The many rejections for military service in two World Wars in a quarter of a century, over-crowding in our institutions for the mentally ill, the wide prevalence of nervous dysfunction, and extensive evidence of avoidable disease in our population are compelling reminders that health education and pre­ventive medicine have much to do if the public 1s to receive the greatest benefit from the biological, chemical, and physical sciences which have been 80 richly endowed through taxation and philanthropy.

SIR:

I have the honor to present herewith the Thirtieth Annual

Re~ort of the activities ot the University Health Service covering the

period of Jul~ 1. 1945 to June 30. 1946.

The year haa been 8 most active one for the Health Service .

The demands upon it have far exceeded ita ability to meet them because

of inability to till vacancies on it, Medical Staff. Sustained effort

haa been made to procure suitable ~er8onnel: the quest continues.

The Health Service 18 seriously handicapped because ita

medical personnel haa been reduced to nearly 50 per cent ot it, normal

strength in 1940. Nothing 18 80 needed by it &8 the filling of vacancies

on ita staff with promising, well-trained physicians between 30 and 45

years of age to make it more effective, proficient and progressive. With

the present shortage of doctors, good office facilities. a thoroughly

modern he~th program, and pleasant living conditions, in addition to

the prevailing stipend, are essential inducements to attract thee.

A greatly reduced number of army trainees hsve received medical

care during: the year. They made a total of 3,213 .,.hits to the Health

Service Station On s ick calls, for immunization, and regular monthly in­

snectlons. Civilian student~ made 29.558 visits to the Health Service,

employees 2.691 and others 281, making a grand total of 32.530 calls for

the year. Complete physical examinations were given to 9,047 students;

of that number statistics haTe been compiled on 6095 men and 2633 women;

78 University High School students; 432 Civil Service employees, and 514

for the Retireoent System.

Medical Examinati ons

1. Service8 to A. S .T . P. Trainees

The Health Service bad 1.914 visits from A.S.T.P . trainees and

made 1,299 physical inspections for them.

11. University Students

There vere 16,614 students enrolled in the University at

Champaign-Urbana during the academic year 1945-46 . All but a very few of

these called at the Health Service Statton one or more times during the

year for a total of 29,558 visits, or an average of 1.78 calls per student.

The average call rate for vomen vas 1.50; and for men, 1 . 94. A total of

9,047 calla were made by students because of the required physical

examination .

III. University High School Students

A total of 78 pupils seeking admission t o the University High

School were examined. Of t hese 31 vere boys and 41 were girls.

IV. Retirement Syatem

The Health Service examined 534 academic and non-academic

personnel preliminary to beginning participation in the University Retire­

ment System of Illinois.

V. Employees

Because of the labor turn-over ' and the number of workers inci­

dental to servicing the Union Building and Residence Halla, 432 prospective

domestics and laborers were examined. Of these 268 were men and 164 Were

women. The University emplolees made 2,691 visita to the Health Service.

VI. Student ~ Private Pilots

With the inauguration of instruction in flying at the University

airfield, the Health Service was requested by the Director of the Insti­

tute of Aeronautics to examine prospective students of aviation in accord­

ance with the requirement of the Ctvil Aeronautics Administration. A total

of 119 were examined of whom 3 Yere women.

VII. Foodhandlers

During the year, 298 foodhandlers vere examined to determine

their physical fitne.s and disease carrier status .

VII1. Chauffeur isamln&tlona

A. JDplolees

A total of 44 individuals requested chauffeur examinations

required for driving University automObiles. Of these, all Were considered

physically able to drive .... ithout involving unusual risks to themselves

and the public.

B. Student 8

During the year, 4 students requesting motor vehicle permits were

examined: all were found to oe phySically capable of driving an automobile.

IX. Applicants for Marriage Certificates

Pre-martial examinations have been available to students since

the enactment of the Saltiel Law. During the year, 76 of them received

the benefit of this service.

X. Laboratory Service

In caring for students and employees, 11,885 laboratory tests

were made; 9,783 at the Health SerTice Station and 2,102 by the State

Branch Laboratory at 505 South Fifth Street, Champaign, Illinois.

I. General

When neV students take their physical examinations, a briet

summary of the abnormal findings, if any, 1s made on their recorde.

Subsequently, the, are r ecalled for confer ences concerning the defects

discovered . and are given adTlce regarding such correction or alleviation

aa ~ be possible. This phase of "follow-up· vas much restricted during

the year because at the reduction i n the medical staff of the Health Ser-

vice and great increase in the number of students. emplo~ee8. and faculty

members to be examined in accordanCe vith University regulations .

