Earlier this year, Brenner FIT® celebrated 10 years of helping families in North Carolina and beyond lead healthier lives. I think this is an amazing milestone given our journey in the past decade. Brenner FIT started out as a clinical program: a doctor, dietitian, nurse, family counselor, physical therapist, and an administrative secretary. We learned quickly that helping families navigate an unhealthy world required us to expand beyond a clinical setting.

Brenner FIT has grown from an outpatient clinical program to encompass so much more: a Spanish-language clinical and outreach program; locally- and nationally-recognized research; community engagement and outreach; experiential cooking and activity programs; telemedicine; undergraduate, graduate and professional medical education and internships; volunteer programs, culinary medicine… the list goes on and on. But at the heart of Brenner FIT, we remain a clinical program focused on helping families lead healthier lives through evidence-based, family-centered care.

Ten years and going strong — we could not have done this alone. We are blessed to work with dedicated and equally passionate partners. This anniversary year, we accomplished some amazing goals thanks to our partners. Most notably, we strengthened our partnership with Northwest Area Health Education Center (NWAHEC) to further develop and enhance our Culinary Medicine program, which trains health professionals in “nutrition through cooking”. We also expanded our community-based cooking and educational classes. We could not have asked for a better partner, as we plan to collaborate even more in the years ahead.

Our work with Kohl’s Cares and The Mebane Foundation led to the development and launch of the Kohl’s Cooks Mobile Kitchen, allowing us to offer our highly successful cooking classes to various parts of the city. We have maintained our strong partnership with the YMCA of Northwest North Carolina, where the Brenner FIT Teaching Kitchen is housed. We have addressed hunger and food insecurity in our clinic through our partnership with Mary’s House of Greensboro, which has led us to working with Wake Forest Baptist Health’s Downtown Health Plaza and their Food Pharmacy, targeting food insecurity and other social determinants of health in a clinical setting. Individual partners, such as our medical student volunteers, allow us to expand the work we do while they gain valuable experience through service learning.

We do not look at the coming year as our 11th anniversary, but rather, as the second decade of our mission. As we endeavor to help children and families in our clinic, we will work side-by-side with our partners to fulfill the greater mission of Brenner Children’s and Wake Forest Baptist Medical Center: to improve health, to teach the next generation of clinicians, and to research new and innovative approaches that deal with childhood obesity. Join us in our mission to redefine pediatric obesity care as we aspire to set new standards in treatment, research, education, and community outreach. 0.5%Brenner FIT ClinicFor the past 10 years, Brenner FIT has provided clinical care to children, ages 2 to 18, and their families. The clinical team for each family includes a Pediatrician, Dietitian, Family Counselor, Activity Specialist and Physical Therapist, with a Family Support Specialist available as needed. Additionally, Spanish-speaking families are partnered with the Program Manager for Latino Health.

The treatment model focuses on family needs and schedules to find the right match of habit changes for them. Social Determinants of Health are a daily reality in this program. The ability to follow through with habit change is largely determined by other factors outside of the children’s and their families’ desire to make a change. Brenner FIT works with each family to address some of these challenges, such as transportation, food availability, medical or mental health needs, and recreational opportunities. Brenner FIT’s families benefit from the variety of expertise provided by their treatment team, as well as the classes offered to support clinical treatment goals.

Experiential Education

In addition to one-on-one treatment in clinic, Brenner FIT families are encouraged to participate in experiential education through nutrition/cooking classes, activity events and parenting classes. Through these hands-on experiences, families enhance their cooking skills, sit down to eat meals together, get active by playing together, and learn parenting tools to use at home between clinic visits. Classes designed just for Brenner FIT families help to create a supportive relationship between parents and children as they navigate making health habit behavioral changes at home.

Clinical Teams (English and Spanish)Two teams of experts provide treatment and support for English-speaking Brenner FIT families. Through the teams’ focused efforts, more families remained in the program, as compared to the prior year.

A third team of bilingual experts provides treatment and support for Spanish-speaking Brenner FIT families. Taking into consideration multiple sociocultural barriers, such as language differences, transportation challenges, work schedules, navigating a new culture and holding onto familial traditions, this team provides space and support for families to seek treatment in Spanish through individual visits, group trainings, and telemedicine.

