Other Consumer Reports

Initiatives

• Began to expand healthcare coverage, content,

advocacy and ratings in 2004-05; big commitment!

• Provider ratings – insurers, hospitals, doctors in four

states including heart surgeons

• Care/treatments – prescription drugs, preventive

care, and Choosing Wisely campaign

• More than a dozen organizations as partners

• Our advocacy wing: www.ConsumersUnion.org

• Our main content site: www.consumerreports.org

The 16-Page Guide

• Broad overview

• Written for consumer – minimal jargon

• Focus on 3 areas of positive change

with most direct consumer impact:

- payment changes

- info/data on quality and

performance

- HIT/EHRs, digital health

• Practical advice, resources, links,

glossary (more on web)

Our Hope for Guide Use

• As a supplement to other materials distributed to

consumers

• PDF and written

• As tool to explain changing system and ACA

initiatives other than insurance

• To expand awareness of websites with info and tools

to help consumers choose providers and treatments

• To expand awareness of resources at

ConsumerHealthChoices.org and Consumer

Reports.org

Major Trends

• Emerging from intense political acrimony over ACA

• Coverage – acceptance growing

• Broad agreement on wasteful spending,

unnecessary care, inefficient system

• New models of care taking hold - if not exactly

catching fire

• Shift of costs to consumers – deductibles, out-of-

pocket, % premium

• Consumers accessing info online – engagement!

Consumer Engagement –

of One Kind

• U.S. News got a million hits in one day after August

release of “best hospital” ratings

• Two recent surveys probe use of physician ratings

• Findings: 65% aware of online reviews and rankings;

40% have “seen” or “used” them; 5% have rated a

doc.

• Healthgrades – largest provider ratings site: close to

1 million/day access it; 4 million posted reviews;

95,000 added each month over last 12 months

“Engagement” Shifts

• Passive patient ——————› Active patient

• Price/cost naïve ——————› Price savvy and sensitive

• Patient as patient —————› Patient as partner

• Hard-to-understand

info/data on quality&price —› 5 star ratings, composite scores

• Doc group level reporting ——› Individual doc reporting

• Process measures —————› Outcome and functional status

measures, and condition/

procedure specific

• “Raw” patient reviews ————› Validated reviews, CAHPS

Shifts Needed….continued

• Separate info/data on

cost and quality ——————› Integrated cost + quality

• Registries for QI ——————› Registry data for choice

and payment initiatives

• “Some” access to EHRs ———› Universal access

and notes

• Doc data entry ———————› Automated data entry

and patient-entered data

with outcomes!

• Large employer cost

+ quality data (e.g. Castlight) —› More $$$ for initiatives

that reach all consumers

1. Check medical bills and EOB forms

2. Compare prices for medical services

3. Bargain with hospitals and doctors for

reduced fees

4. Get a surgical safety checklist and a

hospital safety checklist

5. Compare treatment options with your

doctor or other clinicians.

6. Check out doctor and hospital ratings

and report cards online.

7. Track your medical history in a notebook,

computer document, or file. Or create a

PHR.

8. Create an advance directive
