INTRODUCTION
During July 1969-June 1970 an estimated406.7 miIIion acute illnesses or injuries requiringeither medical attention or restriction of daiIyactivities occurred in the civilian, noninstitu-tional population of the United States (table 1).The incidence of acute conditions was 204.8conditions per 100 persons per year. Thisincidence rate is very similar to that of 206.8 per100 pers~ns per year reported for the 12 monthsending in June 1969. Although the number ofinfluenza-like conditions per 100 persons peryear (41.1) was about 25 percent lower duringthe current year contrasted with the prior year’sincidence rate of 54.7 when the Hong Konginfluenza epidemic occurred, increases in othercondition groups compensated to offset thisdecline.
This report is based on information col-lected in the Health Interview Survey and is oneof an annual series of such reports on theincidence of acute illness or injury in the civilianpopulation, exclusive of inmates of institutions.The most recent report, Series 10, No. 69,covered the epidemiologic year, July 196 8-June1969; other “Acute Conditions” reports inSeries 10 are nos. 1, 10, 15, 26,38,44, and 54.The incidence of acute conditions on a calendaryear basis is summarized in the “Current Esti-mates” reports, Series 10, Nos. 52, 60, 63, and72 for 1967 through 1970.

SOURCE OF DATA
The information from the Health InterviewSurvey presented in this report is derived fromhousehold interviews of a probability sample ofthe civilian, noninstitutional population of theUnited States. The sample is designed so thatinterviews are conducted during every week ofthe year by trained personnel of the U.S. Bureauof the Census. However, from January throughMarch 1970 interviews were conducted everyother week due to budgetary restrictions. DuringJuly 1969-June 1970, the sample was composedof about 37,000 households containing about115,000 persons Iiving at the time of theinterview. Comparability with previous a.mmdestimates is not affected by the reduced samplesince the weighting procedure employed for theHealth Interview Survey adjusts for the missingweeks.
A description of the design of the survey,of the methods used in estimation, and ofgeneral qualifications of the data obtained fromthe survey is presented in appendix I. Sinceestimates shown in this report are based on asample of the population rather than on theentire population, they are subject to samplingerror. Therefore particular attention should bepaid to the section entitled “Reliability ofEstimates.” Sampling errors for most of theestimates are of relatively low magnitude. How-ever, where an estimated number or the numera-

tor or the denominator of a rate or percentage issmall, the sampling error may be high.
Because of the reduction in sampling dur-ing January-March 1970, the estimates for thisquarter are subject to larger sampling error thanthose for the remainder of the year.
Certain terms used in this report are de-fined in appendix IL Some of the terms havespecialized meanings for the purpose of thesurvey. For example, estimates of the incidenceof acute conditions include, with certain ex-ceptions, those conditions which had startedwithin the 2 weeks prior to the week ofinterview and which had involved either” medicalattention or restricted activity. The exceptions,listed in appendix II, are certain conditions suchas heart trouble and diabetes which are alwaysconsidered chronic, regardless of duration oronset.
Annual estimates of the number of disabili-ty days associated with each acute condition arederived from the number of such days experi-enced during the 2-week period prior to week ofinterview and include all those reported, even ifthe illness causing the disability had its onsetprior to the 2-week period. Since any particularday of disability may be associated with morethan one condition, the sum of days for allconditions adds to more than the total numberof person-days of disability.
The questionnaires employed by the HealthInterview Survey during July 1969-June 1970are illustrated in the “Current Estimates” re-ports for 1969 and 1970 (Series 10, Nos. 63 and72). Appendix III shows the probe questionsand condition pages used during January-June1970.
COMPARISON WITH EARLIER YEARS
During the 12 months ending in June 1970the annual incidence of new acute illnesses orinjuries per 100 persons was 204.8, closelycomparable to the incidence rate of 206.8 forJuly 1968-June 1969. Table A presents compar-ative rates of incidence and associated disabilitydays for 7 data collection years. The datacollection procedure was the same for’ the first 4years shown in the table. During July 1967-June1968 the sample was split during a test of a new
collection procedure, the “person approach”described in the “Current Estimates” report for1967 (Series 10, No. 52). The “conditionapproach” used in earlier years was used inone-half of the sample. The person approach hasbeen employed as the collection method for thelast 2 years in the table. Also, medical codingemploying the 8th Revision of the InternationalClassification of Diseases as it has been adaptedfor use in the United States began in 1969.Heretofore medical coding was done using the7th “Revision of the International Classificationof Diseases. These two changes in collectionmethod and medical coding cause somle loss incomparability over the 7 years. However, in thebroad groupings shown in the table, the loss incomparability should be minimized. 1
Although the total incidence of acuteconditions per 100 persons per year was aboutthe same for July 1968-June 1969 and July1969-June 1970, several of the condition groupschanged substantially. The incidence rate ofinfluenza-like conditions dropped by a fourthfrom 54.7 to 41.1 conditions per 100 personsper year and the annual incidence in the group“other respiratory conditions” rose from 3.8 to6.1 conditions per 100 persons. The incidencerate of upper respiratory conditions remainedabout the same for the 2 years. The reportedincidence of injuries increased by about 14percent from 24.2 to 27.7 per 100 persons peryear. An additional probe question (appendixIII, question number 14) contributed to the risein rate.
The only age group to show a significantchange in incidence rate was that for personsunder 6 years of age, where the rate rose from317.3 to 346.6. The geographic distribution ofacute conditions was substantially the same forthe two time periods.
In general, acute conditions caused fewerdisability days per person per year during thecurrent year than in July 1968-June 1969. Theannual rates of restricted activity amd bed

