, health care provider and medical director contracts) and negotiating disputes pertaining to same, analyzing and preparing advisory opinions for compliance with applicable health laws (such as HIPAA, STARK law, and the Florida Patient Self-Referral Act), and representing clients in all phases of litigation in both business litigation as well as medical malpractice defense (presuit and litigation).

Dr. Brummel-Smith is Professor Emeritus in the Department of Geriatrics at the Florida State University College of Medicine. He founded and served as chair of the Department of Geriatrics from 2003 until 12015. He served as chief of the division of geriatrics at two medical schools (University of Southern California and Oregon Health Sciences University). He is a co-editor or author of five textbooks and has written numerous book chapters and articles in the area of geriatrics, ethics and geriatric rehabilitation. He has been selected 16 times by his peers as a member of the “Best Doctors in America.” He was a member of the National Advisory Council on Aging for the National Institute on Aging and a Health and Aging Policy Fellow with the Senate Special Committee on Aging in Washington DC. He is a past-president of the American Geriatrics Society. He serves on the Aging, Disability and Independence Forum of the National Academy of Medicine.

He graduated from the University of Southern California School of Medicine, did a residency in family medicine at Glendale Adventist Medical Center, and a fellowship in the Department of Medical Education at the University of Southern California School of Medicine. He is board certified in Family Medicine and has a Certificate of Added Qualifications in Geriatrics. He retired from FSU in 2017 and is enjoying riding, hiking, and consulting.

Orlando Florete, Jr., MD

The Honorable Nikki Fried, J.D.

Orlando G. Florete, Jr, MD, has received numerous scholarships and awards, among themthe Winthrop Stearns Scholarship Award and the Community Service Award from theDepartment of Anesthesiology at the University of Florida College of Medicine. In 2011,Dr. Florete was recognized by the International Association of Anesthesiologists as one ofthe Leading Physicians of the World.

Nicole “Nikki” Fried, Florida’s 12th Commissioner of Agriculture and Consumer Services, is a lifelong Floridian, attorney and passionate activist.

Born and raised in Miami, Commissioner Fried graduated from the University of Florida, where she received her bachelor’s, master’s and juris doctorate degrees. While in law school, she served as student body president, the first woman to hold the position in nearly two decades.

Prior to being elected, Fried worked as an advocate in Tallahassee representing at-risk children, the Broward County School Board, and for the expansion of patient access to medical marijuana.

As an attorney, she was head of the Felony Division at the Alachua County Public Defender’s Office and worked at law firms as a government consultant, advocating on behalf of clients before the Florida Legislature. Working in private practice in South Florida, she defended homeowners against foreclosure during the 2007-2008 housing crisis.

Throughout her career she has served with numerous organizations including the Young Lawyers Board of Governors, Broward Days Board of Directors, Legal Needs of Children Bar Committee, LeRoy Collins Institute, Girls Scouts of Southeast Florida, University of Florida Governmental Relations Advisory Committee, University of Florida Board of Trustees, Florida’s Children First, and others. Fried is a member of Florida Blue Key, the oldest and most prestigious leadership honorary in the state of Florida.

Greg Gerdeman, PhD

Barry Gordon, M.D.

Dr. Gerdeman is a neuroscientist and educator with expertise in the physiological actions of cannabis and the endocannabinoid system (ECS). This PhD dissertation in Pharmacology from Vanderbilt University was one of the first studies to discover endocannabinoids acting as “retrograde messengers” that regulate synaptic plasticity in the brain, findings that have become a foundation to modern understanding of the ECS and the neuroprotective actions of c annabis. Greg’s research has been widely cited in the scientific literature, and highlighted in such diverse publications as Science, National Geographic, Men’s Health, and The New York Times. He has been a member of the International Cannabinoid Research Society (ICRS) since 1999, and has received research funding through grants from the National Institute on Drug Abuse (NIDA) and the National Science Foundation (NSF). His scholarly research has spanned topics ranging from the synaptic physiology of the ECS; behavioral pharmacology of CB1 receptors and their regulation of habitual memories; the physiological activation of ECS signaling in exercise; and the evolutionary and comparative biology of cannabinoid receptors through studies on invertebrate marine chordates.

