MONTHY FILE MAINTENANCE: A 6 HR outage of CMS, the VRU, and the WEB will occur on 01/23. The outages are planned to occur between 00:00 AND 06:00 local SDF time on those dates.

IVR FUNCTIONALITY REMOVAL: With the recent removal of some of the IVR options, please visit the Flight Operations website for the new procedures pertaining to the IVR options that have been removed. Please send an email to [email protected] if you have any questions or comments. In Addition, please see the Flight Operations website for information on future IVR functionality removal. FLOW PROCESS ERRORS – It has been brought to our attention that some crewmembers have experienced “flow process” errors on the web when submitting LITT requests. This is most likely due to multiple requests being submitted simultaneously. To avoid this problem in the future, please submit only one LITT request at a time.

FIRST CLASS MEDICALS – All crewmembers including the First Officer must hold a first class medical if operating a 2+1 augmented international flight segment or is part of a double-crew. Previously only the Captain and the IRO were required to have a 1st Class Medical. LITT & INTERNATIONAL TRIPS – During LITT, First Officers who trip trade for a trip that contains an International flight segment that requires an augmented crew or double crew within the trip needs to select FFP if the crewmembers 1st Class Medical is set to expire prior to the end of that trip. Schedule Requests during LITT will not approve any waivable legalities unless FFP has been selected within the request.

BIDDING 1. BIDDING: WE STRONGLY ADVISE YOU TO CHECK YOUR BIDS ON FILE THROUGH THE WEB AND REQUEST A COPY OF YOUR BIDS. When bidding through the WEB, be certain to record the “bid verification number” which is provided with each bid entry. Contests to a bid require a “bid verification number” that is received upon placing your bid on file. To contest your line bid award, call the Crew Planning Transition Team and speak to any of the transition schedulers or leave them a voicemail. See the cover letter calendar for cut-off times. You are responsible for your bids on file! Insufficient or erroneous bid choice entries may result in non-desirable assignments. PLEASE BID CAREFULLY!! a. VTO, IOE Support AND BLANK RESERVE LINE BIDDING: Please note that VTO, IOE Support and Blank Reserve lines may only be printed once in the bid package but may have a range of lines (ie Lines 112 – 124). On the prescribed dates (see cover letter calendar) all constructed VTO, IOE Support and Blank reserve lines will be available for bid by those crewmembers who were awarded a VTO, IOE Support, or Blank reserve line in the initial schedule award process. Copies of these secondary lines will be available on the Flight Ops web page under Crew Applications Data / CMS Main Menu. If you view these lines on the web you will be able to see the detail of all the pairings. Once you have reviewed these secondary lines, you must submit your preferences using the “VTO Bid” tab under the Bidding options on the WEB. The Secondary LINE AWARDS will be available on the “View Bid Awards” option on the web, following the Schedule and Training Awards, per the timetable on the cover letter calendar. Please be sure to submit sufficient bid choices, and remember that the secondary line numbers always begin with line 1, not your initial Line Award. The Secondary Line Award number that you receive as your initial schedule award does not correlate to your relative seniority as compared to other Secondary Line bidders. Refer to the cover letter calendar for dates and times of the VTO line adjustment periods. b. Support Lines. These lines will be published as Blank Lines and will be populated with Call Reserve assignments (RICA Reserve IOE Call A-side, RICP Reserve IOE Call P B-side). Each will contain 14 duty days per pay period. These secondary lines will be built and awarded during the VTO award process (see coverletter calendar) Flying assignments will be made from the IOE CPT’s Schedule where support of flight legs >8hrs is needed to perform IOE. If you are awarded one of these lines please contact George Coleman for MD11 or Debbie Nagel for 747-400 in Transition/IOE Scheduling. VRU options (4 : 6 : 1) c. ONE-PAY PERIOD VTO’S: One pay period VTO lines may be published on some fleets and seats to utilize excessive available open-flying which results from the difference of planned block hours and/or staffing variables between pay/bid periods. A simple explanation is that one of the two pay periods will contain scheduled flying and the other pay period will be published without assignments (blank). Using the time table outlined in Article 13.C.3, those awarded 1 pay-period VTO lines will have the opportunity to bid for the corresponding pay period’s VTO schedule from identified choices within the published VTO schedules. (See Cover letter calendar for availability dates). Please note that the originally awarded flying contained in the published line is retained, you only bid for the corresponding “VTO” pay period. When viewing published VTO lines, one of the two pay periods will contain ”XXX” on each day of the schedule, the other pay-period will contain assignments. You may select choices that coincide with the VTO status pay-period of your original award... you may not bid for portions of the regular 2 pay-period VTO lines. PLEASE NOTE: Numerous departure time and routing changes take place during the transition process. A VTO line could contain a pairing that has been modified since it was originally published in the bid packet. Please be certain you confirm all departure times once you obtain your final VTO line for the bid period. d. HOT STANDBY: If you are an FFDO and are awarded hot standby, ensure you reference your FFDO SOP before reporting for duty for important information regarding FFDOs on standby status.

