A Message from Dr. Michael D. ShonrockPresident, Lindenwood University

Fellow Lions,

I am delighted you have chosen to join me in my first year at this outstanding university. This will be an exciting year for Lindenwood athletics in the field of play, as well as in the classroom.

With outstanding coaches and some of the best facilities in the nation, I know that we can perform at a high level in our competitions. Also, our dedicated faculty and our academic success programs for student-athletes will help you continue Lindenwood’s tradition of outstanding academic achievement.

Now it’s up to you. You will not be alone. Our entire Athletics Department is behind you, as are my wife Karen and I. You will see us a lot, cheering you on from the sidelines.

Those of you who are returning have set the bar high for the incoming freshman, but I trust they are up to the challenge this year, academically and athletically. It’s a great day to be at Lindenwood.

Go Lions!

Michael D. Shonrock, PhDPresident, Lindenwood University

Message from the Director of Student Life Sports

Lindenwood University has a rich history and tradition of combining academic and athletic success spanning 187 years. It is important that we do not lose sight of the things that have not only made Lindenwood athletics special, but made a difference in the lives of countless young men and women. Student Life Sports and Competitive Programs (SLS) has always been the cornerstone of success for Lindenwood Athletics with over 50 team National Championships and close to 100 individual national champions.

Our commitment to the concept of the “student-athlete” is resolute. Lindenwood offers nationally recognized academic programs, personal attention, outstanding coaches and outstanding facilities. Our coaches’ concern for the total development of each student-athlete is evident from the first day they walk on campus to the day they graduate and beyond. Our student-athletes are involved in a variety of campus activities and service opportunities that add diversity to their college experience.

We are all competitive people who want to win, and certainly winning and contending for championships is important at Lindenwood. But, we never compromise the integrity and principles of sportsmanship in order to win. The development of character and strong personal habits for our student-athletes is the most important aspect of their athletic experience. There is a great deal more to being a champion than winning a championship.

All students who participate in the SLS programs at Lindenwood are required to fully comply with all policies which apply to all students in admission, curriculum, degree requirements, class attendance, personal conduct and other matters.

All athletic contests, practices, and related activities will be conducted in compliance with the principles of fair play and amateur athletic competition, as defined by the University. All rules of the University and other conferences/organizations in which Lindenwood has membership, will be strictly observed and enforced.

Our goal is to create an atmosphere for all student-athletes to be able to participate in activities that they are passionate about while pursuant of their post-secondary education degrees. We pride ourselves on creating “Lions for Life” and hope Lindenwood will become a place where you will build bonds for life.

I trust that you will take the time to read this policy manual thoroughly as it outlines the policies and procedures pertinent to all Lindenwood student-athletes. You are responsible for knowing these rules and abiding by them. Should you have any questions, please do not hesitate to contact me.

Best wishes for a successful season, both on and off the range, court, table, rink, lane, pool, road, studio, mat, or field.

Mike Elam,Director of Student Life Sports

Athletics Department Mission StatementIt is the mission of intercollegiate sports at Lindenwood University to advance learning, enliven campus life, develop leadership skills, and foster the personal growth of the student athlete through offering a high quality athletics program. Athletics Department personnel strive to provide model programs that develop meaningful standards of scholarship, athletic performance, leadership, community service, and sportsmanship conduct. Through sports, Lindenwood seeks to contribute to a sense of collegiate identity and positive perception in the community. The Athletics Department shall strive for excellence in the competitive environment in accord with the philosophy, rules, and regulations of the sports’ governing bodies in which Lindenwood has membership. The University is committed to sponsoring and promoting a gender equitable sports program where the physical, emotional, and social well-being of the student-athlete is of primary concern.

Lindenwood Athletics PhilosophyLindenwood University is committed to the development of the whole person within a value system that acknowledges our Christian heritage and focuses on a four-dimensional personal growth: mental, social, physical and spiritual.

The actions of the student-athlete affect both himself/herself and others. In choosing to be a part of the Lindenwood learning community, the student-athlete agrees to be responsible and to choose his/her actions in such a way that they reflect favorably on the student-athlete and the Lindenwood community.

Champions of CharacterThe Champions of Character program was designed and implemented in 2000. The primary objective of this program is to educate student-athletes and instill core values that aid in the development and establishment of essential character traits. The five core values are:

RESPECT“Those associated with sport show respect by treating themselves, other persons, institutions and their sport according to the highest standards of conduct. It implies civilized and gracious behavior to players, coaches, fans and parents.”

Page 10: 2015-2016 Lindenwood University Student-Athlete Guidebook ...

10

Champions of Character (continued)

RESPONSIBILITY“Athletes show responsibility by making academic progress toward graduation their top priority. They solve problems rather than make excuses and are reliable team players. Students should be accountable for their actions and decisions, and coaches and administrators should maintain high standards of competence and conduct.”

INTEGRITY“For student competitors and their coaches, integrity means keeping commitments and conducting honest behavior. Student-athletes must know and understand the Champions of Character Student-Athlete Pledge.”

SERVANT LEADERSHIP“This core value refers to putting the group first and becoming responsible for personal and group roles while performing at your best. Students demonstrating servant leadership have a primary purpose of serving others while striving to become a personal and team leader. The servant-leader provides a critical service to society and the great gift of good example.”

SPORTSMANSHIP“The conduct of educational sports according to the highest standards is our expectation for sportsmanship. Administrators, coaches, students, fans and parents are expected to act correctly even when others do not, and demonstrate fairness and equity in all contests and relationships.”

The Champions of Character program educates student-athletes on these values through various presentations and achieves its objectives through the commitments of student-athletes. All student-athletes are required to sign and adhere to the Champions of Character Student-Athlete Pledge which symbolizes a commitment to exhibiting the five core values when representing the University on the court and/or field as well as the classroom and the community.

Academic and Student Services

REGISTRATIONEach student-athlete is assigned an academic advisor in his/her academic discipline to assist with the registration process and provide guidance in your career direction. You should plan to meet with your advisor at least once a semester. In preparation for meeting with your advisor regarding your class schedule, the following key tips will aid you in a productive and successful registration process.