disability were reduced by about a half day to8.5 and 3.8 days per person, respectively.Among children 6-16 years old the number ofdays lost from school per person per yeardeclined from 4.9 to 4.5 days during July1969-June 1970. However, among currentlyemployed persons the number of days lost fromwork per person per year was essentially thesame for the two periods—3.9 and 3.8 days,respectively. A currently employed person is onewho had a job or business or worked at any timeduring the 2-week period prior to the week ofinterview.
INCIDENCE OF ACUTE CONDITIONS ANDASSOCIATED DISABILITY DAYS
During July 1969-June 1970 there was anincidence of 406.7 million acute illnesses andinjuries which were medically attended orcaused at least 1 day of restricted activity (table1). These conditions caused an estimated 1.7billion days of restricted activity or an averageduration of 4.2 days per condition (tables B and2). They also caused an estimated 748.7 milliondays spent in bed or an average duration of

about 1.8 days per condition (table 3). Theaverage duration in a given condition category isdetermined from the total number of disabilitydays during the year divided by the incidenceduring the year. It can also be computed bydividing the particular rate of disability days bythe incidence rate.
Females experienced a higher incidence ofacute conditions per 100 persons per year thandid males-212.2 for females and 196.9 formales. Similarly, the average duration of re-stricted activity per condition was substantiallylonger for females than for males-4.4 days percondition for females compared with 3.9 daysper condition for males (table C). The averageduration of bed stay per condition was 2.0 daysfor females and 1.7 days for males.
Table B shows that the condition group“other respiratory conditions” had the longestaverage duration of disability and that “upperrespiratory conditions” had the short~est dura-tion.
About 19.4 percent of acute conditionswere medically attended but did not causereduction in daily activities (tables D and 4).Thus about four of each five acute conditions

caused activity restriction. About three of eachfive conditions were medically attended ( 19.4percent medically attended only and 38.6 per-cent medically attended and activity restricting).
As age increased, the incidence of acuteconditions per 100 persons per year declined forall acute conditions (table 5). However, thispattern was noted primarily for the infective andparasitic diseases and respiratory illnesses. Theincidence rates of digestive conditions and in-juries were more evenly distributed across theage span shown in the table. The averageduration of disability days per acute conditionwas longest for persons 45 years and over with7.3 days of restricted activity and 3.1 days inbed (tables C, 6, and 7). Correspondingly, theaverage duration was least for children under 6years of age–2.8 days of restricted activity and1.2 days in bed.
During July 1969-June 1970 acute condi-tions caused an estimated 203.0 million dayslost from school or 4.5 days per child aged 6-16years (table 8). As shown in table 5 there wereabout 118.3 million acute conditions reportedfor these children, so the average duration ofabsence from school per condition was short—1.7 days per condition.
The currently employed population experi-enced about 1.7 acute illnesses or injuries perpm-son per year during the year ending in June1970 (table 9). These acute conditions resultedin 3.8 days Iost from work per currentIyemployed person (table 10). When these figuresare used to show average duration per condition,they show that there were 2.3 days absencefrom work per condition.
GEOGRAPHIC DISTRIBUTION
The incidence of acute illnesses and injuriesper 100 persons per year was quite similar forpersons living in metropolitan areas and nonfarmresidents outside of metropolitan areas (table11). However, farm residents outside metropoli-tan areas experienced a lower incidence ratethan either of the other two groups. In eachresidence category the pattern of age and sexdistribution was much the same as that for the