e. BIDDING FOR PAY PURPOSES ONLY: Certain crewmembers that are in long-term training, acting as instructors, or have been bypassed for a system award, have the option to bid for pay and day off protection. . If you are eligible to Bid in a Pay Only status, you will receive a letter in your v-file on the same day that the bid packets are distributed. This letter will detail your bidding options as a pay-only bidder. This letter will describe pay-bid fleet and seat options, and will direct the pay-bidder to submit their pay-bid via the WEB or via FAX/e-mail. Pay-only bid awards are completed by the 2nd Friday following the regular line awards, at which time a web notification is posted to your CMS Main Menu page confirming your pay-only bid award. Please allow us this time to complete all the Pay Awards. A copy of this pay-only bid award will be forwarded to the Quality Assurance group. It is important for all pay-only bidders to submit their bids during the same time frame as that which applies to regular line bidders. The Pay awards are processed against the entire population. Its purpose: "If I were Flying the line for this BID which line could my seniority hold" All questions regarding pay-only bidding should be directed to the IOE/Schedule Adjustment supervisor, (see Dept. Phone Listings). BY-PASS BIDDERS VACATION OPTION: Per Article 14.E.3.b. You must select “Apply vacation to bypassed line” during the Schedule Bidding process in order for your vacation to be applied as a Pay Only Status toward your Pay Only By-Pass Line. AGE 65 and RETIREMENT Bidding: Please note that if you are turning 65 and

retiring in a bid period for which you are still able to bid to fly, you will not be paid for the portion of flying on your bid line that occurs after you reach your 65th birthday (FAA mandatory retirement date).

2. SDF International Domicile Crewmembers: This is a reminder for First Officers who may be awarded a line containing IRO duties. Lines containing First officer only trips constructed to support IRO requirements are labeled as being available for First Officers only in the bid package. These lines are available for First Officers only and will not be awarded to Captains. IRO duties within trips are identified by the "IRO" comment (this is placed in the "DUTY" column of the trip) published in the bid package. One exception is when a trip carries-out from one bid period into the next bid period. Due to system limitations, not all IRO duties are identified for the carry out period of these trips and may be communicated to you at a later date. 3. ADMINISTRATION LIST: An updated seniority list per seat/fleet is supplied with the bid packages. Updates to the Seniority BID Lists are published on the Flight Ops web sight on Wednesday 01/06 and Friday 01/08. This list is to be used for schedule/pay bidding purposes only. The 'NB' code is used to designate a "no-bid" status. This code appears to the left of a pilot's name under the "CD" (code) column. If the "NB" code is present, that pilot is not expected (at the time of publication) to be bidding a schedule for the bid period. It is advisable to bid additional choices in case these pilot's status' change. There is a field which indicates with asterisks (**), crewmembers who are not projected to have achieved 75 hours by the beginning of the next bid period (field identifier is "<85"). (A 10-hour buffer is used, actual tracking is to 85 hours) These columns will also appear on the Bid Award lists. If you are new to a seat/equipment (requiring consolidation) and have not achieved, or are projected to receive 75 hours of flying by the first day of the bid period, you will not be awarded a line that would pair you with a consolidating captain. This is requirement by FAR "Crew Pairing" issue 121.438b, and IPA Article 13.L.3.d. In the 11 day period from when the bid package is sent to print until the closing of the Captain schedule award, some pilot's projection of completion may change. For this reason, projections are re-calculated at the time the First officer lines are awarded ("real time"). Viewing the Captains' Bid Award list to check on a specific pilot's Crew Pairing status on the day the captains award is posted may help First officers (<75) to reassess their bid choices on file and potentially avoid a non-desirable bid line award.

