    1) INTRODUCTION

    Choithram Hospital & Research Centre is not only a hospital to treat disease but acomplete institution for care of whole Society. This hospital was established in the year 1979

    by Late Shri Thakurdasji in the fond memory of his father Shri Choithramji Pagarani .Since its inception the founder of this hospital knew the problems of large section of oursociety, especially the rural masses who couldnt afford treatment. He laid the principle offree OPD , where any patient can consult any specialist doctor, and made available allspecialities of medical sciences with finest investigation facilities under one roof.

    AIMS At Choithram hospital quality patient care is the prime objective. Caringattitude with precise skills are interwoven with modern technology to offerhighly specialised diagnostic and therapeutic service. Our aim is to bringworld class medical facilities within the reach of a common-man.

    Sickness is a frightening experience. When a patient comes to a hospital he isentrusting himself to a team of strangers. At Choithram we stretch ourselvesto create an atmosphere of mutual trust and confidence. All patients receivethe same consideration regardless of cast creed or religion, socio-economicstatus or sex. Choithram is a referral hospital covering a large spectrum ofspecialities at one place.

    HISTORY OF THE HOSPITAL

    16 Jul 1979 The Choithram Hospital is commissioned

    1981 Physiotherapy block is brought up.Fibre Optic Endoscopy is initiated which today carries out morethen 200 endoscopies and cystoscopies per month.Haemodialysis department is also started for renal failure cases.

    1983 Gamma Camera (GAMMATONE T-900) for nuclear imaginginstalled.Radioimmunoassay [RIA] installed in pathology for diagnosis of

    endocrine diseasesInstalled Computerized Blood Gas and Electrolyte AnalyzerCorona Analyzer in Biochem and Coulter System in haematologyinstalled.Microbiology lab was set-up.

    1984 Cardiac Cath lab and Echocardiogram set up.Department of Neurosciences stated with EEG, EMG, ECT andmicro-neuro surgery.

    1985 Open Heart Surgery started.

    Percuteneous Renal Lithotripsy started.Microsurgery of middle ear started.

    1986 Renal transplant started, so far 262 transplants done.Diagnostic Clinical Immunology initiated.Ultrasonography unit set up.Total Body CT Scanner was installed.

    1987 Total Hip Replacement started. More than done so far.Corneal Transplant Surgery undertaken.Departments of Neurosurgery and Onco surgery were updatedSeveral OPD Chambers added to cater for growing daily OPD

    1988 60 Bed Paediatric wing added with department of Neonatology.Paediatric ICU started.fully equipped with incubators, ventilators,infusion pumps, central monitoring and oxygen system.Fully equipped 11 Bed ICCU and 7 Bed post operative ICU started.6 Bed Burn Unit commissionedTotal bed strength goes up to 300Recognised for aids surveillance and conducting research withICMRSolar heater installed for 24-hr hot water supply.Recognized for House Job by DAVV Indore.

    1990 Indigenous CO2 Laser was given to us by CAT for trial anddevelopmentAutomated Haematology counter was installed in Path dept.Dr. Vinod Naneria trained under Dr Ranawat at Connel Uni, USAfor TKR surgery.

    INTERNAL RADIATION (Breaking area)-

    1. Needle, beats, seeds are implanted in the body.2. Nucleation machine- In this machine cancer of cervix, buccal mucosa.3. Patient is kept for 10 to 15 min. for internal radiation.

    One TRS Room (treatment plan section).One Countering station.One mould room-

    a. Only head, neck mould is made. b. It is made of plastic sheet.c. Plastic sheet is kept in water bath at 60 degree temp.d. Then it become flexible it can be shaped according to patients head and neck.e. With the mould, patients go for CT scan and through laser the area is marked

    and then radiation is given.

    ENDOSCOPY LOCATION: Ground floor, left hand side of day care center, near corporate

    desk.

    LAYOUT: One reception room, two rooms for maam and sir, one room forcapsule endoscopy , one room for colonoscopy, one room for seminar, one autoresearch room, one ERCP and EVS room.

    OBJECTIVE: To diagnose the problem inside the body. STAFF : four nursing staff, one technician, two clerk, one word boy, three junior doctors.

    WORKING HOUR: For nursing it is 8 am to 4 pm and procedure happens between 10 am to 4 pm.Endoscopy hardly takes five minute.And colonoscopy takes thirty minute.

    EQUIPMENT:Two monitor (one for endoscopy image).

    Anesthesia traully.ECG leads.One endoscopy traully.Sphygmamometer.Saturation prob.

    Suuction.Hydroxide .

    AVERAGE PATIENT: 18 to 19 patients per day but someday 5 to 10 patients per day.

    CHARGES: Endoscopy charges are 1600 Rupees and if biopsy is done thencharge will be 750 Rupees.

    MortuaryLOCATION :-Outside the Choithram Hospital near Physiotherapy unit.

    LAYOUT:- 1 Room

    OBJECTIVE :-A morgue or mortuary (as in Choithram hospital) is used for the storage ofhuman corpses awaiting identification, or removal for autopsy or disposal by burial, cremation or otherwise. In modern times they have customarily been refrigerated to delaydecomposition.