Brenner FIT en Español is proud to share that this year, clinic capacity increased by almost 76%, compared to last year, through the creation of a new intake protocol that did not incur additional costs, nor required the hiring of more staff.

Quality Improvement Activities

While Brenner FIT managed across-the-board participation increases in fiscal year 2017, there was an increase in families dropping out before completing the program. Brenner FIT took care to assess retention carefully and improved this measure for fiscal year 2018.

Through phone surveys with parents, mock intake assessments to analyze the intake visit, and data review that identified the majority of families dropped out right after intake, the team identified the need for more contact with families after intake and better alignment of the Brenner FIT intake process with treatment concepts. After making these changes, retention measures improved.

Quality Improvement Activities include:

• Assessed root causes of increased attrition in FY17. Implemented changes and demonstrated improvement of 15.5%

• Centralized the referral process within the team to a single person for both English and Spanish language programs

• Revised the Brenner FIT en Español treatment program to respond to the long waiting list by increasing access to intake appointments — this was implemented in June 2017

TeleFIT TeleFIT is an innovative telemedicine program that extends Brenner FIT’s proven approach to families who are looking for support outside of Winston-Salem. In addition to two Brenner Children’s satellite clinics, Brenner FIT’s telemedicine program exists in six primary care clinics and two public health centers north and west of the Piedmont Triad.

In fiscal year 2018, the TeleFIT program received 26 referrals. Of those referrals, 21 families attended orientation in Winston-Salem or by phone with a Brenner FIT phone coach, 20 families attended FIT 101 and 21 families completed initial intake visits, with 12 of those families still active in the program. Although there were fewer referrals than last year, these statistics demonstrate that 80% of the families who were referred began intensive treatment; whereas, only 36% of the families entered Brenner FIT’s treatment program the previous year.

Metabolic Syndrome and Prevention Clinic This clinic meets the needs of pediatric patients requiring specialized management of lipids, glucose, metabolic syndrome, and abnormal weight. Dr. Skelton leads this clinic and offers medical visits along with a Brenner FIT registered dietitian. In October 2017, a satellite clinic office opened in Greensboro to meet the needs of patients who desire care closer to their homes. In addition, medical and dietitian visits are offered in Spanish. Patients are followed as long as needed and may participate in Brenner FIT for a more intensive treatment option, if they qualify. This year, the clinic completed 249 visits, and return patient visits increased by 59%.

Kohl’s CaresIn 2017, Brenner FIT was awarded a two-year grant through Kohl’s Cares. The grant focuses on four key areas:

• Teach nutritional competency

• Share Brenner FIT’s expertise with rural communities

• Train medical professionals

• Strategically use social media to promote awareness of Brenner FIT within the community

Through the Kohl’s Cares grant, Brenner FIT designed and launched a mobile cooking class called Kohl’s Cooks. Brenner FIT Academy was introduced in different areas of Wilkes County to share Brenner FIT’s expertise with rural communities, and Dr. Skelton, along with the Brenner FIT Team, led experiential cooking activities and didactic presentations for medical students to create a long-term change in the community. Brenner FIT filmed 15 nutrition and recipe videos this fiscal year, which reached more than 45,000 Facebook users. Social media advertisements, designed to promote awareness of Brenner FIT, reached more than 700,000 people. These four strategies will continue to be the focus of the grant in year 2 with the goal of improving nutritional competency of community members and care providers by 10%.

Northwest Area Health Education Center of North Carolina Brenner FIT has had a long-standing partnership with Northwest AHEC. This year we completed the first year of a larger partnership with Northwest AHEC. Throughout this inaugural first year, summer camp was held and wrapped up in August, community nutrition education classes were executed under the partnership agreement, and a culinary medicine program was created with 3 full pilots successfully executed. The next steps in this partnership are:

• To collaborate and develop a family obesity certification and learning network for area physicians

• To strengthen Brenner FIT’s volunteer and internship program

• To improve community engagement by focusing on community requests

• To continue offering and perfecting the Culinary Medicine continuing education activities

Kohl’s Cooks Mobile Kitchen This year, Brenner FIT formally launched an exciting community outreach initiative called the Kohl’s Cooks Mobile Kitchen. This initiative, made possible through a Kohl’s Cares grant and the Mebane Foundation, takes cooking instruction into the community where people live. The Mobile Kitchen is staffed by a community health program manager and a Brenner FIT program assistant — both of whom are bilingual, as well as two chefs — one of whom is bilingual. After designing and purchasing the van, installing necessary items such as the lift, cooking equipment, and four state-of-the-art mobile cooking carts, the Kohl’s Cooks Mobile Kitchen kicked off its first community cooking class in March 2018.