4. CARRY-IN TRIPS INTO RESERVE LINES: reserve days will be adjusted per article 13.D.10.f(1 - 3). If you call after office hours then leave us a voicemail. (see numbers listed on page 9 Department Directory)

Crewmembers must notify Transition Scheduling of any request to not move reserve days to cover a carry-in trip prior to Self Adjustments ending.

5. VACATION TRIP ADJUSTMENTS: Vacation conflicts are adjusted per Article 13.D.5 Per Article 11.E.7, you may elect to remain on a trip that extends up to (2) days into the vacation period. You must notify the Transition Scheduling Team at 502-359-5516 (1-800-228-2739 prompt 4 : 4 : 9) prior to the close of your respective seat’s adjustment period (see cover letter calendar). If you call after office hours then leave us a voicemail. (see numbers listed in Department Directory) Per ART 13.D.5.c: For conflicts with Vacation and/or Training you may request, after the bid lines are awarded but prior to the adjustment period, a positioning or de-positioning deadhead that would keep you on your original trip that does not transit the domicile. You must contact the Transition Scheduling Team at 502-359-5516 (1-800-228-2739 prompt 4 : 4 : 9) prior to your respective seat’s self adjustment period (see cover letter calendar). If you call after office hours then leave us a voicemail 6. TRANSITION “CREDIT DEFICIT” for REGULAR AND VTO LINE HOLDERS: Crewmembers that have an FAR, carry-in, training, or vacation trip conflict may be subject to reschedule to re-establish lost credit which precedes or follows these events, if the credit deficit for the pay period is greater than 5 hours. “Credit Deficit” information will be available on the WEB and will be displayed to the right of the Broadcast Message information. When your “seat’s” open time adjustment period begins (refer to cover letter calendar), your deficit information will be displayed (ie. "Credit Dropped for PP01 due to TRN or VAC Conflict is HH:MM… PP02 is HH:MM). You must access an open time report and submit open flying requests through the “Pick Up from Opentime Tab” option on the WEB. Credit can be restored only in the pay period affected. If you do not elect to submit requests for these adjustments, you will be subject to assignment during the transition period for balance of the trip/s dropped coded “DCRY”, “DTRN”, or “DVAC”. You may access your future schedule during your credit deficit restoration period. Outside of this period, schedules will remain unavailable until Thursday, 10 days prior to the new bid period. Vacation and training deficits must be restored to within 5 hours but cannot exceed the deficit amount. Deficits resulting from a carry-in conflict must be restored to not less, nor more than 5 hours. On the days the open time adjustment periods close, open time will be awarded in seniority order by fleet and seat for those crewmembers electing to self-adjust. REMEMBER, ALL REQUESTS MUST MEET CONTRACTUAL AND FAR GUIDELINES. "Important note: With the automation of deficit self-restoration, please bear in mind that your deficits are not final until the contractual times noted in Article 13.C.3.h. for captains and 13.C.3.i for first officers (1700 LDT eight days prior to the next bid period for captains, and five days prior to the next bid for first officers). Until these windows of time are reached, any deficit-restoration transaction(s) that have been posted to your line are subject to review and revision by the Company. For example, if your deficit is 20:00 and you self-restore for only 5:00 (and ASA approves the request and posts it to your line), the Company has the prerogative to 1) add more restoration flying to your line in addition to the 5:00 trip, 2) remove the 5:00 trip and replace it with a trip or trips with greater credit, or 3) leave it as it is. 7. TRANSITION SCHEDULE CHANGE ADVISEMENT: When each seat’s Transition Period ends, you may access your schedules to view all adjustments. Crew Scheduling will either make personal contact or leave a message at the Primary Phone Number listed in CMS, and the Secondary Phone Number if personal contact is not made at the Primary Number. If Crew Scheduling is not able to leave a message, then flagging the crewmembers line shall meet the notification time table of Art.13.C.3.h-i and 13.D.5.d. Please Contact Crew Scheduling to confirm any adjustments that may or may not have been made to your line during this period. To facilitate dissemination of this information, you may request a detailed schedule summary through the Flight Ops web sight. Please note that Crew Scheduling may not notify you for any flight departure schedule revisions that are less than 10 minutes. If you review your schedules during the transition period, please remember to review the cover letter calendar to determine when your adjustment period is complete.

