A celebration of Sefton LINk’s activity

2008 – 2009
Sefton LINk was established in April 2008.
Set up as an independent volunteer network of local people and groups to play a key part in
enabling effective citizen engagement in Health and Adult Social Care decision making and representing the views and concerns of all
residents, patients, carers and groups across Sefton.

Sefton LINk spent a lot of this time establishing the network, building up contacts
and relationships with key partners.
An interim steering group was set up in May 2008 to establish the LINk and get LINk
activity started with a democratically elected steering group in place by November 2008…

The Elected Steering Group had 8 representatives from organisations, 8 individual
members and 4 Younger Peoples Representatives…
2009 – 2010
Sefton LINk received additional funding from Sefton MBC to help with recruitment and raise awareness of Sefton LINk with 2
four day road shows held in North and South Sefton...

Over 900 local people were given the opportunity to find out more about us and fill
out a survey to share their views on local services.
A 90 second promotional DVD was produced and used to help promote and raise awareness
of Sefton LINk…

During May 2009, working in partnership with a local service provider we co-hosted the ‘LINk 09’ event at the Floral Hall in Southport. Over 150 independent Care/Nursing/Home Service
Providers attended the event.
This was a chance to share our statutory role in monitoring and scrutinising services by

During June 2010 members of the Adult Social Care Task Group met with Mark Waterhouse and members of the Care Management Teams at Sefton Health and Social Care Directorate to help them understand how services are provided…
Sefton Young Advisors helped us to make sure that our Annual General Meeting held in 2010 had a focus on our work with young people…
Kim Cooper, a Young Advisor from Sefton presented information to guests at the event and got them involved in an engagement exercise to get them to appreciate the challenges which face young people in different scenarios.
Following this event Young Advisors also helped us to set up a ‘young person led advisory group’.

This was the year which brought the vision for changes to our work and the concept of
Healthwatch.
On Friday 21st January 2011 Sefton LINk worked alongside Merseyside and Cheshire LINk
colleagues to host the ‘LINks to the Future’ event which was held at Aintree Racecourse.

Throughout September 2011 Sefton LINk Support helped us to hold 3 community road
shows.
At the roadshows members asked the support team to look at how the network could work more meaningfully with communities so that
local people could learn about the work of LINk and provide a greater voice for the residents of
Sefton…

We asked Sefton LINk Support to develop a model of engagement and from the work we were undertaking with the South Sefton Clinical Commissioning Group a locality model was developed and community champions introduced…

Over 1000 patient experience forms were completed and returned to Sefton LINk Support. Patient Experience feedback boxes were launched which could be found out and about in the local community…

Key Achievements
Patient Safety
An early contribution in 2009 to patient safety was the agreement of NHS Sefton to place hooks for outdoor clothing on the outside of cubicle doors on Ward 35 at
Aintree Hospital. This idea originated from seeing hooks on the outside of cubicle doors for visitors to hang outdoor clothes on to minimize the carrying of
infections to the general population outside of hospitals in other Acute and Specialist Trusts.

During 2010 we had the pleasure of accepting ‘The Chief Executives Award’ on behalf of all
the work undertaken by LINk members…

Hospital Discharge
Hospital Discharge was identified by patients and Carers during 2008 as an idea needing review and following an engagement exercise LINk member
Kate Taylor produced a research document ‘Discharge from Hospital for Patients with Social
Care Needs ’in March 2009.
Key Achievements
This group represented an approach to working in a supportive way with all professionals
involved in the discharge of patients.

This research showed communication between services and the patient/carer as an issue of
concern.
An event was held 3rd November 2012, followed by a further report sharing recommendations…

A key outcome from this work was that Aintree University Hospital NHS Foundation Trust
decided to incorporate the findings from this work into their work on a new Hospital
Discharge policy .
Other pieces of work included the review of Hospital Discharge Summaries and a
communication and information review of Hospital Discharge leaflets…

The work of the Hospital Discharge Task Group was used by the Department of Health in their Annual Report of LINks Activity 2009 – 2010.
It was evidenced that the work of LINk and Local NHS Trusts in reviewing the patient experience of discharge from hospital had reduced the number of delayed discharges .

of Health ‘Healthwatch Transition Plan’...

Our work with the Care Quality Commission (CQC)
During 2010 – 2011 we worked with Care Quality Commission (CQC) to look at how both organisations could work together and share information.

Our work in reviewing Patient Advice and Liaison Service (PALS) reports was chosen
by the Care Quality Commission to be included within a learning set, a piece of work in which 8 LINks worked locally with them to
develop relationships…

“We have focussed on our role in monitoring local services and ensuring that concerns raised by local people are recorded and share them locally to commissioners and nationally to the CQC.
By aligning our findings to the ‘Essential standards of quality and Safety’ we can send information/intelligence to the CQC, which helps them to use it more effectively and effectively”

Transforming Community Services.
Following concerns from Sefton LINk members about the changes that were being made to
community health services across Sefton, we helped Liverpool Community Health NHS Trust to
draft and produce a poster to help share information with local patients and carers .

Eagles Rest Care Home - CQC Rating of 1 (adequate)Visited on 6th September 2010.
What we found:
• Caring home with many residents living there for a number of years
• Residents clean and well cared for by staff
Issues of Concern:
• Fabric and upkeep of the building and toilets• Poor lighting• Some issues with the cleanliness of toilets and
dining area• Dog hair on floor and furnishings• Fire Equipment tags out of date• Third Party and Liability Insurance certificate on
display.

Citing the LINks work as evidence, the Care Quality Commission (CQC) conducted a follow
up visit to the visit.
Sefton LINk were praised for their ‘enter and view’ work and further action was taken against
the home…

Southport and Formby District General HospitalVisited 1st November 2010.
Information from PALS reports and evidence
provided at 2009-2010 Trust Boards prompted Sefton LINk to work in partnership with the Trust. Wards 9a and 15b were visited whilst a snapshot
review of hand gel use of people entering and leaving a further 5 wards was undertaken.
Southport and Ormskirk Hospital NHS Trust commended Sefton LINk on the successful
enter and view Visit.
One major success from this work was a service change - increased rehabilitation input for
patients on a Saturday. This service is being provided 8.30am - 4.30pm.
