Box 14084 Seattle, WA 98114-0084 2016 HONOREE NOMINATION FORM Honoree Criteria MANDATORY CRITERIA Required for consideration AUXILLARY CRITERIAPROCESS TIMELINE AND CONFIRMATIONS The Robert Chinn Foundation or representative will confirm receipt via e-mail of any submitted nomination forms.The honoree selection process will be ongoing throughout the fall as candidates are contacted.The Robert Chinn Foun-dation will advise the nominator of the outcome of his/her nomination either prior to contacting the nominated honoree or at the conclusion of the recruitment process. 1.ActionsofthenomineemustpositivelyreflectRobertChinnFoundationvaluesandhaveimproved,ina positive manner, the nations civic, cultural, educational quality of life for all Asian Pacific Americans while contributing to the American experience. 2.AUnitedStatescitizenofAsianorPacificIslanderdescent.Thoseofmixedheritageareeligibletobe nominated. 3.Demonstrated an historic achievement or precedent in the experiences of Asian Pacific Americans. 4.Demonstrated significant achievement in the field of which they have been honored. 5.Selection is irrespective of future, current or past contributions to the Robert Chinn Foundation. 6.Honorees must be available to be honored at the Asian Hall of Fame awards in May 2016. 7.In case of posthumous awards, close family members must be available to accept the Asian Hall of Fame award on the honorees behalf. 1.Honorees must serve to do public relations, projects and visits benefitting the community, the Asian Hall of Fame, and the Robert Chinn Foundation. 2.Honoreesagreetogiverightstotheirimagesandlikenessesforthepurposesofbeinghonoredatthe Asian Hall of Fame. 1.Inputonnomineeswillbetakenfromthecommunityat-large,staff,RobertChinnFoundationBoardand those advising the Asian Hall of Fame Committee. 2.Staff, with cooperation of volunteers or the Asian Hall of Fame Committee, will then compile candidates to be selected by the Robert Chinn Foundation Board of Directors.3.The Robert Chinn Foundation Board of Directors selection is final.
