Patient-centred care

Primary care patient pathways: Community Pharmacy Services (Minor Ailments, MURs and NMS, Referrals to GPs)Working with GP practices, Medicines optimisation in Care Homes

Hospital patient pathways: Pre-admission clinics, hospital admissions, pharmaceutical care on the wards, discharge planning and processes
