Abstract
Background: Infection is an invasion of pathogenic microorganisms to whatever
part of the body exposed to them .these microorganisms find a good habitats to
increase causing illness, the boy starts fighting against these microorganisms, but
when the resistant is not enough interference is necessary controls infection .
Infection control, refers to policies and procedures used to prevent infections.
The study aims at evaluating the effectiveness of nurse midwives training
regarding their performance on infection control in labor rooms of government
hospitals in Khartoum State during the period from March 2015 to March2018.
Methods: this was a quasi-experimental study (pre-test and post-test carried for
the same group). The method was conducted in fifteen labor Rooms in
government hospitals in Khartoum state. One hundred and thirty five nurse
midwives on duty in labor rooms were chosen for total coverage. Different
procedure were done: face to face interviews questionnaire and observation
checklist. The Data collection was taken by the researcher during pre and post
training intervention. A follow up test was made three months later using the same
data collection tools. The researcher used "Descriptive Statistics", the paired
sample "t" test, 95% and confidence interval (C .I.) to do Statistical tests Results:
The overall knowledge average score at pre-test measurement was 60.7% after
educational program the average knowledge score rose to 91.9 % (Figures 2 and
3).there was statistic significant difference between the two average scores (p
value 0.00). The second component of the research was to assessment of practical
skills regarding infection control. the average overall score achieved in practical
skills was 60.7% at pre-test measurement. It was 93.3% at posttest measurement
after training intervention (Figures 4 and 5). the statistical difference in
improvement was highly significant (p value 0.00). There was further
improvement in the practical skills score, (P= 0.01) in the follow up test.

Conclusions: the study showed that structured training program regarding
infection control had significant impact on the nurse's midwives knowledge and
practical skills.

CAHPTER ONE
1.1. Introduction
Infection is defined as invasion and multiplication of pathogenic
microorganisms in a bodily part or tissue, which may produce subsequent tissue
injury and progress to overt disease through a variety of cellular or toxic
mechanisms (1).
Prevention and control of infection are essential cornerstones of clinical
care in all health setting; it could be more cost effect if we give emphasis to
effective control infection precautions to face the changing pattern of infection,
and to reduce the use of antibiotic. It's necessary for all health care workers to
have sound knowledge about infectious microorganism and good understanding
of infection control and evidence based practice these are essential and important
factors in delivery of a quality patient focused services (2).
Labor rooms is the place where pregnant woman stay from the beginning of
labor till two-six hours after delivery of the fetus and placenta. Throughout this
time the delivery of the fetus and placenta may need some minor surgical
procedure. In this regard, infection control refers to policies and procedures used
to minimize the risk of spreading infections (3).
1.2 Problem statement:
Preventing maternal and neonatal death is an international priority. This
reaffirmed by united nation at MDGs, unfortunately at global level every minute
a woman dies somewhere in the world. In Sudan maternal mortality rate is132
per100000 life birth in a year, in Khartoum state maternal mortality rate is 88per
100000 life birth in a year (4).

The majority of maternal death causes are avoidable (hemorrhage, sepsis,
eclampsia, obstructed labor and other indirect causes) maternal death due to
sepsis is estimated about (15.3%) of all causes (5). To achieve the MDGs of
reducing maternal and neonatal mortality rate we need a process to promote safe
motherhood. Sepsis is one of these preventable causes, need to address infection
control process in labor room. There is ongoing training programmed of infection
control in health facilities comprising of theoretical training to care providers
,unfortunately the care providers in labor rooms are not enrolled in this training
from the curative department and reproductive department.
1.3 Justification:
Maternal death due to sepsis is increasing in Khartoum state (15.3% in
2015, 16% 2016 respectively) of causes of maternal mortality in Khartoum state
(5). However, unfortunately sepsis is increasing every year. There was a previous
study conducted in Khartoum state in 2010 done by researcher in Khartoum
MOH, about assessing infection control in labor rooms, it explore the gap of
infrastructure, availability of equipment and supplies, also to assess the
knowledge of care providers. The result found poor knowledge and practice
regarding infection control and lack of equipment and supplies, also the degree
of contamination about 72% from deferent sites in labor room (table, instrument,
scissor, baby bed and scale, umbilical tie and suction machine) (6) .

1.4 Objectives:
1.4.1 General Objective:
To evaluate the effectiveness of performance for nurses midwives
regarding Infection control in labor Rooms in governmental hospitals at
Khartoum state 2015-2018.
1.4.2 Specific Objectives
1. To assess the basic general knowledge and practice of nurses midwives
regarding Infection control precaution in labor Rooms.
2. To develop and implement training program regarding the infection
control precaution.
3. To evaluate the effectiveness of the training programs for nurses midwives
performance regarding infection control precautions.

1.5. Research Questions
1. What i s t h e e x i s t i n g of knowledge and practice of nurses midwives
regarding Infection control precaution in labor Rooms?
2. Is there any improvement of nurses midwife level of knowledge and practice
regarding infection control precaution after application the program?
1.6. Hypotheses
H0: There will be no significant difference between the pre and post
intervention of nurses midwives performance regarding infection control
precautions.
HA: Training p r o g r a m will improve the performance of nurses' midwife after
intervention regarding infection control precautions.

Literature review
Introduction
Infection control is the discipline concerned with preventing nosocomial
or healthcare-associated infection. As such, it is a practical issue rather than an
academic one. Infection control is an essential part of health care. Both Infection
control and hospital epidemiology are akin to public health practice, within the
confines of a particular health-care delivery system rather than directed at society
as a whole.
In 1985 due to emergence of HIV/AIDS, guidelines for protecting
healthcare workers from becoming infected with HIV and other blood borne
infections were quickly developed and became known as Universal Precautions
(UP) (7).
At nearly the same time that UP were being introduced, a new system of
precaution for health worker and patients was proposed as an alternative to the
diagnosis-driven UP. This approach, called Body Substance Isolation (BSI),
emphasized the need of protecting patients and health personnel from all moist
and potentially infected body substances secretions and excretions, and not just
blood. BSI was based primarily on the use of gloves. Personnel were instructed
to put on clean gloves just before touching mucous membranes or intact skin, and
before anticipated contact with moist body fluids (e.g., blood, semen, vaginal
secretions, wound drainage, sputum, saliva, amniotic fluid, etc.) (8).
A survey was conducted in the largest hospital in Albania to estimate the
prevalence and risk factors for nosocomial infections (NIs). A one-day prevalence
survey was carried out between October and November 2003 in medical, surgical
and intensive care wards. Centers for Disease Control and Prevention definitions
were used. Study variables included patient and hospital characteristics, surgical

