HOW TO STOP CURRENT MEDICATION
__ 1. You meet detox criteria and withdrawal criteria to have induction today
__ 2. You DO NOT meet criteria and have to come back in the morning or some other
identified time for induction
__ 3. This information is being given for training and planning for Opiate dependence
after chronic opiate therapy for chronic pain condition, and possible Opiate induced
Hyperalgesia.
INDUCTION MEDICATION INFORMATION: SEE MARKED MEDICATIONS TO BE
GIVEN
A. ___ Suboxone 8mg/2mg film or ___ Suboxone 2mg/.5mg film

B. The prescription will be given as:
___ one film every 8 hours
___ one film every 12 hours
C. ___ Subutex 2mg pill was used or _____ Subutex 8mg was used and dose

be 1/2 or 1 whole every 812 hours
D. The above dosing is to be followed for the first 710 days or until you are seen by
the provider again. Max dose is 3 films per 24 hours. Mimimum advised dose is
1/2 (half) film under tongue every 12 hours
E. ___ Clonidine pills 0.1mg po every 12 hours as needed for aggitation or ____
Clonidine TTS1 patch every 7 days
* Take as ordered by the provider, unless blood pressure is too low or doctor

F. ___ High dose Vitamin C to dose up to 4000 to 5000mg daily to optimize withdrawal
symptoms is advised. If you develop loose stool, reduce dose until it resumes normal
G. ___ Phenergan 25mg one po every 6 hours as needed for nausea or aggitation

* This dose may not be indicated... and again is 2030 minutes after the second dose
under your tongue, if you still felt any feelings of withdrawal like symptoms. This dose
may have increased the withdrawal you were having, or it may be the only other dose
you need to load the medication into your system, or it completely resolved your
withdrawal.
PART 2 INDUCTION DAY/STILL DAY ONE SCHEDULED DOSING1. After the induction in the office is done you will begin daily doses as listed.2. Your daily dose is determined by your provider and your response during induction. 3. Your next dose on the induction day is done at home, and is 68 hours from the very first dose given in the office by the provider. 4. YOUR NEXT DOSE IS: AT _______________

5. CONTINUE THIS DOSE AS PRESCRIBED. UNLESS THE DOSE MAKES YOU TOO SLEEPY. If symptoms of agitation and/or sweating, skin creepy crawling symptoms, or other agitation symptoms and not pain occur before the 6 hours is up you may use the Clonidine for that. If you are lightheaded or the medication makes you feel racing or highlike, this is very rare and means you are extra sensitive to the dose, and you must reduce all doses by half and contact the office. If you are not sure what you are feeling you must stop using this medication and contact the office and report symptoms as URGENT and FROM NEW SUBOXONE INDUCTION PATIENT to the staff. 813.999.3030 or email if provided to you.

These include: 1) understanding and reinforcement of safe use conditions, 2) the importance of psychosocial counseling, and 3) screening and monitoring patients to determine progress toward treatment goals.
