A Male patient of aged 35 came with Severe

Bony defect in the upper central incisor

..due to CHRONIC PERI APICAL INFECTION and

the associate incisor got RESORBED till the

neck of the tooth. The entire root of the

incisor got resorbed..due to the infection..

This kind of COMPROMISED BONE

SITUATIONS..

1.Method 1-Bone augmentation

2.Method 2- Replacing without any bone

grafts using basal implants

Page 4: 15mm defect in upper central incisor replace by basal implants without sinus augmentation

Using bone augmentation techniques,,and waiting for 3-5 months for bone graft to integrate and then placing the impant..then again waiting for 4 months for implant to integrate..then fixed tooth can be given

The disadvantages of this approach

1.Multiple surgeries needed

2.Need for second surgery if autograft is planned( e.g from chin or from ileac crest)

3.Risk of bone graft failure

4.Risk of infection of bone grafts

5.Additional cost due to bone grafts used

6.Additinal cost to multiple visits/surgeries needed

7.The success of implants in a bone grafted site is always less

Advent of basal implants has made possible to restore this kind of boneless situation with implants and fixed teeth.

Concept of using basal implants without bone grafts..HOW?

The bone defect is only involving the alveolar bone.

While the conventional implants take anchorage form alveolar bone..BASAL IMPLANTS TAKE ANCHORAGE FROM BASAL CORTICAL BONE..in this conditon..NASAL CORTICAL BONE..

which is pure and strong and uninfected..and good quality bone...

The basal implants are DESIGNED in such a way..that they are LONG ENOUGH to get SUPPORT FROM NASAL CORTICAL BONE and they have POLISHED SURFACE so that it wont get any infection.

So basal implants RESISTANT TO INFECTION..
