Struck By: February 8, 2016

Three employees were changing signage on several off ramp billboards, when finished and loading tools a semi-truck exited the expressway and hit the back of an arrow board striking two of the three employees. One sustained injuries and was treated and released from the hospital, the other received fatal injuries.

An operator of an excavator was clearing trees from a drainage ditch to level and install a drain pipe. The operator was trying to bring down a tree that had been topped off on March 14, 2016 by a tree service company. A tree fell on to the excavator, crushing the cab and killing the operator.

Fall: May 6, 2016Employees were performing roofing tear off activities and had finished a section of roof and were cleaning up the debris. While pushing a wheelbarrow full of debris an employee fell through a section of the concrete roof deck and landed on the floor of an industrial plant. The employee fell approximately 27 feet and died from the injuries he sustained.

Fall: April 23, 2016Owner/Roofer fell approximately 30 feet from a pallet elevated by a rough terrain forklift while installing an ice guard on a roof.  The forklift began to tip and the decedent jumped to the roof but was unable to secure himself and slipped off of the roof, falling 30 feet to the dirt below.

Electrocution: July 22, 2016

A 35-year-old laborer was performing arc welding on a private home-owner's metal dock. It appears that the laborer accidentally dropped the welding terminal lead onto his chest. EMS arrived, performed CPR, and transported him to the hospital, where he was later pronounced dead.

Fall: August 10, 2016A roofing contractor employee, near completion of a two week project, was working outside of the warning line. The accident victim was the foreman for the project and had not assigned any of the other employees as a designated safety monitor that day. The employee fell approximately 17’ to the concrete area below, surviving initially, but died later that day from the injuries he sustained.

Fall: August 16, 2016Two employees were installing an exterior light fixture 24’ above the ground. They were standing on two extension ladders that were placed on a mobile scaffold platform. The two employees fell to the ground when the mobile scaffold platform and ladders fell away from the building. The employee/owner died from the injuries he sustained when he landed on the concrete below. The other employee landed on sandy ground, received only a broken arm.

An employee installing elevator equipment was working from an extension ladder across the hoist shaft. The employee was relocating the ladder to allow rigging to be installed when the ladder and employee fell 22 feet to the ground floor level. The employee died from the injuries he received from the fall.

Electrocution: September 6, 2016

An employee engaged in gutter installation on a residential home was found slumped over a running generator, inside the company box truck. The employee was pronounced dead at the scene. The autopsy report indicated the cause of death was electrocution.

Employees had constructed a job made wooden window jack scaffold on the residential job site. The wooden scaffold was constructed 20 foot above the ground and collapsed when two employees were performing soffit installation. One employee received a broken tail bone and hip, the other employee died from the injuries received in the 20 foot fall to the ground level.

Electrocution: September 15, 2016

An employee was using a metallic bull float during installation of new concrete flatwork at a residential project. The employee contacted an energized 7200 volt overhead electric line with a twenty-nine foot long aluminum handle and was electrocuted. The employee later died from the electrical shock he received.

Firefighters lowered the employee (roofer) to the ground after he was found unconscious on the roof of an apartment building. The employee was taken to a local hospital where he died.
