Abstracts must be no more than one page (8.5” x 11”) including diagrams and tables.

Participants in the Duke Preceptorship will spend three days in the regional block area, operating rooms and on the floor with post surgery patients observing ul-trasound guided single shot nerve blocks and catheter techniques in a wide variety of clinical scenarios. They will learn how to set up the block area for maximum efficiency in the OR environment, improve their decision making ability and make choices in the performance of regional anesthesia. A one-on-one discussion with the Duke Faculty member reviewing cases, scanning techniques and the image library as well as covering handout materials will augment the preceptorship experience.

Participants in the Duke Intraoperative TEE Preceptorship spend one three-day session in the cardiac operating suites, observing techniques of intraoperative TEE and interpretation of images. Preceptors will participate in active discussions with cardiothoracic anesthesia faculty and fellows, and learn the basic TEE exam. They will also learn how to troubleshoot difficult cases and enhance their decision-making skills in the operating room. Cases will be reviewed with fellows and faculty and images from pathology libraries will be used to augment the preceptorship experience.

100 Miles. one DayWarwick Ames, MD

Some of you may know that I like to run. Long story but new venture and it has been my salvation to a certain degree. The distances have gotten longer and reached a peak last weekend at the ‘Umstead 100 mile Endurance Race’, a 12.5 mile loop of the Umstead trails that has to be

completed 8 times, ideally within 24 hours. I thought it would be fun. I was wrong. First my preparation was poor. It is hard to train for any race like this whilst working at Duke and trying to keep the family happy. Some may have spotted me early morn, walking up the wrong way of the parking lot escalator in preparation for the uphills. A couple of days before the race I was up all night with a pediatric heart transplant which, not to sound ungrateful, really messed with my sleep schedule. As for the race I knew that running 8 loops would be dull but each loop was in fact very different. The first was fast and I blew my quadriceps, which tends to happens on down hill racing. Umstead isn’t mountainous but it is not exactly flat. On the second lap, I realized that this was going to be tough. On the fourth my bowels let go, which was unexpected but fortunately I reached the toilets every time it happened. After the first fifty miles, someone, without my prior knowledge or consent, inserted coarse sandpaper between my butt cheeks and that particular agony began in earnest. I prayed at the end of each loop to get a pacer (someone not racing but runs with you to encourage you to run). I began to slow horribly and started to panic. Towards the end of lap 6 (approaching mile 75) a few running friends found me shuffling along and injected me with hope. But my troubles started to mount. I began feeling unbelievable nausea, frantically tried to vomit and each step was a disaster. I couldn’t eat or drink anything. Because I was

reduced to a shuffle at 1 am in the morning, with the temperature in the mid 30’s and dressed in seemingly just a thong, I began to get hypothermia. At mile 82, an aid station, I sat down for the first time, by a heater. Realizing that hyponatremia was also ongoing, I ate some salt tablets and finally laid down. I decided to quit and leave the fight for another day. My pacer on the other hand was not so keen to quit. Wackus is a cherished running buddy who, for a living, sells Hondas at the Southpoint dealership. He found me clothes and fed me chicken soup. Almost an hour later, I felt better. But it was 2:20 and we had only

3 hours and 40 minutes to complete over 18 miles. Somehow somewhere we spouted virtual wings and made it with 6 minutes to spare. I cannot thank my pacer enough but will try with words and endless beer vouchers (and possibly the purchase of a new Honda) .

The reward? Apart from the satisfaction of completing 100 miles, finishers of any registered 100 mile

race get, not a medal, but a belt buckle (another long story). The Umstead buckle is not glorious and simply reads ‘100 miles. One day’. I am very proud of mine.
