Instructions Welcome to the 2012 Clinical Competency Guide. In response to team member requests the process for completing and tracking clinical competencies has been simplified. Competency assignments will be released quarterly in this publication. To register:
1. Go to the department index (page 4 of this document) and click on your department 2. Click on your role, (i.e. RN, LPN, etc.) you will be taken to your assigned competencies 3. Click on a competency title, the KDMCU login screen appears, login to the system 4. You will be taken to the registration page for that competency 5. Register and complete the training.
For team members required to demonstrate skill based competencies, you will find these listed in this guide as well. Once registered, you will be guided through the process by the Web Based Training (WBT) module. When you complete a skill validation in the presence of your unit trainer, you will login to KDMCU online, find the validation you previously registered for, and open the Required Skill Validation (RSV) competency statement. Your unit trainer will enter their name and id, and then you do the same. The trainer then clicks the ‘YES’ button to validate the skill. Skills validation checklists may be found by clicking here. You will now have the tools you need to view what is required and what you have completed. From time to time your leader may ask you to report on your progress in completing your competencies. A simplified reporting process is outlined on the next page. If you experience technical difficulties please call the Help Desk at ext. 4357 (HELP). Reporting Information To track your progress, just login to KDMCU, click on “My Reports” then select the “My Class Credit Report by Credit Type”. Don’t forget to enter the start date of 10/1/11 to include only information for FY 12. Click the report graphic on the right for a brief demo of the process.

Demonstrates the collection of the appropriate equipment: Betadine swabs, alcohol prep pads, 23 gauge
Butterfly needle, 3-way stopcock, 20 cc syringe, occlusive dressing and sterile gauze/OB sponges

o Locates nipple on side of Pneumothorax at fourth intercostal anterior axillary line or
o Second intercostal at mid clavicular line
o Cleans insertion site prior to insertion

o Aspirates air/fluid from pleural space
o Begins preparation for CT placement per physician
o Covers insertion site with petroleum gauze and small dressing unless ordered otherwise

Expected knowledge and technical ability:
Demonstrates equipment selection: 23 gauge Butterfly needle, Betadine swabs, alcohol prep pad, syringes (3 cc and/or blood gases syringe) and OB sponges/sterile gauze
Demonstrate appropriate puncture site on infant Demonstrate the correct assessment of collateral circulation by utilizing the modified Allen’s test. Demonstrates correct method of placement
o Attain proper positioning and stabilization of wrist to allow puncture against the direction of the arterial flow
o Prep skin puncture site with iodophor prep pad and alcohol o Puncture skin with a needle at a 15 – 45 degree angle. Using the shallower angle for the smaller
infants or more superficial artery. o Advance needle slowly to puncture artery. o At the onset of blood into the tubing attach a syringe and gently aspirate to collect sample o Occlude artery with pressure just proximal to puncture site and withdraw needle o Apply pressure to site for five minutes or until hemostatis is ensured

Notify the Primary Care Provider, Physician’s Assistant and/or Nurse Midwife of the results
Take the solvent vial by its cap and shake well to make sure all liquid in the vial has dropped on the bottom.
Open the solvent vial and put it in a vertical position.
Obtain sample of vaginal secretion by using a sterile polyester swab.
Ensure that the swab tip does not touch anything prior to insertion.
Hold the swab in the middle of the stick and, while the patient is lying on her back, carefully insert the tip of the swab into the vagina until the fingers contact the skin no more than 2-3 inches (5-7 cm) deep.
Withdraw the swab from the vagina after one minute.
Place the polyester tip into the vial and rinse the swab in the solvent by rotating for one minute.
Remove and dispose of the swab.
Tear open the foil pouch at the tear notches and remove the AmniSure® test strip.
Dip the white end of the test strip (marked with arrows) into the vial with solvent for no less than 5 minutes and no longer than 10 minutes. (Strong leakage of amniotic fluid will make the results visible early approximately 5 minutes but with a very small leak will take the full 10 minutes).

