Diagnosticimagingofsepticarthritishaslittlespecificsigns.Inplainx-rayisseenincreasingvolumeanddensityofsofttissues,withpartialortotallossoftheperi-articularfatlines,withoutbonealterationsintheearlystagesmainlyinchildren.Inordertoavoidirreversibledamageofarticularcartilageandjointismandatorytoperformajointpuncturetopreparesmearandcultureandthereforeestablishthediagnosisandearlytreatment.Inthelaterstagesastheplainx-rayimagingmethods,US,CT,MRI,PET-CTandSPECTareofgreatvalue,aloneoralltogethercanestablishthediagnosis.MagneticresonanceimagingMRIisverysensitivebutlittlespecific.Intheappendicularskeletonmorefrequentlyinshoulder,kneeorhipcanincreasejointfluidandsynovitisoritscomplicationssuchasosteomyelitisorabscesses,whileintheaxialskeletoninfectionmanifestsasspondylodiscitis,usuallyaffectingonlyoneintervertebralspaceandwhichcanbeextendtosofttissuesorthespinalcanalthatbecomesmoreevidentwiththeapplicationofgadolinium.Thesedataestablishdiagnosisbutthedefinitivesettleswiththesmear,cultureorbiopsy.InallcasesshouldbeestablishthedifferentialbetweenpyogenicandTBdiagnosis.
Osteomyelitishasthreetypesofacute,subacuteandchronicpresentation.Theinfectionreachesbonebyhematogenousroute,bycontiguityorbydirectinoculation.ThemostcommoncausativeagentbothinchildrenandinadultsisStaphylococcusaureusfollowedfromahugelistofpathogensincludingMycobacteriumtuberculosis,anaerobes,fungi,etc.Itoccursatanyageandriskfactorsarewoundsinskin,exposedfractures,surgery,prosthesis,diabetes,immunosuppressedandothers.Acuteosteomyelitishasnomanifestationonplainx-rayorinsingleCTbonescanandMRIcanshowitwithhighsensitivitybutlowspecificity.TheMRIshowsedemainboneandsofttissuewithgreaterintensityaftertheapplicationofgadolinium-basedcontrastmaterial,butonlybiopsyandculturegivethedefinitivediagnosis.SubacuteosteomyelitisisrepresentedbytheBrodie`sabscesswhichislocatedintheepiphysisorthemetaphysisowingtovascularpatterninchildreninthoseplaces,whileinadultsthelocationismetadiafisiaria,itsetiologyisusuallypyogenicbutitcouldalsobetuberculous.Asseenintheplainx-rayasgeographicalwithsclerosusedge,thicknessandradiopaque,withsimilarimageintheTAC,butMRIismoresensitiveandwiththeapplicationofgadolinium-basedcontrastmaterialsensitivityandspecificityincreasegivingreliabilitytothediagnosis.Chronicosteomyelitisiscomposedoftheinvolucrum,sequestraandfistuloustract,demonstratedaltogetherbyplainx-ray,CTandMRIthattogetherwiththeapplicationofgadolinium-basedcontrastmaterialcandemonstratethefeaturesofeachofthemaswellasitscomplicationslikeanabscesses,whichcanbesupportedwithmethodssuchasSPECTorPET-CTincasesofdoubt.Ifthediagnosisandtreatmentofosteomyelitisissetproperly,theresultcanbethecure.