procedures, invasive devices, antibiotic treatment, microbiological and
radiological examinations, infection signs and symptoms. Risk factors were
determined using logistic regression. In all, 185 NIs were found in 163 of 968
enrolled patients. Urinary tract infections (33.0%), surgical site infections
(24.3%), pneumonia (13.0%) and venous infections (9.2%) were the most
frequent NIs. The prevalence of NIs was higher in intensive care units (31.6%)
than in surgical (22.0%) and medical wards (10.3%). Overall, 132 NIs (71.4%)
were confirmed by microbiological examination; the single most frequently
isolated micro-organism was Staphylococcus aurous (18.2%). the prevalence of
NIs was higher than in western European countries and in some developing
countries (9).
There are universal precautions and guidelines regarding infection control
globally and at national and state level follow the same guideline of the Centers
for Disease Control and Prevention which started firs with: Hospital surveillance,
that born in the 60s as a result of the Staphylococcus aurous pandemic of the
1950s.
Training courses were first offered in the 70s by CDC . The Hospital
Infections Program (which is now the Division of Healthcare Quality and
Promotion) is a federal advisory committee created to provide guidance to the
CDC This is the re-vamped NNIS system and the first comparison raters were
published in 2007.
2.2 Hand washing:
For generations, hand washing with soap and water has been considered a
measure of personal hygiene (10). The concept of cleaning hands with an
antiseptic agent probably emerged in the early 19th century. As early as 1822, a
French pharmacist demonstrated that solutions containing chlorides of lime or
soda could eradicate the foul odors associated with human corpses and that such

solutions could be used as disinfectants and antiseptics.(11). It is well-documented
that the most important measure for preventing the spread of pathogens is
effective hand washing (12).
During the 19th century, women in childbirth were dying at alarming rates
in Europe and the United States. Up to 25% of women who delivered their babies
in hospitals died from childbed fever (puerperal later found to be caused by
Streptococcus pyogenes bacteria. In the late 1840's, Dr. Ignaz Semmelweis was
an assistant in the maternity wards of a Vienna hospital. There he observed that
the mortality rate in a delivery room staffed by medical students was up to three
times higher than in a second delivery room staffed by midwives. In fact, women
were terrified of the room staffed by the medical students. Semmelweis observed
that the students were coming straight from their lessons in the autopsy room to
the delivery room. He postulated that the students might be carrying the infection
from their dissections to birthing mothers. He ordered doctors and medical
students to wash their hands with a chlorinated solution before examining women
in labor. The mortality rate in his maternity wards eventually dropped to less than
one percent.
As early as 1843, Dr. Oliver Wendell Holmes advocated hand-washing to
prevent childbed fever. Holmes was horrified by the prevalence in American
hospitals of the fever, which he believed to be an infectious disease passed to
pregnant women by the hands of doctors (13).
In 1961, the U.S. Public Health Service produced a training film that
demonstrated hand washing techniques recommended for use by health-care
workers (HCWs) (14).
In 1975 and 1985, formal written guidelines on hand washing practices in
hospitals were published by CDC (15).

Not only can frequent hand washing reduce the spread of infection from
the hands of health workers, but from everyone else’s as well! For example, it is
estimated that persuading people, especially young children, to wash their hands
with soap and clean water after going to the toilet, handling or changing a dirty
baby, or doing other tasks that potentially contaminate hands (cleaning
vegetables, fresh meat or fish) can reduce diarrheal diseases by 45%, saving the
lives of a million children a year (16).
Moreover, in a large study, the US military found that when troops washed
their hands five or more times daily, sniffles, coughs and common “colds” fell by
43%.
Hand hygiene significantly reduces the number of disease-causing
Microorganisms on hands and arms and can minimize cross-contamination (e.g.,
from health worker to patient). The indications for hand hygiene are well known,
but guidelines for best practices continue to evolve .The purpose of hand washing
is to mechanically remove soil and debris from the skin and reduce the number
of transient microorganisms. Hand washing with plain soap and clean water is as
effective as washing with antimicrobial soaps (17) in addition, plain soap causes
much less skin irritation (18).
Hand washing should be done before:
Examining (direct contact with) a patient.
Putting on sterile or high-level disinfected surgical gloves prior to an
operation, or examination gloves for routine procedures such as a pelvic
examination.
Eating food.

Hand washing should be done after:
Arrive from home (before start working)
Any situation in which hands may become contaminated, such as:
Handling soiled instruments and other items;
Touching mucous membranes, blood or other body fluids (secretions or
excretions);
Having prolonged and intense contact with a patient; and
Removing gloves.
Using bath room.
Hand hygiene can be accomplished by:
Routine hand washing (with or without antiseptic agent).
Surgical hand scrub using (water and soap) or a waterless,( alcohol-based
antiseptic agent).
The purpose and way to do each differs slightly, for the steps of each type
(see annex 3)
2.3 PERSONAL PROTECTIVE EQUIPMENT:
Protective barriers, now commonly referred to as personal protective
Equipment (PPE), have been used for many years to protect patients from
Microorganisms present on staff working in the healthcare setting. Today, the
most common occupational risk faced by healthcare personnel is contact with
blood and body fluids during routine patient care. This exposure to pathogens
increases their risk for serious infection and possible death. HWs in some
occupational settings, such as surgery and labor rooms, have a higher risk of
exposure to these pathogens than in all other departments combined (19).

Personal protective equipment includes:
2.3.1 GLOVES:
Hand hygiene, coupled with the use of protective gloves, is a key
component in minimizing the spread of disease and maintaining an infection-free
environment (16). In addition, understanding when sterile or high level disinfected
gloves are required and, equally important, when they are not, can reduce costs
while maintaining safety for both patients and staff.
Healthcare workers wore gloves for three reasons:
1. To reduce the risk of staff acquiring bacterial infections from patients.
2. To prevent staff from transmitting their skin flora to patients.
3. To reduce contamination of the hands of staff by microorganisms that can
be transmitted from one patient to another (cross-contamination).
Although the effectiveness of gloves has been proved in prevention
contamination of healthcare workers’ hands has been repeatedly confirmed.
Wearing gloves does not replace the need for hand washing. For example,
even the best quality latex surgical gloves may have small in apparent defects,
gloves may be torn during use and hands can become contaminated during
removal (20) .
When to Wear Gloves:
Depending on the situation, clean examination or utility Worn by all staff
when:
There is reasonable chance of hand contact with blood or other body
fluids, mucous membranes or non intact skin.