Through deep study of the science of cannabis, Dr. Gerdeman became a vocal advocate for reclaiming this plant as a safe and effective natural remedy. Over the last 10 years, he has progressively studied the chemical ecology and genetics of cannabis, and the extraction of cannabis biomolecules to create medicinal formulations.

For more than three decades, Barry Gordon M.D. built a career working on the front lines of the American health care system.

After years of seeing the same faces and resurrecting the same addicts daily in Ohio emergency rooms, he thought there had to be a better way.

Today, Florida is just one of 30 states that recognize the medicinal value of marijuana for the relief of symptoms associated everything from neurological illness to chemotherapy.

Dr. Barry, as he is affectionately known, is at the forefront of the medical cannabis movement.

Hinanit Koltai, PhD

Prof. Hinanit Koltai, PhD, is a Senior Research Scientist at the Agricultural Research Organization, Volcani Center, Israel. She is the Editor of books and a member of Editorial boards in international scientific journals. She is a leading Author of more than 80 peer reviewed publications and more than 30 book chapters and invited reviews and she holds 9 patents. She teaches plant development and medical cannabis courses in Bar Ilan University, Israel.

Research in Koltai lab is focused on deciphering the “entourage effect” between cannabis compounds while specifying active pharmaceutical ingredients (API) from cannabis and their medical activity at the physiological and molecular levels in human cells and tissues. The lab combines state of the art chemistry and cellular and molecular biology tools, making novel discoveries in this field. Research results are IP protected; they are published in international scientific journals and form a scientific basis for the development of the new generation of cannabis-based medical products.

Jade Green

Jade is the CEO of Jade Green Consulting, an all-female canna-business consultancy, and a member of AMMPA’s Board of Directors. She was appointed to AMMPA’s Board in 2019, bringing nearly five years of cannabis industry leadership and executive management experience to her role with the company. Jade started college at sixteen years old and graduated from the University of Florida with an honors degree in Biochemistry. A career entrepreneur, Jade founded her first company in the allergy and immunology sector at age twenty-one, which she operated as CEO for nearly two years. In 2015, Jade was asked to consult on a medical marijuana license application for San Felasco Nursery, which ended up winning one of only five vertically integrated licenses awarded in Florida. Jade moved to Los Angeles in 2016, where she and her team now advise large cannabis companies on their operational and expansion strategies. Jade’s education and experience in biomedical sciences allows her to understand and advocate for cannabinoid-based therapies in highly regulated environments, and her entrepreneurial proficiency affords her the perspective to analyze and advance cannabis businesses in emerging markets.

Michael Lowe, JD

Michael R. Lowe, Esq. is board certified in health care law by The Florida Bar. As Managing Partner of Lowe & Evander, P.A., his practice focuses on all aspects of business, corporate, transactional, litigation, regulatory, operational and administrative matters in the health care law arena. Emphasizing the representation of physicians and physician group practices, Michael regularly represents clients in litigation, medical records and HIPAA privacy regulations issues, managed care contracting and reimbursement matters, the preparation, review and negotiation of physician employment agreements, Medicare/Medicaid fraud and abuse prevention, federal Stark Law matters and analysis, defense and reimbursement issues, ACA matters, medical staff privilege cases, professional licensure and disciplinary actions, voluntary self-disclosure cases, physician-hospital contracts, compliance plan development, and health care regulatory analysis and counseling matters. Michael’s practice areas also includes the representation of hospital medical staffs, IPAs, ambulatory surgery centers, durable medical equipment providers, diagnostic imaging centers, hospitals, long-term care facilities and other health care professionals and providers.

Dr. Ralph Nobo is a member of the Board of Directors of AMMPA and an OB/GYN and past President of the Florida Medical Association. He also serves as Board Member and Chair of the Private Practice Committee of The Physicians Foundation. He has held multiple positions at the Florida Medical Association, including President, Vice President, Secretary and Delegate to the American Medical Association. Dr. Nobo is also a Medical Student Advisor and liaison for the American Medical Association.

Julio Paez, M.D.

Michael Patterson, NHA, OTR/L, CEAS

Julio Paez, MD, takes a comprehensive approach to pain management services at South Lake Pain Institute in Clermont and Orlando, Florida. He combines his background in anesthesiology and pain management with a deep passion for helping his patients to lead healthy, active and pain-free lives. Dr. Paez provides every patient a highly customized plan of care that relieves pain and improves overall wellness.