Since then, the Kohl’s Cooks Mobile Kitchen van has been on the move in the Winston-Salem/Forsyth County area, logging more than 700 miles, traveling to area clinics, schools and universities, and Wake Forest Baptist Medical Center.

The Kohl’s Cooks Mobile Kitchen travels directly into neighborhoods and communities to reach those who cannot access resources. Classes target lower-income families who live in the zip codes of 27101, 27105, and 27107. Through mobile kitchen education, Brenner FIT coordinates and delivers cooking classes; provides hands-on instruction to participants learning new cooking skills and recipes; and builds participants’ confidence in feeding their families.

During its first six months, the Kohl’s Cooks Mobile Kitchen held fourteen community cooking classes, six of which were offered in Spanish. Families who participated in the Kohl’s Cooks Mobile Kitchen classes learned about nutrition and increased their cooking skills in these hands-on cooking classes led by a Brenner FIT chef. Meals have included turkey sloppy Joes with roasted asparagus and watermelon slices, black bean taco pitas, and summer veggie burritos.

Brenner FIT Teaching Kitchen Brenner FIT had another successful year in the Teaching Kitchen at the William G. White, Jr. Family YMCA, offering classes to community members. Community cooking classes consisted of a mixture of hands-on classes, as well as demonstration-based classes, to learn specific nutrition and cooking skills. Community members were taught how to prepare quick vegetable ideas, balanced breakfasts, a weekly meal plan and more.

Partnership

Brenner FIT continued partnerships with groups within Wake Forest Baptist Health and the William G. White, Jr. Family YMCA to expand the utilization of the Teaching Kitchen. These include:

• Weight Management Center

• Outpatient Nutrition

• Medical students within the Wake Forest School of Medicine

• YMCA staff – YMCA staff completed training in the Teaching Kitchen, as well as the Brenner FIT approach, and led monthly cooking classes for community members

Wake Cooks

In November, Brenner FIT piloted cooking classes at the Teaching Kitchen called Wake Cooks. Designed as a team-building cooking class, Wake Cooks was marketed to various Wake Forest Baptist Health departments. The pilot ended in April 2018 after completing 11 classes across 10 departments. Feedback from the pilot classes was analyzed to create a more formal program, which will launch in the near future.

COMMUNITY ENGAGEMENT

Brenner FIT Academy Brenner FIT Academy is a six-month program designed by Brenner FIT for families with children (ages 2 to 18) who have concerns about a child’s weight and health.

This program increases families’ access to Brenner FIT by training community organizations in Brenner FIT’s family health and wellness approach. The Wilkes County Health Department completed their fourth offering of the Academy in August 2018 (first two sessions were pilots). With financial support from the Kohl’s Cares grant, Brenner FIT revised the Academy curriculum to reflect advances in understanding the best ways to help parents and families make health habit changes.

Additional Community Outreach and Education Latino Community

Brenner FIT is able to offer group classes, led by Ms. Angelica Guzman, at the LCS Buen Pastor in Winston-Salem. Classes focus on empowering parents through education and development of support networks. At the end of eighteen sessions, 12 participants graduated with enhanced parenting tools and the support of one another. In the Brenner FIT Teaching Kitchen and at Amos Cottage Gym, Brenner FIT en Español staff provided 55 classes to 397 participants, a 25% increase in reach.

Food Insecurity

Brenner FIT maintains a partnership with Mary’s House of Greensboro and the Produce Box Company. One hundred and nine families received frozen meats from Mary’s House and/or fresh produce from the Produce Box Company. The Produce Box Company donated 74 community boxes filled with a variety of local fruits and vegetables to families looking for ways to increase the variety of foods offered in family meals and snacks.