Perform invasive medical procedures (e.g., inserting vascular devices such
as peripheral venous lines.
Handle contaminated waste items or touch contaminated surfaces.
A separate pair of gloves must be used for each patient to avoid cross
contamination (21). Wearing the same pair of gloves and washing gloved hands
between patients or between dirty to clean body site care is not a safe practice.
Doebbeling and colleagues (1988) recovered significant amounts of bacteria on
the hands of staff that were just washing their gloved hands, not changing gloves
between patients.
Types of Gloves:
There are three types of gloves used in healthcare facilities: surgical,
examination and utility or heavy-duty household gloves:
1. Surgical gloves: should be used when performing invasive medical or
surgical procedures.
2. Examination gloves: provide protection to healthcare workers when
performing many of their routine duties.
3. Utility or heavy-duty household gloves: should be worn for processing
instruments, equipment and other items; for handling and disposing of
contaminated waste; and when cleaning contaminated surfaces
2.3.2 Caps:
Caps are used to keep the hair and scalp covered so that flakes of skin and
hair are not shed into the wound during surgery. Caps should be large enough to
cover all hair.
While caps provide some protection to the patient, their primary purpose is
to protect the wearer from blood or body fluid splashes and sprays.
In many countries caps, masks, gowns and drapes are made of cloth or paper.

The most effective barriers, however, are made of treated fabrics or
synthetic materials that do not allow water or other liquids (blood or body fluids)
to penetrate them.
2.3.3 Surgical gowns:
Surgical gowns were first used to protect patients from microorganisms
present on the abdomen and arms of healthcare staff during surgery. They are
made of fluid-resistant materials play a role in keeping blood and other fluids,
such as amniotic fluid, off the skin of personnel, particularly in operating,
delivery and emergency rooms.
2.3.4 Aprons:
Aprons made of rubber or plastic provide a waterproof barrier along the
front of the health worker’s body. An apron should be worn when cleaning or
during a procedure in which blood or body fluid spills are anticipated (e.g.,
cesarean section or vaginal delivery).
2.3.5 Foot wear:
Footwear is worn to protect feet from injury by sharps or heavy items that
may accidentally fall on them. For this reason, sandals, “thongs” or shoes
made of soft materials (cloth) should not be worn. Rubber boots or leather
shoes provide more protection, but they must be kept clean and free of
contamination from blood or other body fluid spills
.2.3.6 Respirators are specialized types of masks, called particulate
respirators,
that are recommended for situations in which filtering inhaled air is deemed
important (e.g., for the care of a person with pulmonary tuberculosis). They

contain multiple layers of filter material and fit the face tightly. They are
considerably more difficult to breathe through and more expensive than surgical
masks.
2.3.7 Eyewear protects staff in the event of an accidental splash of blood or other
body fluid by covering the eyes. Eyewear includes clear plastic goggles, safety
glasses, face shields and visors. Prescription glasses or glasses with plain lenses
also are acceptable . Masks and eyewear or face shields should be worn when
performing any task where an accidental splash into the face is likely (e.g.,
performing cesarean
section or vaginal delivery or when cleaning instruments). If face shields are not
available, goggles or glasses and a mask can be used together
2.4 Decontamination:
Decontamination is the first step in processing soiled surgical instruments,
Surgical gloves decontaminate and other items. It is important, before usage to
immerse the items in 0.5% chlorine solution cleaning for 10 minutes. It’s a highly
effective infection prevention measures that can minimize the risk of transmission
of these viruses to healthcare workers, especially cleaning and housekeeping
staff, when they handle soiled medical instruments, surgical gloves or other items.
These measures are also important steps in breaking the infection transmission
cycle for patients.
WHO (1989) recommends 0.5% chlorine solution for decontaminating
instruments and surfaces before cleaning because potable (clean) tap water often
is not available for making the solution. In addition, because of the potentially
high load of microorganisms and/or other organic material (blood or other body
fluids) on soiled items, using a 0.5% solution for decontamination provides a
wider margin of safety (22).

For HLD, a 0.1% chlorine solution can be prepared provided boiled and
filtered (if necessary) water is used for dilution, and the items have been
thoroughly cleaned and rinsed.
More than 20 years ago it was shown that decontamination markedly
reduces the level of microbial contamination of surgical instruments. For
example, in the study by (24), 75% of previously soiled instruments had fewer than
10 microorganisms and 98% had fewer than 100 after being decontaminated prior
to cleaning. Because of these findings.
2. 5 Surgical Antisepsis:
Antiseptic or antimicrobial agents are chemicals that applied to the skin or
other living tissue to inhibit or kill microorganisms (both transient and resident)
thereby reducing the total bacterial count. Examples include alcohols (ethyl and
isopropyl), dilute iodine solutions, iodophors, chlorhexidine and triclosan.
Antisepsis:
Process of reducing the number of microorganisms on skin, body tissue or
mucus membrane by applying an antimicrobial (antiseptic) agent.
USE OF ANTISEPTICS:
Hand Hygiene:
Antimicrobial soaps or detergents are no more effective than plain soap and clean
water in reducing the risk of infection when used for routine hand washing,
provided the water quality is satisfactory (23).
Skin Preparation Prior to Surgical Procedures
Although skin cannot be sterilized, applying an antiseptic solution minimizes the
number of microorganisms around the surgical wound that may contaminate and
cause infection.
2.6 Sterilization:

Sterilization destroys all microorganisms, including bacterial endospores.
Sterilization is a process, not a single event. All components must be carried out
correctly for sterilization to occur. It should be used for instruments, surgical
gloves and other items that come in direct contact with the blood stream or
normally sterile tissues (23). It can be achieved b High-pressure steam (autoclave),
Dry heat (oven),
Chemical storylands (glutaraldehydes or formaldehyde solutions).To be
effective, sterilization requires time, contact, temperature and, with steam
sterilization, high pressure. The effectiveness of any method of sterilization is
also dependent upon four other factors:
The type of microorganism present. Some microorganisms are very
difficult to kill. Others die easily.
The number of microorganisms present. It is much easier to kill one
organism than many.
The amount and type of organic material that protects the microorganisms.
Blood or tissue remaining on poorly cleaned instruments acts as a shield to
microorganisms during the sterilization process.
The number of cracks and crevices on an instrument that might harbor
microorganisms. Microorganisms collect in, and are protected by,
scratches, cracks and crevices such as the serrated jaws of tissue forceps.
Standard of sterilization steps see (annex).
2.7 PROCESSING LINEN:

Linens:
Cloth items used in healthcare facilities by housekeeping staff (bedding
and towels), cleaning staff (cleaning cloths, gowns and caps) and surgical
personnel (caps, masks, scrub suits, surgical gowns, drapes and wrappers) as well
as by staff on specialty units such as ICUs and other units performing invasive
medical procedures (e.g., anesthesiology, radiology or cardiology).
Housekeeping and laundry personnel should wear gloves and other personal
protective equipment as indicated when collecting, handling, transporting,
sorting and washing soiled linen.
When collecting and transporting soiled linen, handle it as little as possible
and with minimum contact to avoid accidental injury and spreading of
microorganisms.
Consider all cloth items (e.g., surgical drapes, gowns, wrappers) used
during a procedure as infectious. Even if there is no visible contamination,
the item must be laundered.
Carry soiled linen in covered containers or plastic bags to prevent spills
and splashes, and confine the soiled linen to designated areas (interim
storage area) until transported to the laundry.
Carefully sort all linen in the laundry area before washing. Do not presort
or wash linen at the point.
2.8 High-level disinfection:

High-level disinfection can be achieved by boiling in water, steaming
(moist heat) or soaking instruments in chemical disinfectants. Although
sterilization is the safest and most effective method for the final
Processing of instruments, often sterilization equipment is either not
available or not suitable. In these cases, HLD is the only acceptable alternative.
The HLD process destroys all microorganisms (including vegetative bacteria,
tuberculosis, yeasts and viruses) except some bacterial endospores (24).
2.9 INSTRUCTIONS FOR CERVICAL OR VAGINAL PREPARATION:
For cervical and vaginal antisepsis, prior to inserting a uterine elevator for
minilaparotomy or doing an endometrial biopsy, select an aqueous (waterbased)
antiseptic such as an iodophor (povidone-iodine) or 2–4%
chlorhexidine gluconate (e.g., Hibiclens or Savlon if properly prepared).
Do not use alcohols or alcohol-containing preparations, such as Dettol.
Alcohols burn, and they also dry and irritate mucous membranes that in
turn promote the growth of microorganisms. In addition, hexachlorophene
(pHisoHex®) is neurotoxin and should not be used on mucous membranes, such
as the vaginal mucosa, because it is readily absorbed (25).
2.10 Skin Preparation for Injections:
According to WHO and its Safe Injection Global Network (SIGN),
“Swabbing of clean skin—with an antiseptic solution—prior to giving an
injection is unnecessary,” because in controlled trials no infections werenoted. In
addition, a review of microbiologic studies did not suggest that wiping the skin
with an antiseptic before giving an intradermal, subcutaneous or intramuscular
injection reduced the risk of infection (26).

If the injection site is visibly soiled, wash the site with soap and water and dry
with a clean towel, and then give the injection.
2.11 WASTE MANAGEMENT:
Wastes from hospitals and healthcare facilities may be contaminated (potentially
infectious) or non-contaminated, it divided in to:
Non-contaminated waste includes paper, trash, boxes, bottles, plastic containers
and food. They can be disposed of by the usual methods or sent to the local
landfill or dumpsite (27) .
Contaminated wastes include blood, pus, urine, stool and other body fluids
(human tissue, blood or blood soaked sponges, gauze or cotton)
Soiled medical devices or items that can inflict injury (e.g., used needles and
scalpel blades) .
Cytotoxic waste (e.g., drugs typically used in cancer chemotherapy).
The purpose of waste management is to:
protect people who handle waste items from accidental injury
prevent the spread of infection to healthcare workers who handle the waste,
prevent the spread of infection to the local community.
safely dispose of hazardous materials (toxic chemicals and radioactive
compounds.
Steps of waste manage: Solid contaminated waste (e.g., surgical specimens, used
dressings and other items contaminated with blood and organic materials) may
carry microorganisms or during transport.
STEP 1: Wear heavy-duty or utility gloves when handling and transporting solid
wastes.
STEP 2: Dispose of solid wastes by placing them in a plastic or galvanized metal
container with a tight-fitting cover.

STEP 3: Collect the waste containers on a regular basis and transport the burnable
ones to the incinerator or area for burning.
Note: If incineration is not available or waste is non burnable, bury it.
STEP 4: Remove utility gloves (wash daily or when visibly soiled and dry).
STEP 5: Wash and dry hands or use an antiseptic hand rub as described above.
2.12. Health Education Program
Health Education Program The society's most valuable resource is its people. By
protecting and promoting the health of its members, the best interferes of society
will be severed. The higher level of health in a population, the most likely that
individuals will be productive. They can share in the responsibilities of family
life and they can contribute to the wellbeing of the communities in which they
live and the country as a whole (28).
2.13. Health Education is an Integral Part of Nursing Roles
Nursing is a dynamic, therapeutic and educative process in meeting the health
needs of society. Today education about preventive health practices and health
promotion is considered an essential component of comprehensive health care.
Education is also a mean of improving health status of the public, (29).
2.4. Definition of Health Education Program
Health education is the process of providing learning experiences for the purpose
of influencing knowledge, practices and attitudes relating to health. It is the part
of health care which concerned with promoting healthy behavior, (30).
In other words, health education is the instruction that addresses physical, mental,
emotional and social dimensions of health; develop health knowledge, attitudes,
and skills; and is tailored to each age level, (31).Health educational program is a
planned sequential program of experience that is designed to motivate people to

maintain and improve their health, prevent disease, and reduce health related risk,
(31).
2.15. Aim and Subjects of Health Education Program
The aim of health education program is to make people will be able to achieve
health by their own action and efforts and available resources and develop the
sense of responsibility for their better health as individuals, members of families
and communities and governments in additionally to the objectives of health
education. They are informing into action, (30.31).
2.6. Principles of Health Education
To conduct health education, certain principles should be followed. They include
motivating the learners a motivation is the key to learning it people benefit from
an action, they will be encouraged to repeat that action. Fear is not a motivator, it
destroys the learning process. Other principles are: helping learners to define and
specify learning needs; encouraging the learning process by using all senses;
assessing and alleviating learning difficulties among precipitants; conforming to
beliefs and culture; creating a willing atmosphere for learning; reinforcing
progress; having fun; giving feedback; presenting the message clearly;
encouraging the active participation of the learners; avoiding prejudice and bias;
deciding on priorities; objectives and actions, (30,31.32). Suggested three factors
work in client health education. The first one is health education includes a variety
of strategies, such as lecturing, modeling, or providing printed material, and all
methods are effective in increasing skills or knowledge level. The second one is
the individualization and the adult learner. Individual characteristics such as age,
social status, cultural issues, and educational level influence teaching
effectiveness and long-term health behavior so it should be put into consideration
when health teaching and the educational program should be individualized to
meet learner's needs. In addition, the health educator needs to assess the learner's