8. TRIP TRADE / OPENTIME AVAILABILITY: When requesting trip trades through the WEB you will receive a confirmation number. If you do not get confirmation number, then your requests were not processed by the interface program and you should resubmit the request. As with all bidding options you must record your confirmation number. Please be certain you confirm all departure times of open time trips, as changes may have occurred after bid publication. Please see calendar on cover page for specific dates. Trade or pick-up requests for trips which cross over into the new bid period may be submitted after 2000 local domicile time 17 days prior to the next Bid. Open time may be viewed on the FLIGHTOPS web site. You can list open time by Pay Period for any seat/fleet. TRIP TRADES: When trading a trip with open time one request will include a series of trips to be dropped followed by a series of trips to be picked up. If any trips to be picked up are not available, C/M is not legal, or trade does not meet contractual guidelines, then the entire trade will be denied. All trip trades with open time are worked as ALL or NONE per request. It is best to submit a combination of separate trades in order for all possible combinations to be considered. TRIP TRADES used to restore Sick (ART 9.A.4) and/or Military (ART 9.G.3-4): You have two pay periods to restore sick or military after the pay period in which the leave occurred. You have the standard pick up from open time option or you may elect to trip trade and utilize the additional credit generated to restore your sick bank and/or military accruals lost. Requests for reserve day-off movements should not be submitted more than 10 days prior to the day(s) of reserve you wish to move to or from. Operational changes, illness, training requirements etc., require us to remain as close to the original published reserve schedules as possible. In most cases you can not break up a block of reserve duty. Art.13.B.3.b. Procedure for Trading VTO CA/CP days for Trips A VTO Lineholder who wishes to trade CA/CP reserve days for a trip may perform this function on the web utilizing the VTO Trade with Open Time option under Schedule Adjustments. The person working the trade will assume ALL requests submitted in this manner are trades of CA/CP for a trip and will approve or deny these based on the merits of the two requests together as if they are one. The Reserve days will be dropped with DTO (Drop Trade Opentime) and the Pickup will be coded PTO (Pickup Trade Opentime)Art.13.B.4.f. Domicile Hots : Hots can only be traded/picked up in an entire block with other standby line holders or open time. In addition, airport standby line holders may trade entire block(s) of airport standby for trips in open time or with other crewmembers. However you may not trade trips for blocks of hots unless the trade is with another crewmember. To review open time schedule adjustment requests use the “Review Requests” option on the WEB under Schedule Adjustments. During non-transition weeks all trades are generally completed by 1300lcl SDF. During transition weeks the volume of trades are high these trades are not completed until late in the evening (Transition weeks Jan 18 – Jan 30) Note: Open time pickup requests for multiple domiciles: Due to current software limitations, when a crewmember submits an open time pickup request that contains choices from more than one domicile, we are unable to determine which requested trip is the crewmember’s first choice. When you submit such requests, please contact the Transition Team Schedulers or leave them a voice mail message to clarify your intended sequence of trip choices. They may be reached at 502-359-5516 (1-800-228-2739 prompt 4 : 4 : 9). This is only necessary when trips from more than one domicile are part of the same request. LINE IMPROVEMENT TRADE REQUESTS, 13.D.1.i. OF THE AGREEMENT. Each seat has a day for line improvement trades with open time for the new bid period. Please submit only trip trades with open time on these days, as all open time pick-up requests will be denied. The intent is to improve ones line, it is NOT intended to have crewmembers significantly increase their credit time. SEE COVER LETTER CALENDAR FOR ALL DATES AND TIMES.

9. TRIP TRADES, OPEN TIME PICK-UPS & FNP/PNF NOTES: 1. Crewmembers who wish to cancel a previously approved PNF/FNP trip trade must contact the

Transition Scheduling department 2. Crewmembers who trip trade and or pick up open time will not be awarded two separate trips that can

be flown in one duty period without intervening domicile rest unless the “Reduce minimums and forfeit pay protection” box is checked

3. Crewmembers who have difficulty submitting an ASA request needs to submit a Schedule Request Exception Form under Schedule Adjustments on the Crew App Website.

4. Crewmembers who question the denial of an ASA request need to submit a General Inquiry under Schedule Adjustments on the Crew App Website. Please include the confirmation number associated with the ASA request you would like reviewed by Transition Scheduling. An email response will be sent as a response to the review.

