2014 Online Enrollment Guide
Step 1:
Access the AflacAtWork enrollment system by typing “aflacatwork.com” in your internet
address bar. The following log-in screen will load. Note: To view and sign forms, you must
have Adobe Acrobat or Adobe Acrobat Reader and Adobe Flash Player installed on your
computer. The minimum version of Acrobat Reader required is 5.1.
Enter the following log in credentials:
Username: Social Security Number
Personal Identification Number (PIN): last four digits of social and last two digits of your birth
year
Example: SSN 123-45-6789 of and date of birth of 1/31/1958
Employee ID or Social Security Number: 123456789
Personal Identification Number (PIN): 678958
If you have forgotten your password, enter your SSN and click the “Have you forgotten your
PIN?” link listed on the log-in page. Please note: This option is only available after you have
reset your initial password listed above.

To reset your password, enter your Canyons School District work email address, type in the
Verification code listed and click “Continue.”
You will be prompted to answer the Security Question that you selected during your initial log-in
and provide your Date of Birth. Once all information has been entered, click “Continue.”
A confirmation code will be sent to your Canyons School District email address. Copy and paste
the confirmation code provided in your email into the “Confirmation Code:” field, designate a
new PIN and click “Continue.” A successful reset confirmation message will display. Click
“Continue” to return to the log-in page.

Step 2: The presentation screen shown below will load welcoming you to the enrollment system.
You should not move forward with this enrollment if you do not have the following:
All of your dependent’s SSN
Your dependent’s DOB
If you are not covering dependents this information is not needed.
Click “Next” to begin the enrollment.
Step 3: Review the Employee tab and make any necessary changes.
Please note: Fields in italics are not required.

Step 4: Review the Dependents tab and make any necessary changes by clicking on the dependents first
name. Recent changes to federal regulations require us to gather Social Security Number for all
covered individuals. Please provide or verify the social security numbers for yourself and all of
your dependents receiving coverage.
To add new dependents, select the “Add” button and enter the dependent information.
Please note: Fields in italics are not required.
Click “Next” to begin the enrollment of your medical plan.
Step 5: To expedite the enrollment process, a Quick Enroll feature has been added for this year’s
enrollment. This feature can be used in the following ways:
1. If you wish to keep the same elections listed under the “New Coverage” section of the
chart, place a checkmark next to each plan (including waived) and click the "Next" button
to continue.
2. If you wish to keep a portion of the elections listed under the “New Coverage” section of
the chart, place a checkmark next to each plan you would like to continue and/or waive
and click the “Next” button to continue.
3. If you are a new employee enrolling for the first time, you will see “Waived” for all
plans. If you wish to waive any plans, place a checkmark next to the plan(s) and click the
“Next” button to continue. Beneficiaries are required for some of the Life plans;
therefore, you may be routed to the application screen to designate the beneficiary. Click
“Next” to continue through the enrollment process.
4. If you would like to review each plan in detail or waive a plan that was enrolled last year,
click on the “Next” button to continue through the enrollment process.
Please note: The Basic Life & AD&D and LTD plans are employer paid benefits; therefore, they
are auto-enrolled. A beneficiary designation is required on the Basic Life & AD&D plan. Also,
the Selman Group Accident and Washington National plans are informational only. You will
only see a premium if you are currently enrolled in this plan. Please contact HR for any details
regarding these two plan offerings.

Step 6: The Health plan application screen will load providing information regarding the plans for
election.
To elect a plan, click the button next to the requested coverage tier and “I wish to apply for this
coverage.”
To waive a plan, click the button next to “I wish to DECLINE this coverage.”
Click “Next” to verify the dependents to be covered under this plan, if applicable.
Click “Next” to go to the next plan.
Continue Step 6 to apply or decline the Dental and Vision plans.
The Basic Life & Accidental Death & Dismemberment (AD&D) and the Long Term
Disability (LTD) plans are employer paid; therefore, the screen will not have an option to
waive. Click “Next” to continue.

Step 7: If you are an existing employee enrolling in the Voluntary Employee or Spouse life plans for the
first time or increasing your current benefit amount, you will be required to submit an Evidence
of Insurability (EOI) form.
Once you have made your election and designated a beneficiary, the following Evidence of
Insurability screen will prompt you to print the form and submit to your insurance carrier. The
EOI form can be accessed by clicking on the “Evidence of Insurability” hyperlink on the screen.
Click the “Back” button in your browser to go back into the enrollment.

Step 8: If you are enrolling in the Health Care FSA or Dependent Care FSA plans, the application screen
has a calculator function that will allow you to enter the per pay period or annual contribution
amount. Enter the amount you wish to contribute per pay period or annually and click
“Calculate” the system will then give you the total annual amount or per pay period amount based
on the number of pay periods in the year.
Click “Next” move to the next plan.
Step 9: The Selman Group Accident (Hartford) and Washington National (Conseco) plans are
information only. If you have current benefits with either of these providers, your information
will display on the screen. If you are interested in changing or enrolling in this coverage, please
contact the District Insurance Office at 801-826-5428.

Step 10: Once you have completed the enrollment of your core and Aflac benefits, a Sign & Submit screen
will present recapping your enrollment elections. Once you have completed you review, click
“Next.”
Click “Next” to sign the Benefit Summary confirming your elections.

Step 11: Please review the statement and verify all elections, waives and benefit amounts are correct and
enter your PIN (the new PIN you entered in Step 1) to sign the form.
To complete your enrollment, you must click “Sign Form.”
Once you have completed enrollment, you will be taken to a “Congratulations!” screen to recap
all of your elections one last time. At the bottom of this screen, you can click the hyperlink to
open your Benefit Summary in a new window and print.
Click “Logout” to exit the system.
If you need to stop and come back to your enrollment at a later time, all the changes you
have made up until that point will be saved.

To make changes once you have completed enrollment:
Step 1: Log back into the enrollment system as you did in Step 1 using your new PIN.
Step 2: Hover over “My Benefits” and select the plan you wish to change from the drop down.
Step 3: Click the “Unlock” button. Make the necessary change and click “Next.”
Step 4: The Sign and Submit screen will display for you to review your elections.
Click “Next” to sign the Benefit Summary. Review the Benefit Summary to ensure all elections,
waives and benefit amounts are correct, enter your PIN and click “Sign Form.”
Once you reach the “Congratulations!” screen, scroll to the bottom to print your updated benefit
summary and click “Logout” to exit the system.
