Requisition for Temporary Facilities
1. GENERAL
This requisition defines the minimum requirements for the Temporary Facilities (Office,Canteen, Toilet etc.) to be installed for EVOLUE No.2 Line Project at Jurong Island inSingapore.
2. SITE LOCATION
(refer to the attached DWG)
3. BIDDER’S SCOPE
(refer to the attachment of 12. Cost Allocation Form)
4. BUILDING EQUIPMENT
Building shall be equipped with :
electric distribution panel and cablingair conditioninglightingelectric outletplumbing and sanitary piping (bidder shall propose 1 line diagram)small kitchen for serving tealightening protection shall be installed for every building roofsLong tables & Benches for canteen as per drawing.Other required equipment, if any by Singapore regulation (Ex. Fire extinguishers).
a. The air conditionings shall be installed in all rooms.Inside of the room, the temperature to be maintained below 26ºC
b. Water meter shall be installed at water supplying point for each building.
c. Suitable numbers of fluorescent lamp shall be installed in each room and corridor,lighting intensity on the floor in each room shall be at least 400LUX, warehouseshall be at least 200LUX, others shall be at least 100LUX.
d. The small kitchen shall be installed as per attached drawing.

5. GENERAL SCOPE
a. Develop necessary construction drawings from the conceptual drawings attached.Those drawings are subject to approved by MES.
b. Provide all drawings and documents which are required for permits approval forTemporary Facilities from Singapore Government, and obtain allGovernment approval for Temporary Facilities.
c. Apply WATER SUPPLY to PUB Water Dept., POWER SUPPY to JTC/Power Grid,TELEPHONE LINE/INTERNET LINE to Singapore Telecoms, Canteen Facilityto ENV, Sanitary Facilities to concerned authorities.
d. Supply all required materials, equipments, all necessary fitments and accessoriesto complete the work specified herein.
e. Carry out material control such as transportation to the job site, unloading,unpacking, storing, warehousing etc.
f. All Facilities to install, such as Sanitary system, electrical system and etc., shallcomply with Singapore Regulations.
g. Clean up all areas used for the construction work as per the contract, and alsocollect and remove all materials and equipment which are neither surplus,defected for other reasons after completion of the work.
h. Provide transportation system for workers and staffs from Mainland Singapore toand from Construction Site.
6. MES’S SCOPE
a. issue to the Subcontractor conceptual drawings and documents for the work, ifnecessary.
b. Supply to the Subcontractor a land to be used for laydown space of equipment andmaterials for the work.

7. GENERAL REQUIREMENTS
a. AcknowledgmentWithin three (3) days after receipt of this requisition, the bidder shall e-mail MESadvising whether the bidder intends to submit his quotation, using the attachedRECEIPT OF BIDDING DOCUMENTS.
b. Delivery of TenderBidder ’s proposa l shall be e-mailed to the following attention:

c. Closing DateThe bidder shall submit to MES his proposals on or before 10 Dec.2012.
8. SCHEDULE OF THE WORK
The bidder shall submit MES detailed construction schedule based on the attachedschedule.
9. SITE VISIT
The bidder shall be deemed to have inspected and examined the site and it’ssurroundings and to have satisfied himself before submitting his bid as to the nature ofthe ground and sub-soil, hydrological and climatic conditions, the form and nature of thesite, the quantities and nature of the work and materials necessary for the completion ofthe work and the means of access to the site by land and sea and, in general, shall bedeemed to have himself obtained all necessary information as to risks, contingenciesand other circumstances which may influence or affect his bid.
No claim for additional payment will be allowed on account of the failure of the bidder toobtain complete and correct information on all matters relation to the site. The biddershall be responsible for any misunderstanding or incorrect information except for thosegiven in writing by MES.
Any neglect, delay of failure on the part of the bidder of obtain reliable information uponthe above or any other matters shall not relieve him of any risks and liabilities or of the

responsibility for the completion of the work including in the contract.
10. COMMERCIAL REQUIREMENTS
a. Commercial ProposalThe bidder shall offer his bid prices for each cases as fixed lump-sum contractaccording to the following cost breakdown;
Equipment and Material CostLabor Man-Hour and its CostField Management CostConsumableConstruction EquipmentTemporary FacilityField Office ExpenseInsuranceTaxes and DutiesOverhead and Changes
The bidder shall show the unit rates of materials, installation work and others onhis proposal.
b. Payment ScheduleThe Subcontractor shall invoice MES monthly for the contract price referring to theprogress % authorized by MES. MES will pay Subcontractor every month ninetypercent (90%) of the amount calculated with the progress % above, within 45 daysafter receipt of the invoice by MES.
It is agreed that the remaining ten percent (10%) of the contract price shall beretained for a minimum 3 months from the date of completion to the work and shallthereafter become payable only if the work has been found to be satisfactorycompleted.
c. ChangesNo increasing and decreasing to the contract price shall be made except for theevent that change in the scope and/or conceptual requirements caused by MES’sinstructions which do not include the failure and slippage of the Subcontractor.The unit rates to be stated on the cost breakdown in the commercial proposal shallbe fixed and unchanged during the contract period.