II. Albuminuria

A t otal of 211 new students exaained on matriculation in 1945-46

Were found to have albuminuria. Repeated examinations shoved the ,condit ion

to be pers1~tent 1n 76 instances and transient in 135 instances.

Ill. Heart Disease

Of the 8. 728 students examined, 104 were found to have cardiac

abnormalities. These have been kept under observation , have been repeatedly

examined, and have been excused from physical education where the condition

was marked or assigned to a program of adapted exercise which will protect

them against undue strain.

IV . Tuberculosis

During the year, 57 students requested tuberculin tests. of which

approximately 30 per cent were positive. The percentage of positive

reactors 19 about what should be expected in a group of the age, environ­

mental background, and composition of Illinois students. Those who reacted

positively have been urged to have roentgenograms of their cheets to

establish, 8S far 86 possible, the extent and nature of their infection

and to have a record for compar1son w1th future X-ray films.

V. Mental Hyg1ene

During the year 1945-46 . the Health Service had the part-time

services of Dr. Richard L. Jenkins. a psychiatriet. as consultant in

Mental Hygiene. Th1~ service was establiehed on a one-fifth time basis

on April 15. 1945. Until March 15 , 1946, these services were continued

on a one-fifth time basis. Thereafter. they were half-time. One hundred

twenty-six stUdents and etaff members of the University have conferred with

him on a wide variety of problems .

liThe number of appointments per case has varied from one to

t wenty. The most common problem for which consultation has been sought

has been chronic anxiety . Of cour se . the intensity of the anxiety varies

from case to case. a8 do its causes. In some cases, the anxiety is pro­

voked by difficulties in adjust1ng to the University. In other case s. it

i. related primarily or entirely to problems within the personal life of

the student. In 80me cases. it 1s a residual of combat fatigue or other

w.r-occ8aloned anxiety state in returned veterans . Many returning veterans

are keenly desirous of getting through their University work &s rapidly

as possible and 80me are inclined to push their academic load vith too

little regard for their own need to become acclimated to the University.

Consultation haa been sought for man¥ other psychoneurotic or personality

problema. There have been a few instances of Serious mental disorders. I

COMMUNICABLE DISEASE

1. Student'

The incidence of communicable disease in the student body

shOWed an increase over last year; the total caSes in 1945-46 were 226,

and in 1944-45 there v~re 156.

A. Military Classification

During the year . 130 men were excused from Military permanently.

and 15 men were excused temporarily.

B. '91s1cal EdUcation Classification

During the 1ear, 6 men and 143 women Were excused from Physical

E4ucation permanent11, and }6 men and 308 women were deferred temporarily.

The number of students who were aSSigned adapted eXercise instead of

Regular Ph18ical Iducation was 967 men and 28 women. There were 13 women

transferred from one type of Regular Physical Education to another, and

7 men were re-assigned to Regular Physical Education.

Students suffering from certain forms of dermatitis, 8inusltls,

infection of the middle ear, or perforation of the ear drum were trans-

terred from 8wi5ming to forml of exerci8e le8s likely to aggravate their

condition. A total of 1,008 recommen4ations were made to change students

from one type of PhySical Education to another or for modification of the

exercise they were taking.

INSTRUCTION IN HYGIENX

Compilation of the results of 2,196 questionnaires filled out

by freshmen registered in Hygiene 2 and 5 revealed that only 103 teachers

in hTgiene in high schools attended by those students gave full-time

instruction in health education. The greater percentage taught the sub-

ject as a "side line". The State of Illin01s is still far behind in

having its teaching of hygiene and sanitation of a quality comparable to

their importance to the individual, to hiB community, and to the state.

The survey (see Appendix H) reveals, furthermore, that student.

who do not attend college do not receive sufficient training in high

high acbool attend institutions of higher learning, and that Bcience can

only be applied to the general welfare in a democracy by support of the

people. This educational deficiency assumes the alpects of a menace in a

weekly and travel in the tropics 1s common-place.

I. Proficiency Examination

A total of 56 students passed the proficiency test in Hygiene

and received credit in the subject . Their distribution by colleges 1s

given in Table IV.

Ill. Hygiene 10, !£! Coaches ~ Teachers

The advanced course in hygiene for coaches , pnyslcal education

majors, and teachers had a total registration for the year of 99 students.

IV. Hygiene 16, For Occupational Therapy Students

A total of 14 students who were enrolled in the Occupational

Therapy curriculum receiVed instruction in Hygiene 16 during the past

academic ,year .