learning-teaching style and other and ragogy principles. (33) Finally, the third
factor is the support system. The presence of a peer group can enhance learning
by providing encouragement to the learners. Additionally, teaching a supportive
family instead of just one family member is more effective in achieving learning
objectives and modifying behavior. For groups of community learners, special
efforts to include culturally appropriate information and the use of culturally
sensitive materials may enhance participation and learning. (33)
2.17. Barrier Facing Health Education of People
Receiving information does not, in and of itself, guarantee that learning will
occur. Several barriers can impede the learning of healthy behavior. They stop
against achieving health education and changing of people's behavior into a
healthy one. Some of these barrier are general such as illiteracy, faulty values and
beliefs, place and environment, the influential practice of key persons, such as
older family member, friends. In addition to absence or inadequacy of
infrastructure such as water and sanitary conditions, other factors are considered.
They are related to the human behavior as feelings ideas, knowledge, experiences,
values and traditions, cultures and attitudes. Besides absence or inadequacy or
resources such as low budget, health team, time certain skills health service and
educational skills, equipment and supplies, (32, 34). On the other hand, other specific
factors are identifies to learning, teaching process. In nursing situation, learning
barriers can be classified as either internal (psychological and physiological) or
external (environmental and socio-cultural). The psychological barriers are
anxiety, fear, anger, depression, and inability to comprehended, compared to the
physiological barriers. They are pain, fatigue, sensory deprivation and oxygen
deprivation. Whereas, the environment are interruption lack of privacy and
multiple stimuli compared to socio-cultural barriers, they are language, value
system, and educational background,(29).Large-print materials, good lightening

and appropriate are a few easy modifications than can reduce or eliminate barriers
to learning, (33, 34).
2.18. Steps of Health Educational Program
Health educational program is a systematic ongoing process involving four
sequential but interrelated steps which begins with identifying learners' learning
needs, planning of the program (formulating objectives, selecting and organizing
learning, learning materials, selecting teaching stratifies and evaluation policies),
planning budget and finance, designing systems of reports and records,
implementing the program, and evaluating the program. The development of
content, strategies for teaching and evaluation of the effectiveness of health
educational program should be carried out in a systematic manner to achieve the
most effective results. This systematic method is the teaching learning process.
The teaching learning process parallels the nursing process. The nurse will use
both the nursing process and the teaching learning process to intervene for
community health process. Both usually should start with needs assessment, (33).
2.9. Learning Needs Assessment
To create a health education program for a community, both the needs of
the community and the learning needs of individual participants should be
assessed. The assessment of the community is based on epidemiologic and
demographic data, observations of health care personnel in the community and
survey and conversations with community members. A real need is one that is
based on a deficiency that actually exists, an educational need is one that can be
met by a learning experience; a real educational need indicates that specific skills,
knowledge, and attitudes are required to assist the client in attaining a more
desirable condition; a felt need is recognized as important by the leaner,(33).

Assessment of the leaner is essential to planning the educational program.
It is also helps facilitate the learner's acceptance and use of the information being
offered. Within the community, the leaner may be individual, family or group.
Initial assessment of the leaner is often referred to assessment of the learner's
readiness to learn. In the group of learners, background, skills, abilities, and
motivation are different for each group's members. In addition several factors
need to be considered during assessment including: learning style, potential
learning needs, ability to learn, client strengths, previous experience and
knowledge base (29.33).
2.20. Planning the Education Program
The next step in developing educational program is the planning which
based on certain facts. Budget is the major factor put into consideration when
planning health educational program, and the cost of resources whether human or
UN human. Also planning includes determining the time needed to complete the
program whether period of sessions and their starting time. In addition to
determining the actions phases, the responsible persons whether administrative
or health educators, the setting and learning environment with certain
characteristics and the construction of health education lessons plan, (32, 33, and 34).
In the initial phase of planning teaching. Learning process in the assessment of
needs which is followed by creating the behavior goals and objectives which
reflect changes in the learner's behavior that are observable and measurable.
When the behavioral objectives are properly written, they will be useful tools in
evaluating educational outcomes. Behavioral objectives are classified into here
domains of learning cognitive, psychomotor and affective. All three domains of
learning are usually necessary to incorporate a new health behavior into the
learner's life. Behavior objectives lead to development of the content of the
program. The health educator needs to be selective in planning content so the

most important concepts and knowledge about how to use the information are
included in outset of the educational program. Additional content is included if
time or money permit. The nurse also needs to plan using educational materials
the suitable methods of teaching and audiovisual maternal and degree of patient
participation in his learning in addition to evaluation of leaner progress and
teaching performance. (29, 33)
2.11. Implementing the Program
Implementation phase is put the program into action. During this phase the
learning teaching process is going on the health educator uses suitable teaching
technique that is congruent with size, composition, and learning abilities of the
group. Consideration of cultural differences, barriers of learning and community
values are necessary. (29, 33)
2.22. Evaluation of the Program
Evaluation of health education program has two focuses. The first one is
geared toward the learning teaching process itself. This evaluation has two types
of evaluation: outcome evaluation and evaluation of teacher performance.
Assessment of leaner outcomes has traditional been based on achievement of the
behavioral objectives. Evaluation includes determining if the teaching session
was successful and if the client learned the intended information. This can be
observed, there is a change in client behavior related to learning activities. The
evaluation of teaching performance is geared toward assess the effectiveness of
the teaching activities and decided which modifications, if any, are necessary.
Several activities can evaluate teaching effectiveness, including feedback from
leaner and colleagues and self-evaluation, (29, 33). On the other hand, the second
focus of health education program is geared toward evaluate all steps of
developing the health educational program, such as the educational objectives,
the resources, budget and cost, the time, the evaluation tools for learning teaching

process, the learning environment, evaluation of performance of all responsible
persons. Satisfaction questionnaires, chart audits and budget reviews are
examples of evaluation tools of educational and program, (29.33).
2.23. Educational Tools
Facilitating the learning process and retraining the learned information and
stills are related to the educational materials. Educational materials may be
classified into printing and non-printing materials. The printing group includes
photographs, sketches, booklet, and pamphlets, whereas the non-printing
materials include the audiovisual materials such as overhead transparencies,
slides, motion pictures, videotapes. Each group has its specific different purpose
and selection, (33).
Previous studies:
First study carried on Nigeria regarding infection control measures. The
objective of this study was to investigate the nature and pattern of existing
policies and practices relating to infection control in maternity care centers
in Edo state, South-South Nigeria, cross sectional study in public and
private health care facilities were done. Which resulted in of the 63 health
facilities, 68% (43) reported that they had infection control procedures in
place, while only 25% (16) reported that they documented these as manuals
or charts? Only 13% (8) of facilities had infection control committees; 11%
(7) routinely carried out audits of maternal deaths, while 33% (21) reported
that they had an ongoing programmed for staff training on infection
control. A high proportion of the health facilities reported that staffs
routinely wash their hands before and after sterile procedures, but only half
of the facilities were observed to have 24-h running water and only two-
thirds had soap and antiseptic solutions in delivery and operating theatre
areas. Although more than 90% (57) of the health facilities reported that

they use sterile gloves routinely, unused sterile gloves were found in only
60% (38) of these facilities, and recycled gloves in 11.1% (7).(35)
The second study done In Ghana, infection has been identified as a major
cause of birth‐related mortality. Results of a 2‐month observation of
infection control practices among Ghanaian obstetric nurses and midwives
indicated that most personnel did not practice basic rules of asepsis.
Problems included frequent breaks in technique, inadequate sterilization
and disinfection, and repeated exposure to large amounts of blood and
vaginal secretions. Supplies were limited and, even when available, not
always used appropriately. The situation in developing countries is
different from that in the United States. Therefore, an observational needs
assessment is essential to plan relevant and practical measures for change