1. Plan Ahead. If you plan to register for classes on a certain date, contact your academic advisor at least two weeks in advance to meet with him/her regarding your schedule. Only your advisor can open your portal for on-line registration.

2. Come Prepared. Review your degree requirements in advance and have an outline of the courses that you plan to take prior to meeting with your academic advisor to expedite you meeting.

3. Communicate. Prior to meeting with your academic advisor, speak with your coach and identify any concerns regarding taking courses during season and missing class. If you have any concerns regarding your eligibility status, contact the Assistant Director of Student Life Sports (Jason O’Dell).

STUDENT ACADEMIC SUPPORT SERVICESLindenwood University complies with the Americans with Disabilities Act by making our programs and services accessible to students with disabilities. Student Academic Support Services ensures that these students receive assistance by providing auxiliary aids and accommodations. We strive to give students and equal opportunity to succeed in earning their degrees.

Accommodations are provided to qualified students with a variety of disabilities, including, but not limited to, the following: attention deficit disorder (ADD), learning disabilities, mobility impairment, hearing loss, and visual impairment. Students with temporary physical, sensory, medical, psychological, or other temporary impairments may receive services on a short-term basis.

Reasonable accommodations are provided on an individual basis and are determined based on submitted documentation as well as discussions with students. Accommodations will reflect the diagnosis provided on your documentation. Some accommodations include assistance with note taking, double time on tests, private testing areas, tutoring, extended time on writing assignments, and preferred seating. Lindenwood also provides accommodations for those with mobility disabilities. Because our campus does have classroom buildings that may not fully accommodate students with mobility issues due to the age of the buildings, we will move classes for these students to rooms in an accessible building.

In order to receive accommodations, students need to follow the appropriate process established by the Student Support and Disability Coordinator. The student needs to contact the Student Support and Accessibility Coordinator, Jared Conner, at (636) 949-4510 or by email at [email protected]. Next, the student will be asked to provide appropriate documentation with diagnosis. The coordinator and student will discuss proper accommodations based on the documentation. Once accommodations are decided, the coordinator will give the student Campus Accessibility Faculty Notification Forms to give to their professors. The student and professors will then discuss how to arrange the accommodations.

14

Nine-Hour RuleAll student-athletes, including transfers, are required to earn a minimum of nine semester credit hours in the previous full-time term of attendance to be eligible for competition in the next semester. This rule does not apply to graduate students and students seeking a second bachelor’s degree. If a student-athlete is in the final year of his/her degree program, the nine hours may be acceptable toward any of the institution’s degree programs.

24-Hour RulePrior to each fall semester, a student-athlete will be certified based on earning 24 credit hours during the last academic year. Of those 24 credit hours, 18 of those must be earned during the previous fall, J-Term, or spring terms.

Designation of Degree Program and Progress Toward DegreeDuring the first two years of enrollment, a student-athlete can use credits acceptable towards ANY degree program. After a student-athlete’s fourth semester of full-time enrollment, he or she must have declared a specific degree program (major) prior to the beginning of the next semester. It is the student-athlete’s responsibility to declare a major no later than the completion of the fourth semester of full time enrollment. A Designation of Degree Form must be filed with the Registrar’s Office. A student-athlete may view the requirements of his/her declared major(s) by accessing the Program Planning Worksheet in Lindenwood University’s catalog. The Registrar must approve and document all changes of degree program. Beginning with the student-athlete’s junior year, or fifth semester of full-time enrollment, at least 24 semester credits MUST count toward the student-athlete’s designated degree program. For questions regarding academic eligibility, contact the Compliance Office.

TransfersIf you transferred to Lindenwood from another institution, your eligibility is determined by your academic record in existence at the time of transfer. Once Academic Services receives an official transcript, it will be evaluated and added to the student-athlete’s Lindenwood transcript. If the University has already had an equivalency set for a specific course, then Academic Services will upload the equivalency.

Page 15: 2015-2016 Lindenwood University Student-Athlete Guidebook ...

15

Transfers (continued)If the transfer course does not have an equivalency set, the course will be sent to the department chair and he or she will make the determination of its equivalency and notify Academic Services. The student-athlete may be asked to provide documentation, such as course description or syllabi from the previous coursework. Transfer eligibility can vary. Contact the Compliance Office to discuss your specific academic record and how transfer rules apply to your specific situation.

TermsThe maximum numbers of terms (semesters) that a student-athlete can compete for one sport is determined by the governing body of the sport. Part-time terms/semesters do not count and the normal number is ten full time semesters.

SeasonsThe maximum number of seasons of competition for one sport is determined by the governing body of the sport. The normal number is four seasons, however some organizations allow additional years if the student-athlete is completing a master’s degree.

One-Time AppealStudent-athletes who fall below GPA and credit-hour requirements are authorized to write an appeal letter to the Vice President of Student Development, Dr. Ryan Guffey. While the appeal needs to be directed to Dr. Guffey, it should be sent to Mike Elam, Director of Student Life Sports, for review first. The appeal should contain an introduction and an explanation for why the academic standards as prescribed in the Student-Athlete Guidebook were not maintained. After consideration, the Director of Student Life Sports will notify the coach and the student-athlete with the appeal decision. Every student-athlete is given ONE opportunity for appeal during their career at Lindenwood University. Appeals are granted on an individual basis and are never guaranteed. Also, any institutional appeals are subject to the required academic standards of the governing body of each sport.

TutorsTutors are generally available in each academic division. Student-athletes should contact their advisor and/or professor to see if tutoring is available.

16

Student-Athlete Code of ConductThe University expects student-athletes to represent themselves and Lindenwood with the highest degree of respect and integrity. Student-athletes are considered to be valuable role models on the court, in the classroom, and within the community. As a result, certain expectations have been established in addition to the expectations of traditional students as outlined in the student handbook. Failure to meet these expectations may result in the dismissal of the student-athlete from the athletics program and/or the University.