V. Hygiene !l, Extension Course

In cooperation with the Director of the University Extension

Service. a correspondence course in Hygiene has been offered . which had

a registration of 23 students . The quality of work presented by those

taking it is most satisfactory. This training should better prepare them

to protect themselves and their families and to function more intelli­

gently as citizens interested in improving living conditions in their

community.

SANITATION

Immediate attention has been given to reports which have been

received concerning the conditions of the swimming pools and water supply.

The pools and drinking water of the University have been well supervised

by an able Sanitary Engineer and maintained at a high level of sanitation.

FIRST AID CABINET.

A total of 175 first-aid cabinets are being maintained in the

various buildings on the campus. They are much used and are visited

weekly or twice weekly~ depending upon their location and use, to replace

supplies needed .

SPECIAl. SERVICE Kl UNIVERSITY EVENTS

It has been customary for a number of years for members of the

Health Service Staff upon request to attend certain University functions

in order to render emergency medical treatment if necessary. This service

18 given at the Physical Education Tournaments, Farm and Home Week

Programs, Commencement Exercises, and certain sports eventB.

In cooperation with other departments, the offices of the Health

Serv~e were also made available in emergenCies to guests of the Univer­

sity. It s facilities were offered to those attending Farm and Home Week,

4-H Club Conventi ons, and short courSeS given by the Univer sity .

STKlE LABORATORY SERVICE

Various laboratory services were given to students and CIvil

Service emplorees a s a part of the routine work of the Health ServiCe .

In many instances , these teats Were eSSential either in making effective

the regulations of the UniverSity concerning foodhandlers or in diagnosing

and controlling communicable disease. Bacteriological tests (see Table I)

Were made by the Branch Laboratory of the State Department of Health on

specimens submitted by members of the medical staff of the Health Service.

It is a pleasure to acknowledge our indebtedness to it.

lIDl,UESTS FOR INFOMION

A total of 29 citizens of the State have requested information

on various aspects of public health. Requests Were also received for

reprints of articles by members of the Department and for copies of forma

used by the Health Service. ~estionnaires fro~ numerous Sources have

been many, varied. and often quite comprehensive . These forms of survey

leem to be increasing in popularity and in the demands they make upon the

time of those r equested to fill them out.

The Medical Staff of the Health Service has had the most helpful

cooperation of local and family p~sicians in caring for studente and

injured employees. A total of 238 letters have been received concerning

the p~.iC&l conditions of students and employees who are or have been

patients of these doc t ors .

HOSPITAL IZATION

The number of students admitted to local hospitals during the

academic year was 3,128. Of this numbe r, 2,611 Were sent to the hospital

by local doctors, and 511 went directly from the Health Service Station.

The 511 who were sent to the hospitals from the Health Seryice Station

chose 59 different docto r s to att end them.

Sickness insurance was available to all students during 1945-46

to cover both medical and hospital care , as a r esult of its cost being

made a regular University fee.

1. The Student ~

The facilities of the McKinley Hospital are a great insurance

for the student body. A total of 2,794 stUdents were admitted to the

Hospital for 9 . 673 days, an average of 3.46 days per patient.

Of all the students hospitalized. 89,3 per oent were admitted to

McKinley Hospital and only 10.1 per cent to other hospitals.

lrom Table 6. it will be seen that 434 students were admitted to

the McKinley for communicable disease and remained fo~ a total of 1.577

days or an average stay of 3.63 days per patient . The common cold caused

about nine-tenths of the cases and nearly 75 per cent of the hospital days.

Mumps vas second in incidence with 16 csses and 140 patient days. There

were 5 csses of scarlet fever with a total of 70 hospital days . Four

students ~ere admitted because of influenza and remained a total of 25 days

for an averace of 6.25 days per case .

During the year. University employees suffered 304 accidents

in the line of duty. It vas necessary to have 34 roentgenograms made in

order to determine the presence or abSence of bone injury. Of those

injured 210 required only minor surgical attention and 94 vere referred

to outside surgeons or specialists for prolonged treatment. It vas

necessary to hospitalize 20 employees.

Ill. Local Hospitals

There were 47 students admitted to t he Mercy Hospital for a

total of 193 days. an average stay of ~.ll; to the Burnham Hospital

146 tor 579 days or 3.96 per student. Students vho enter the local

hospitals usually do BO for surgical treatment aDd. therefore, remain

longer on the average than in the McKinley Hospital.