Third study regarding infection control measures carried on South West
Ethiopia. Objective: To assess knowledge, attitude and practice towards
infection control measures among Mizan-Aman general hospital. Methods:
The study was conducted at Mizan-Aman General Hospital, Southwest
Ethiopia. Cross-sectional study design was employed, Results: Out of 135
respondents, 57(42.2%) of HCWs think that they apply standard precaution
always. About two-third (65.6%) of them had ever participated in training
program. All of the respondents know that dirty needle and sharp materials
could transmit disease causing agents. More than three fourth (76.3%) of
health care workers think that they were at risk of acquiring HIV in their
work place. Among HCWs 59(43.7%) of them disposed sharp materials in
open pails, 91(67.4%) in sharp and liquid proof container without
removing syringe. Ninety five (70.4%) HCWs know that gloves and gowns
were required for any contact with patients. Among respondent 63(46.8%)

of HCWs practice standard blood and body fluid precautions always. One
hundred three (76.5%) of HCWs wear gloves last time while they took
blood sample. Ninety two (68.7%) of HCWs wash their hands before
examining the patients and 84(62.5%) of HCWs recap needle immediately
after using them (37).
Fourth study regarding infection control measures carried on in Zimbabwe
This study investigated the availability of infection control policies and/or
guidelines while observing midwives' practices in relation to peripartum
sepsis prevention. A participant observational study collected data, in May
and June 2014, by means of checklists from 37 purposively selected
midwives working in the labour and postnatal wards of two central
hospitals in Zimbabwe. Descriptive statistics and SPSS version 20 were
used during data analysis. The results indicate lack of resources necessary
for good midwifery practice and poor standards of infection control among
midwives. Strengthening infection control policies and practices, ensuring
resource equitable distribution and training of midwives in per partum
sepsis prevention are essential interventions (38).
Fifth study in Iran about Infection Control Practices and Program
Management in Labor and Delivery, Objectives: This study aims to
investigate infection control program management and midwives’
practices in labor and delivery units. Objectives: This study aims to
investigate infection control program management and midwives’
practices in labor and delivery units. Results: The evaluation scores for all
aspects of infection control were suboptimal; infection control program
management 38.1% to 71.4%, healthcare workers post-exposure measures
58.5% to 92.7%, medical waste management 73.9% to 87.0% and infection
control related standard infrastructures and equipments 55.9% to 82.8%.

Chapter Three
Methodology
3.1. Research design:
It is a quasi-experimental research design was used in the current study (pre-
test/post-test design).
3.2.Study area:The study was conducted in labor rooms - govarmental Hospitals -
Khartoum State M2015 – 2018.
Labor rooms is the place where pregnant woman stay from the beginning of
labor till two-six hours after delivery of the fetus and placenta. Throughout this
time the delivery of the fetus and placenta may need some minor surgical
procedure. In this regard, infection control refers to policies and procedures used

Back ground: Khartoum is the capital, located in approximately the center of
Sudan and shares borders with north kordofan, AlGazeera, gadarif and the river
Nile state. Covers a total area of about 28000 square Km. the total population
was estimated 5968069(according to population census 2010).. Khartoum state
composed of seven localities: Ombadah, Karary, Omdurman, Bahrey, Sharg
Elneel, Gabal Awlia and Khartoum Locality There are 157 governmental and
247nongovermental health centers, all of them provide antenatal care services.
3.3.Study population:
Total coverage of nurses' midwife (n: 135) working in Governmental
hospitals labor rooms (n: 15).
Back ground about nurse midwife:
At 1933 the nurse midwives program was established at Omdurman
midwifery school, the first batch included 12 certified nurses graduated from
Omdurman hospital nursing school. The course duration was one year, the ones
who met the requirement of the course were awarded the midwifery certificate

and they were working at the hospitals obstetrics and gynecological wards giving
nursing care and conducting normal deliveries. The program continued to train
certified nurses who were sent from the provinces hospitals and after they were
(40).awarded the nurse midwife certificate go back to their hospitals

-the skill of staff nurses midwife for performing technique of infection cotrol for
each participant pre and post intervention.
3.7.1.Close ended questionnaire: To assess the knowledge of the nursesmidwife
about infection control before and after the educatin program. It is filled by each
nursemidwife enroled in this study. The same questionnaire is filled before and
after the structured traing program. This questionnaire items included the
nursesmidwife study and control group demographic data regarding age,
educational level, duration of experience,perment job, Immunization against
hepatitis B and the source of knowledge about the infection control. A set of
questions assessing the knowledge about the infection control asking the study
candidates about the ways of infection transmission; the types of Hand washing;
timing of wash your hands; type of gloves, personal protective equipment ,
Infection control processes and Ways of sterilization.
It composed of 13 questions each with division from 1-9 answers using ascoring
system of the phrases (correct=1, incorrect=0)
Theinterval scale was was used for evaluationof the scores for the level of
knowledge items which was evalusted by : poor (0-49), acceptable (50-59), fair
(60-69), good (70-79), very good (80-89) and excellent (90-100).
3.7.2. The observation check list:

