Remember you can onlyget an idea of the costs forthese fees. From place to
place they can be different.So do not look at these pricesas set in stone. They are justto give you a picture of whatto expect. You should findthese listed on your GoodFaith Estimate.
Below is a list of some ofthose non-recurring one-time fees for home closingcosts:
Application Fee - This is afee that the lender requiresto get your application start-ed. This fee can include yourcredit report. They can rangefrom $100 to $400. This fee isnegotiable.
Origination Fee - This feeis for the work of preparing
your loan and is sometimesreferred to as "points". Yourloan officer is usually paidfrom this fee. You can expecta range of 1% to 5% (or 1 to 5points) of the loan amountfor this fee. Definitely nego-tiate this fee.
Mortgage Discount Points- The name for this fee canbe confusing. When you hearthe word discount you think
you are getting a bargain orsomething being lowered for
you. These fees are paid to
buy you a lower interest rate.So it is money you put upfront to lower your rate. In asense it is a bargain when
you consider you will payless interest for your loan.
Where it is confusing is youput money up front to getthis bargain. Mortgage dis-count points can range from.5% to 2% of the loanamount. Be sure to negotiatethis fee.

When life seems to be bearing down on you and the thought
of running is over whelming remember the race in which we
run. The scriptures teach us to persevere and run the race for
the win. The win in Life is what God has called us to do and
our obedience to do just that. Sometimes we take our eyes offthe one who leads the way and that is only our own fault.
When we remember what awaits us at the end we will be more
eager to stay in the battles of life even if we lose a few along the
way. Losing these battles should only make us stronger and
prove our dependence is on God alone.
When our past (Satan) sneaks up behind us and show us our
loses, we need to remind +Satan that it is he who loses in the
end and our forgiveness is sealed in red.
The precious blood of Jesus leads our tomorrows.

Gaylord -- The Otsego Memorial Hospital
(OMH) Medical Group would like to welcome
two new physicians to their team of
providers. Lori M. Burke, MD, and Jessica A.
Kiley, MD, joined David W. Miner, MD, and
OMH Medical Group Obstetrics &
Gynecology the first week of August.
Lori M. Burke, MD, received her under-
graduate degree in biology in 2003 from the
University of Michigan in Ann Arbor,
Michigan, and went on to complete her med-
ical education at the University of Michigan
Medical School in 2007. She completed her
residency at William Beaumont Hospital in
Royal Oak, Michigan, and is a member of the American College of Obstetrics and
Gynecology.
Dr. Burke has been interested in medicine
since a young age after a bicycle accident sent
her to the hospital with a broken collar bone.
From this first experience with healthcare she
knew she wanted to be a physician. Anatomy
and surgery were two areas of special interest
for Dr. Burke, and throughout medical school
she found that she enjoyed working with
women and addressing their healthcare
needs through a range of ages.
She has had several research papers pub-
lished and has delivered professional presen-
tations on a wide range of topics including
Gynecologic Issues of Adolescents with
Downs Syndrome, Autism, and Cerebral
Palsy, Maternal Prompts to Eat, ChildCompliance, and Mother and Child Weight
Status, and Fertility Sparing Management of
Cervical Cancer. She received several awards
while attending school, including being
named a Galloway Resident by the Memorial
Sloan Kettering Cancer Center for
Gynecology Oncology Service.
What is most appealing to me about
OB/GYN is that I can work with women in
every stage of their lives, commented Dr.
Burke. My goal is to be on the same level as
my patients; ensuring they are comfortable
with me and feel confident in asking any and
all questions they may have. Dr. Burke has
special interest in adolescent gynecology
needs and issues, as well as young womens
healthcare issues in general.
Originally from a small town, Dr. Burke is
happy to return to a smaller community, andis excited to be living in Gaylord with her
English Bulldog puppy.
Jessica A. Kiley, MD, received her under-
graduate education in biology at Michigan
State University in East Lansing, Michigan.
She completed medical school at the Saba
University School of Medicine in Saba,
Netherlands, Antilles. She completed her res-
idency at Sparrow Hospital in Lansing,
Michigan.
