Log Into CROWNWeb and

Change Facility User

(Same process can be used to log

into QIP 2.0)

1. Open www.qualitynet.org

2. Click Log In

3. Choose End-Stage Renal Disease Quality

Reporting System

4. Click I Accept

5. Enter your User ID and click Next

6. Enter your Password7. Select MFA Device Type8. Enter Security Code received9. Click Log In

Security Code is sent through the MFA

Device Type you selected (Phone, Text,

Email or Symantec VIP)

10. Check the top right corner to make sure you are logged in as

[Facility Name]: CROWNWeb Facility Check the top right corner to make sure you have selected the appropriate role and facility for what you are trying to do

If logging in to ESRD QIP instead of CROWNWeb, look for display

[Facility Name]: ESRD QIP Facility

11. Click Arrow to the right of Quality Programs

12. Select End Stage Renal Disease Quality Reporting System

13. Click CROWNWeb SUI under

CROWN Applications

If logging in to ESRD QIP instead of CROWNWeb, application will be different. Select Inquiry.

14. Welcome to CROWNWeb

• User Profile Menu: User Preferences

- Used to switch back and forth between

organizations and systems

- Change from Facility A to Facility B

- Change from CROWNWeb user to QARM Org

Security

- Change from CROWNWeb user to QIP 2.0 User etc.

Change Organization, Role or

System

1. From User Profile Menu, select drop down arrow

2. Select User Preferences

3. Click on Organization Switch Tab

4. Select EQRS from Change Program5. Select Organization/Role from Change Organization

6. Click Update

7. Answer Yes to pop-up. You want to switch current organization

Quick View: Switch Roles and Facilities

Step 3: Change Program to EQRS and

Change Organization to the appropriate