AcademicsIn addition to maintaining the academic standards established by Lindenwood, all student-athletes are required to achieve a specific level of academic achievement approximately every four weeks throughout the semester.

Week FourIf the student-athlete is attending classes, but has six or more hours of “F”, they will be given the option of signing an Athletic Success Contract to continue to participate until mid-term grades are reviewed. This applies to both in season and out of season participation. Failure to sign an Athletic Success Contract: In season student-athletes will be allowed to practice, but not compete until mid-terms. Out of season student-athletes will not be allowed to participate in any practices until mid-terms. If the athlete is NOT attending classes and has six or more hours of “F”, both in season and out of season student-athletes will be ineligible to participate until mid-term grades are reviewed.

Mid-Term GradesIF out of season student-athletes have six or more hours of “F,” they will not be allowed to participate in out of season practice or conditioning for the rest of the semester. If in season student-athletes have six or more hours of “F,” they will be ineligible for participation, but can practice and attend team study halls. Coaches may choose to appeal for a student-athlete, based on academic progress (not athletic need) by asking the Director of Student Life Sports to request an updated from an instructor. The student-athlete may also ask his instructor to email the Director of Student Life Sports an updated grade. If the appeal has merit, the student-athlete may be allowed to remain part of the team until week 11.

Week 11If the student-athlete has NOT lived up to the agreement and still has failing grades, he/she will be ineligible to participate or practice but may attend team study halls. At the end of the semester, eligibility reverts back to Lindenwood eligibility rules.

Student-Athlete Policies

Drug Use and Testing PolicyAll student-athletes are required to follow the drug use and testing policy outlined in the student handbook. Use or possession of drugs, or alcoholic beverages by any student-athlete while on campus or at a university sponsored event is prohibited, and will result in an immediate suspension from their team, and will be subject to expulsion from the University.

Student-Athlete Drug Use and Testing PolicyLindenwood University is committed to the physical and mental well-being of its student-athletes. The University holds a strong belief that the use of drugs and alcohol has a negative effect on the academic and athletic performance of its student-athletes. This testing and education policy is designed to be in accompaniment to the NCAA Drug Testing Policy and University policy. This policy is intended to set a minimum standard by which sanctions are to be enforced. Additional team policies, University policies or past rules infractions may increase the severity of incurred penalties.

SelectionAll Lindenwood University student-athletes listed on a current NCAA squad lists are eligible for drug testing. Drug testing can be random, at the request of a coach, or if there is reasonable suspicion. For random drug testing, random numbers will be selected from a numbered roster. The student-athletes whose names correspond to these selected numbers will be tested.

In addition, the term reasonable suspicion means that information has been received by a member of the coaching staff, athletic training staff, staff personnel, or others, in good faith, from a reliable source, regarding the student-athlete’s use of banned substances. Furthermore, a student-athlete’s possession of, or use of a prohibited substance, arrest, or conviction related to the possession of, or use of or trafficking of banned substances, all rise to the level of reasonable suspicion.

Method of TestingStudent-Athletes will be informed about the drug testing program -- a copy of this program will be available to the student-athlete as part of the Student-Athlete Guidebook and is available on the Lindenwood University athletic website. Student-Athletes are required to annually sign a drug testing consent form. Student-Athletes may be inconvenienced during practices, meetings, strength and conditioning sessions, etc. The Student-Athlete may even be tested in off hour times such as early morning, meal times, and/or late evenings.

Page 18: 2015-2016 Lindenwood University Student-Athlete Guidebook ...

2015-16 Student-Athlete Guidebook and Planner

18

Method of Testing (continued)Tests will be primarily conducted for, but not limited to, drugs of abuse, such as, amphetamines, cannabinoids, cocaine, and other controlled substances. All other substances on the NCAA Banned Substance List may also be included in testing. Drug testing can consist of the collection of urine or hair samples from the student-athlete, or other similar drug tests, under the direct supervision of trained personnel. Analysis of the sample will be performed by a laboratory contracted by the University to do such testing. All samples having positive results, indicating the presence of prohibited drugs or their metabolites, will be further analyzed. All Student-Athletes refusing to provide a urine sample will be sanctioned as if their results were positive. All student-athletes refusing to provide a urine sample will be sanctioned as if their results were positive.

Positive Drug TestsThe Lindenwood University Athletic Department will make every effort to keep the test results confidential and will oppose disclosure thereof to any unauthorized persons within or outside the University.

The Head Coach, Athletic Trainer, Compliance Office, Sport Supervisor and Athletic Director will all be notified of all positive drug tests or alcohol related infractions. The Athletic Director will inform the student-athlete of the presence of a substance in his/her sample. The Head Athletic Trainer and Associate Athletics Director for Compliance and Academic Support will be present and a witness to procedure. The Head Athletic Trainer will advise the Student-Athlete of the nature of the substance and the negative health effects of the use of the substance. The Associate Athletic Director for Compliance and Academic Support will inform the Student-Athlete of the right to an appeal and the process for doing so.

Any test that comes back positive for an NCAA banned substance will be subject to following: 1. An initial offense, whether tested at random or for reasonable suspicion,

will result in mandatory enrollment in University Drug Intervention Program, resulting in loss of play and all team activities until they produce a clean drug screen. Student-athlete will be required to sign Success Contract with the Dean of Students. (Agreement attached)

2. A second offense will result in expulsion from the University for failing to maintain their agreement under the Success Contract.

Appeals

1. All Student-Athletes who incur sanctions under the terms of this program will be entitled to an appeal hearing before the Drug Policy Grievance Committee.

2. Appeals will only apply to sanctions under the terms of this program and will not apply to individual team sanctions.

3. Members of the Drug Policy Grievance Committee shall include the Director of Athletics, Dean of Students, Head Athletic Trainer, Associate Athletics Director for Compliance and Academic Support and Faculty Athletics Representative. The Committee will meet at the convenience of all members’ schedules.

4. The request for an appeal must be made in writing to the Director of Athletics by the end of the second business day after the Student-Athlete is notified of their results.

