Declaration I hereby accept appointment as an Authorised Person(LV) for the systems, installations and locations indicated above. I acknowledge receipt of certificate No WS 015 As my authority to act, while on duty, as an Authorised Person(LV) for the systems, installations and locations indicated on that certificate. I note that, while on duty as an Authorised Person(LV), I will be responsible for the practical implementation of the company’s safety policy for the LV systems and installations for which Skanska Rashleigh Weatherfoil has control, and for which I have been appointed. I will to the best of my abilities follow the procedures for LV systems, as set out in the Skanska Rashleigh Weatherfoil “Operational Health and Safety Manual” and any written local variations notified to me by or agreed with the Authorising Engineer. Job Profile for an AP (LV)
• Prepare, revise and update the site-specific electrical safety plan.
• Appoint CP (LV), ensure CP (LV) carries out his duties and is given appropriate support and guidance.
• Appoint CP (LV), and supervise his work and / or take on his duties.
• Approve all method statements and associated risk assessments with work to be carried out on electrical equipment and systems.
• Approve, issue, cancel all electrical permits.
• Carry out all major LV switching operations in a safe energisation procedure.
• Maintain a system for the issuing and return of access keys with a key allocation register, together with a
lockable key box, accessible only by AP (LV), that contains all keys to areas subject to access control permits, and all keys to the safety locks fitted to the un-energised switches, isolators.
• Control the safety of LIVE equipment or systems when the area where equipment or system is located is
not subject to restricted access control.
• Ensure all earthing arrangements are in place.
• Ensure all safety signs and notices are in place.
• Monitor the work being carried out under the issued permit.
• AP (LV) to be in attendance on site at all times while electrical permits are being carried out.
• Ensure all permits are closed at the end of each day with keys returned.
• If permits are not returned at the end of the day, the AP (LV) will take actions to ensure all personnel have left the place of work and the area is in a secure and safe condition.
• Vet all electrical workers are appropriately competent for their assigned work.
• Maintain a permit system register / logbook with copies of all issued and cancelled permits
• Ensure on-site training takes place.
• Maintain and Display all work in progress with up to date location drawings, single line diagrams, circuit
schedules suitably marked to indicate LIVE equipment / systems.
• Issue safety violation notices and implement disciplinary action for failure to observe the requirements of the safety policy, plan, procedure or permit.
• The AP (LV) will adjudicate on all matters electrical and his decision is final.
• Make notes, take photographs and maintain records of all electrical accidents and incidents.
• Where a trade contractor is required to control parts of LIVE electrical equipment or systems , the AP
(LV) shall have the additional responsibilities:
• Ensure that the trade contractor provides a safety plan and procedures document for the parts of the system under their control to the approval of Skanska AP (LV).
• The trade contractor’s safety plan is in addition to the Skanska site electrical safety plan.
• Put in place and operate a system of management for ensuring that the trade contractor carries out his
work in full compliance to his safety plans and procedures.
• Undertake an auditable procedure for monitoring the operation of the trade contractor’s permit system.