10. JUMPSEAT ADMINISTRATION: Published UPS must ride jumpseat requirements for crew positioning are automatically booked 13 days prior to the flight. Personal requests for jumpseats for commuting or personal reasons may be booked 12 days prior to the flight for crewmembers, and 10 days for other qualified individuals. Please use the WEB or CMS terminals found in the crew ready room to book or delete any unneeded jumpseats. DST +4 Standard +5 11. RESERVE DAY DESIGNATION: 08MAR15 –01NOV15 01NOV15 – 13MAR16 “RSVA” 2400 – 1159 Local Domicile Time “RSVB” 1200 – 2359 Local Domicile Time “RSVC” 1600 – 0359 Local Domicile Time “RSVD” 0400 – 1559 Local Domicile Time

“RSCA” 2300 – 1100 Local Domicile Time beginning the day prior to going into a reserve day. “RSCP” 1100 – 2300 Local Domicile Time PLEASE NOTE: The above representations are in local domicile time, reserve assignments are normally given in “Z” (UTC) time. If you are awarded a reserve line, refer to your schedule in CMS for the zulu times of your reserve assignment. Each “A”, “B”, “C” and “D” type reserve will be assigned a 24 hour free from duty period as follows: This duty free period will be assigned only when required (7 or more consecutive days of reserve). If you are assigned to a trip, which contains a rest period that is greater than 24 hours, that rest period will serve as a required break. Crewmembers on reserve status must report to Crew Scheduling upon termination of any assignment. Reserves are required to use assigned positioning and de-positioning flights. Deviations from these procedures must be approved by a Crew Scheduling representative. 12. RESERVE MOVE-UP LINES (RMUL’S): Please refer to the cover letter calendar of the Bid Packet for the RMUL time lines. Submit bids via the RMUL bidding option on the web under the Crew Applications bidding options. Job Share and RDG line holders may not bid for RMULS. NOTE: The choices available for RMULs are as follows: PAY PERIOD 1 ONLY: You will only be considered for a RMUL for the 1st pay period of bid. PAY PERIOD 2 ONLY: You will only be considered for a RMUL for the 2ND pay period of bid. BOTH PAY PERIODS: You will only be considered for a RMUL if a line can be constructed for both pay periods. ANY PAY PERIOD: You will be considered for any combination of pay periods, i.e., pay period 1 only, pay period 2 only or both pay periods. We will build a move up line for both pay periods if possible. 13. TEST FLIGHT AVAILABILITY OPTION: If you are interested in being called for test flight support, you may indicate your desire to be “on” or “off” the availability list during each schedule bid collection period. THIS OPTION IS NOW AVAILABLE ON THE WEB UNDER THE PREFERENCES OPTIONS.

14. SHORT TERM TRAINING PREFERENCE BIDDING: If you are scheduled for Recurrent Training (SVT, PT, CQ or Captains Leadership Workshop) you will receive a training bid package with scheduled training events that specifically meet your training requirements. This package will be placed, along with your schedule bid package, in your “V” file at the ASC. Your preference for training dates will be confined to the TRAINING bid period (not the BASE month) shown on your individual training notification. A listing of those pilots scheduled for training during your assigned bid period will be provided with this information. Recurrent Training bid choices may be submitted through:

1. The FLTOPS web page:http://ras.ups.com/flightops Short term training bids will close twenty-four (24) hours after the posting of the schedule bid awards (refer to the cover letter calendar). Training dates will then be awarded in SENIORITY order. If you are a crewmember with an ADVANCED ANC, MIA or ONT domicile base, a DH to and from will be provided. Those crewmembers should contact the assistant training scheduler for details regarding hotel and travel arrangements once awarded a Training date, whether it’s Recurrent or Captains Leadership Workshop.

