OverviewCommunity Solutions is opening up limited new spots in our Built for Zero Collaborative for 2018.

In 2017, more than 70 communities (including eight Canadian communities) participated in this proven model, which we have adapted from the healthcare sector to help your local team drive down chronic homelessness.

Your community will achieve breakthrough results through a mix of in-person peer learning events, virtual coaching and exclusive digital content.

Over the course of 12 months, your community will set and reach sustainable stretch goals around ending chronic homelessness. Our trained coaching team will help you move as far along this continuum as possible.

The Collaborative is a proven peer learning model that revolves around two mutually reinforcing components:

● Learning Sessions: ○ Gather with communities from across the US and Canada to learn the science of rapid

cycle testing○ Swap ideas, set clear goals and build an Action Cycle work plan under the guidance

of expert coaches

What’s Included: Resources + Support

In-Person Learning SessionsJoin communities every four months for an in-person peer learning session. Each consecutive in-person session will build on the content and learning from previous sessions. Your community will:

● Share a summary of your work with other participants● Train on Quality Improvement and rapid cycle testing strategies● Consult experts from federal and provincial agencies and partner

organizations to bring home expert guidance● Build an action plan focused on testing specific change ideas

throughout the following Action Cycle

Action CyclesYour team will focus on implementation and accelerating toward your goals between each Learning Session. You’ll leverage a curated menu of virtual opportunities to support your work:

● Virtual, cohort-based coaching with peer communities and a dedicated Improvement Advisor

● Group innovation calls spotlighting key bright spots from participating communities— maybe even yours!

● Virtual training opportunities to learn advanced data and quality improvement strategies

● Support sessions with expert brain trust to bust myths, explain government rules and clear away obstacles

Data + Performance Management Infrastructure

Your community will receive and learn to use a custom data and performance management infrastructure to design and articulate your strategy, run tests of change and drive measurable progress to zero using your by-name list.

Our data team will work with your local data leads to make the best and easiest use of this infrastructure, which is designed for learning and improvement, not judgment!

Designing Your Collaborative Team

Who Do You Need on Your Local Team?Your team should include key people in your community who are already working together on ending chronic and veteran homelessness:● No limit to the number of people that can participate● Every team needs a leader and a commitment to set clear,

measurable goals● Four to eight key leaders will travel to the three Learning Sessions to:

○ Participate in breakthrough training and expert advisory sessions○ Set goals and develop high level community action plans○ Prepare to translate key learning and decisions to the rest of your

team

Community Representative / Key Leader - Person overseeing ending chronic homelessness for your community.

List manager - This is often the community data or information systems lead, but may be anyone who will own primary responsibility for managing your community’s by-name list.

Coordinated Systems Lead- Person responsible for leading the implementation and management of the coordinated access system in your community

Learning Session Team - By-Name List Cohort

Don’t waste time! You’ll build a by-name list for all single individuals at once, even if you’re primarily focused on chronic homelessness. You’ll need two to four key people at the Learning Session to build an A+ by-name list.

Community Representative / Key Leader - Person overseeing ending chronic homelessness for the local community.

Local measurement guru responsible for collecting and sharing data - This will likely be the Data Lead for Built for Zero already identified. This person will be responsible for monthly reporting of chronic data to the Built for Zero initiative.

1-4 People Overseeing Programs Specifically for chronic homelessness. This may be (but isn’t limited to):

Homeless Service Provider - Person that represents a local organization serving people experiencing chronic homelessness.

Street Outreach Program Administrator - Person that represents an organization leading outreach and engagement of people experiencing chronic homelessness.

Permanent Supportive Housing Provider - Person that represents an organization that operates a portfolio of permanent supportive housing units.

Housing Authority Representative - Person that represents a local housing authority’s rent assistance program that is preferred or specific to people experiencing homelessness.

Deadlines and Membership Rates

The Collaborative runs from February 2017 to February 2018.

The registration deadline is January 12th.

The cost is $10,000 US (currently $12,730 Canadian) per community for the full year of support. This does not include travel costs.