Dr. Paez is a renowned, double board-certified Medical Doctor with specialization in anesthesiology and pain medicine. He completed his anesthesiology residency at the University of South Florida in Tampa, Florida. He went on to complete additional training in pain management at the State University of New York (SUNY).

Dr. Paez has decades of experience in helping patients find relief from painful conditions including fibromyalgia, headaches and migraines. His expertise also includes diagnosis and treatment for conditions causing back and neck pain as well as, issues stemming from cancer, auto accidents and surgical procedures.

In addition to daily patient care, Dr. Paez oversees and manages the research division at South Lake Pain Institute. Mr. Patterson is 25-year health care executive who has managed successful US healthcare corporations of all shapes and sizes in the nation’s most regulated industries. Michael’s experience includes being Chief Operating Officer of Avante Group Inc, a national skilled nursing and therapy corporation with annual revenues in excess of $225 million, and Chief Development Officer of a national medical ancillary services company focusing on development and management of pharmacies, laboratories, assisted living centers, and home health companies across the United States with annual revenues of $300 million.

Mr. Patterson is the founder and CEO of US Cannabis Pharmaceutical Research and Development (US Cannabis). US Cannabis was established to develop the legal cannabis/hemp/CBD markets across all platforms (education, investment, regulation, cultivation, production, dispensing, research and development, payments/cryptocurrency/banking, advocacy)

Lawton Tang, M.D.

Tara L. Tedrow, Esq.

Dr. Lawton Tang is an award winning plastic surgeon from the American Society of Plastic Surgery and Plastic Surgery Educational Foundation and was recognized with the First Place Award in Body Contouring Mentoring at the Breast and Body Symposium at Dallas Texas. He is a published author of multiple journals and textbooks and has been named Los Angeles “Top Docs”, Patients’ Choice Award, and 2010 Compassionate Doctor Recognition Award. Dr. Tang started his medical training at State University of New York.

After winning several awards and research grants he relocated to Mount Sinai in Miami to train in General Surgery and Trauma Critical Care. He attended the University of Pittsburg, completing an Aesthetics Breast and Body Contouring Fellowship and served as clinical instructor in their Department of Plastic and Reconstructive Surgery. He then completed another Plastic and Reconstructive Fellowship at MCG, allowing him to complete his surgical training from five academic institutions of excellence.

In addition to his practice, Dr. Tang teaches surgeons in training from Huntington Memorial Hospital and University of Southern California. He continues to publish his research in many of the top plastic surgery journals, including articles and book chapters in the Plastic and Reconstructive Surgery and Cosmetic Surgery periodicals. Dr. Tang continues to contribute to the body of literature of biomaterials being designed for skin, bone, and nerve regeneration, with a strong focus on polymeric materials, both native and synthetic.

Tara L. Tedrow is a shareholder in the Lowndes Law firm’s Real Estate Transactions, Development, Land Use, Zoning & Environmental practices. Tara’s practice focuses on land use and development, as she assists clients statewide on entitling projects for commercial, residential, industrial, office, and mixed uses. Tara works with local governments and other regulatory authorities in order to address the needs of her clients related to environmental permitting and compliance, zoning, comprehensive plans, concurrency, site plan approval, variance and waiver requests, due diligence, and property rights. Additionally, Tara is the Chair of the Cannabis and Controlled Substances Group, where Tara assists clients in complex regulated industries such as alcoholic beverage and medical marijuana licensing. Tara works with physicians, lenders, real estate developers, landlords, ancillary service providers, licensed Medical Marijuana Treatment Centers and license applicants, helping them to navigate the ever changing regulatory landscape of medical marijuana. Tara is a regular presenter and speaker at industry seminars and conferences. As of the fall of 2018, Tara was the first professor in the state of Florida to teach a law school course on medical marijuana law and policy at the University of Florida Levin College of Law.