    Types of cold chambers:- There are two types of mortuary cold chambers:

    1.Positive temperature

    Bodies are kept between 2C (36F) and 4C (39F). While this is usually used for keeping bodies for up to several weeks, it does not prevent decomposition, which continues at aslower rate than at room temperature.

    2.Negative temperature

    Bodies are kept at between -10C (14F) and -50C (-58F). Usually used at forensicinstitutes, particularly when a body has not been identified. At these temperatures the body iscompletely frozen and decomposition is very much reduced.

    HOUSEKEEPING

    LOCATION- inside the hospital campus, in front of central store room..

    OBJECTIVE The main aim of the housekeeping department is to provide additionalor external help to the hospital by giving their services.

    SERVICES - All services of hospital is coming under housekeeping. Services includetechnical as well as non technical . Patients requirement like bed, in summer seasonarrangement of AC and cooler is all seen by housekeeping.

    Technica l- Electrical or Electronic problems (fan, cooler, AC), biomedicalmachinery or waste.

    Non Technical - dealing with painter, carpenter, civil vendor, plumber andmaintaining all fourth class worker (word boy, maid, sweeper.), CSSD,Sanitary department..

    Priority of managing all this facilities is decided by housekeeping manager.

    STAFF - There is 13 members in total staff.

    ETO ROOM-One Ethylene oxide gas sterilizer:- In this machine along with the materialwhich has to be sterilized, ethylene oxide gas cartridges are kept. Gloves, all

    plastic material, catheter, OT devices etc are sterilized. ETO machine is aautomated machine.

    Pressure of ETO machine is 935 milli bar.

    There are two cycles occurring in this machine:

    Cold cycle: i. Temperature is 40 degree centigrade.

    ii. Exposer time is 360 minute.iii. Aeration time is 360 minute.

    Hot cycle: i. Temperature is 50 degree centigrade.

    ii. Exposer time is 240 minute.iii. Aeration time is 360 minute.

    DUMP ELIVATOR ROOM-Dumb waiter is a small lift which directly opens into operation theatre.

    OBJECTIVE :-

    In this department those gases used in hospital are stored mainly oxygen &nitrousoxide is stored.

    2 big tanks filled with liquid oxygen. Nearly 50-60 cylinders are there in which oxygen & nitrous oxide gas is filled. These gases are supplied to hospital through pipes or through cylinders. The capacity of cylinder is 875 Litres.

    STAFF :- 4 Technicians

    WORKLOAD :-70-90 Cylinders are used per day.

    1 nitrous oxide cylinder used per day.

    Record :-Log book(hand over book & take over book).

    B.ELECTRICAL DEPARTMENT:-

    OBJECTIVE:-

    It consists of 33 Kvsubstation called as power house. MPEB supply 3300V electricity to this power house which gets converted into 440V

    then is supplied to transformer. 3 phase line are there:500Kv

    320Kv250Kv

    Auto manual setup is there i.e. as supply of electricity stops so within 12 sec generatorstarts.

    In the morning everyday 10 min trial of generator done.

    EFFLUENT TREATMENT PLANTThe ETP plant was designed and erected at the hospital by the firm. The construction

    work was completed in 6 months at the cost of rupees 14 laces and commissioned in November 2001.

    LOCATION ETP is situated outside the hospital building.

    LAYOUT There is large free space in which number of different tanks is made inwhich water can be collected.

    OBJECTIVE Main purpose of ETP is to recycle the water and filter dirty water.

    PROCEDURE Water can be filtered by effluent treated plant by follows various stages.

    1. Dirty water from hospital, school, and college is collected in Effluent collectiontank.

    2. Then collected water goes to screen bar through channels and slug is and slugis separated.

    3. Now from aeration tank water goes into CLARIFIER TANK and then itsupplies to the holding tank.

    4. From clarifier tank water goes to the INTERMEDIATE HOLDING TANK.Here water is collected.

    5. From holding tank water can goes to the PRESSURE FILTER 6. From pressure filter water can be totally filtered and remove the fine suspended

    matter from effluent tank.7. Now water goes through pipe into CHLORINE CONTACT TANK. Here

    chlorine is added continuously to inactivate the microbial population.8. Now finally the purified water comes in TREATED WATER TANK. In this

    tank water can be collected before lifting to high level storage tank, then thiswater can be supplies into garden, toilets.

    STAFF Total Five members in total staff of ETP. One in charge- Chouhan sir4 operators.

    PROCEDURE- (1) Assembling assembling of all sheets or record.ie; admission sheet,history sheet, doctors sheet, operation sheet, consent sheet, anesthesia sheet, nursing sheet,medication sheet etc.

    (2) Coding coding of the disease according to WHO, It is for to check out how many

    patient comes for a particular disease or to bring out the statical data for researches. (3) Indexing for save data according to coding.

    MLC form filling, certificate distributions is done through this department. Certificate offitness, fever, estimate are given.

    Choithram Hospital & Research Centre is not only a

    hospital to treat disease but a complete institution forcare of whole Society.Since its inception the founder ofthis hospital knew the problems of large section of oursociety, especially the rural masses who couldnt affordtreatment. He laid the principle of free OPD , where any