5. The Director of Athletics will notify the Student-Athlete of the Committee’s decision in writing.

6. The results of the hearing are final and binding.

Policy on Excused Absences For Students Engaged in LU Authorized EventsBasic Policy: Lindenwood University allows students (including student-athletes) engaged in official sports contests, arts events, approved field trips, approved conference attendance, etc., to be excused from class for university-sponsored events in which those students are officially representing the University. Students are required to communicate with their professors and make arrangements for missed work in advance of their University authorized absences. When make-up work is pertinent, students who have provided advance notice should be provided the opportunity to complete that work or an educationally equivalent and substitutable activity at a time and place mutually agreed upon by the student and the affected faculty member.

Page 20: 2015-2016 Lindenwood University Student-Athlete Guidebook ...

20

Premises of the Policy

1. Lindenwood University exists principally for the education and development of the student.

2. Intercollegiate athletics, field trips, special performances and recitals, faculty supervised travel to conferences, and other such co-curricular and extended experiences are necessary and beneficial components of modern university campus cultures, and they support the education and development of the student.

3. Students miss academic and student-work activities to participate in scheduled, organized intercollegiate athletics and other co-curricular pursuits because they have been asked to do so by the University and are serving the University via that participation.

4. Students must not be punished for authorized and proper participation in Lindenwood-sanctioned events for which they would normally be excused under prevailing University policy. They must not be placed at an academic or other disadvantage because they are fulfilling their commitment to represent the University in scheduled, organized co-curricular events.

5. Lindenwood University and its faculty have an obligation to afford reasonable effort and accommodations to any student who must miss class to represent the University in any scheduled, organized activities. If circumstances prohibit reasonable accommodations, the decision between academic responsibility and the conflicting activity resides with the student. If a student feels an appeal is in order, the Provost will receive input from both the student and the faculty member and then render a decision.

6. Student-Athletes are also authorized to miss school for non-institutional sponsored events (EG. National team events). Students must file the appropriate paperwork and be approved prior to the event.

Responsibilities

All members of the Lindenwood community have responsibilities to ensure that the Students’ Excused Absence Policy functions properly and efficiently for students, staff and faculty. Below are the responsibilities of each party affected by or involved with the student-athlete Excused Absence Policy:

(1) Department of SLS: The Lindenwood Department of SLS will provide at least 48 hours notice of necessary, upcoming student-athlete absences for a sporting event or related, excused sport activity. This notice will be sent through CAMS and student-athletes will appear in red on faculty portals. The notice is to inform faculty that the student-athlete will be absent from class or other University related event because the student-athlete is representing the University in an athletic event or function. If a situation arises where 48 hour notice is not possible, the Department of SLS will notify the faculty of changes or additions to the student-athlete excused absence list as soon as possible.

(2) Academic Departments: A faculty member or academic department sponsoring a special co-curricular event or outing should first complete a field trip form and submit it to the office of the Provost for approval. This form should include a list of the students attending the outing and a sponsor signature stipulating that all students who will be traveling have signed a travel waiver. Once the outing is approved, the sponsor must provide at least 48 hours written notice to the University community by sending a listing of each student who will be absent from class or other University related event because the student is representing the University at an event or function. This listing is to be sent to the Director of Executive Communication for distribution. If a situation arises where 48 hours’ notice is not possible, the professor or department should notify the faculty of changes or additions to the excused-student list as soon as possible.

(3) Professors: Each affected professor should make reasonable accommodations for excused students who are missing class or other academic activities because of their participation in an event or other function related to their participation in a scheduled, organized co-curricular event. Professors should communicate, in advance, a written stipulation of expectations and procedures for students who must miss class for authorized purposes. Each professor’s expectations and procedures should meet the parameters outlined in this Excused Absence Policy.

Page 22: 2015-2016 Lindenwood University Student-Athlete Guidebook ...

22

Responsibilities (continued)

(4) Students: Students who must miss class for a Lindenwood event must provide personal advance notice to all of their affected professors regarding their specific, upcoming absence. If possible, this notice should be given to professors at least 72 hours in advance of the affected class meetings. If 72 hours’ notice is not possible, then the student should notify his/her professor as soon as possible. This personal notice should be given in addition to the general notice provided by the Lindenwood Department of SLS or a professor or academic department via the Director of Executive Communication. In each case, the student is also expected to rigorously prepare for and promptly appear to engage in any substitute or “make-up” assignment or activity.

Best Practices

1. The Lindenwood Department of SLS will update the on-line excusal report, that will notify all professors of student-athletes’ absences at least 48 hours in advance by email.

2. A sponsoring faculty member or his/her academic department will provide a general student absence notice via the Director of Executive Communication to professors at least 48 hours in advance.

3. Professors will provide a written stipulation of expectations and procedures for students who must miss class for authorized University purposes. This policy will be written in each professor’s syllabus and reviewed with the students during the first week of class.

4. Students should individually communicate an upcoming absence to the affected professors either in person (during office hours or before/after class) or via email using their lion-mail accounts. If possible, the students should make the professors aware of the absence at least 72 hours in advance. It is highly recommended that student-athletes communicate verbally and via email.

5. At the time that a student personally communicates his/her absence to a professor, the professor will communicate the steps that the student should take to make-up any missed class work or assignments, when “make-up” work is pertinent.

23

Team Travel

1. Regarding class absence, student-athletes are excused from class for all approved away competitions as well as related, approved home competitions. Although the faculty will make every effort to work with student-athletes, it is ultimately the student-athlete’s responsibility to contact the instructor(s) at least one week prior to the competition and make arrangements to make up the work in a timely manner. The Faculty Athletics Representative will serve as an intermediary between the student-athletes and faculty to insure that this process is taking place. In addition, a weekly excuse notification report is forwarded to all faculty confirming a student athlete’s participation.

2. Student-athletes are expected to be on time for departure.

3. Student-athletes must travel to and from competitions via University-authorized transportation, unless they complete the alternative travel form and obtain approval from the Director of Student Life Sports 72 hours in advance.