test region (T). A negative results means that H.pylori IgG specific antibodies were not found in the sample or are below the detection limit of the test.
b. Positive -Two distinct red lines appear. One line should be in the control region ( C) and another line should be in the test region (T). A positive result means that H. pylori IgG specific antibodies were detected in the sample
Invalid – No line appears in the control region ( C). If this occurs, read the directions again and repeat the
test with a new test strip. If the result is still invalid, stop using the test kit and contact Inverness Medical
Technical Support at (800)637-3717
Allow the test device, sample, buffer and controls to reach room temperature (15-30 degrees C.) before testing.
Remove the test device from the foil pouch and use it as soon as possible. For best results, perform the test immediately after opening the foil pouch.
Place the test device on a clean and level surface.
For Whole Blood (Venipuncture) samples: o Hold the dropper upright and add 2 drops of whole blood (about 50 uL) to the sample well (S) of the
test device. o Then add 1 drop of Sample Buffer to the sample well. o Start timer. o For Whole Blood (Fingerstick) samples: o Add one capillary tube of blood (about 50 uL) to the sample well (S) of the test device. o Then add 1 drop of Sample Buffer to the sample well.
Start timer. Wait for the red line(s) to appear. The result should be read at 10 minutes. The background should be
clear before the result is read. NOTE: Low levels of H.pylori IgG specific antibodies might result in a weak line in the test region (T) after a
long period of time. Do not read the result after 15 minutes.

Place monitor on a flat, stable surface.

The monitor will prompt you to confirm the strip’s code.
Open a test strip foil pouch and remove test strip
Insert the test strip into the monitor with “INRatio” readable at the bottom When a 5 digit combination of letters and numbers XXXXX appears on the display, check that this strip code matches the
Strip Code printed on the test strip foil pouch.
If it matches, press the OK
If it does not match, press the up/down arrows to select the correct digit, and press OK after each digit is entered.
After the fifth (5th
) digit is entered, check that the entire strip code matches the one on the foil pouch, and then press OK.
The monitor will count down for a few seconds and display the temperature symbol (a thermometer) as it warms up for the test
The monitor will beep and the display will prompt you to apply a blood sample (blood drop over a test strip).
The green sample target light will appear through the test strip sample well.
To improve circulation, prepare the hand by warming (using a hand warmer, rubbing hands under warm running water or
by rubbing hands briskly together).

Select a finger-stick site on a middle finger, on the side of the finger tip that faces the floor Clean the selected finger tip with alcohol. Allow to air dry completely.
***NOTE: Do not puncture the finger tip until the blood drop and test strip symbols appear on the INRatio2 Monitor Screen***
Perform the finger-stick, following the lancet instructions and the directions in the User’s Guide.
***NOTE: Avoid excessive “milking” (repetitive pressure) of the finger. Do not wipe away the first drop of blood Do not move the monitor during or after sample application***
Collect a large hanging drop of blood (approx. 15ul) by using gentle continuous pressure. Apply the blood to the sample well on the test strip above the green light by lowering the finger until just the tip of the blood drop touches the sample well on the test strip.
Wait while the monitor performs the test and calculates the results (approximately one (1) minute). The monitor will beep and the results will appear on the display when the test is complete. If an error message appears,
see User’s Guide Troubleshooting Section for proper action. Dispose of all materials that have come in contact with blood following proper infection control guidelines.
Reporting Results: Results are reported in units equivalent to the plasma reference method. Results may be displayed as INR units
alone, INR + PT seconds, INR + QC, or INR + PT + QC. The measuring range of the INRatio2 System is 0.7 to 7.5 INR units. Repeat testing. Any INR greater than 7.5 must
be sent to the main core lab for analysis. Expected values: Normal Range. The normal range for the INRatio2 System was determined by testing 126 normal (not anticoagulated) subjects: INR: 0.7-1.2 INR PT: 6.5-11.9 seconds
Critical Result: Refer to Critical Result Reporting Policy 4.5 or greater
Limitations of the procedure: The INRatio2 system is designed for use with fresh capillary whole blood. Plasma or anticoagulated whole blood or
venous blood should NOT be used. The drop of blood must be a minimum of 15 ul.
In vitro studies show the INRatio2 System to be sensitive to levels of heparin and low molecular weight heparin.
This test should not be used for patients on heparin therapy. Hematocrit ranges between 30-55% will not affect test results.
In vitro studies show no significant effect in samples containing up to 20 mg/dl of bilirubin and 500 mg/dl of
hemoglobin (hemolysis). No significant effect was seen in samples containing up to 1500 mg/dl of triglycerides (lipemia).

urine and remove immediately to avoid dissolving out reagents.
While removing, run the edge of the entire length of the strip against the rim of the urine container to
remove excess urine. Hold the strip in a horizontal position to prevent possible mixing of chemicals from
adjacent reagent areas and/or contaminating the hands with urine.
Compare reagent areas to corresponding Color Chart on the bottle label at the time specified.

Urine Pregnancy Test
Competency Statement: Care Delivery: Demonstrates the ability to perform QuickVue One-Step HCG Urine Test which is a sensitive immunoassay for the qualitative detection of Human Chorionic Gonadotropin (HCG) in urine for the early detection of pregnancy.