(filled by the researcher) is inteded to assess skill of staff nurses midwife for
performing technique of infection cotrol. The items of this checklist included the
pre- procedure.Number of questions are 6An observation check list (apendix 4)
also by the reasrcher . it was composed of main 6 observed items each item ranged
from 1-10 parts.
The scale for check list was scored into (done =1, Not done = 0).
3.8.Pilot study
This phase was ended by conduction of the pilot study. This was done to test
clarity, applicability, feasibility &relevance of the tools used, to estimate the
length of the required time for data collection.A modification on tools was made
based on the results of the pilot study. Hence, pilot study sample was
excluded from the final sample.
The pretest was obtained by evaluoting ( 20) nursesmidwivs working in
maternaty hospital and ibrahim malik hospital . Test and retest was repeated after
two week . According to the "knowledge and skills test tool" reliability of the
instrument determined through the use pearson correlation coefficient reliability
was (r = 0.071) for nurses midwives level of knowledge and (r = 0.76) for nurses
midwives practices. The level of the P value was (p<= 0.03)which indicates
statistical acceptable of the format.
Both the questionnaire and the observation check list were carried out by the
reseacher.
3.9..Th Training program procedure:
The training programwas stated in atime table and was applied in three phases
(preparatory phase, implementation phase and evaluation phase) as follows:
3.9.1. The Preparatory phase:
orientation about the program was given first to give participans the full idea
about the phase of the applicationof the program in order to facilitate for their
contribution . pretest for assess knowldage for nurse's midwife carried out to the

intervention. Using the interveiw questionnaire format , time offered was half an
hour for each participant , which was consideredenough time for the participant
to answer the questions and was calculated as follows: (1\2×135=67.5) .
pretest for existing skills using the obseration check list using format , time
offered was( half an hour for each participant , this part this part is carried out
by the reseacher . which was calculated as follows: (1\2×135=67.5).
The educational program was developed by the researcher: detected needs,
requirements and deficiencies were translated to aims and objectives of the
program. Moreover, the researcher will prepare a place, materials, audiovisual
aids, and time table to implement the program. The in- serves training will be
used in implementing this program. together with handouts that covered
theoretical and practical information.
2. The Implementation & evaluation phase
The program consisted of two parts:
Orientation to the educational program , including : the lecture s time table ,and
the training materials which was calculated asfollows:(6hrs× 15 labor rooms= 90
hours).
1.The first part: Theoretical part:
Sessions time:
Ateaching sessions were conducted, each session takes around one hour and
half and there was about half hour time offered for discussion after each lecture
for fruther clarification about what is missingor not understand by the participants
.And the breaks times was excluded. Teaching methods were lectures
andTeaching aids were utilized, posters( guidelines), handouts, pen & paper test.
Lectures titles were related to:
Introduction about infection control program.
Definition of infection control

Purpose of infection control.
Ways of infection transmission.
The types of Hand washing (HW).
Timing of wash your hands.
Types of glove.
The personal protective equipment.
The infection control processes.
Ways of sterilization.
Total time of 6 hours to each group which calculated as follows: (15×6= 90 hours)
Each nursemidewife was supplemented with the knowledge booklet, ahead and
utilized each session to assure understanding and clear any misconception or
misunderstanding. The researcher continued to reinforce the gained information,
answer any raised questions and gave feedback. Communication
channel was kept open between the researcher and the study group subjects. Then,
immediately post, and after three months, knowledge tests were carried out.
- apottest was obtained from the participants at the end of the program (same
interveiw questionnaire and observation check list format were used . the time
offered for the participants to obtain the answers was (30min ×135 =67.5 hrs)
Total collection time for questionnaire (pre and postest) knoweldge and
Theoretical part(lectures) was calculated as 225 hrs .
2.The second part: The practical part
For practical contents, each nurse’s midwife performance as regards the pre
determined procedure was evaluated before provision of any information (pretest)
utilizing the formulated checklists (second tool) and (third tool).

Practical booklet was given by interveiwing each nursemidwife and the
immediate post practicetest was done. Then after three months tests were carried
out. Each skill was evaluated 3 times and the mean was calculated. The practical
part was achieved in the clinical area. Teaching methods were in-servese training
in the clinical area, for each hospital included nursemidwife , Befor start the
training the reseacher prepared the supplies and equement when used for training
form ministry of health , khartoum state (Reproductive health Department). For
example soaps used for hand washing procedure ,guaze ,gloves , clorex, plastic
kedeny dishes used for saluted clorex regard disinfection also prepared guideline
about who to wash hand and sterilization.The produre contented of:
Hand washing (the nursemidwife who to washed hand routinlyandsurgicaly)
Wearing the glove( suitable and correctly).
Stepes of Disinfected of instrument.
Stepes of sterilition of instrument.
manages the waste in appropriate way
Total collection time for observation check list for skills of nurse midwives (pre
and postest) was calculated as(2×135=270 hrs) and training program was
calculated as (1×15=30 hrs) .
3.The third part: Follow –up evaluation of state of knowledge and practice
Test the degree of maintenance of knowledge and practical skills gained from
our training intervention, a follow-up evaluation of knowledge and practice were
performed three months after the study.
7. Data analysis:
The plan of data analysis included both descriptive and inferential statistics.. The
collected data was organized, tabulated and analyzed based on the objectives of
the study by using descriptive Statistical tests , the paired sample "t" test, 95%
confidence interval (C .I.) and the Pearson correlation test were used. The data

was processed using SPSS version i.e. percentage, mean and standard. The 't' test
was used to find out the differences in the scores of knowledge and skills
between pre-test and post-test. Significance tests were performed when
necessary taking 0.05 as a “p value”. The findings of the study were presented
in the form of tables and figures.
8.Ethical Considerations
The written Permission for data collection and implementation of protocol in
govarmental hospitals was obtained from the heads of the hospital and from
nursemidwife were taken. The researcher emphasized that the participation is
voluntary. As well anonymity and confidentiality were assured through coding
the data. these data will use for the purpose of research only and if it will be
reused another agreement will be taken. The reactions of the administrative
personnel were very supportive for the program and they offered all available
facilities that might help in the success of the program.

Results
Demographic Data:
The study was conducted in fifteen government hospitals-labor rooms in
Khartoum state according the availability of nurse's midwives. The calculated
sample size was 135 nurses' midwives.