NOTE: IF YOU HAVE MULTIPLE SHORT TERM TRAINING EVENTS IN THE SAME BID PERIOD: There is only one training bid on file per crewmember. If you are bidding for multiple training assignments, you must submit your preferences for all events in the same bidding transaction. If you resubmit a training bid, you must input ALL bid choices again. 15. RDG BIDDING (Reduced Guarantee): RDG awardees for the bid will be identified

in the seniority lists with RDG next to their name. RDG lines are identified in the bid packet and appear after regular VTO lines. Only those crewmembers who were awarded RDG for the bid period during the LOA award process may preference these lines. If you are awarded an RDG line please do not submit a primary bid. You will automatically be awarded one of the blank RDG lines during the primary bid. You DO need to bid in the secondary RDG bid once the RDG lines are published after VTO restorations and prior to LITT. There will be no vacation conflict for these lines. Vacation will be paid as if on days off. Conflict only exists for carry-in and short term training. Restoration of deficits may occur on any days off. If you choose to NOT have your deficit restored, you must notify the Transition Team of that decision between 1500-1600 SDF lcl time 5 days prior to the bid for CPTs and 3 days prior to the bid for FOs. IF you choose to NOT have deficits restored, your line credit/guarantee will be adjusted accordingly. Please see the cover letter calendar for the RDG timelines.

inquiry will need to be submitted to bid on the different RDG Lines available. 16. FLOW PROCESS ERRORS – It has been brought to our attention that some crewmembers have experienced “flow process” errors on the web when

submitting LITT requests. This is most likely due to multiple requests being submitted simultaneously. To avoid this problem in the future, please submit only one LITT request at a time.

WHEN TO MAKE TRAVEL ARRANGEMENTS: Per Article 13.H.5.b. deviation commercial arrangements MUST be made no later than seven (7) days prior to the show time of your originally scheduled deadhead if you would like to utilize your travel bank. Exception is if the trip assignment is made with less than seven (7) days notice. This policy does not guarantee a commercial ticket if there is a schedule change where a commercial is no longer authorized. The crewmember is responsible for canceling any unused reservations through the company’s travel agent. Any unauthorized charges will result in a payroll deduction. Most domestic commercial travel can now be reserved 24/7 by using the Crew Online Tool. Be prepared with your flight schedule information. If you are not signed up for auto issue of airline tickets and wish to fly as scheduled, download the Ticket Policy Form on the UPS Flight Ops website (Travel/Hotel link). Complete and forward the form to Crew Logistics to avoid calling the company designated Travel Agency. (To confirm the “Fly As Scheduled” (FAS) flight information, the Travel Agency will still need to be contacted.) 2. AMERICAN EXPRESS AIR TRAVEL CREDIT CARDS: Travel Cards may be used for Company business related to time-sensitive travel as authorized by the Company. The Travel Authority (TTA) will make all reservations for such travel. When warranted, Crew Logistics or Crew Scheduling will direct the affected crewmember to go to a ticket counter to pick up a ticket and pay for it with the crewmember’s AMEX Travel Card. The AMEX card is provided for emergency airline ticket purchases, additional baggage fees, and approved train usage (see below). The card cannot be used for airline deviation travel, airline upgrades, hotel, other transportation, or miscellaneous charges.

The AMEX card can be used for all FRA-CGN-FRA train tickets, regardless of whether you are scheduled for co-terminal ground transportation or deviating. Crewmembers are authorized to purchase a second or first class ticket (no sleeper accommodations). Crewmembers are also permitted to use their company issued AMEX card to purchase a train ticket and/or light rail service between CDG-LHR-STN as part of deviation travel, which includes the Paris Metro to CDG and the London Underground LHR-STN. All train tickets will be purchased at the train station just prior to travel. 3. All CML arrivals – Domestically, it will be the responsibility of the crewmember to contact the ground transportation vendor for all commercial deadheads. Crew members traveling on a CML DH should contact the layover hotel or ground transportation company (vendor) to confirm pick up at the airport. This should be done well in advance of arriving at the airport, 24 to 48 hours preferably. Please provide the vendor your inbound flight number and ETA. This allows the vendor to properly plan your scheduled arrival time. Waiting to call a vendor after arrival could result in a lengthy wait due to vehicle workload or travel distances. Please prevent post-flight pick up delays by planning in advance.

Internationally, if requested by the crewmember, the company will contact and arrange ground transportation. Request for international ground transportation arrangements may be emailed to: [email protected].

If a crewmember encounters difficulty in arranging ground transportation for domestic or international deadheads he may contact Crew Scheduling for assistance.

Transportation that is associated with a co-terminal DG ride for all UPS scheduled flights and UPS scheduled jump seats will be set up by the company.

