Camper’s GradeChildren must be entering the grade of the camp by 9/1/15.

Fees and DepositsResident and nonresident fees are listed for each camp. A $50 or $100 nontransferable, nonrefundable deposit is due at registration. This deposit is applicable to tuition. Camp fees can not be prorated to accommodate vacations. Please see individual camp de-scriptions for deposit amount.

Camp Payment OptionA camp payment plan is again being o� ered for the 2015 camp season. Your camp fees can now be paid in fi ve (5) equal monthly payments through an automatic installment billing process.

Installment billing must be set up as an au-tomatic charge/debit to a credit card or from a bank account. All automatic (or electronic) account withdrawals (ACH) and credit card charges will be processed on the 15th day of Feb., March, April, May and June of 2015.

Please be aware that this is the only payment plan that will be o� ered for camp fees. All camp fees must be paid in full prior to the start of the fi rst day of camp for participants to attend camp.

Camp Sites

Summer camp locations listed in this brochure may change due to Deerfi eld pub-lic school scheduling. We will advise you of changes should they occur.

Parent Manual Parent manuals will be mailed the fi rst week of April. Inside you will fi nd a health history form, carpool and buddy list information as well as information about the camp your child will be attending.

Buddy RequestsInformation about buddy requests can be found in the day camp parent manual, due to be mailed the fi rst week of April. There are no buddy requests for Sports Camps.

Camp RegistrationSummer camp registration was held in January. If you wish, you may send in your camp registration forms and fees when you register for winter programs in November. Camp confi rmation will be sent after the January lottery. For details about the lottery registration procedures, please see the Lottery/Registration Information pages at the back of this brochure.

Registration ProcessWhen registering for our Day Camps, please use the regular Park District registration form. Registration for our general winter programs is in December. Lottery for day camp registration was in January.

You must include a camp deposit or total payment with your registration form. Your full payment for all sessions of all camps is due on April 24, 2015.

Camp Refund PolicyOne week prior to the start of camp, you may receive a refund on a camp fee less than your $50 or $100 non-refundable, non-transferable deposit. Once camp begins, refunds will be granted only for medical reasons or if you are moving outside of the park district. In these cases, the $50 or $100 deposit will be deducted and the refund will be prorated based on when participation ends.

If your child is unable to continue in a program because of a medical problem, you must submit a letter from your doctor on his/her letterhead in order to receive a refund. If you are moving out of the district, in order to obtain a refund, you must submit proof in writing that you are moving.

Big Camp - #424001So Big Camp is a wonderful transition from home to a day camp setting. The experi-ence helps young children make an easy adjustment to preschool activities o� ered by the park district. Our goals are to expose children to many new experiences as well as help develop their social skills. Children will learn new songs, crafts, and games. A small snack and beverage is provided daily. At So Big Camp, a child will get to know children in the community and develop lifelong friendships.

Our wonderful sta� will make camp a warm and exciting place to spend a summer. Please bring a sack lunch and drink for your camper each day.

Sun ‘N’ Fun - #424002Sun ‘N’ Fun features lots of outdoor play, including sand and water activities, outdoor games and crafts and trips to the wading pool in a low-key setting. We emphasize the social, emotional and physical development of our young campers. A snack and bever-age will be provided each day. Our day will end with a sack lunch and singing. Trained, experienced leaders will guide your young-sters through this sun and fun-fi lled Sun ‘N’ Fun experience. Please send your child with a lunch each day.

Little Scampers - #424003This camp has a low-key, noncompetitive yet structured atmosphere geared to the needs, interests and skills of the young child. Pathfinders Camp - #454001A relaxed, noncompetitive atmosphere allows campers to be among their peers doing activities geared to their interests and skill levels. Small group ratios are main-tained as Pathfi nders participate in athletics and recreational games, art and more. Swim lessons are a part of each camp day. Trailblazers Camp - #454002Children will love this noncompetitive camp designed just for third graders. Campers participate in activities from athletics and rec-reational games to art and dramatics. Art Sampler Camp - #454007New 5-Day option and new start time!