4. Student-athletes are expected to conduct themselves appropriately as representatives of Lindenwood.

5. Student-athletes are expected to travel to/from competitions in University-issued travel suits.

6. Any hotel damage must be reimbursed by the student-athlete.

7. In addition to Athletics Department policies, student-athletes must adhere to all University travel policies when traveling.

8. Alcohol is not allowed at any juncture during University-sponsored travel.

9. When traveling coed, hotel rooms must remain open.

To ensure adequate parking for home competitions as well as daily classes, any team traveling to other locations for competition must park at the designated location for charter bus departures. Any vehicle violating this rule may be fined or towed.

24

Attendance of Non-Lindenwood Athletic Events PolicyStudents who wish to be excused for a non-Lindenwood athletic event, such as an international competition for their home country, must complete the Documented Absence Request form completely prior to their attendance. This form may be acquired from your coach or through the SLS office. The student-athlete must have the form completed and signed by all of their class instructors and then submit it to their coach or SLS office for final approval by the Provost. With an approved Documented Absence Request form, all classwork is required to be completed in advance, or made up in a timely manner after returning, as agreed to by each class instructor.

HazingHazing is defined as “…an often ritualistic test and a task, which may constitute harassment, abuse or humiliation with requirements to perform random, often meaningless tasks, sometimes as a way of initiation into a group. The definition can refer to either physical (sometimes violent) or mental (possible degrading) practices…” Hazing includes discomfort, embarrassment, harassment, humiliation, or destruction of property for the purpose of initiation onto a team. The person’s willingness to participate in such an activity is irrelevant. Lindenwood has a zero tolerance policy on the hazing of any student-athlete. Any such exploitation will result in immediate action by the administration.

Date RapeRape is any sexual intercourse occurring against an individual’s will (i.e. rape does not require the brutalizing of a person or sexual intercourse at knife-point). “NO” simply means “NO!” Date rape often occurs in conjunction with alcohol use and/or abuse. Student-athletes should not misread or give out misleading signals. Student-athletes should also protect themselves from risky situations and be aware of drugs such as GHB or “Liquid G” and rohypnol, which can cause blackouts and are commonly used by perpetrators of date rape.

25

Sexual Harassment or MisconductSexual harassment is any unwelcome sexual advance, request for sexual favors, or other verbal or physical conduct of a sexual nature when • An individual’s employment or education is put in jeopardy if that individual

should refuse to submit to such conduct; submitting to or rejecting such conduct is used in making decisions about an individual’s employment or education; the conduct unreasonably interferes with an individual’s work or educational performance or creates a hostile environment.

Sexual harassment includes but is not limited to• Suggestive/insulting sounds, leering/ogling, and obscene gestures; sexually

explicit statements, questions, jokes, or anecdotes; remarks of a sexual nature about a person’s clothing or body, about sexual activity or previous sexual experience; unnecessary touching, patting, hugging, or brushing against a person’s body; physical assault or rape.

If you are sexually harassed• Know your rights; avoid isolation and self-blame; keep a written, dated record

of what took place and who else was present; tell the harasser to stop; if necessary, seek a formal resolution through the grievance process.

If a student is aware of sexual harassment or feels as if he/she is a victim of harassment, one of the University officers listed should be contacted: Dr. Deb Ayres-Vice President for Student Development, Dr. Ryan Guffey-Vice President for Student Development, Terry Russell-Dean of Students, or Rev. Timothy Butler-Chaplain.

Disciplinary Action and GrievancesUniversity decisions regarding disciplinary actions will supersede any decision of any coach. Coaches may also assess additional punishment where applicable.

If a student-athlete has a grievance or an issue with a teammate, the matter should first be discussed with the head coach. Should the matter not be resolved in a satisfactory manner, all parties will meet with the Director of Student Life Sports. Should the matter not be resolved at that level, all parties shall submit the issue in writing to the Vice President for Student Development, who shall review the matter within 30 days.

Page 26: 2015-2016 Lindenwood University Student-Athlete Guidebook ...

26

Disciplinary Action and Grievances (continued)If a student-athlete has a grievance with a coach or any employee of the Athletics Department, he or she should present the grievance in writing through the grievance reporting mechanism available on the Athletics Department website. The matter is submitted automatically to an employee of the University outside of the Athletics Department, who shall present the grievance for review through the Department of Human Resources, which shall gather appropriate information and review the matter within 30 days.

Social Networking and Public Forms of Communication (Facebook/Twitter)Lindenwood respects each student-athlete’s right to free speech and expression. However, student-athletes are responsible for the manner in which they choose to use this expression. The University’s primary concern regarding social networking sites such as Facebook, MySpace and Twitter involves the safety of students as well as the integrity of Lindenwood and its community. Potential employers, clients, and family members may access this information. Unfortunately, the information can also be accessed by predators and criminals. Once information is posted to the web, it becomes part of a public realm accessible to anyone with internet capabilities. You forfeit any right to privacy when you submit pictures, blogs, and/or comments on these websites. In addition, these items can be archived forever. Remember, what you do represents you and your team, as well as Lindenwood University.

While Lindenwood has no specific policy prohibiting the use of these web sites, as it recognizes that they can be educational, the University does possess resources to determine whether items posted defame the University, its students, faculty, and staff. Student-athletes will be held accountable for any such items, which may result in dismissal from the program and/or the University. Listed below are a few guidelines to follow:

1. Avoid posting personal information like addresses, residence hall locations, cell phone numbers, etc.

2. Do not make references to alcohol or drugs in photos or blogs.

3. Do not post explicit pictures.

4. Do not post negative references to students, your teammates, instructors, staff, or your opponents.

5. Logos and pictures posted on the University’s website are copyrighted and cannot be used without University permission.

Co-Curricular Residential Requirement PolicyLindenwood is by design a residential University. Living-on campus provides educational living and learning experiences that lead to a student’s growth and development and an enhancement of one’s total education at LU. Because residential living is such an integral component of the collegiate experience, only in very rare and special incidences would a student be exempted from the University housing requirement for institutionally funded co-curricular participation. The residential requirement is a commitment between the students and the University to enhance the student’s experience and success. As a result, all students participating in institutionally funded co-curricular programs, e.g., NCAA and Student Life Sports, must live on-campus. When we do not have sufficient space for all residential applicants on-campus, requests for authorization to live off-campus will considered on a case-by-case basis, as with all such requests.