Exceptions on city to city transportation: ORD-RFD scheduled bus service is optional from gateway to gateway when associated with a commercial ticket. Bus tickets may be picked up at SDF jumpseat desk or mailed to crew member’s home. For any questions with ground transportation, please contact Crew Logistics at 800-228-crew (Prompts 3-3). Day Room Information Article 5.H.1.f (1)‐(5) Crews need to request dayrooms at least 24 hours in advance in order to ensure availability. If less than a 24 hour notice is provided, the Company will make best efforts to obtain room(s).   Crew Logistics request that you make your dayroom request before your trip starts or best case scenarios make your request once your bid is awarded.  Internationally, Crews should not contact the gateway for dayroom requests.  All requests must go through Crew Logistics. In order for things to go as smoothly as possible we ENCOURAGE CREWMEMBERS TO GIVE CREW LOGISTICS AS MUCH NOTICE AS POSSIBLE, PREFERABLY 72 HOURS prior to your dayroom need.  You may email [email protected] for dayroom requests or contact Crew Logistics directly.  You may contact Crew Scheduling after normal business hours Monday‐Friday or on the weekends for dayroom needs.   Intransit stops that are scheduled greater than 5 hours are planned as part of the bid hotel requirements and are booked automatically.  If you desire a day room for a scheduled intransit stop between 2:01 and 5:00 hours, and the gateway does not meet the contractual facility requirements, then please use the above procedures for requesting a room.   HOTEL CANCELLATIONS Please Note: Due to a change in hotel management vendors any requests for hotel cancellations need to be emailed to: [email protected]. Please provide your name, gems id # along with the date and location you want cancelled.

Crew Hotel Room Cancellation Rules: Crewmembers must contact UPS at least 24 hours prior to local date of arrival for domestic layovers and 48 hours prior to local date of arrival for international layovers. Crewmembers should contact UPS as soon as possible to reinstate their original room reservation if needed. This program does not include system changes and rescheduled events. Layover changes created by these events are currently recognized in the system and no crewmembers action is required for these circumstances.

Reimbursements: **Effective Jan. 1, 2015, IRS rate for mileage is 57.5 cents. Crewmembers should use a Travel Bank / Crewmember Reimbursement Form for all crew reimbursements. This is located on the UPS Flight Ops website (Travel/Hotel link). Any incorrect or incomplete forms will be returned to your v-file. After completion of your travel, the appropriate forms should be forwarded to UPS Crew Logistics via fax (502-359-5966), housemail (Crew Logistics/GOC), USPS (Crew Logistics, 825 Lotus Avenue, Louisville, KY 40213), or you can elect to scan and email your reimbursement forms and receipts to [email protected]. Please include your last name and gems id in the subject line. UPS Crew Logistics appreciates your assistance in helping them to provide timely and accurate processing of your Travel Bank Forms. FLIGHT ADMINISTRATION/CREW VAN ADMINISTRATION 1. V-File/Bid Request: To request your V-file contents or bidding information to be sent

to your home or other location, call or stop by our administrative office. 1DA or 2DA shipping is available. The cost will be based on the consignee zip code and will be payroll deducted.

2. CREW TRANSPORTATION: For van service, dial 359-1634 then “press 1” for crew vans. When the van driver acknowledges your call, give your location and service request.

3. Crewmember Edgewood service - Continuous service is available Monday morning through Saturday morning, between the hours of 08:00 to 16:00 and 23:00 to 04:00. You do not need to call for service during these hours. For service during off-hours, use the directions listed in item #2 above.

4. Passenger terminal service - To obtain transportation to the SDF terminal, call 359-1634 (Instructions are listed in item #2 above). For transportation to the Edgewood facility, you may use our direct dial telephone at the SDF passenger terminal. This phone is located on the 2nd Level across from the American Ticket Counter. This phone will automatically dial the crew van radio line. Passenger terminal runs depart the ASC on the half-hour BY REQUEST ONLY between the hours of 05:30 and 23:59 local time. Between the hours of 00:30 AND 05:30 local time it is advisable to utilize Yellow Cab for transportation to the ASC.

5. Training Center service - You may request van service to or from the Training Center ONLY when you are in training from out of town. THIS SERVICE IS BY REQUEST ONLY. (Instructions are listed in item #2 above).

QUALITY ASSURANCE GROUP Quality Assurance is responsible for the auditing of CMS data pertaining to pay credit calculations. The objective of the Quality Assurance Group is to ensure accurate information is provided to Flight Payroll for the processing of your paychecks. Our auditing process will be continuous throughout the pay period so we may address discrepancies as they happen.