Art Camp is designed specifi cally for the creative child who enjoys all aspects of art. This camp will expose young artists to a va-riety of media including painting, cartooning, design, and ceramics. Campers have daily free swim Monday - Thursday. Explorers - #454003Each day begins with an all-camp gathering. Each day ends with an all camp event. In between each camper will eat lunch and go to the pool for open swim. The remainder of the day designed to allow each explorer to choose what activities they want and how long they want to do them. Counsel-ors supervise all activities and all areas so campers can move between each activity at camp at their pace. Explorers develops an atmosphere of acceptance and pride in the camp and creates a balance of free choice geared toward the interests of the older el-ementary school child. From sports, games, arts, crafts, acting and improv, each camp-er’s daily activities will be designed by each individual child with the sta� supporting kids in whichever way they need to connect with each other throughout the dozens of daily activities. Wherever campers turn, they will fi nd a helping hand. Special events, theme weeks, and weekly fi eld trips are among the activities planned. Please bring a water bottle to camp with your child’s name on it.

Counselors-in-Training (C.I.T.) - #454008The purpose of this program is to provide an opportunity for teens in this age group to develop leadership skills through a summer camp experience. Duties will include assist-ing with games, art projects, and leading an activity of his/her choice. C.I.T.s will be contacted in April to determine their camp and site preferences. Every e� ort will be made to place C.I.T.s at their preferred site, however, this can not be guaranteed. Man-datory all-sta� training takes place the week of June 15. All C.I.T.s must attend. Transpor-tation is the responsibility of the C.I.T.

SPORTS CAMPS

These camps are designed for those inter-ested in participating in competitive summer sports including softball, soccer, football, kickball, dodgeball, basketball and fl oor hockey. Camps include fi eld trips and free swim (not lessons). Rookies Camp, however, does include swim lessons. Competitive sports are a part of each camp, but the focus will vary by age group; i.e., Rookies focus-es more on fundamentals, Middle School Camps focus more on fi ne tuning skills and strategies and competing in tournaments, and the other camps fall in between to vary-ing degrees.

To keep groups competitive, no buddy requests are accepted except for Rookies Camp. Groups at other sports camps are put together by counselors and based on athletic ability. Note: Campers are in their groups between two and three hours per day and can spend the rest of the day with their friends.

BUS SERVICE OPTION - FOR RESIDENTS ONLY

Bus service is available for the following camps.

» Trailblazers » Explorers » Sports Camp, excluding Tennis Camp

and All Star Sports Camps

Some children could be on the bus up to 45 minutes each way. We will do our best to plan routes so either the morning or afternoon route will be shorter than the other for participants. However, we cannot guarantee this depending on the addresses of the children who register for this service.

Parents are encouraged to accompany their child(ren) to the bus stop and also to meet their child(ren) at the stop when camp is over.

Bus stop location information will be mailed in early June to those who register for this service.

Important: Those interested in the bus option must register for it by 12:00 PM on Saturday, May 16, 2015.

No refunds will be issued for bus service after June 2 for the fi rst session and after June 20 for the second session.

Children who register for the bus transportation option will be picked up each morning at convenient stops within park district boundaries only. Depending on the number of children registered for this service and the route the bus follows, pickup time may begin 45 minutes to one hour before the starting time of the camp program. Once the last child is picked up on the route, the children will be delivered to their camp locations.

Campers will be returned to the same stop after camp is over. Pickup and return times will vary based on the bus routes.

NOTE: If you move residences between the time you register and the time camp begins, please notify the Park District.

EXTENDED CAMP CARE - #454009

Parents may choose morning Extended Camp Care hours, afternoon hours, or both. Children in the morning session will be transported from the Jewett Park Commu-nity Center to their camp site. Those in the afternoon session will be picked up at their camp sites and dropped o� at the commu-nity center. To register, please fi ll out the registration form and include full payment (in addition to camp deposits) with your form. All participants must attend a Deerfi eld Park District day camp listed in this brochure.

Extra Added Attraction - #454010Two additional WEEKS of August Activities!

Designed to help working parents, Extra Added Attraction extends our regular camp season. The camper’s day will include open swim, arts and crafts, athletic and recre-ational games. Special theme days and fi eld trips are among the other fun-fi lled activities planned. Parents may not drop o� and leave children unsupervised before 7:30 am. All children must be picked up by 6:00 pm. Please send your child with a drink and a sack lunch.