Students may apply and appeal to live off-campus if they meet one or more of the following qualifications:

1. Current active military or veteran status: Students must verify by submitting a copy of the DD Form 214, Certificate of Release or Discharge from Active Duty (if a veteran) or written proof of current military active duty.

2. Married and/or with legally dependent children: Copies of legal documentation will be required to accompany the exemption form request.

3. Medical and disability circumstances: Exemption requests for medical or disability reason will be forwarded to the Institutional Appeals Committee and reviewed with the Student Support and Accessibility Coordinator. This committee will render a decision and notify the Department Residential Life Office and the applicant of the outcome.

4. Other: Special circumstances can be considered when a significant personal hardship is worsened by living on-campus and cannot be remedied by normal means. Verification of special circumstances will be required.

Page 28: 2015-2016 Lindenwood University Student-Athlete Guidebook ...

2015-16 Student-Athlete Guidebook and Planner

28

Co-Curricular Residential Requirement Policy (continued)In each case, a student must submit a written appeal to the Vice President for Enrollment Management. In medical and psychological/psychiatric cases, students must provide documentation from a physician, psychologist, or psychiatrist, including diagnosis, specific needs related to condition, and recommended treatment along with an explanation of why alternative housing is necessary. Students must also submit a written statement authorizing the attending physician, psychologist, or psychiatrist to discuss the student’s case with University staff. In cases of financial hardship, students must submit appropriate documentation including such items as tax returns, financial aid statements, and payroll receipts. Students may also be required to meet with an Admissions/Financial Aid Officer to review his/her financial status and to explore the possible availability of additional aid in response to changes in status.

All requests for exemption are reviewed by the Vice President for Enrollment Management and the Appeals Committee. Students requesting an exemption are responsible for assembling and presenting any and all information to assist the Committee in the decision making process. Students are informed in writing of the decision.

Signing an off-campus lease without housing approval does not exempt a student from financial responsibility for an assigned space in on-campus housing.

All students who live on or off campus have the responsibility to uphold the Code of Conduct. Any student whose behavior does not fall within the standards of the Lindenwood University student handbook will be required to return to campus housing, as well as face additional conduct consequences.

Scholarship/Financial Grant Information

In order to maintain your scholarship/grants, you must maintain satisfactory academic progress. Please review the student handbook for detailed information concerning scholarship, federal and state grant eligibility.

New Student Worker ProgramA new Student Worker Program will be implemented during the 2015-16 year. This new program COMBINES the current GA and Work and Learn Programs into a unified program, the Student Worker Program, to better meet workforce needs while ensuring compliance with federal and state (Missouri and Illinois) regulations. Students will still apply their payment for work towards tuition, in most cases, but this is no longer a guaranteed financial award program. Rather, this program will utilize an employment model.

Here are the main changes that can be expected:Student Workers will NOT be defined by their terms (5-term/semester/quarter)In other words, a student worker can be hired for available assigned positions for any authorized period, no matter what their type of enrolled degree program. Instead, each position will be assigned a maximum number of work-weeks (36, 40, 52 weeks, etc.)

There are no official paperwork due dates or hire dates• This means that if you have a vacant student worker position, you can hire

a student worker and start them as soon as they have been approved and inducted by the Student Worker Office.

Change in requirements• A student can be a student worker as long as they are in good standing with

the university.• If a student wishes to be in the program their very first term at Lindenwood,

they simply need to be unconditionally admitted. If they aren’t, they can apply after their second term.

• To be removed from the program, a student worker can quit, be terminated, or no longer in good standing with the university.

Most Student Workers will work either 20, 15 or 40 hours per week. Federal workers will still be assigned 10 hours per week.52 week 40 hour Student Workers will have PTO • 52 week 40 hour Student Workers will be granted Personal Time Off (PTO) 52 week full time workers will receive 12-8 hours days off each year They will also receive paid holiday time off

Page 30: 2015-2016 Lindenwood University Student-Athlete Guidebook ...

2015-16 Student-Athlete Guidebook and Planner

30

New Student Worker Program (continued)No set number of hours to reach before the term ends• Student workers will not be penalized for not reaching a certain number of

hours each term. Since they are no longer connected to term types, they will be paid for what they work. Supervisors will be responsible for monitoring task completion and tardiness.

• There will be specific hours a week that cannot be exceeded. However, if the student worker wants to take some time off, they can, pending supervisor approval.

One united student worker program• All student worker candidate selection will be done through one centralized

location.• Certain jobs will be designated as jobs requiring interviews, while others will

be designated as placement positions.• All candidates must apply through the Student Worker Office. They can

no longer go in search of positions on their own. Since most positions are placement positions, they are on a first come first serve basis.

Since the program is based on workforce needs, the number of workers (previously Work and Learn) will decrease significantly. The most important takeaway is that the student worker population are employees of the University.

Timeline of changesJune 19, 2015: Notice to students of the changesAugust 2015: Finalize collection of W4 formsOctober 2015: All GA’s, Cops for Credit, and Work and Learns will be utilizing a time keeping systemJanuary 2016: All GA’s, Cops for Credit, and Work and Learns will be running through payroll and paid an hourly wage. As a condition of employment, students will still apply their payment for work towards tuition, in most cases, but this is no longer a guaranteed financial award program, but an employment model. NOTE: All students associated with the Work and Learn program will continue in their current position through the Spring Semester change. However, they will no longer receive any Work and Learn award, but instead be paid an hourly amount.March 2016: Those students who were participating in the Federal Work Study Program or the Work and Learn program will need to reapply to take part in the new Student Worker Program.NOTE: Graduate Students who were taking part in the GA program will not need to reapply to the Student Worker Program.April-May 2016: Applicants will be placed in the new positionsAugust 2016: Transition to new Student Worker Program complete

Physical Exams/Insurance/Medical CoverageTom Godar-Assistant Athletic Trainer636-949-4628 [email protected]

All Lindenwood University student-athletes must have a complete folder on file in the Athletic Training Department and be cleared through the Student Life Sports office before athletic participation is permitted. Intercollegiate athletic competition includes any official team practices, voluntary workouts with coaches or other athletes, any team structured activities, and any use of the university fitness centers.

