Comments: The said well by Express Energy's top hole rig with a 100' x 36" conductor borehole being drilled and 100' of 30" conductor being set. Express Energy cement service cemented the casing using 275 sacks of class A -2o/o calcium .50 pound per sack loss circulation material 50°/o open hole excess cement with good cement returns witnessed to surface and the cement mix water tested for chlorides and PH. 09/17/2016 -On site inspection to witness the 18.625"surface-mine string combination with Patterson Drilling rig# 801 on location and a 24" borehole drilled to 770' with the mine seam encountered at 690' to 697', a 18.625" 87.5 lb/ft J-55 Buttress thread casing string was ran to a depth of 742' with a hydraulic operated inflatable packer set at 456' and a DV tool set above the packer at 447'. Allied Cement service was on location to cement the casing with the bottom section between the casing shoe and mine void cemented by first pumping 50 barrels of 6°/o gel followed by 260 sacks of class A-2% calcium cement that was displaced with 226 barrels of fresh water; the swell packer was the inflated by increasing the internal casing pressure to 1,047 psi and then the DV tool was then opened with circulation established to surface before beginning the upper section cement stage by first pumping 50 barrels of 6% gel spacer followed by 1079 sacks or 230 barrels of class A-2% calcium cement with the cement displaced with 196 barrels of fresh water and casing integrity verified by pressure during the landing of both cement plugs on both cement sections with cement returns to surface on the last stage.

Allied OFS, LLC performance of services and sale of materials Is expressly conditioned upon the applicability of the Terms and Conditions listed on the service ticket provided at the job site. The Terms and Conditions Include, among other things, an Indemnity In favor of Allied OFS, LLC from Customer for damage to the well bore, reservoir damage, loss of the hole, blowouts and loss of control of the well, even If caused by the negligence or other fault of Allied OFS, LLC. The Terms and Conditions also limit the warranties provided by Allied OFS, LLC and the remedies to which Customer may be entitled In the event of a breach of warranty by Allied OFS, LLC. For these reasons, we strongly recommend that you carefully review a copy of the Terms and Conditions.
By requesting that Allied OFS, LLC perform the services described herein, Customer acknowledges that such Terms and Conditions are applicable to the services. Further, by requesting the services, Customer warrants that Its representative on the well location or other service site will be fully authorized to acknowledge such Terms and Conditions by executing a service ticket or other document presented by Allied OFS, LLC containing such Terms and Conditions.
In the event that Customer and Allied OFS, LLC have executed a Master Services Agreement covering the work to be performed, such Master Services Agreement shall govern In place of the Terms and Conditions.