Dr. Kiley grew up in a medical family; her
father was an orthopedic surgeon and her
step-mother an OB/GYN, through whom she
became familiar in obstetrics and very inter-
ested in labor and delivery. She holds a spe-
cial interest in infertility issues as well as gen-
eral obstetrics.
She has training in both low- and high-riskobstetrics, Maternal Fetal Medicine,
Reproductive Enocrinology with Infertility,
DaVinci Robotic surgery training, and super-
vised junior residents and medical students.
She also delivered a Case Presentation on
Adolescent Behcets Disease.
My goal is to be as approachable, calming,
and caring as I can for my patients. Dr. Kiley
is excited to be working for a community hos-
pital that sees a high volume of patients
needing obstetric care and guidance.
She is looking forward to spending time in
northern Michigan with her young daughter,
Cora, and her husband. Dr. Kiley was very
involved with gymnastics and qualified as an
Olympic gymnast in 1992, and enjoys hiking,
tennis and snow shoeing.Drs. Burke and Kiley join David W. Miner,
MD, at OMH Obstetrics & Gynecology. Dr.
Miner completed medical school at the
Tulane University School of Medicine in New
Orleans, LA, in 1985; his internship at the
National Naval Medical Center in Bethesda,
MD, in 1986, and he completed his residency
at the University of Louisville School of
Medicine, Louisville, KY, in 1993. He also
served in the United States Navy.
Dr. Miner received Board Certification from
the American Board of Obstetrics and
Gynecology in 1995, and is a Fellow with the
American College of Obstetrics and
Gynecology. He has been in practice since
1993, and is also qualified to perform lasersurgery, microsurgery, and laparoscopic pro-
cedures.

Anxiety or nervousness is a common brain and nervous dis-
order that many experience sometime or a nother. It is charac-
terized by extreme panic and fear, uncontrolled and obsessive
thoughts, nightmares and could even have symptoms like
nausea, muscle tension, headaches and stomachaches. A per-
son could feel unconnected, dizzy, uncontrolled and suffer
from palpations, chills and hot flushes.
There are several prescription drugs which are used in the
treatment of anxiety disorder. Lorazepam, alprazolam, and
buspirone are often prescribed along with antidepressants.
Sometimes hormonal secretion from the thyroid or adrenal
glands cause nervousness and need specific treatment.
Psychological counseling may also be useful in the treatment
of nervousness.
Food and well-being have been associated with mood since
ancient times. Carbohydrates are known to be good mood ele-
vators. They act as mild tranquilizers through increasing sero-
tonin which is a calming neurotransmitter in the brain. It is
better to eat complex
carbohydrates such as
potatoes, whole
wheat bread and
pasta as they take
longer to digest andthus one feels full for
longer.
Vitamin B, especial-
ly B-6, also helps in
the manufacture of
serotonin in the brain.
Supplements of
Vitamin B6 have been
used in the treatment
of anxiety disorder.
Food rich in antioxi-
dants together with
essential fats help in
improving energy lev-
els and may relieve
the feelings of anxiety.
Dehydration may also
add to anxiety andnervousness, so it is
advisable to ensure a
good fluid intake. Hot drinks
may have a calming effect,
but try to avoid excess caf-
feine. Herbal teas and hot
milky drinks may be especial-
ly helpful, especially before
bed.
There is ongoing research
regarding specific vitamins
and minerals in the treatment
of nervousness. Inositol, a
cyclic polyalcohol, plays an
important role as a second
messenger in a cell. Though it
is classified as a B vitamin, it
is not really a vitamin as the
human body can synthesizeit. It has been used to help
people suffering from panic
attacks and relieves the
symptoms of obsessive com-
pulsive disorder. A trial has
shown that when it is supple-
mented with multivitamins,
it can reduce anxiety or nerv-
ousness. It is found in many f oods, especially those with high
bran content. Magnesium has also proved to be an important
reliever of anxiety and so may present another useful nutrient
to supplement in the treatment of nervousness.