A completed folder will include the Eligibility Forms identified below, as well as a copy of the student-athlete’s current health insurance card. The student-athlete will also be required to complete his/her SportsWare online account with personal information and emergency contact information. Each student-athlete needs to meet with the athletic trainer assigned to their sport for confirmation and verification of all required medical/insurance paperwork prior to any type of athletic participation. For a list of forms required, please refer to the “Student-Athlete” section of the Student Life Sports website at www.lindenwoodlionssls.com.

NOTE: Because many Lindenwood University sports program compete at the NCAA Division II level, most forms and requirements for Student Life Sports athletes are the same.

Eligibility for Athletic Participation

Physical Examination

• Every Student Life Sports athlete must have the Lindenwood University Physical Form completed by a physician (MD or DO), physician assistant, or certified nurse practitioner.

• New athletes must have a completed physical form conducted within the past six months.

Page 32: 2015-2016 Lindenwood University Student-Athlete Guidebook ...

2015-16 Student-Athlete Guidebook and Planner

32

Physical Examination (continued)• Returning athletes must have a completed physical form conducted within

the past year. (1) Beginning in the 2015-2016 school year, returning student-athletes to Lindenwood University will only need to have a physical examination completed every other year. (2) Returning student-athletes who submitted a new physical examination form during this past school year will need to complete a medical questionnaire and have some basic stats recorded (i.e. blood pressure, height, weight, etc…). These forms will be provided at the team’s assigned medical screening day. a. Pending a review of this information, student-athletes may still be required to see a physician for further evaluation prior to being cleared for participation. b. If a returning student-athlete submits a physical form completed within the last calendar year, it can replace the medical questionnaire, however an annual meeting with the assigned athletic trainer is still required.

• Prior to each team’s first official practice, a date and time will be scheduled for each SLS program on campus providing students the opportunity to have a physical examination conducted by a team physician. There will be a $15 charge for all physicals performed at the University.

• Physicals may be completed prior to the athlete’s arrival on campus. If completed off campus, the Lindenwood University Physical Form must still be used.

Past Medical History

Student-athletes who have sustained injuries to any of the following are required to report these injuries during the annual physical examination: head, neck, back, shoulder, knee, ankle, and/or internal organs. A student-athlete reporting such an injury is subject to examination by the Team Physician prior to being given permission to become a team candidate.

Student-athletes who have had an infectious disease during the previous calendar year must report this illness during the annual physical examination. A student-athlete reporting such an illness is subject to examination by the Team Physician prior to being given permission to become a team candidate.

33

Past Medical History (continued)Failure to report such pre-existing injuries or illnesses relieves the University of all liability, in the event that the athlete sustains a subsequent injury or re-injury to the affected body part, provided the original injury was a contributing factor to the subsequent injury in the opinion of the Team Physician.

SportsWare OnlinePrior to participating on a team at Lindenwood University, all student-athletes must provide the Athletic Training Department with personal information and emergency contact information. To expedite this process, Lindenwood University uses an online data entry system. This system is also used for recording injury and treatment data, and analyzing yearly trends to make future recommendation. Specific step-by-step instructions for accessing and utilizing SportsWare Online are located on the Student Life Sports website under the “Student-Athlete” menu.

To enter your information, visit www.swol123.net.

Each student-athlete will meet with a member of the Athletic Training Department to review all forms and information submitted via SportsWare Online. These meetings are scheduled by team. The following essential information is required: 1. General student-athlete information 2. Current mail address3. Emergency contact information4. Current insurance information5. Medical information (i.e., allergies, medications)6. Questionnaire

Insurance/Medical Coverage/Reporting RequirementsThe Physical Examination Form and Emergency Information Sheet are available on the Lindenwood University Student Life Sports website. To ensure the highest quality of healthcare, it is imperative that all student-athletes utilize these forms and complete them in their entirety. If you have any questions concerning the forms or their content, please contact the Athletic Training Department at 636-949-4628.

InsuranceParticipation in intercollegiate athletics involves the inherent risk of injury. For this reason, it remains necessary for all student-athletes to maintain a personal health insurance policy. This policy must remain in effect throughout the entire school year, thus covering all in-season and out-of-season training, practices, and competitions.

Students will not be allowed to participate in any team activity or training until proof of insurance is provided to the athletic training department and the acknowledgment form is completed by the policy holder. It is highly recommended the student-athlete’s health insurance policy has a reasonable deductible, as this may be the responsibility of the student-athlete and/or policy holder. Furthermore, this policy must meet the following criteria:

1. Coverage of interscholastic athletic-related accidents2. Coverage in the state of Missouri

*Out-of-state student-athletes with health insurance that only covers emergencies in the state of Missouri may need to return home, at their own expense, for non-emergency, elective medical care. While the university does retain a secondary insurance policy for student-athletes, it must be understood that necessary steps may need to be taken to ensure the primary insurance policy is properly utilized and fully exhausted prior to receiving benefits from the secondary. In addition, if it is determined that a student-athlete incurred out-of-network expenses not covered by his/her primary insurance due to lack of coverage, benefits from the secondary insurance policy may not be available.

In addition to the specific criteria stated above, out-of-state Medicaid plans DO NOT meet Lindenwood University Athletics requirements.

Student-Athletes Needing InsuranceStudent-athletes who do not currently have a health insurance policy, or have a policy that fails to meet the above criteria, are advised to visit the official site for the Affordable Care Act at www.healthcare.gov. This site provides information regarding your available health insurance options. Pending your current personal and family situation, as well as your specific needs for health insurance coverage, please choose the policy that works best for you and meets the University’s student-athlete participation criteria.