SPECIALTY SPORTS CAMPS

Tennis Camps 2015Dana Joseph and Chris Cabanban of First Serve Management and their team of in-structors provide tennis services indoors at Sachs Recreation Center (SRC) and outdoors at Mitchell Park. First Serve’s mission is to nurture the love of the game of tennis. Family-friendly programming encourages students to play the sport for a lifetime and provides a path to pursue the level they wish to attain. From beginning recreational play-ers looking for fi tness and fun to tournament competitors, instructors understand people play tennis for di� erent reasons. The quality and experience of their teaching distinguish-es First Serve Management from their peers. Their entire sta� is certifi ed by either the USPTA or the USPTR and is constant from the start of a lesson session to the end.

First Serve Half Day Tennis Plus Camp - #434060A smash hit! Campers get 10 hours of on-court tennis instruction per week and enjoy energetic o� -court activities such as swim-ming, basketball, volleyball and much more. Increase your self-confi dence in tennis and other areas. Camp is at Mitchell Park and will also utilize indoor space at SRC. Instructor to camp ratio is 1:8.

Hoops Education Basketball Camp – #494015

Presented by David Adelman & Play Hard HoopsIn its third year, this Co-ed camp is dedicat-ed to teaching the game of basketball while stressing sportsmanship, teamwork, en-thusiasm and a positive attitude on and o� the court. The goal is to have a great time regardless of ability while learning skills and fundamentals to be a good player. Camp consists of stations, contests, game play and the ability to learn from coaches with col-lege and pro experience. Fee includes camp T-shirt, prizes and individual team awards.

GENERAL WAIVER & RELEASE - IMPORTANT INFORMATIONThe Deerfi eld Park District is committed to conducting its recreation programs and activities in a safe manner and holds the safety of participants in high regard. The Deerfi eld Park District continually strives to reduce such risks and insists that all participants follow safety rules and instructions that are designed to protect the participants’ safety. However, participants and parents/guardians of minors registering for this program/activity must recognize that there is an inherent risk of injury when choosing to participate in recreational activities. You are solely responsible for determining if you or your minor child/ward are physically fi t and/or skilled for the activities contemplated by this agreement. It is always advisable, especially if the participant is pregnant, disabled in any way, or recently suff ered an illness, injury or impairment, to consult a physician before undertaking any physical activity.

WARNING OF RISKDespite careful and proper preparation, instruction, medical advice, conditioning and equipment, there is still a risk of serious injury when participating in any recreational activity/program. Understandably, not all hazards and dangers can be foreseen. Participants must understand that certain risks, dangers and injuries due to acts of God, inclement weather, slipping, falling, equipment failure, in supervision, premises defects and all other circumstances inherent to recreational activities/programs exist. In this regard, it must be recognized that it is impossible for the Deerfi eld Park District to guarantee absolute safety.

WAIVER AND RELEASE OF ALL CLAIMS AND ASSUMPTION OF RISKPlease read this form carefully and be aware that in signing up and participating in this program/activity, you will be expressly assuming the risk and legal liability and waiving and releasing all claims for injuries, damages or loss which you or your minor child/ward might sustain as a result of participating in any and all activities connected with and associated with this program/activity (including transportation services, when provided). I recognize and acknowledge that there are certain risks of physical injury to participants in this program/activity, and I voluntarily agree to assume the full risk of any and all injuries, damages or loss, regardless of severity, that my minor child/ward or I may sustain as a result of said participation. In the event of an emergency, I authorize District offi cials to secure from any licensed hospital physician, and/or medical personnel any treatment deemed necessary for me or my minor child’s immediate care. Participants and parents/guardians of minor participants are solely responsible for any and all expenses associated with emergency medical treatment, including but not limited to, transportation services to the nearest available medical facility/provider or to an alternative medical facility/provider requested by the participant or parent/guardian. I further agree to waive and relinquish all claims I or my minor child/ward may have (or accrue to me or my child/ward) as a result of participating in this program/activity against the Deerfi eld Park District, including its offi cials, agents, volunteers, and employees (hereinafter collectively referred to as “Deerfi eld Park District”). I do hereby fully release and forever discharge the Deerfi eld Park District from any and all claims for injuries, damages or loss that my minor child/ward or I may have or which may accrue to me or my minor child/ward, arising out of, connected with, or in any way associated with this program/activity.
