Please note that Mid-Pacific Institute usually does not provide the F-1 student visa or English languagesupport or Homestay for international students below Grade 7. Any international student below Grade 7should apply through the regular application and provide proper documentation of U.S. citizenship,permanent residency, or other visa.

A student is considered International at Mid-Pacific unless he/she falls under both of the categoriesbelow. Documentation of U.S. Citizenship or Permanent Resident of U.S. must be verified.

1. The student is a U.S. Citizen or a Permanent Resident of the U.S. AND2. At least one Parent is a U.S. Citizen or Permanent Resident of the U.S.

4. STEP 4: Apply to Mid-Pacific Institute

Type of application

• For Grade 6, choose Middle and Upper school application process.



• For Grade 7 or above, choose International application process.

Domestic application is for the following: applicant AND at least one parent is a United States (U.S.)citizen or permanent resident of the U.S. If you do not meet this criteria, please fill out the InternationalApplication.

5. STEP 5: Complete Preliminary Application Steps by October 30

Application and Application Fee

Once you have completed all requiredquestions in the form you will return to this page and the "Go to Pay & Submit" button to the above rightwill turn green. Once you click on that button your Application will be submitted and you will beprompted to pay the application fee of $125 (non-refundable). .



If you have completed the application form and the "Go to Pay & Submit" button does not turn greenplease re-open the form and click on "Highlight Missing Questions" at the top left of the form. Scrollthrough the form to see if any questions are highlighted. If not, please contact Ravenna Solutions directlyat [email protected].

Mid-Pacific will receive the data exactly as entered, so please make sure that you enter the informationaccurately in terms of spelling and punctuation. If you are unable to complete the application in onesitting, you may select Save and Close so that you may return to your application later. Once theapplication is submitted, you will receive an email confirmation that it was successfully processed.

International Application Supplement

If you are a student needing an F-1 visa, please allow at least 2 weeks to create and receive your I-20.You will need your I-20 to apply for the F-1 visa and to enter the U.S. You may only enter the U.S. up to30 days before start date of school with the Form I-20.

If student plans to live with a guardian other than a relative, Guardianship Form must be submitted.

Homestay Application

The Mid-Pacific Homestay Program is for any International student in grades 7 through 12 who are inneed of housing. The program has two goals: first, to find a home on O'ahu where the student will



become part of the family, and second, to help make sure that his/her time at Mid-Pacific is successful,happy and healthy. Homestay application form must be submitted.

We want to get to know you, your family and your child better. Please take some time to thoughtfullyanswer the questions in the form below. You can re-visit the form and continue to make changes to youranswers until you finally submit it. After the form is submitted, no changes may be made. This form willbe due by January 31.

The strength of Mid-Pacific education is based, in part, on an open and trusting partnership between theSchool and Parents. For this reason, we are asking you to submit this confidential Parent Questionnairewhich will provide the Admissions Committee with valuable insight about your family. This is anotherimportant piece to your childʻs unique picture. Our school will do our best to support your child in a waythat is consistent with our mission and academic programs.

Teacher Reference Report (2)

After December 1st, please submit a Math Teacher Reference Report and English Teacher ReferenceReport completed by your child’s current teachers.

Click on the link and input your child's current teacher's name and email address. The teacher will receivean email with a link to a secure Teacher Reference Report. Additional forms can be requested using theAdd Person button. Once the teacher fills out the form, she or he will submit it directly to our school.After we have received the form from the teacher we will mark this step as complete. Please allow timefor processing.

Submit all forms by January 31, but no earlier than December 1.

By emailing or delegating this recommendation form to your child's teacher, you waive any right toaccess this recommendation and authorize the teacher to provide an evaluation and all relevantinformation to the school for purposes of your child’s application to the school. By submitting thisevaluation form and your application for consideration by the member of the Hawai`i Association ofIndependent Schools (HAIS), you hereby release the HAIS school, its employees and representatives, theevaluator and the evaluator’s employer from any and all claims and liability that may arise frominformation provided. All information provided on the attached evaluation form will be held in strictestconfidence and will not be shared with students, parents, or guardians. This evaluation will remainconfidential and does not become part of the student’s permanent academic record.

Required Testing for International Applicants

International applicants must take a Language Assessment Test.



• Grades 7-10: TOEFL Jr., iTEP SLATE, or CaMLA• Grades 11-12: TOEFL, iTEP SLATE, or CaMLA.

The Cambridge Michigan Language Assessment (CaMLA) test can only be taken on campus byappointment. Mid-Pacific's CaMLA test fee of $100 US includes the exam, an interview and a writingsample.

Additional Testing

If offered in your home country, students are also encouraged to take the Secondary School AdmissionsTest (SSAT). Designate Mid-Pacific to receive the scores by adding our code below.

Mid-Pacific is more than just a place where your child will learn math and English and Science. After you submit your application and fee, we will assign you to an Interview date and time. Individualinterviews are required for our international applicants. Students who require the Form I-20 to study at Mid-Pacific on the F-1 visa must verify sufficient fundsfor one year of study. Financial assistance is not available to international students. Students must verifya minimum of: $23,185 (Tuition) + $18,000 (Housing) + $2,000 (International service fee) + $2,275 perELD class (Will be determined through the admissions process). Bank statement requirements:

• The bank statement or bank letter must be on the financial institution's• letterhead.• The account holder's (person liable for financial responsibility) name must• be written in English.• The type of account. The account must verify liquid assets (checking, savings, or deposit

School Tour (highly recommended)

After completing your application form and fee, please feel free to sign up for a School Tour by clickingon the link below. This is for the parent / guardian as well as the applicant student. During this time theparent / guardian will be able to meet with admission office staff and the student will be able to visit aclass in session. Exact activities will depend on your student's grade level. More information will beemailed to you after registration.



Register for current monthʻs tour dates. The following monthʻs dates will be available on the 15th of theprevious month. (October tour dates will be available on September 15. February tour dates will beposted in January).

School Administrator or Guidance Counselor Evaluation

Input your child's current school administrator or guidance counselor's name and email address. They willreceive an email with a link to a secure Administrator / Counselor Evaluation form. Once they fill out theform they will submit it directly to our school. After we have received the form we will mark the step tothe right as complete. Please allow us time for processing.
