What are the Strategic Directions for the IHSP 3?Improve access to care by• working to improve your access to a doctor, a

nurse practitioner or another health care provider as close to home as possible

• working with community partners to expand education and self-management programs to help you understand and manage chronic diseases with a focus on diabetes

• expanding mental health and substance abuse programs through partnerships with community partners, with an emphasis on youth

• supporting community-based services for seniors

Streamline transitions and navigation of the system by• working with Health Service Providers to improve

linkages among primary care and community Health Service Providers so residents have the right care when they need it

• understanding and addressing barriers to accessing health care services by working closely with hospitals and community Health Service Providers

• improving system navigation resources so residents can move more easily from one Health Service Provider to the next

• supporting the use of modern information technology to ensure Health Service Providers and clients have access to the patient information they need

Drive quality and value by• working closely with Health Service Providers

to improve the quality of services they provide

• reducing avoidable hospital visits by freeing up the ER and hospital beds for the most ill patients, which can improve the efficiency of the system

Build on the momentum by continuing to• meet the legislated requirements to engage local

Aboriginal and First Nations peoples to develop priorities and strategies to improve access to health care services

• engage diverse communities to promote equitable access to health care services

• meet the legislated requirements to improve access to and integration of French language services in the local health care system

• improve palliative care services tailored to meet the needs of residents of the Central West LHIN

• improve access to and quality of women and children’s services

What is the Integrated Health Services Plan 3 (IHSP 3)? The IHSP 3 is the strategic plan that outlines the Central West LHIN’s actions aimed at improving local health care services from April 2013 to March 2016.

The IHSP 3 aligns with Ontario’s Action Plan for Health Care, that sets actions to build a health care system that is more responsive to residents and improves quality of care and value for money.

The Central West LHIN did extensive community consultation with local residents and Health Service Providers to find out what they thought the LHIN should work on over the next three years. The feedback was used to develop four strategic directions that will guide the Central West LHIN to improve health care services in the communities of Brampton, Caledon, Dufferin County, Malton, Rexdale and Woodbridge.

Over the next three years, the Central West LHIN will focus its time and attention on four strategic directions: Improve Access to Care, Streamline Transitions and Navigation of the System, Drive Quality and Value, and Build on the Momentum.