There are many herbs associated with the treatment of
nervousness. Herbal chamomile tea is often used to combat
insomnia (www.mitamins.com/disease/ Insomnia.html)
associated with anxiety attacks. Passion flower, bacopa and
kava are also used in herbal
tonics given to reduce anxi-
ety. A drink made of the root
of kava and used extensively
in Polynesia is a well known
relaxant. In one study when
used for one week, it helped
subjects to reduce both
depression and high anxiety
levels and nervousness.
These herbs must be taken
under supervision as they
could have other side effects.
Sometimes dieting though calorie reduction and using diet
pills can contribute to an anxiety disorder. Diets should be
undertaken with care and it may be important to seek profes-
sional guidance. Reducing and perhaps even cutting out alco-
hol and coffee have shown remarkable improvement in the
levels of anxiety for some people and generally assist with the
treatment of nervousness.

We're experiencing a heat wave, andit's not just in Cleveland. Nearly every
person I've spoken to from California to
Massachusetts has told me how unusual-
ly hot these past few weeks have been.
Seniors, the elderly and small children
are susceptible to overheating and this
can cause some serious health problems.
Those with a history of stroke, heart
disease, high blood pressure, kidney dis-
ease and respiratory disease need to take
special care. However, overheating can
happen to any one if you're not careful.
Pay attention to the symptoms of heat
exhaustion and heat stroke. Some symp-
toms are: heavy sweating, pale or clam-
my skin, fainting, and vomiting. A heat
stroke is a true medical emergency. A
delay in treatment can be fatal. If you
know someone who is elderly or fragile,
check on him or her to make sure they
stay cool. The elderly are much more
likely to suffer from heat related illness-
es. If a person becomes dehydrated and
cannot sweat enough to cool their body,
their internal temperature may rise to
dangerously high levels, causing heat
stroke
Are your feet and hands swelling? Heat
edema is swelling of the hands and feet
when blood vessels expand and allow
fluid to pool under the skin. Is your skinitchy? Prickly heat is an irritating rash
that is caused by a blockage of the sweat
pores, usually under clothing.
Your sweat is your body's main system
for getting rid of extra heat. As long as
blood is flowing properly to your skin,
extra heat from the core of your body is
"pumped" to the skin and removed by
sweat evaporation. If you do not sweat
enough, you cannot get rid of extra heat
well. If you have poor circulation it is also
difficult to stay cool because your body
can't get rid of heat as well if blood is not
flowing to the skin.
Dehydration will make it harder for
you to cool of in two ways: if you are
dehydrated you won't sweat as much,
and your body will try to keep blood
away from the skin to keep your blood
pressure at the right level in the core of
your body. But, since you lose water
when you sweat, you must make up that
water to keep from becoming dehydrat-
ed. If the air is humid, it's harder for your
sweat to evaporate. This means that your
body cannot get rid of extra heat as well
when it's muggy as it can when it's rela-
tively dry.
Other precautions to consider include:
Increase your intake of water, avoidalcoholic, carbonated, sugar and caf-
feine drinks
This may sound crazy, avoid ice cold
drinks. When your tummy receives fluid
that is ice cold, your internal body tem-
perature must rise in order to compen-
sate, leading to further internal heat
An old farmers trick (you learn this
quickly if you've ever helped to bail hay
in August) Wet a bandana (or scarf) and
put it in the freezer. Wrap this around
your neck, once the fabric is thawed,
wrap the cool scarf around your fore-
head. My grandma use to have a largewrap that she would position around her
bust line. (Bailing hay is a family event J)
Wear clothing that is light in color and
loose fitting (let your skin breathe)
Avoid outdoor activity during the after-
noon hours
Eat less protein and
more fruits and vegetables
(proteins increase your
metabolism which heats
you up inside) this leads to
further water loss. The
fruits and vegetables will add minerals
and salts that you are loosing in your
sweat
Treat yourself to a cool bath
If you don't have air-conditioning, stay
in the lowest section of your home if pos-
sible. Keep in mind, heat rises. If you
have window fans, set the fan to pull hot
air out of your home, rather than pulling
more hot air in from the outdoors.
Seniors living without air conditioning
may want to consider spending the
hottest part of the day where relief from
the heat can be found. Senior centers,
the mall, libraries, churches and muse-
ums are all (almost always) air-condi-tioned.