entire population (figure 1 and table 11). Non-farm residents outside standard metropolitanstatistical areas (SMSA) had a somewhat longeraverage duration of restricted activity per condi-tion than did residents of the other areas (table12). The average duration of bed stay percondition was the same for the residents ofSMSA’S and nonfarm areas (table 13) and the

average duration of disability was lowest amongfarm residents.
The incidence rates for most conditiongroups were quite similar by place of residence(table 14). However, although the average dura-tion of disability was lowest among farm res-idents for most categories. it was higher for
restricted activity days due to injuries (tabIes C,15, and 16).
The incidence of acute conditions per 100persons per year was highest in the West Regionand very similar in each of the other threegeographic regions (table 17). The average dura-tion of disability per condition was remarkablysirniIar in each region-average duration of re-stricted activity ranged from 4.0 days in theNorth Central Region to 4.3 days in the North-east and South Regions and the range of days inbed was 1.7 to 2.0 days (tabIe C, 18, and 19).Figure 2 shows the incidence rates by age foreach region. The higher rates for the WestRegion occurred for most age groups.
Respiratory conditions were primarily re-sponsible for the excess incidence per 100persons per year for the West Region (tabIe 20).The incidence rate of upper respiratory condi-tions was highest in the West Region. Theincidence of conditions reported as influenza per100 persons per year was about the same in theNorth Central and West Regions; in each ofthese regions the rate was about one-third higherthan the incidence rate for the Nation as awhole.
Even though the incidence rate of in-fluenza-like iIlness was lo~ti in the Northeast andSouth Regions, the average duration of disabilityper condition was higher in these regions thaneIsewhere, as the following figures derived fromtables 20-22 show:

APPENDIX I
NOTES ON METHODS
This report is one of a series of statisticalreports prepared by the National Center forHealth Statistics (NCHS). It is based on informa-tion collected in a continuing nationwide sampleof households in the Health Interview Survey(HIS).
The Health Interview Survey utilizes aquestionnaire which obtains information onpersonzil &d demographic characteristics, ill-nesses, injuries, impairments, chronic conditions,and other health topics. As data relating to eachof these various broad topics are tabulated andanalyzed, separate reports are issued which coverone or more of the specific topics. The presentreport is based on data collected in householdinterviews during July 1969-June 1970.
The population covered by the sample forthe Health Interview Survey is the civilian,noninititutionalized population of the UnitedStates living at the time of the interview. Thesample does not include members of the ArmedForces or U.S. nationals living in foreign coun-tries. It should also be noted that the estimatesshown do not represent a complete measure ofany given topic during the specified calendarperiod since data are not collected in theinterview for persons who died during thereference period. For many types of statisticscollected in the survey, the reference periodcovers the 2 weeks prior to the interview week.For such a short period, the contribution bydecedents to a total inventory of conditions orservices should be very small. However, thecontribution by decedents during a long refer-ence period (e.g., 1 year) might be sizable,especially for older persons.
Statistical Design of theHealth Interview Survey
General plan.–The sampling plan of thesurvey follows a multistage probability designwhich permits a continuous sampling of thecivilian, noninstitutional population of the Unit-ed States. The sample is designed in such a waythat the sample of households interviewed eachweek is representative of the target populationand that weekly samples are additive over time.This feature of the design permits both contin-uous measurement of characteristics of samplesand more detailed analysis of less commoncharacteristics and smaller categories of health-reIated items. The continuous collection hasadministrative and operational advantages aswell as technical assets since it permits fieldworkto be handled with an experienced, stable staff.
The overall sample was designed so thattabulations can be provided for each of the fourmajor geographic regions and for urbap and ruralsectors of the United States.
The first stage of the sample design consistsof drawing a sample of 357 primary samplingunits (PSU’s) from approximately 1,900 geo-graphically defined PSU’S. A PSU consists of acounty, a small group of contiguous counties, ora standard metropolitan statistical area. ThePSU’S collectively cover the 50 States and theDistrict of Columbia.
With no loss in general understandirlg, theremaining stages can be combined and treated inthis ‘discussion as an ultimate stage. WithinPSU’S, then, ultimate stage units called segmentsare defined in such a manner that each segmentcontains an expected six households. Threegeneral types of segments are used.

