Types of Gifts
One-Time Gifts of Cash These gifts are available immediately for use by Community VNA and are tax deductible to the fullest extent allowed by law for donors who itemize their federal income taxes.
Recurring Gifts of Cash A recurring gift is also available immediately for use by Community VNA and provides ongoing support with a scheduled monthly or quarterly gift. To set up a recurring gift simply visit our secure online giving form or call 508.222.0118, ext. 1303 or 800.220.0110.
Gifts of Securities You may enjoy substantial tax savings by transferring gifts of appreciated securities or mutual funds to Community VNA. Gifts of long-term appreciated assets (securities owned for

What is community?
The Oxford English Dictionary defines community as “A feeling of fellowship with others, as a result of sharing common attitudes, interests, and goals.” For more than 100 years, Community VNA has fostered a fellowship with the residents of Attleboro and the surrounding region by focusing on compassionate caregiving and meeting the changing health needs of the population.
As we review 2015, we continue to honor our founding mission by providing health care in the home while diversifying our service offerings to address unmet needs in the community. One new program that arose out of the community’s need is our Palliative Care Consultative Service (see pages 10 & 11). The goal of this program is to improve a patient’s quality of life during a chronic, progressive illness by addressing symptoms such as pain, anxiety or fatigue.
The addition of palliative care, along with the growth of our other programs over the past year, closely aligns with predictions about the nation’s increasing need for home health care. According to the Alliance for Home Health Quality and Innovation, 10% of all Medicare Beneficiaries in Massachusetts use home health. Of these patients, 89.3% have three or more chronic conditions. Clearly, the need for home health care is evident and is growing.
There are many studies that attest to the fact that patients heal better at home in their own environment, in the community. One such study examined the recovery of stroke patients. According to the journal, Stroke, “…patients who rehabilitated at home after only 10 days in the hospital had a faster recovery and, after three months, were farther advanced in assimilating back into their familiar surrounding than those who weren’t sent home.”

In 1911, community leaders of Attleboro were becoming concerned about public health in the face of threatening communicable diseases. A community campaign was launched to raise funds to establish the Attleboro Anti-Tuberculosis Society. They recruited a trained nurse from Providence and created a “district nursing service” (home visiting nurses) to care for the community’s most vulnerable members, including the elderly, the disabled and the growing number of low wage workers and their families who increasingly were drawn to Attleboro’s early manufacturing enterprises.
Known today as Community VNA, the Anti-Tuberculosis Society was the first organized healthcare service in Attleboro, pre-dating the local community hospital, Sturdy Memorial, by two years.
The little district nursing agency grew and flourished over the decades. In the 1960s and 70s, in order to meet more rigorous requirements under newly enacted Medicare legislation, a number of area towns merged their town visiting nurse services into Attleboro’s larger Community VNA, to create the region-wide, non-profit home health and hospice agency we know today.
In 1983, Hospice Care was formally added to Community VNA to provide skilled and compassionate end of life care to those in need. In 1984, a non-profit sister agency, HealthCare Options, was created to provide other types of community-based health services and care. Today this includes Help at Home care, health and wellness education, public health nursing, worksite flu clinics, and adult day health centers located in Norwood, Mansfield and Taunton.
In 1984 a non-profit parent organization, Community Health Systems, was established to coordinate and oversee the two non-profit subsidiaries, Community VNA and HealthCare Options.

Community VNA and its affiliates provide services and resources that enhance the community’s capacity to achieve optimal health, wellness and quality of life.
Community VNA includes the following:

Skilled nursing and rehabilitation care, certified by Medicare
Community VNA Hospice & Palliative Care
Hospice and Palliative Care are specialized programs that provide advanced care for individuals and their families who are facing any type of serious, life-limiting illness.
Community VNA Private Care
Private Care, or Help at Home, is the extra assistance you prefer to help you or your loved ones to live more safely and comfortably at home, thanks to our wide range of personal care and in-home services.
Adult Day Health CentersSupport during the day for frail elders and disabled adults is offered at our Centers located in Mansfield, MA; Norwood, MA; and, Taunton, MA.
In 2015 Community VNA once again was awarded HomeCare Elite Status by the National Research Corporation for placing in the top 25% of home care agencies. This national survey, conducted by an independent research firm, awards this Elite Status based on quality of care, patient experience, and financial management.
Additionally, 2015 was the 3rd consecutive year Community VNA received the prestigious Home Health Care CAHPS Award for having demonstrated superior performance in overall patient satisfaction results.

August 2, 2015: The Runaways Running Club hosted its annual 5K benefiting Community VNA. Thanks to this generous community support, we are able to provide services and supports to all those needing our Hospice Care. With proceeds from 2015, the Run has generated over $50,000 for Community VNA Hospice and Palliative Care over the past 13 years.
The courtyard at our Mansfield Adult Day Health Center, located at 300 Branch Street is a bit greener, having benefited from the gardening expertise of three Sensata Technologies engineers on a sunny Friday in late August. Participating in The United Way of Greater Attleboro/Taunton Day of Caring were Jacob Supron, Zenun Qi, and Nik Lal who spent several hours planting, weeding and refreshing mulch in the garden area of the enclosed courtyard at the adult day health center.