JCAHO requires a temperature log to be used to ensure all reagents and test kits are kept at a suitable temperature. (59-86°F. – 15-30°C.) out of direct sunlight.
Notify the Primary Care Provider, Physician’s Assistant and/or Nurse Midwife of the results.
This test requires blue-spectrum differentiation and should not be attempted by individuals with blue
colorblindness. If you have not been tested for colorblindness, contact Employee Health before proceeding.
QuickVue® Urine hCG kit is to be stored at room temperature (15-30°C), out of direct sunlight. Stable until the expiration date listed on the box.Urine hCG controls should be stored at room temperature and must be at room temperatore prior to testing; stable until expiration date on box.
The person who sets up the test must also read the test and report the results.
Remove a QuickVue Test Cassette from the foil pouch just before use and place it on a clean, dry, level surface.
Label the cassette with patient name or control identifiers.
1 ml of urine collected in a clean, dry plastic or glass container is acceptable but first voided, morning specimens are usually the best because they typically contain the highest concentration of hCG. Bloody urine specimens and specimens exhibiting visible precipitates should be centrifuged and the supernatant used for testing. Specimens must be at room temperature prior to testing. Specimens must be gently missed prior to testing. (Note: A 70 ml urine specimen will take about 1 ½ hr to reach room temperature from chilled 8° C.)
Using a transfer pipette supplied with the kit, dispense three (3) drops of urine specimen into the round sample well on the Test Cassette. The Test Cassette should not be handled or moved until the test is complete and ready for reading.
Note: A new transfer pipette is used for each specimen and control.

Urine specimens: Read result at 3 minutes. Results read after 3 minutes are to be considered invalid and the test must be re-run. This should be noted in the test log.
Record the results on the Urine hCG QuickVue® Test log
Positive results – the test is positive if one red colored line appears in the T (test) region and one blue colored line appears in the C (control) region .
Note: The line in the T (test) region may be lighter, darker or the same intensity as the line in the C (control) region.
Negative results – the test is negative if a blue colored line appears only in the C (control) region.
Equivocal results – if uncertain whether even a faint strip of color has developed in the T (test) region, don’t attempt to decide by committee. Call the test “Equivocal” and suggest retest on a follow-up sample.
Unreportable results – the test is invalid if no blue colored line appears in the C (control) region even if a red colored line appears in the T (test) region.
“Expected” or “normal” results are completely dependent upon the clinical circumstances. Healthy males should be “negative”.
Quality Control: 1.Internal Control – described in the test procedure above, is a function check endogenous to the analytical system, which provides effective, real time, quality control on a unit use basis. For every single analysis, Testing Personnel will verify with a check mark in the space provided on the requisition that the blue colored line appeared in the C (control) region of the device and the background was clear.
A. Internal Positive Control – The appearance of a blue Control Line in the C region of the device is a positive procedural control. Correct procedural technique, specimen flow and device performance are confirmed when a blue colored line appears in the C (control) region of the membrane. If the blue colored line fails to appear in the C (control) region, the test result is invalid.
B. Internal Negative Control – A clear background is an internal Negative procedural control. The background color should be white to light pink and should not interfere with the reading of the test result. If a more intensely red background color appears, it may interfere with the ability to read the test result, therefore the test should be repeated.
C. If the endogenous control fails, do NOT report the patient result! The specific failure will be documented by leaving the appropriate check box on the Log blank or empty. Notify your supervisor and/or POCC. Indicate in the designated Urine hCG QuickVue® Test log who was called along with the date and time of the call.
2.External Controls: A. CMS “Waived status
B. Nursing personnel authorized to perform the testing will run the positive and negative urine hCG controls on a rotational basis for each lot number of each shipment received regardless of whether the lot number has changed. Nursing Service or the Supervisor will ensure that those individuals who are authorized to perform the test run the assigned controls on a rotational basis.
C. These control trials must generate the expected reaction response.

Record results on the Urine hCG QuickVue® Test log. D. If a control fails to generate the expected reaction response, do not use the kit; complete the following
procedure: c. Repeat the Q.C. test d. Document the failed test result and repeat test on the designated laboratory test log. Notify the
Supervisor and the POCC immediately. e. Indicate in the designated Urine hCG QuickVue® Test log who was called along with the date and
time of the call. 3.When a new kit is opened, record the lot number in the designated laboratory test log. Check the new kit lot # against the previous lot #. Indicate on the log if the lot # is the same as the current lot or if it is a new lot #. Per KDMC Laboratory’s Quality Control Procedures and JCAHO Standards WT 1.3-1.5, Two (2) levels of an external control must be tested with each new QuickVue® hCG Urine kit and on each day of patient testing.