Census address listings were used for allareas of the country where addresses were welldefined and could be used to locate housingunits. In general the list frame included thelarger urban areas of the United States fromwhich about two-thirds of the HIS sample wasselected.
The usual HIS sample consists of approxi-mately 8,000 segments which yield a probability ysample of about 134,000 persons in 42,000interviewed households in a year. However, thefirst quarter of 1970 contained a sample reduc-tion of 7 full weeks of interviewing due tobudgetary restrictions , whereas the remainingquarters each contained the full 13 weeks.During the 45 weeks in 1970 the sample wascomposed of approximately 37,000 householdscontaining about 116,000 persons living at thetime of the interview. Comparability with pre-vious annual estimates is not affected by thereduced sample since the weighting procedureemployed by the Health Interview adjusts forthe missing weeks although the samplingerrors are somewhat higher for 1970 estimates(see statement on adjustment of relative sam-pling error on page 47).

Collection of data. –Field operations forthe survey are performed by the U.S. Bureau ofthe Census under specifications established bythe National Center for Health Statistics. Inaccordance with these specifications the Bureauof the Census participates in survey planning,seIects the sample, and conducts the fieldinterviewing as an agent of NCHS. The data arecoded, edited, and tabulated by NCHS.
Estimating Procedures. –Since the design ofthe HIS is a complex multistage probabilitysampIe, it is necessary to use complex proce-dures in the derivation of estimates. Four basicoperations are involved:

Inflation by the reciprocal of the probabili-ty of selection.–The probability of selec-tion is the product of the probabilities ofselection from each step of selection in thedesign (PSU, segment, and household).
Nonresponse adjustment. —The estimatesare inflated by a multiplication factorwhich has as its numerator the number ofsampIe households in a given segment andas its denominator the number of house-holds interviewed in that segment.
First-stage ratio adjustment. –%,mpling the-ory indicates that the use of auxiliaryinformation which is hlghIy correlated withthe variables being estimated improves thereliability of the estimates. To reduce thevariability between PSU’S within a region,the estimates are ratio adjusted to 1960population within six color-residenceclasses.
Poststratification by age-sex-color. –Theestimates- are ratio adjust;d within each of60 age-sex-coIor cells to an independentestimate of the population of each ceil forthe survey period. These independent esti-mates are prepared by the Bureau of the