Left: Community VNA Hospice and Palliative Care hosted our annual Tree of Life and Remembrance Ceremony on December 6, 2015. The theme, Gifts of Winter, featured a quiet, reflective, gentle atmosphere, with thoughtful readings and music.
For some, such as the Andrews family pictured here, this is an annual ritual which has become a strong part of their personal holiday tradition. For others, it was the first time they joined us, having experienced a recent loss of a loved one. Many commented on the peaceful opportunity to remember those who have passed. Placing stars on the tree, reflecting the names of their family and friends who have passed, is an important part of the ceremony.

With over 50 wreaths to choose from, there was a design for every taste, in a variety of colors which were generously donated by local businesses and community organizations. The event featured live music and generous refreshments as a compliment to the wreath silent auction. Community VNA helps us to be there for those who need us the most and helps us bridge the gap for those who
either lack insurance or are lacking the necessary health benefit coverage. Other times, what we may receive in
reimbursements from insurance companies does not cover the true cost of providing such high-quality,
compassionate care. In addition, your gifts help to continue to offer free health screenings, a variety of community
health education programs, and critically needed caregiver support and respite.

New Program:Palliative Care
In early 1900, the average life expectancy in the U.S. was 47 years and most deaths were due to a sudden and acute illness. In the years since, life expectancy has increased to 77 years (a 64% increase). The majority of deaths are now due to chronic and progressive illnesses and tend to occur after a slow decline in function. People are living longer, but often with multiple medical conditions, related symptoms such as pain, fatigue or depression, and the challenge of complex medical decisions regarding treatments, advanced directives and goals of care.
Palliative Care is a medical subspecialty that has evolved in response to this changing demographic that incorporates hospice, but can be offered at any time to patients and families who are dealing with a serious illness. It is an added layer of support that can be integrated into the plan of care along with active medical treatment. It aims for achieving the best quality of life possible at any phase of a disease. Initially, palliative care programs were centered in hospitals. The current initiative is to expand palliative care into the community to improve access across all health care settings.
Community VNA has initiated a new palliative care consultation service and hired a nurse practitioner to work collaboratively with their home care and hospice teams, primary care providers and additional specialists, to provide palliative care to patients and caregivers who are living at home, in nursing homes, as well as in assisted living facilities.

By the Numbers
Our medical model adult day health centers are all certified by the Department of Public Health.
They provide care and socialization to hundreds of clients in our service area.
Most of private care service hours are provided by certified home health aides and homemakers.

How can we identify patients who may benefit from a palliative care consultation? There are patients who are often experiencing recurrent hospitalizations and emergency room visits due to acute exacerbations of chronic diseases. Some of these diseases can be cancer, heart failure, advanced COPD with oxygen dependency, neurological disease, end stage renal disease, or multiple chronic conditions.
A patient may also be experiencing uncontrolled physical symptoms such as pain, dyspnea, anxiety and/or depression, nausea, agitation, weight loss, fatigue, or functional decline. We can help by providing a comprehensive assessment considering the patient’s and family’s wishes, and maximizing the quality of life at any stage of a progressive illness.

Families who are caring at home for loved ones who may be experiencing a range of issues, from minor memory loss to full dementia, can turn to Community VNA for help. We are a compassionate agency that is skilled and highly experienced in caring for individuals with these special needs. At Community VNA, our specially trained and understanding staff is experienced in providing the extra support needed by caregivers to care for their loved one and to learn more about the caregiver skills they need.

Skilled Home CareHaving professional home health care in your home will help you or your loved one to recuperate faster and more completely. You will also enjoy the comfort and security of being at home. Our care team includes registered nurses, home care aides, social workers and rehabilitative therapists. These highly trained specialists provide our patients with a variety of clinical services. This team works hand-in-hand with the patient and family caregivers, the physician, and our other clinical staff to ensure we produce superior patient satisfaction and the highest-quality health outcomes.
Our specially trained and experienced homecare registered nurses (RNs) provide skilled nursing care which includes patient assessment, patient and caregiver education, medication administration and training, and skilled care and treatments per your doctor’s orders. Our telehealth monitoring system helps our patients learn to manage their chronic illness to achieve and maintain good health.
Additionally, we provide professional nursing for acute and chronic diseases; physical, occupational and speech therapy; wound care, IV therapy, pain and symptom management, medical social services, and more.
The Elder Dental Program is a community-based, non-profit program dedicated to helping low-income adults, aged sixty and older, access quality dental care at greatly reduced costs. The program relies on a network of local dentists who volunteer by donating the costs of providing care to these seniors including their time, equipment, office space, and staff.
The Elder Dental Program advocates for greater access to quality, affordable oral health care for individuals of all ages. We sponsor free dental screening clinics for anyone age sixty or over — regardless of financial status — and host these screening events throughout our Elder Dental program service area.

For those who need a little more help at home, Community VNA offers Private Care services. Home care aide and CNA services include: personal care, bathing, dressing, and transportation. Homemakers help with laundry, light housekeeping, meal preparation, shopping and errands. Companions are available for walks, conversation, reading, games, letter writing, and other needs. Nurses and therapists help with medication management, reminders and administration; medical care for most illnesses; as well as, physical and other therapies.
What differentiates us from our competitors? Because our Private Care is affiliated with our VNA, our staff are especially vigilant to report an exacerbation of symptoms that may need medical intervention.
