ADPI JIM PAGE MEMORIAL SCHOLARSHIP

DESCRIPTIONA one year grant, of $1,000 minimum, awarded to the dependent of an employee or to an employee

whose company is a member of the American Dairy Products Institute.

REQUIREMENTS/CRITERIA1. Applicants will complete and file an official application and any required materials by the deadline

date as pre-determined by the American Dairy Products Institute Scholarship Committee.

2. The American Dairy Products Institute Scholarship Committee for the selection of this award shall

include a member of the American Dairy Products Institute Board of Directors, Affiliate Committee

and the Chief Executive Officer of the American Dairy Products Institute.

3. The recipient of the scholarship will receive a letter of congratulations and a check, made payable

to the school they are currently attending, from the Committee and their name will be announced

at the ADPI/ABI Annual Conference to be held in Chicago, IL.

4. This award may be used for any educational expenses that a student may have in furthering

his/her education. (Such as, but not limited to: tuition, books, room & board etc.).

5. Application and attachments must be typewritten and can be submitted by E-mail, fax or US mail.

6. Award of the scholarship in any given year is strictly subject to the discretion of the ADPI

Scholarship Committee.

RECIPIENTA. Must be a dependent of an employee or an employee whose company is a member of the

American Dairy Products Institute.

B. Must be currently enrolled in a four-year, nationally-accredited college or university, pursuing a

course of study that will lead to a bachelor's degree, have completed at least one full semester (or

the equivalent thereof) and must have a cumulative GPA of at least 3.0 on a standard 4.0 scale.

C. All qualified persons who submit an application form would be eligible for the scholarship

regardless of gender, race, creed, color or nationality.

piece of paper.

2. A signed letter of character reference, from a person unrelated to the applicant, must be

submitted with the application.

3. A current, official transcript must be submitted directly from the college or university the

applicant is currently attending.