Census. Both the first-stage and poststrati-fied ratio adjustments take the form ofmultiplication factors applied to the weightof each elementary unit (person, house-hold, condition, and hospitalization).
The effect of the ratio-estimating process isto make the sample more closely representativeof the civilian, noninstitutional population byage, sex, color, and residence, which therebyreduces sampling variance.
As noted, each week’s sample representsthe population living during that week andcharacteristics of the population. Consolidationof samples over a time period, e.g., a calendarquarter, produces estimates of average character-istics of the U.S. population for the calendarquarter. Similarly, population data for a year areaverages of the four quarterly figures.
For prevalence statistics, such as number ofpersons with speech impairments or number ofpersons classified by time interval since lastphysician visit, figures are first calculated foreach calendar quarter by averaging estimates forall weeks of interviewing in the quarter. Preva-lence data for a year are then obtained byaveraging the four quarterly figures.
For other types of statistics–namely thosemeasuring the number of occurrences during aspecified time period—such as incidence of acuteconditions, number of disability days, or num-ber of visits to a doctor or dentist, a similarcomputational procedure is used, but the statis-tics are interpreted differently. For these items,the questionnaire asks for the respondent’sexperience over the 2 calendar weeks prior tothe week of interview. In such instances theestimated quarterly total for the statistic is 6.5times the average 2-week estimate produced bythe 13 successive samples taken during theperiod. The annual total is the sum of the fourquarters. Thus the experience of persons zkter-viewed during a year–experience which actuallyoccurred for each person in a 2-calendar-weekinterval prior to week of interview-is treated asthough it measured the total of such experiencedura”ng the year. Such interpretation leads to nosignificant bias.
General Qualifications
Nonresponse.–Data were adjusted for non-response by a procedure which imputes topersons in a household which was not intewiew-ed the characteristics of persons in households inthe same segment which were interviewed. Thetotal noninterview rate was about 5 percent– 1percent was refusal, and the remainder wasprimarily due to the failure to find an eligiblerespondent at home after repeated calls.
The interview process. –The statistics pre-sented in this report are based on repliesobtained in interviews with persons in thesample households. For children and for adultsnot present in the home at the time of theinterview, the information was obtained from arelated household member such as a spouse orthe mother of a child.
There are limitations to the accuracy ofdiagnostic and other information collected inhousehold interviews. For diagnostic informa-tion, the household respondent can usually passon to the interviewer only the information thephysician has given to the family. For conditionsnot medically attended, diagnostic informationis often no more than a description of symp-toms. However, other facts, such as the numberof disability days caused by the condition, canbe obtained more accurately from householdmembers than from any other source since onlythe persons concerned are in a position to reportthis information.
Rounding o~numbers.–The original tabula-tions on which the data in this report are basedshow all estimates to the nearest whole umit. AUconsolidations were made from the originaltabulations using the estimates to the nearestunit. In the final published tables, the figures arerounded to the nearest thousand, although theseare not necessarily accurate to that detail.Devised statistics, such as rates and percentdistributions, are computed after the estimateson which these are based have been rounded tothe nearest thousand.
Population figures. –Some of the publishedtables include population figures for specifiedcategories. Except for certain overall totals by

age, sex, and color, which are adjusted toindependent estimates, these figures are basedon the sample of households in the HIS. Theseare given primarily to provide denominators forrate computation, and for this purpose are moreappropriate for use with the accompanyingmeasures of health characteristics than otherpopulation data that may be available. With theexception of the overall totals by age, sex, andcolor mentioned above, the population figuresdiffer from corresponding figures (which arederived from different sources) published inreports of the Bureau of the Census. (Forpopulation data for general use, see the officialestimates presented in Bureau of the Censusreports in the P-20, P-25, and P-60 series.)
Reliability of Estimates
Since the statistics presented in this reportare based on a sample, they will differ somewhatfrom the figures that would have been obtainedif a complete census had been taken using thesame schedules, instructions, and interviewingpersonnel and procedures.
As in any survey, the results are alsosubject to reporting and processing errors anderrors due to nonresponse. To the extent pos-sible, these types of errors were kept to aminimum by methods built into survey proce-dures. Although it is very difficult to measurethe extent of bias in the Health InterviewSurvey, a number of studies have been conduct-ed to study this problem. The results have beenpublished in several reports. 6-9
The standard error is primarily a measureof sampling variability, that is, the variationsthat might occur by chance because only asample of the population is surveyed. As calcu-lated for this report, the standard error alsoreflects part of the variation which arises in the

measurement process. It does not include esti-mates of any biases which might be in the data.The chances are about 68 out of 100 that anestimate from the sample wouId differ from acomplete census by less than the standard error.The chances are about 95 out of 100 that thedifference would be less than twice the standarderror and about 99 out of 100 that it would beless than 2% times as large.
The relative standard error of an estimate isobtained by dividing the standard error of theestimate by the estimate itself and is expressedas a percentage of the estimate. For this report,asterisks are shown for any cell with more than a30-percent relative standard error. Included inthis appendix are charts from which the relativestandard errors can be determined for estimatesshown in the report. In order to derive relativeerrors which would be applicable to a widevariety of health statistics and which could beprepared at a moderate cost, a number ofapproximations were required. As a result, thecharts provide an estimate of the approximaterelative standard error rather than the preciseerror for any specific aggregate or percentage.Since the sampling error charts are based on thefull HIS design, the sampling errors derived fromthe charts for 1970 estimates must be adjustedto reflect the sample reduction .~ade during thequarter of 1970. For annual statistics the adjust-ment factor is 1.08, and for first quarterestimates it is 1.5.
Three classes of statistics for the healthsurvey are identified for purposes of estimatingvariances.
Narrow range. –This class consists of (1)statistics which estimate a population attribute,e.g., the number of persons in a particular

