Starting the Evaluation Process
Start by clicking on the Activity icon on the main icon taskbar.
Staff Activity Screen
After selecting the Activity icon,
the staff activity screen will open.
For a probationary evaluation, use the code PRE in the Default
Activity Code.
For a step up evaluation, use the
code SE in the Default Activity
Code.

After entering the Default Activity Code select the Add button to add
the participant detail record for the staff member being evaluated.
Participant Detail Screen
After the Participant Detail screen opens,
enter the Staff ID for the person being evaluated.
Enter the Position for the person being
evaluated.
Next select the Other button to start entering the evaluation record.
NOTE: The Attendance Required field is disabled when you start each evaluation record. This field will be used later in the evaluation process to hide the evaluation record in the user fields table.

Next select the User Fields button to continue.
Entering the Employee Evaluation
The Evaluations(1) tab is where you will enter the information relating to
staff member evaluation.
NOTE: The tab will not turn red; you need to select the Evaluations(1) tab.
Complete the Evaluation Type field by selecting one of the options.

48 Hour, Training Academy, or
Step Up
The Evaluators Staff ID will be
automatically entered.
Next, enter your evaluation scores in
the appropriate evaluation area.
NOTE: At least one field in each of the rating areas must be entered. The areas include - Station Work Public / Personal Relations and K/S/A

The comment fields on the
Evaluations(2) tab cover a spectrum of activities the evaluated staff member
may have encountered during their shift.
If the staff member did not respond on any calls during their shift, a zero value
can be entered in the Number of Incidents Responded On field.
The evaluation criterions for each comment area are shown next to the
comment field; this is the minimum expectation for comments on all
evaluations. Any additional thoughts or comments can be recorded in these
fields.
Before leaving each of the comment
fields, you should push the F7 button to spell check your work.
The example, on the left, shows the type of comments that are expected, by
senior staff, for staff member evaluations.
Once you have completed all the
required fields in the user fields, select
the Save and Close button to close the record.

Finishing the Employee Evaluation
After the employee evaluation has been
completed, select the Save button and then the Close button to close out of the
Participant Detail screen.
After selecting the Save button you will
get a Validation Results message on your screen.
Select the OK button and return to the
Participant Detail record.
The Attendance Required field is now open and needs to be checked to complete your
staff evaluation record.

Once you check the Attendance Required field, the field will be disabled. Once you
check the field your evaluation is considered locked and cannot be modified.
After the field becomes disabled, the only
staff personnel, who can look at the user field
information after it is locked, hold the rank of
Chief, Deputy Chief or
Battalion Chief
Note that there are 11 total tabs across
the top of the user fields, including the Evaluations(1) and Evaluations(2)
tabs.
In the example on the left, note the two
Evaluation tabs are missing from the
user fields tab list.
The information that you entered is still there, it is just now in view for anybody
to read.

You can create multiple staff
evaluations on one activity record.
Once you have entered all your
evaluations, you will need to lock the staff activity.
Select the Other tab to prepare the
activity record for locking.
Select the Record Lock Status button to open the lock record.
Once the record lock status screen opens, check the
Record Lock field and then select the OK button to close this screen.

Select Save and Close to finish
entering your daily evaluation.
You have now completed entering a probationary or step-up employee evaluation.

Printing Blank Forms
To print blank forms, you can access the reports in one of two ways.
Use the Emplyee Eval button on the left task bar
or
Select Reports from the main Firehouse
taskbar,
Select Favorite Reports,
Select Administrative Reports, and
Then select Employee Evaluation
When the Output Report To screen opens up do the following,
Make sure Default Printer is checked,
Check the Print a Blank Form with No Data box,
Select the number of copies you want to print, and
Then click the OK button
Make sure that all the boxes are checked for each form that you want
to print blank copies of and then select the OK button to print the forms.

Printing Completed Evaluations
To print completed forms, you can access the
reports in one of two ways.
Use the Emplyee Eval button on the left task
bar
or
Select Reports from the main Firehouse
taskbar,
Select Favorite Reports,
Select Administrative Reports, and
Then select Employee Evaluation
When the Output Report To screen opens up do the following,
Make sure Default Printer is checked,
Check the Print a Blank Form with No Data box,
Select the number of copies you want to print, and
Then click the OK button
Make sure that all the boxes are checked for each page of the report that you want to print blank copies of and then select the OK button
to print the forms.
To proceed with the printing process, select the OK button. The
report Query Filter screen will open.

To run the report, the following information
needs to be entered in the query filters.
Staff ID is the staff ID for the person being
evaluated. You can enter more than one staff ID on this filter line.
Evaluation Start Date is the date that was
entered in the main staff activity screen.
After entering this information click on the
Run Query button to run the report.
You have now completed entering and printing a probationary/step up employee evaluation.