Brenner FIT staff, volunteers and interns continued to support the staff and residents managing the Pediatric Food Pantry at the Downtown Health Plaza (DHP) through the transition to become the Food Pharmacy. During that transitional period, Brenner FIT provided up to 4 hours of assistance each week with essential tasks such as grocery shopping, stocking shelves, packing bags and tracking inventory. The Food Pharmacy increased the amount of bags distributed from the pantry from 60 to more than 100 bags per month. Brenner FIT staff now serve as a resource to the Food Pharmacy Care Coordinator who manages the food pantry and assists families identified by the clinic as chronically food insecure.

Volunteers

In order for Brenner FIT to provide experiential health education, committed volunteers are essential. Each year Brenner FIT relies on more than 1,200 hours of volunteer time to help with and lead clinical and community programs. Essential volunteers include community members, Wake Forest medical students and students from other area universities. Medical Student Education Educating future physicians on the prevention and treatment of childhood obesity is a key focus of Brenner FIT, particularly in identifying weight-related comorbidities and preventing weight bias. Through the Kohl’s Cares grant, Dr. Skelton taught medical students through 10 hands-on cooking classes, which focused on basic culinary skills and nutrition knowledge. In the classes, 30 first- and second-year medical students created affordable, quick, and tasty meals. The cooking classes included lessons in knife skills, roasting vegetables, and even grilling. With the support of Kohl’s Cares, Brenner FIT also reached 67 third-year medical students by doing a multidisciplinary presentation every other month to improve students’ knowledge of childhood obesity treatment.

PROFESSIONAL EDUATION

As part of an academic medical center, Brenner FIT is dedicated to educating the next generation of healthcare providers. In fiscal year 2018, Brenner FIT provided 28 presentations to 700 students and medical professionals.

Culinary Medicine In partnership with Northwest AHEC, Brenner FIT created a Culinary Medicine continuing education course in September 2017. Culinary Medicine was developed to provide evidence-based education around nutrition through hands-on cooking experiences, didactic presentations and cooking demonstrations with Chef Jeff Bacon, a local professional chef with the Triad Community Kitchen. The continuing education course is intended for medical providers, such as doctors, physician assistants, nurses and nurse practitioners. The knowledge and skills gained from the course offer practical approaches to healthy cooking and meal preparation for both providers and their patients. Since beginning this program, Brenner FIT has completed two pilot courses and one official course, educating close to 100 medical providers from around North Carolina.

Internships Brenner FIT worked with 8 different colleges and a variety of majors to help educate both undergraduate and graduate school students on childhood obesity treatment. This year, 13 interns worked alongside Brenner FIT staff dedicating approximately 2,360 hours. CollaborationsBrenner FIT has maintained active research collaborations locally and nationally. In fiscal year 2018, Brenner FIT actively partnered with researchers in Public Health Sciences, Family and Community Medicine, and the Weight Management Center. Nationally and internationally, Brenner FIT has worked with Duke University, The Ohio State University, and the University of Alberta (Canada). Listed below are highlights of Brenner FIT’s collaborative research activities:

• Sugar sweetened beverages as a vital sign: Dr. Skelton worked with Principal Investigator Dr. Kristina Lewis in instituting a screening process for sugar sweetened beverage intake into primary care practices.

• Weight loss and the family system: collaboration between Ohio State and Wake Forest Baptist Weight Management Center to investigate how spouses and children influence weight loss attempts by parents.

• Y-HIIT: an exploratory study of using games to increase the physical activity of children with obesity. In partnership with Family and Community Medicine.

• Las Familias: focus group study, also with Family and Community Medicine, studying the use of technology and family-based weight management.

Obesity CareBy leveraging Brenner FIT’s expertise in family-based weight management, many projects are focused on improving the care of children with obesity, as well as extending Brenner FIT’s family-based approaches to the community. Projects from this past year include:

• Predictors of engagement with Brenner FIT: retrospective project with Dr. Callie Brown studying what predicts patient engagement after referral.

• Impact of summer cooking camps on family meals: community-engaged study of the Brenner FIT teaching kitchen, exploring if teaching children recipes will influence home meals.

• Weight stigma in inpatient settings: collaborative project with Dr. Elizabeth Halvorson evaluating the potential impact of weight bias on inpatient care.

• Food is Health: observational study of intervening on food insecurity in a primary care setting.