income group, and (2) statistics for which themeasure for a single individual during the refer-ence period used in data collection is usuallyeither O or 1 or on occasion may take on thevalue 2 or very rarely 3.
Medium rarzge. -This class consists of otherstatistics for which the measure for a singleindividual during the reference period used indata collection will rarely lie outside the range Oto 5.
Wide range. –This class consists of statisticsfor which the measure for a single individualduring the reference period used in data collec-tion can range from O to a number in excess of5, e.g., the number of days of bed disability.
In addition to classifying variables accord-ing to whether they are narrow-, medium-, orwide-range, statistics in the survey are furtherdefined as:
Type A.–Statistics on prevalence and incidencedata for which the period of reference inthe questionnaire is 12 months.
Type B.–Incidence-type statistics for which theperiod of reference in the questionnaire is 2weeks.
Type C.–Statistics for which the referenceperiod is 6 months.
Only the charts on sampling error applica-ble to data contained in this report are pre-sented.
General rules for determining relative sam-pling errors. –The “guide” on page 50, togetherwith the following rules, will enable the readerto determine approximate relative standard er-rors from the charts for estimates presented inthis report.
Rule 1. Estimates of aggregates: Approximaterelative standard errors for estimates ofaggregates such as the number of per-sons with a given characteristic areobtained from appropriate curves onpages 51-53. The number of persons inthe total U.S. population or in an age-sex-color class of the total population isadjusted to official Bureau of the Cen-sus figures and is not subject to sam-pling error.
Rule 2. Estimates of percentages in a percentdistra”bution: Relative standard erro~sfor percentages in a percent distributionof a total are obtained from appropriatecurves on pages 54-56. For values whichdo not fall on one of the curves presentedin the chart, visual interpolation willprovide a satisfactory approximation.
Rule 3. Estimates of rates where the numeratoris a subclass of the denominator: Thisrule applies for prevalence rates orwhere a unit of the numerator occurs,with few exceptions, only once in theyear for any one unit in the denomina-tor. For example, in computing the rateof visual impairments per 1,000 popula-tion, the numerator consisting of per-sons with the imp airment is a subclassof the denominator which includes allpersons in the population. Such rates ifconverted to rates per 100 may betreated as though they were percentagesand the relative standard errors ob-tained from the chart P4AN-M. Ratesper 1,000, or on any other base, mustfirst be converted to rates per 100; thenthe percentage chart will provide therelative standard error per 100.
Rule 4. Estimates of rates where the numeratoris not a subclass of the denominator:This rule applies where a unit of thenumerator often occurs more than oncefor any one unit in the denominator.For example, in the computation of thenumber of persons injured per 100currently employed persons per year, itis possible that a person in the denomi-nator could have sustained more thanone of the injuries included in thenumerator. Approximate relativestandard errors for rates of this Kindmay be computed as follows:
(a) Where the denominator is the totalU.S. population or includes all personsin one or more of the age-sex-colorgroups of the total population, therelative error of the rate is equwdentto the relative error of the numeratorwhich can be obtained directlly fromthe appropriate chart.

(b) In other cases the relative standarderror of the numerator and of thedenominator can be obtained fromthe appropriate curve. Square each ofthese relative errors, add the resultingvalues, and extract the square root ofthe sum. This procedure will result inan upper bound on the standard errorand often will overstate the error.
Rule 5. Estimates of difference between twostatistics (mean, rate, total, etc.): Thestandard error of a difference is ap-proximately the square root of the sumof the squares of each standard errorconsidered separately. A formula for thestandard error of a difference d=X1 -X2 is

