International Application Supplement

If you are a student needing an F-1 visa, please allow at least 2 weeks to create and receive your I-20.You will need your I-20 to apply for the F-1 visa and to enter the U.S. You may only enter the U.S. up to30 days before start date of school with the Form I-20.

Teacher Reference Report (2)

After December 1st, please submit a Math Teacher Reference Report and English Teacher ReferenceReport completed by your child’s current teachers.

Additional forms can be requested using theAdd Person button. Once the teacher fills out the form, she or he will submit it directly to our school.After we have received the form from the teacher we will mark this step as complete. By emailing or delegating this recommendation form to your child's teacher, you waive any right toaccess this recommendation and authorize the teacher to provide an evaluation and all relevantinformation to the school for purposes of your child’s application to the school. By submitting thisevaluation form and your application for consideration by the member of the Hawai`i Association ofIndependent Schools (HAIS), you hereby release the HAIS school, its employees and representatives, theevaluator and the evaluator’s employer from any and all claims and liability that may arise frominformation provided. All information provided on the attached evaluation form will be held in strictestconfidence and will not be shared with students, parents, or guardians. The Cambridge Michigan Language Assessment (CaMLA) test can only be taken on campus byappointment. Mid-Pacific's CaMLA test fee of $100 US includes the exam, an interview and a writingsample.

Additional Testing

If offered in your home country, students are also encouraged to take the Secondary School AdmissionsTest (SSAT). Designate Mid-Pacific to receive the scores by adding our code below.

After you submit your application and fee, we will assign you to an Interview date and time. Individualinterviews are required for our international applicants. Once the Admissions Office has scheduled yourchild's appointment, a message will be sent confirming the appointment. Skype/Facetime interviews areconducted for international applicants.

Students who require the Form I-20 to study at Mid-Pacific on the F-1 visa must verify sufficient fundsfor one year of study. Financial assistance is not available to international students. Students must verifya minimum of: $23,185 (Tuition) + $18,000 (Housing) + $2,000 (International service fee) + $2,275 perELD class (Will be determined through the admissions process). Bank statement requirements:

After completing your application form and fee, please feel free to sign up for a School Tour by clickingon the link below. This is for the parent / guardian as well as the applicant student. During this time theparent / guardian will be able to meet with admission office staff and the student will be able to visit aclass in session. Register for current monthʻs tour dates. The following monthʻs dates will be available on the 15th of theprevious month. (October tour dates will be available on September 15. February tour dates will beposted in January).

School Administrator or Guidance Counselor Evaluation

Input your child's current school administrator or guidance counselor's name and email address. They willreceive an email with a link to a secure Administrator / Counselor Evaluation form. Once they fill out theform they will submit it directly to our school. After we have received the form we will mark the step tothe right as complete. Please allow us time for processing.