Discussion:
This study took place in Khartoum State during period between March2015 and
March 2018. It covered fifteen labor Rooms in government hospitals. A similar
study was conducted in three states of the USA. Other studies were conducted in
India in 2009 and in the Sudan 2010 to assess the knowledge and practices (6)
without intervention.
The total number of nurse midwives who participated was (n=135) .the
majority of participants were more than 45 years old and they had work
experience over 10 years', That category of the subjects were not university
graduates, but (63%) were Intermediate school leavers and the others had their
basic education. only (73%) obtained their information about infection control
form their daily contact with surrounding job Before attending the program. As
for immunization against HBV only (35%) were immunized. Although nurse
midwives are at risk of needle stick injuries not only once but for many time still
there no training for those participants available regarding post exposure
prophylaxis, a study was conducted in Sudan 2010 to assess the knowledge and
practices in labor rooms of government hospitals (6) ,the findings were all most
the same (the care providers more than 40 years, work experience above 10 years,
about (88%) didn't receive any training courses, regarding, (41%) were immunize
against HBV). Another study was done in the Sudan - Khartoum state in 2014 to
assess the knowledge and practices in government hospitals obstetric department
operating theater, the results were( care providers are above 40years old. more
than (68%) of them did not receive any refreshment or in-service training
regarding infection control .Only (37.8%) have been immunized against hepatitis
B virus infection HBV). (47) If we compare this to other study conducted in the
Caribbean area With respect to training, (64.4%) of the85 participants had

completed basic medical training within the last 10 years. Of those remaining, 13
had completed basic medical training within the last 11–15 years, 15 within the
last 16–20 years, and 19 respondents had completed basic medical training for at
least 20 years (48).
The present study showed Tables (4-10) that the Knowledge of the
participants regarding infection control before and after implementation of the
education program. It is clear from these tables that the correct answers of the
participants increased significantly from pre-test measurement to post-test
measurement. For example regarding the blood as way of infection transmission,
the correct answer was mentioned by 91(67.4%) of the participants at pre-test
measurement .It increased to 120(88.9%) at post measurement. Regarding the
mean value of the answers of participants on this mode of infection transmission,
the mean score of their answers about the item of blood as one of the ways of
infection transmission was 0.67±0.47 at pre-test measurement, and at post-test
was 0.89±0.32; T value was (-4.4).This indicates significant differences between
pre and posttest measurements regarding this item. Then, the overall correct
knowledge and very good knowledge grade were 91.9% at posttest measurement
compared to 60.7% at pre-test measurement shown in (Figures 4 and 5).
Moreover, similar study was done in Indonesian about awareness of Health Care
Workers Regarding Infection Control, after cross tabulation against last on job
tainting the results are X2 =90.000 df =1 P .value =0.000 which mean
significant association with 95.5%confidence interval (41).
The effect of intervention on nurse midwives` practice Figures (5 and 6)
shows that a clear improvement in the level of expertise in handling various
aspects in infection control after the training intervention which attained highly
significant difference. Results in Tables (11-16) indicate significant differences
in the level of practices of the studied population regarding infection control in
labor room after attendance of the educational program. For example, washing

hands after arrival from home was practiced by 87(64.4%) of the participants at
pre-test measurement and by 121(89.6%) at post-test measurement. Regarding
the mean value of the practices of hand washing after arrival from home, the mean
score of this item was 0.64±0.478 at pretest measurement, and at posttest was
0.90±0.31; T value was (-5.14) indicates significant differences between pre and
posttest measurements regarding this item of practice. The overall high practice
and very good practices grade were 93.3% at posttest measurement compared to
60.7% at pretest measurement (Figures 5 and 6).to compare to Different studies
conducted in different countries revealed that poor hand washing practices can be
improved by educational programs and training. A study conducted in three
states of the USA revealed overall lapse in infection control elements. The study
focused on hand wash and found that compliance increase. Also it could lead to
significant reduction in HCAI rates, after two intervention periods for four years
(37). Another study done in Iran the results were, the evaluation scores for all
aspects of infection control were suboptimal; infection control program
management 38.1% to 71.4%, healthcare workers post-exposure measures 58.5%
to 92.7%, medical waste management 73.9% to 87.0%. The midwives mean
scores for attending infection control training courses and hand hygiene were less
than 40% of the maximum score, but the mean scores for normal vaginal
deliveries scrub, and equipment and instruments hygiene practices were generally
above 70% (39).
A follow-up evaluation of knowledge and practice were performed three
months after the study , Test the degree of maintenance of knowledge and
practical skills gained from our training intervention the test was improvement in
the practical skills score, (p =0.01).
Conclusions

The study concluded that:
The present study was conducted with the aim of evaluating the performance
of nurse's midwives regarding infection control precaution in fifteen
governmental hospitals labor rooms at Khartoum state.
The study found that the majority of participants' age was above 45 years,
65% did not immunize against hepatitis B and most of them are Intermediate
school (63%).
At the pre- educational program stage the evaluation of overall knowledge
was fair, after applying a structured educational program the result was
improved significantly
Overall mean practice score in the pretest stage was 60.7% and 45.2%
respectively and improved in the posttest stage was 93.3% and 80%
respectively (P value 0.00).
The tests were repeated three month after completion of the training program
so as to evaluate the knowledge and practice gained from the program. We
found that the level of knowledge and practice was still significantly high.
Recommendations
From this study the researcher recommends the following:

Make sure that a pre-employment medical examination, investigation and
immunization for all nurse midwives against important infection especially
hepatitis B and C.
Upgrading and qualifying nurse midwives to improve their standard
knowledge and practical skills to be better in midwifery nursing.
Training programs based on the infection control precaution to improve the
performance of nurse's midwives in the labor rooms.
Sufficient medical supplies and equipment of infection control precautions
should be provided for labor rooms to ensure better nurses midwives practice.
Adequate number of nurse midwives for labor rooms should be employed.
The hospital administrators should establish continuing training programs for
nurse's midwives practice regarding infection control precaution.
Guideline and structures should be written e:g posters in labor rooms.
Health education program for mothers in the labor rooms.

عدم ثني الإبرة أو كسرها أو فصلها عن مع مراعاة ولذا يتعين التخل الفوري من الإبر دون تغطيتها، خطيرة،تلك عملية
السرنجة )المحقنة( باستخدام اليد.
إلى إعادة تغطية الإبرة يمكن القيام بذلك بطريقة آمنة باستخدام طريقة اليد الواحدة. رارالاضطعند
الطريقة:وفيما يلي الخطوات المتبعة في هذه

الإبرة داخل الغطاء تماماً اسااااااتخدم اليد الأخرى لضاااااامان إحكام الغطاء فوق تدخلعندما
إذ يجب إمساك الغطاء من أسفل فقط )قرب نهاية الإبرة(. الحذر،الإبرة مع توخي
الأدوات الحادة مناولةعند عن طريق الخطأ وذلك قد يصااااايب العاملون في الرعاية الصاااااحية بعضاااااهم البعض
مثل هذه الأدوات بطريقة لا مناولةمثلاً، لذا يتعين بإجراء جراحيالأدوات الحادة عند القيام منااولة

ويجب أن تكون الأوعية ذات ساعة كافية ويسهل فتحها منها،جوانبها وأسافلها ومضاادة للثقب حتى يتم التخل النهائي
الاستخدام.آمنة أثناء بطريقة
لاستخدامها في التخل من الأدوات لحاوياتالوصول ل: ويقصد بذلك قدرة جميع العاملين على سدهولة الوصول إليها -