Terms Relating to Conditions
Condition.–A morbidity condition, or sim-ply a condition, is any entry on the question-naire which describes a departure from a state ofphysical or mental well-being. It results from apositive response to one of a series of “medical-disability impact” or “illness-recall” questions.In the coding and tabulating process conditionsare selected or classified according to a numberof different criteria such as whether they weremedically attended, whether they resulted indisability, or whether they were acute or chron-ic; or accordhg to the type of disease, injury,impairment, or symptom reported. For thepurposes of each published report or set oftables, only those conditions recorded on thequestionnaire which satisfy certain stated crite-ria are included.
Conditions are classified by type accordingto the Eighth Revision International Classifica-tion of Diseases, Adapted for Use in the UnitedStates, with certain modifications adopted tomake the code suitable for a household inter-view survey.
Acute condiiion.–An acute condition isdefined as a condition which has lasted.less than3 months and which has involved either medicalattention or restricted activity. Because of theprocedures used to estimate incidence, the acuteconditions included in this report are the condi-tions which had their onset during the 2 weeksprior to the interview week and which involvedeither medical attention or restricted activityduring the 2-week period. Onset of condition.–A condition is con-sidered to have had its onset when it was firstnoticed. This could be the time the person firstfelt sick or became injured, or it couId be thetime when the person or his family was first toldby a physician that he had a condition of whichhe was previously unaware.
Incidence of conditions.–The incidence ofconditions is the estimated number of condi-tions having their onset in a specified timeperiod. As previously mentioned, minor acuteconditions involving neither restricted activitynor medical attention are excluded from thestatistics. The incidence data shown in somereports are further limited to various subclassesof conditions, such as “incidence of conditionsinvolving bed disability. ”
A ctiuity-restric ting condition. –Anactivity-restricting condition is one which had itsonset in the past 2 weeks and which caused atleast 1 day of restricted activity during the 2calendar weeks before the interview week. (Seedefinition of “Restricted-activity day.”)
Bed-disabling condition.–A condition withonset in the past 2 weeks involving at least 1 dayof bed disability is called a bed-disabling condi-tion. (See definition of “Bed-disability day.”)
Medically attended condition. –A conditionwith onset in the past 2 weeks is consideredmedicalIy attended if a physician has beenconsulted about it either at its onset or at anytime thereafter. Medical attention includes con-sultation either in person or by telephone fortreatment or advice. Advice from the physiciantransmitted to the patient through the nurse iscounted as well as visits to physicians in clinicsor hospitals. If during the course of a single visitthe physician is consulted about more than onecondition for each of several patients, eachcondition of each patient is counted as medi-cally attended.
Discussion of a child’s condition by thephysician and a responsible member of thehousehold are considered as medical attentioneven if the child was not seen at that time.
For the purpose of this definition, the term“physician” includes doctors of medicine andosteopathic physicians.
Terms Relating to Disability
Disability .–Disability is the general termused to describe any temporary or long-term
reduction of a person’s activity as a result of anacute or chronic condition.
Disability days are classified according towhether they are days of restricted activity,bed-days, work-loss days, or schooI-loss days. AI1days of bed disability are, by definition, days ofrestricted activity. The converse form of thisstatement is, ot course, not true. Days lost fromwork and days lost from school are also days ofrestricted activity for the working and school-age populations. Hence, restricted activity is themost inclusive term used in describing disabilitydays.
Condition day.–Condition days of re-stricted activity, bed disability, and work lossare days of the various forms of disabilityassociated with any one condition. Since anyparticular day of disability may be associatedwith more than one condition, the sum of daysfor conditions may add to more than the totalnumber of person days.
Restncted-actzuity day.–A day of restrict-ed activity is one on which a person substantial-ly reduces the amount of activity normal forthat day because of a specific illness or injury.The type of reduction varies with the age andoccupation of the individual as welI as with theday of the week or season of the year. Restrict-ed activity covers the range from substantialreduction to complete inactivity for the entireday.
Bed-disability day.-A day of bed disabilityis one on which a person stays in bed for all ormost of the day because of a specific illness orinjury. It is considered to be a day only if theperiod of bed disability includes more than halfof the daylight hours. All hospital days forinpatients are considered to be days of beddisability even if the patient was not actually inbed at the hospitzd.
IVo+loss day.–A day lost from work is aday on which a person did not work at his job orbusiness for a least one-half of his normalworkday because of a specific illness or injury.The number of days Iost from work is deter-mined only for persons 17 years of age and overwho reported that at any time during the 2-weekperiod covered by the inten’iew they eitherworked at or had a job or business. (See“Currently employed persons” under “Demo-graphic Terms.”)