2015-2016 Secondary Insurance Information (provided by the University)

The university’s secondary insurance policy is limited to those injuries received, which in the opinion of the team physician and certified athletic trainer, are directly attributable to participation in sanctioned intercollegiate athletics while a full-time student and team member at Lindenwood University.

The current secondary policy is an accident-only policy and an excess policy. This policy carries a disappearing $1,000 deductible and eligible medical expenses payable under any other insurance policy or service contract may be used to satisfy or reduce the deductible. After the student-athlete’s primary health insurance policy is exhausted and the $1,000 deductible has been met for the university policy, the secondary health insurance may provide up to the maximum benefit for eligible medical expenses. This may not include certain medical procedures, special services, durable medical equipment, etc.

In addition, this accident-only policy will not provide coverage for the following:

1. Costs incurred from the care of injuries and illnesses that are not linked directly to a specific athletic accident or event.

2. Costs associated with injuries and illnesses incurred while participating in activities not directly associated with the student-athlete’s intercollegiate program.

3. Costs incurred due to additional testing required as the result of issues or concerns raised during the pre-participation examinations.

4. Costs incurred from the emergency care of medical conditions that are not directly attributable to the participation in intercollegiate athletics (ie. appendectomy).

5. Costs incurred due to dental care not relating directly to an athletic incident.

6. Costs incurred due to injuries associated with fighting (regardless of the setting).

Guidelines for Reporting an Injury/Submitting a Claim1. The athlete must report all injuries and illnesses to the athletic training staff

in a timely manner for appropriate evaluation and referral.2. All medical services and referrals must be coordinated and/or approved

by the athletic training department. Any costs related to medical services scheduled without notification and approval from the athletic training department will be the sole responsibility of the student-athlete: this does not include emergency care. In the event of an emergency visit, the student-athlete should seek assistance from the athletic training staff immediately after returning to campus.

3. The student-athlete must first have all medical expenses filed with his/her primary insurance carrier prior to the secondary policy.

4. Once the incident has been verified by the athletic training staff, a claim form will be submitted to the secondary insurance company by the secondary insurance coordinator.

5. The student-athlete is responsible for ensuring all information is submitted to the secondary insurance company. Secondary insurance company information will be provided to the student prior to the first medical appointment.

6. Requested information may be submitted to the secondary insurance company via:• Direct submission from the healthcare provider by providing them with

the secondary insurance company information• Indirect submission from the student-athlete after information is

obtained from the involved parties (medical providers and primary insurance carrier)

7. The student-athlete is responsible for ensuring all requested information from the secondary insurance company is provided in a timely manner. This may include, but is not limited to: parent employment verification, itemized bills for services, explanations of benefits, etc. Failure to comply with requests for additional information may result in a lack of benefits provided by the secondary insurance company.

All student-athletes must have a completed Primary Insurance Acknowledgment Form and proof of insurance (i.e. copy of insurance card) prior to participation in any sanctioned intercollegiate athletic involvement while a full-time student and team member at Lindenwood University. The Primary Insurance Acknowledgment Form must be completed and signed by the primary health insurance policy holder.

Athletic Training DepartmentAthletic trainers have been recognized by the American Medical Association as an allied health care professional since 1990. Athletic trainers are certified health care professionals who collaborate with physicians and other health care professionals to optimize the participation and activity for patients and clients of all ages and skill levels, including young children, student-athletes, professional athletes, active adults, and military personnel. Athletic trainers provide prevention, examination, treatment, and rehabilitation of acute, subacute, and chronic illnesses and injuries. This commonly includes a wide range of conditions from mild ankle sprains to catastrophic head and neck injuries and mild illnesses to heat stroke. Our mission, as the Lindenwood University Sports Medicine Team, is to provide our student-athletes the best health care possible with the highest standards. As an educational institution, we also strive to provide the most optimal learning environment for our athletic training students. Lindenwood University has over 20 certified athletic trainers and approximately 50 undergraduate athletic training students that are enrolled in our highly respected Athletic Training Program. Along with the team physicians, campus health services employees, local physical therapists, and chiropractors, we provide well-rounded health care for all student-athletes. To assist our staff, there are four very well equipped athletic training facilities on campus located in the Hyland Arena, Field House, Clubhouse, and the Student Athlete Center (SAC).

Athletic Training Services for Student Life Sports Teams (2015-2016)Athletic trainers are available for Student Life Sports programs. Student-athletes that are part of a team (except cheer and dance teams who are assigned their own) will be able to receive treatment if needed in the Field House by contacting any of the following graduate assistants. Scheduling an appointment is required in most cases.

Media RelationsPhil Vida, Student Life Sports Information Director636-627-2535 ~ [email protected]

During your years at Lindenwood, you may be called upon by the Sports Information Office to appear for interviews and photographs. You have a responsibility to the University, your coaches, and teammates to cooperate with these requests whenever possible.

All of these media/institutional requests must be coordinated through the Sports Information Office. If you receive an interview request by telephone or via email, ask that the media representative make arrangements through the Sports Information Office. In general, when speaking to the media, be confident, courteous, and prompt. Always remember when dealing with the media to present an image that is consistent with the ideals and high standards of Lindenwood.

If you have any questions about how to conduct interviews, please feel free to contact the Sports Information Office. Lindenwood makes a strong effort to gain positive exposure for its athletics programs and your efforts in working with the media are appreciated.

Tips for Dealing with the Media1. Always be on time for interviews. Should the interview be set for a time

on the phone, make sure you are available. Absence from class for media interviews is not permitted.

2. Organize your thoughts before a scheduled interview so that you are sure about the points you want to communicate.

3. Be sure to understand the reporter’s question before answering it. 4. Give proper credit to teammates and coaches and never criticize them to the

media.5. Always say complimentary things about your opponents before and after the

game.6. Keep direct eye contact with the interviewer.7. Speak in short, clear sentences.8. Be yourself. Do not be afraid to talk about things of importance to

you outside of athletics. “Conversations” make better interviews then “interrogations”.