Michelle Weiner, DO, MPHDr. Michelle Weiner is double board certified in Interventional Pain Management and Physical Medicine and Rehabilitation and is one of the Pain Physicians for Spine and Wellness Centers of America. She completed her residency and fellowship training at the University of Miami. She is currently faculty at FLorida international FIU and Nova southeastern university. She has an IRB approved research project on using cannabis as a substitute for opioids in chronic pain. She is an advocate for and a leader in the Florida Medicinal Cannabis industry. Dr. Weiner is passionate about educating the public and other medical professionals about the evidence supporting Cannabis, its benefits for a multitude of different conditions as well on the Endocannabinoid System. She individualizes medical cannabis treatments based on patient specific factors with the goal of maximizing quality of life, decreasing overall pharmaceutical use and side effects. She is knowledgeable in diagnosing and treating vitamin and micronutrient deficiencies. Dr. Weiner’s practice also treats spine and musculoskeletal pain as well as chronic migraines, performs ultrasound guided joint injections, uses platelet rich plasma (PRP) and stem cells and is well trained in aesthetic medicine.

Florida Department of Agriculture and Consumer Services, drove into work several weeks ago and realized that she was singing to herself. That astounded her. “I thought to myself, wow, I’m happy,” she says. “I didn’t do anything like that in my last few jobs. I really enjoy coming in, working with the people I work and interact with everyday.”

For Bell, this position is about helping to change lives.

“I field so many calls about people who need help and are looking for someone in Tallahassee that can help them change their lives, help them on

their farms, or help in so many other ways. That’s what I enjoy.

I’m a person who likes to help others and I love to sit back and

watch them flourish as you give them a little help.” Florida Agriculture Commissioner Nikki Fried appointed Bell,

57, to the newly-created position of director of cannabis. Bell’s

office is responsible for expanding patient access to medical mar-

ijuana across the state. Bell was previously a Nashville-based business consultant for

the hemp and cannabis industry. She met the commissioner

through some of her consulting work in the past. When she learned that Fried was running, she want-ed to help on a grassroots level. “I’m in the latter part of my career,” says Bell. “The older you get, the more you really want to think about what matters in life and you want to do some-thing you enjoy and feel passionate about. I listened a lot to what the commissioner was saying at the time, read her articles and watched her videos. I felt inspired. There’s a handful of people I can recall in my life that I listened to and thought I like what you say and I will follow you wherever you go.” When Fried won, Bell was ecstatic. She asked how she could help and Fried said she should apply for the director of cannabis position. “My number one priority is to be here to support

the commissioner and implement the vision she has and that she

ran on during her campaign,” says Bell. One of those visions is to try to get hemp legislation done,

according to Bell. Last year's Farm Bill legalized regulated hemp

production. “We have two bills pending in the House and Senate,” she says.

“After the bills are passed, my goal is to get an infrastructure in

place to help build a hemp industry in the state of Florida to ben-

efit farmers, workers, processors, manufacturers, and retailers. Continued on page 40

with the New Orleans Saints, has launched a new cannabis business now that his playing days are behind him. He brings his own person-al experience with using cannabis for pain man-agement and mental health, along with his vast industry knowledge, to lead his venture. But Williams has also become a fierce advo-cate for medical marijuana use in the NFL after his career was cut short by injury. He is on a mission to dispel misconceptions about cannabis through information, products and services. He says he spends every day in action, making his ‘Saving Lives and Changing Minds’ slogan more than a catchy motto for him and the lives he has

touched and changed. This slogan is empowerment, and

cannabis is the vehicle for the change that drives him and his

company. Following a bout with depression in 2011, Williams went to

rehab at the Crosby Center in San Diego. He also continued to

use cannabis to help him heal, both physically and psycholog-

ically. “Cannabis has really helped me especially when it comes to

the mental side,” he says. “It’s safe and effec-tive. After going through the Crosby Center, I began to do more research on the use of cannabis. Living in California, cannabis was readily available. So you were exposed to it more. That is when I started to use it more as a medication.”

If you think of your brain as having a traffic signal, Williams says that for many years his brain never saw a yellow light. “Cannabis helps you obtain that yellow light before the red, and that’s one way it has helped me,” he says. “Athletes, and likewise, people in the military have been trained to react immedi-ately and not think and process something. Once your playing days are over or you are out

of the service, you still have those traits ingrained in you and

you need to relearn. That’s why cannabis is beneficial — it

helps with that part of the brain that process every day life.” Williams has joined a growing number of former NFL play-

ers, such as Jake Plummer, Nate Jackson, Lorenzo Neal, Marvin

Washington, and Ricky Williams, who have become vocal

advocates for cannabis use among players. They are campaign-

ing for the NFL to eliminate its ban on medical marijuana as

well as to further study its efficacy for helping players recover