School-loss day. –A day lost from school isa normal school day on which a child did notattend school because of a specific illness orinjury. The number of days lost from school isdetermined only for children 6-16 years of age,
Demographic Terms
Age.–The age recorded for each person isthe age at last birthday. Age is recorded in singleyears and grouped in a variety of distributionsdepending upon the purpose of the table.
Quarter.–The quarters used by the HealthInterview Survey are actually 13-week periodsrather than 3 calendar months. Since each13-week period begins on a Monday and ends ona Sunday, the actual dates of the beginning andend of each 13-week period may overlap intoanother calendar quarter. Therefore, the timeperiods in the table headings are the approxi-mate rather than the precise periods duringwhich the interviewing was conducted.
Currently employed persons. –Currentlyemployed persons are all persons 17 years of ageand over who reported that at any time duringthe 2-week period covered by the interview theyeither worked at or had a job or business.Current erpployment includes paid work as anemployee of someone else, self-employment inbusiness, farming, or professional practice, andunpaid work in a family business or farm.Persons who were temporarily absent from theirjob or business because of a temporary illness,vacation, strike, or bad weather are consideredas currently employed if they expected to workas soon as the particular event causing theirabsence no longer existed.
Free-lance workers are considered as havinga job if they had a definite arrangement withone or more employers to work for pay accord-ing to a weekly or monthly schedule, either fulltime or part time. Excluded from the currentlyemployed population are such persons who haveno definite employment schedule but who workonly when their services are needed.
Also excluded from the current employedpopulation are ( 1) persons who were notworking, even though having a job or business,but were on layoff or looking for work, (2)persons receiving revenue from an enterprise in
whose operation they did not participate, (3)persons doing housework or charity work forwhich they received no pay, and (4) seasonalworkers during the unemployment season.
The number of currentIy employed personsestimated by the Health Interview Survey (HIS)will differ from the estimates prepared by theCurrent Population Survey (CPS), Bureau of theCensus, for several reasons. In addition tosampling variability they include three primaryconceptual differences, namely: ( 1) HIS esti-mates are for persons 17 years of age or over;CPS estimates are for persons 14 years of age orover. (2) HIS uses a 2-week-reference periodwhile CPS uses a l-week-reference period. (3)HIS is a continuing survey with separate samplestaken weekly, while CPS is a monthly sampletaken for the survey week which includes the12th of the month.
Place of residence. –The place of residenceof a member of the civilian, noninstitutionalpopulation is classified as inside a standardmetropolitan statistical area (SMSA) or outsidean SMSA according to farm or nonfarm resi-dence.
Standard metropolitan statistical areas. –The definitions and titles of SMSA’S me estab-lished by the U.S. Office of Management andBudget with the advice of the Federal Com-mittee on Standard Metropolitan StatisticalAreas. There were 212 SMSA’S defined for the1960 decennial census.
The definition of an individual SMSAinvolves two considerations: first, a city or citiesof specified population which constitute thecentral city and identify the county in which itis located as the central county; and, second,economic and social relationships with contig-uous counties (except in New England) whichare metropolitan in character, so that the periph-ery of the specific metropolitan area may bedetermined. SMSA’S are not limited by Stateboundaries.
Farm and nonfarm residence.–The popula-tion residing outside SMSA’S is subdivided intothe farm population, which comprises all non-SMSA residents living on farms, and the non-farm population, which comprises the remainingnon-SMSA population. The farm populationincludes persons living on places of 10 acres or

more from which sales of farm productsamounted to $50 or more during the previous12 months or on places of less than 10 acresfrom which sales of farm products amounted to$250 or more during the preceding 12 months.Other persons living in non-SMSA territory wereclassified as nonfarrn if their household paid rentfor the house but their rent did not include anyland used for farming.
Sales of farm products refer to the grossreceipts from the sale of field crops, vegetables,fruits, nuts, livestock and livestock products(milk, WOO1,etc.), poultry and poultry products,and nursery and forest products produced on theplace and sold at any time during the preceding12 months.
