Future Plans – the next 2 years

• A strategic approach to community health development that includes:– Asset based approach to building community enablers– Established programme of community health champions– A vibrant community sector with an increased role in

service planning, and increased share of service delivery– Health improvement embedded within the

Neighbourhood Offer, which is informed by the JSNA and Tameside Insight.

Future Plans – the next 2 years

• A universal, locality based, integrated approach to ‘Wellness’ which delivers on the prevention workstream of the integration programme across the whole life course – Programme of service reviews and transformation

– ‘Specialist’ wellbeing services wrapped around community care provision with defined pathways

– Wide foundation programme of early intervention via community engagement, health literacy development, social marketing and behaviour change

– Public Health Settings approach with initial focus on workplaces, housing, Healthy Schools, Primary Care and hospitals
