The Impact of Diabetes
• More than 1 out of every 5 people aged 60 and older has type 2 diabetes.1
• Diabetes is solely responsible for over half of nursing home admissions among adults with diabetes 45 to 74 years old.1
• Residents with diabetes in long-term care are likely to need to stay longer than 90 days and have additional comorbidities such as cardiovascular issues, depression, and pain.1

Enhancing Diabetes Management • AMDA recommends a systematic approach that addresses
the entire interdisciplinary team:– Medical director – Nursing staff – Dietitians– Director of nursing – Nursing assistants – Consultant pharmacist– Physicians – Therapists
• Includes an interdisciplinary care model1
• Involves patients, families, and caregivers in diabetes awareness and self-management1
• Reviews glycemic control on an ongoing basis and intervenes when necessary1
• Individualizes long-term care diabetes therapy by accounting for factors such as mental and physical abilities, disease severity, and coexisting conditions1

Getting Started
• Familiarize yourself with the program resources.
• Confer with your physicians and nursing managers to select the appropriate resources to incorporate into your current program.
• Update your current policies and procedures.
• Distribute template letters to announce the initiative.
• Schedule an in-service program for staff.
• Schedule a family event for residents, families, and caregivers.
